Plenty of Red Flags I am currently having a problem with Pacific Clear Vision Institute that has led me to file complaints with The Oregon Department of Justice, The Oregon Medical Board and The Oregon Division of Financial Regulation. Below is a summary of my situation. Please contact me at brian.m.litton@gmail.com if you've had a similar experience: 1. Billing Malpractice Delayed Invoicing: There was a significant delay in billing; surgeries performed in early February I was't billed until April 30th. I had no knowledge of an outstanding balance until I received a phone call from billing on April 15th. No bill in the mail, just a phone call asking me if I was aware of my outstanding balance of $4,000. Contradictory Records: My original receipts, from payments I made on the date of surgeries, explicitly stated I was paying for "Toric Lenses" and showed a $0 running balance. Recharacterization of Payments: Two months after the fact, the company claimed the money I paid was actually for "facility fees" and not the lenses, directly contradicting the written descriptions on my receipts. 2. Advertising and Misrepresentation Price Switching: The company originally quoted $3,000 per eye for the *entire procedure* before insurance. They later changed this to $3,000 per *upgraded lens* only, significantly increasing the total cost. False Insurance Claims: They explicitly told us that insurance would cover most of the surgery but later claimed the $3,000 for each lens when they knew from the beginning that lenses are not covered by insurance. Omission of Fees: When offering the North Bend clinic as a convenience to save us from driving to Eugene, they failed to disclose that choosing this location triggered an additional "facility fee." I have two payments I made in North Bend that say they are for Toric Lens but when I asked, they told me, "no those payments are a facility fee". 3. Good Faith Estimate (GFE) Violations Failure to Provide Timely Notice: I did not receive a GFE until April 16th, nearly two months after both surgeries (February 5th and 19th) were already completed. The GFE was dated for January and when I told them I did not receive in January, they told me they sent to me but I did not open the email. Deceptive Documentation: The company backdated the April GFE to January to make it appear as though they had provided it before the surgeries. Mathematical Errors: The numbers provided on the GFE are inconsistent and do not add up correctly. Bad Faith Delivery: The company admitted they knew I had not opened the email containing the GFE, yet they still argued that I had "received" it, bypassing the requirement for informed financial consent. Conclusion of Impact The cumulative effect of these actions was the denial of informed consent. Had the company been transparent about the true costs via a timely and accurate GFE, my wife and I would never have agreed to the surgeries. By withholding this information and providing contradictory billing statements, the company prevented us from making a rational financial decision or canceling the second surgery. read more

I went in with an ophthalmology problem and they told me I was seeing an M.D. He was an O.D. I settled for this as I was worried about the flashes I was seeing. Having previously worked in ophthalmology I realized this was concerning. To make a long story short, they said I was fine and did I want an eyeglass rx. I said yes And was told I needed to pay $75 more up front. I couldn't afford it and left with no prescription or concern about the flashes. Jump forward 4 years. I get a letter they owe me money. Where to begin... my dad needed new glasses because his is about 12 year's old. It started with an eye exam, then oh, he needs cataract surgery. That took place. Then out of nowhere he also needed Retina care. The Retina doctor did an eye exam. Note: they share the same office. I still had to call and bug them for eyeglasses for my dad. They gave me some false information and my dad was taken to the Eugene office. No, the elderly man in the wheelchair needs an appointment for an eye exam first. Why they couldn't use the one from the Retina doctor? OK, then the eye exam took place. Then they told us that my dad didn't have eye glasses covered on his insurance. Yes, it is. Then we had to make yet another appointment for glasses. That turned into a oh, buy the way your out of pocket is $400.00. NO! I called another optometrist and explained what happened. They were awsome! Now he finally will have the new pair of glasses that we went into Pacific Clear Vision at year ago for. ​ read more

I went to Pacific ClearVision Institute because the ophthalmologist who performed my cataract surgery eight years ago told me I would likely develop "secondary cataracts" in the future. When my vision started declining again, I assumed this was the issue and that PCVI could take care of the procedure. When I called to schedule the appointment, the scheduler asked whether I would need "both eyes done." I said that should be determined by the doctor, not me -- in hindsight, that should have been my first warning sign. They seemed more interested in booking procedures than in verifying whether I actually needed one. During my visit, some tests were done, and the doctor recommended procedures for both eyes. I don't have an in-network eye surgeon in the area, so this was an entirely out-of-pocket expense for me. Although the surgeon also owns the clinic where the procedure is done, the estimate I was given only included the surgeon fees. The charges for the clinic itself were missing -- so the final total ended up being double what was quoted. Read the fine print carefully. After the procedure, my vision did not improve at all. At the follow-up appointment, a different doctor saw me. After hearing my vision had not improved they performed an astigmatism test -- something that was never done before the surgery. Why this wasn't checked initially, I have no idea. It felt like they rushed me into an expensive procedure instead of making sure it was the correct treatment. I was told at the follow-up that I could either have another surgery to correct the problem or get glasses. There was no way I was going to schedule another procedure with this provider after the first one made no difference. And yes -- even though this was a follow-up visit, I was charged an additional $75 for the glasses prescription. If I were this provider, I would be embarrassed to have missed something so common and would have offered to correct the mistake at no charge. So that's my experience: why mention a $75 fix when you can perform a $3,000 procedure instead? I would strongly recommend getting a second opinion before agreeing to anything here. read more

Dr. Eric and the staff at Pacific Clear Vision Institute were great during my cataract surgery for both eyes. They were incredibly kind. And if I hadn't had this procedure, I don't know when I would have found out I had A-Fib. Nothing showed up on the first eye. The problem showed up on the next eye procedure. So that was a good thing. They were on it as to contacting my primary care physician. Anyway, all is good. read more

I had exceptional care with Doctor Eric and staff at Pacific Clear Vision Institute during and after cataract surgery in both eyes. They were knowledgeable and patient with all my concerns, questions, and delays in the process. The doctor gave me access to contact him privately whenever I needed help or more information. The nurses on the surgery team were skilled and good humored which eased tensions and reduced the stress of surgery. I highly recommend recommend Doctor Eric and his team! read more

I had a minor eye surgery here (removal of chalazion). I called around several places and found this place to be mid range price wise for paying OOP. They were able to fit me in quickly and the procedure went well and ended up costing exactly what I expected it to. Staff were professional and reasonable, it does seem that the desk staff (at least the days I was there) are unable to handle any billing questions or inquiries as their billing people aren't on-site. I saw several confused and frustrated customers come in to resolve a billing issue and be told instead to call the place. I had the same issue when figuring out the final items of my bill, though it did all work out at the end. I don't know if they're remote or outsourced or what, but for that reason it may make sense to get quotes and billing figured out first via email. read more

We had a 2:15 appointment and was sat in the exam room when somebody came in to do an eye chart and a pressure testing 20 minutes later. We sat another hour before a doctor or other person came in. We had opened the door and tried to find somebody but nobody was around. Highly unprofessional would recommend going to anyone else in town aside from office. At check-in we saw no less than 10 people working so it doesn't make any sense. read more

We were patients at Pacific Clear Vision for 10+years. No more. We have always seen an opthalmologistcs as I am a diabetic, have Sjogren's, and take plaquenil. NEITHER of us wear glasses. Our last visits were handled by very unprofessional staff...reception on up. The OD had the nerve to lecture me on proper patient behavior after the unprofessional ma would not answer my simple question why i was seeing an OD as a recheck on an atypical floater that needed to be removed as had one in my other eye had been done 1-2 years before. She would not answer my question, got up, slammed the door and i guess went crying to the OD. The OD (not an opthalmologist) told me the surgery to remove the floater(remember, I had had it done in the other eye already) was complicated and I would be blind(same surgery in both eyes, second surgery elsewhere, perfect 20/20 vision now). We used to love Pacific Clear Vision but will no longer recommend and have moved my business elsewhere for much better care. Janssen wrote this review: I'm writing a review on Clear Vision, Eugene; Chase Gardens Surgery Center in Springfield; and Clear Vision in Cottage Grove because I have direct experience with the Eugene and Springfield offices and because my ophthalmologist, Dr. Joseph Mando, works for all three offices. I have had two surgery's to date from Dr. Mando on August 2, 2023 and August 16, 2023 on cataracts associated with both my eyes. Let me be quite clear that if you value the quality of your eyesight, as I do, you will NOT get involved with any of these companies or Dr. Mando. While I will try to be brief, I have a lot of information to back up my conclusions regarding avoiding these companies and Dr. Mando. Probably one of the most disturbing aspects of my involvement with this organization was that they scheduled me for surgery's in early July at Clear Vision, 1125 Darlene Way, Eugene and emailed me that the cost would be a little over $100 per eye with my insurance, which is AETNA. Just days before the first surgery they informed me that Clear Vision in Eugene was "Out of Network" and that I would have to wait for weeks to get scheduled for surgery's at Chase Gardens Surgery Center in Springfield since they were "In Network" and that Dr. Mando also worked there. Shortly after these surgery's I contacted AETNA and asked for verification that Clear Vision in Eugene and Dr. Joseph Mando were indeed "Out of Network". I recorded this call and was told in no uncertain terms that Clear Vision and Dr. Joseph Mando were "In Network"! Obviously, this was very disturbing information because it on the surface meant I had been lied to and then manipulated into going to a different company. Once going to Chase Gardens Surgery Center I was informed that the first surgery would cost over $1,200 and the second surgery would cost over $600. I felt like I was being overcharged during a very vulnerable time in my life. Most importantly, during the surgical procedure they used 1/2" tape to secure my head. I felt strongly that this was not proper protocol for such a precise laser surgery and the slightest deviation in my head movement could compromise the outcome of the surgery. I talked with a neighbor of mine who had recently had cataract surgery done by Drs. Fine, Hoffman & Sims, a very reputable company, and was told they used two soft pillows filled with sand to securely position his head with no movement at all. After my first surgery on August 2nd I was seen by Dr. Mando two days later. My vision was really poor and when we met on a Friday he told me that my vision would improve significantly by Sunday or Monday. This was absolutely not true. In fact, when I saw him again on August 17th after my second surgery, both my eyes continued to be extremely blurry and I was continuing to experience significant headaches due to the condition of my eyesight. I had now reached the point where I had very little confidence in the credibility of Dr. Mando. Also, I had expected Dr. Mando to give me an explanation as to why he was working for 3 companies but he chose to offer no information whatsoever. I decided to consult with Drs. Fine, Hoffman & Sims in order to be assured of a responsible, objective opinion due to the fact my eyes were not significantly improving after weeks following the surgeries. Finally, I decided to initiate a lawsuit in the Circuit Court of Lane County so a Judge could hear the evidence and decide on a fair resolution. I'm asking for $6K based on my financial losses associated with their dishonesty and pain and suffering. If possible, I will offer an addendum to my review after the court hearing. Please be aware that the staff at Clear Vision have manipulated many review sights like Endorsal, Facebook, Birdeye, Glassdoor and others to inject their own reviews. When I have attempted to enter my review, these websites totally rejected my review and would not allow me to enter it in spite of multiple efforts to make my opinion known. This is a company you should avoid at all costs. It's been almost two months now since my surgeries and my vision is still poor. There are good companies out there, so find one! I do recommend Drs. Fine, Hoffman & Sims who I will be seeing soon for an evaluation. read more

Blind Right Eye, they tried giving me a hard contact lens , it seamed to improve my vision, but they wouldn't let me wear it for a week to see if that's what I wanted. It's hard to put into my eye and NOT comfortable at that short time they let me try it. They said they would call me about wether the insurance would cover the cost! About $2000 ? So Far I have enjoyed visiting Clear Vision, and the work was fine. MY COMPLAINT is overcharging me. They automatically charge copays, However after my Claim was paid on 2 invoices, there was NO copay due according to my Insurance. I did email On Sept 1, 2023 for a REFUND, and was told they are really backed up as several people are owed Copays. I said ok, and I would wait. After 30 days, I emailed again, and was told by Petra that it could take another 2 weeks for an Electronic check to me mailed!/ 6 weeks is absolutely absurd. You have had my funds LONG ENOUGH. I would like my refund ASAP! I am now going to contact my Insurance Company an complain about my Overcharge and NO REFUNDS SO FAR. Not a great way to do business. I had cataracts in both eyes and was referred to Dr Churney at Pacific Clearvision. He is a wonderful person and a fantastic doctor. I've worn glasses or contacts for over 20 years. I had the most comprehensive eye exam I've ever had and it was decided that I would get both eyes done with different lenses for both near and far and its been wonderful! This was 2 years ago and I've been very happy with my new eyes. I have not had to use any reading glasses at all due to the different lenses. Thank you Dr Churney and staff! read more

My experience at Pacific Clearvision was first rate. I had my cataracts removed and Panoptix lenses inserted. The staff was very friendly and reassuring. The procedure was quick and painless. Dr. Ambiti inspires confidence and was highly recommended by my regular optometrist. It's been two weeks since the first eye was done and one week since the other., and so far the lenses are working as advertised. I golfed within a few days of eye number 2 being done with total comfort and great vision both far and near. No more moving my head around to focus on the ball. Right now I'm writing this review on my iPhone, again with no glasses. Pretty amazing. read more

this place is a circus. very unprofessional staff. i can't stress enough how unprofessional they are. our first time visit was filled with unprofessional attitude from staff. they gave a wrong appointment date and after we came back. the younger receptionist was so rude and keep intruding with our conversation with the other staff. she doesn't even have phone manners. i had observed her taking calls. she feels like that patients are asking favor for her. environment is not friendly, very disrespectful and not courteous! ****kim from optical department banged the phone and hang-up on me after complaining about her job that she has to go to work 2 hrs everday that i dont need to know. if she doesn't like her job then she should quit and not complain and be rude to patients. overall, this place is a joke!!!! read more

Hidden charges! My double lens replacement was elective and 100% out of pocket so I got a total cost amount directly from Dr. Ambati. Please be aware this does not include Pacific Surgery Center costs. Dr. Ambati or someone there should have absolutely made it clear that Pacific Surgery Center charges are in addition to the total cost. Pacific Surgery Center is in the same building and feels totally connected to the business. This resulted in over 30% more charges and gave me a shady feel about the whole operation. read more

Never ever ever go here!! So many issues! Lost the order for my contacts and got them over 6 weeks later. Got a bill that I paid on May 27th on their patient portal and then get a bill in the mail today saying that I did not pay it. The charge is right there on my credit card statement! I'd been living with itchy stinging red eyes for many months. In Jan.2013 I saw Dr.J.deGuire, who told me I had pingueculas & he'd be happy to surgically remove them if I desired. Surgery was NOT what I wanted to hear (the whole idea of surgically removing pingueculas is preposterous - they're not visible or irritating). I went elsewhere - to eventually find out red/itchy/stinging was Blepharitis/Conjunctivitis (a common problem - not mentioned at ALL by deGuire), with exactly the symptoms I was experiencing. And surely easily seen/diagnosed with the opthalmolgic microscope used in all optical appointments. In March 2017 I saw B.Ambati in hopes of getting LipiFlow eyelid cleaning treatment (blepharitis help). He diagnosed contagious viral conjunctivitis & said my immune system will eventually take care of it. No & no! It turned out to be a concretion (tiny rock) embedded in eyelid (another bleph problem), causing small vertical scratch in eyeball & big pain as well: left eye stayed involuntarily closed, and I was virtually incapacitated for a month: couldn't read, drive or do much of anything due to hurt. (I now wonder if LipiFlow treatment I came in for would've caught the calcification in upper eyelid.) Considering the quality education of their doctors I'm more than a little surprised about their lack of blepharitis expertise, & worse, lack of help for it. Especially considering blepharitis affects people mostly of European origin (Lane Co's pop.=83%), so bleph.sufferers in this area must be quite a few. Both times, after no help from deGuire or Ambati, I finally ended up seeing Norman Goo at 18th Ave.Rainbow Optics, who helped big time, fixing both problems(ongoing Doxycycline has helped since 2013; removing the rock in 2017), regaining my health. read more

I recently had cataract surgery at Pacific Clearvision and was told by a staff person that if I purchased multi-focal lenses, I would never again have to wear glasses So I paid $4000 for the lenses and still need glasses -- bifocals and reading glasses. Nobody told me that multi-focal lenses do NOT work for all people. I wrote three letters to the medical director regarding the inaccurate information but have received no reply. read more

Dr Batista, Dr Hunter & Dr Deguira, I seen them all for different eye concerns & would HIGHLY recommend all of them. Dr Batista found wet macula degeneration. Dr Batista is very good at his job & thank God he found what had caused my vision loss. He spent quite a bit of time with me, was very helpful & is a very genuine caring doc. He referred me to Dr Hunter for many tests. Dr Hunter is by far the most gentle of all gents out there! He is highly qualified & very caring. We are still in the process of shots into my eyeball to restore my vision. So far we've got about 75% back! Yesterday we changed med's to destroy the rest of a rogue blood vessel that was growing into my eye. You can trust Dr Hunter with your vision. Dr Deguira found that I had narrow angles & at risk for glaucoma and risk of going blind, which meant changing 4 meds or do a very simple painless solution! Just use his cool laser to drill "pressure relief valves" into each eye & never have to worry about high eye pressure & blindness from glaucoma again! This is the coolest preventive solution ever! And it does not hurt. Dr Deguira is a fun guy for sure & highly skilled. He is going to restore my husbands vision next month! You'll love any 1 or all 3 of these docs! And you'll trust that they truly care about your eye health. Very highly recommended! read more

I recently saw Dr Bala Ambati for cataract surgery. It was an example of total excellent health care. I had two procedures they were not at all alike but both were excellent with the same results, "I have sharp clear vision"! I can't say enough about Dr Ambati, he has a very quiet, calming personality that totally removed any nervous anticipation. He thoroughly explained all that would happen to me in the operating room, before and after. I appreciate all the time and effort to make me comfortable. His staff was always there to assist and offer anything I might need. My surgery was quick and painless..In fact there is no memory of the first one, I had a nap. After the surgery they graciously offered me a contact since my eyes were seeing so entirely different and then proceeded to teach me how to use it. Every staff member I dealt with gave me such exceptional assistance I will recommend this office always. The same results I experienced, from the front office, to the surgery front office, nursing staff, and the clerks who assisted in explaining my fees. I am diabetic and I went to WalMart to get new glasses. After dilating and looking into my eyes the doctor told met there was so much blood that he couldn't see to the back of my eyes. He contacted Clear vision Center who asked how soon I could could come in. It was a Friday at 3:30 in the afternoon and they took me that afternoon. I was diagnosed with Diabetic Retinopathy and the doctor started lasar treatments that same afternoon. I did not have health insurance at that time but they continued with the lasartreatments for a total of eight times over the next year anyway. Each treatment was $8700.00 totalling over $70,000.00. for just the laser treatments alone, not including the all the other stuff. I never received a bill, they never even asked about what I was going to do about my bill They treated my case as a "Hardship case." I did not even owe them a dime. I will always be grateful to Dr Osterlough for saving my sight and it was free... no charges, no bills OMG!. Had I not been treated for this at that time I would have lost my left eye entirely and been blind in the other within a year. I am able to see now because of Clear Vision Center. The staff there are so friendly and compassionate. Thanks guys I owe you my sight, I will be forever grateful for that. read more
