I ordered a Primavera Pizza without prosciutto. Pizza had prosciutto so I sent it back. I watched guy take off prosciutto and grate cheese on it. Brought it back to me. I questioned the guy & the hostess spoke up that it was a new pizza. This vegetarian does not find this acceptable! To start off, the pizza is delicious. We had the margarita pizza and my friend got the Salciccia. They're a good size personal pizza that you can definitely have leftovers. My other friend got the Fresca salad and a side of meatballs. She said the salad was alright but the meatballs were excellent. They have a good amount of drinks to choose from, we got Manhattans. No complaints. The service on the other hand was not the best. It took a while to get our drinks, however our food came out quickly. To get our bill took forever. We had finished eating and we sat for a good 20 minutes before our server came over again. The people at the next to us had been waiting for the check for 40 minutes and finally got up and flagged the server down to pay. We did the same. They were however pretty packed and he was busy with tables. I do however recommend this place. The aesthetics are pleasing and the patio is excellent for a summer night. read more

While in Moab, stopped in to Antica Forma for a good meal after spending a few days on the road. My wife had the meatballs Parma, and I had the chicken Parmesan. We also had an excellent bottle of Sangiovese with dinner. The service was good both dinners were tasty it was a nice meal and experience. read more

Pizza so good we went back two days later. This is pizza in a class by itself. We were visiting Moab and noticed the crowd and thought this is a good sign. So we have it a try. We will never regret the decision, except maybe we won't want any other pizza. And ask about the name; it is apt, but I don't want to give it away. read more

To make a long story short, I had several issues with the service I got here, not just from the waiter, but from the cooks too. I asked to speak to the manager who told me that he'd "take care of [the bill] for me", but I haven't seen a dime since footin' the entire amount. The wood fired oven pizza was amazing. Crestwood soft and chewy just like Napoleon pizza crust should be. We had the calamari for an appetizer which was pretty good and then the Caprese salad which was the worst we've ever had fresh homemade buffalo cheese was completely dried out. Great option for outdoor dining especially during these Covid times. Staff was attentive and very friendly. We ordered the homemade burrata and meatballs as appetizers and both were very tasty. Having been to a solid number of famous naples style pizza places around the country I can honestly say that the pizza here tonight was on par with the best! No joke.. first time out of towner and on an empty stomach. So time will tell, however.. its gonna haunt me for a while that one of my top 5 pizza places is over 1000 miles away! read more

Had a lovely dinner here on a Sunday night in Moab. In general, dining in Moab can be frustrating as many places do not take reservations and so you end up walking in and waiting great amounts of time for dinner - however, when we went to Antica Forma we were seated almost immediately in a lovely outdoor area. Pizzas are described as "personal" which is probably a bit generous (unless you are my husband...) but were delicious! I had a veggie option and he had a spicy pepperoni pie. We also split a salad which exceeded my expectations. Our server, Joe was friendly and attentive. Came in with a group of 11 adults on a Friday night. We didn't have to wait too long so that was a plus. Got 7 pizzas, a calzone, a salad and garlic bread. Nothing to write home about. The menu was hard to read as the ingredients on each of the pizzas was overly complicated and the prices WERE VERY HIGH. One pizza came out wrong and our friend had to wait for her food. No discount was given even though she got her meal when we were all almost done. Automatic gratuity was on the check but the server never told us. read more

First the service was outstanding. Our drinks came out quickly and the appetizers followed in short order. The bruschetta was different than the usual but scrumptious. The food was decent. We had one pizza and lasagna each with a salad. The bill with tip was $71 which I felt was a bit hefty considering we only had water to drink. The decor is a bit hodgepodge and has a diner feel to it. I would expect a classier feel based on what we were charged. The experience did not fit the price that was paid IMO. read more

Pizza was pretty good. The crust is what did it for me. We ordered apps (caprese and some weird looking mozz rolls with arugula) and pizza and I emphasized that we would like to enjoy our apps first. Sure enough Mariah our server put all the food in at once. Pizza came out 5 min before apps. Sent back the caprese and the weird mozz rolls were god awful. Why is that even on your menu? Overall our server was awful. Her name was Mariah. She didn't bother welcoming us or introducing herself or give us any type of suggestion. Come on. I'm a server in Hawaii and I know the steps of service. Y'all need to understand consumers are the reason you are in business. Do better. read more

How on earth this place has an overall good review by people is beyond me. Hands down, the worse restaurant I've eaten at...the service was good but the quality of food was terrible. The caesar salad was decent but the croutons were cheap tasting and only a few were in the bowl. The chicken Alfredo was watery and the taste was flavorless. The pepperoni pizza wasn't eatable. Our tab was $63 I tipped $10 and left with out finishing even half of anything we ordered but the salad. Ruined my drive home. I only gave the second star because the service was good. read more

By far the best food in town. Saturday night, busy as can be, and the food was coming out fast and hot! Good service, very fresh ingredients, and overall a great experience. Went there twice in two days while we were in Moab. My kids were begging for more (and they know pizza). Ingredient combinations were right on and the cheeses were spectacular. But most importantly, this is the best gluten-free pizza you will ever have. Most GF pizza doesn't come close to the real deal, but the crust here is phenomenal and even had some flavorful char like the neopolitan crusts. I was absolutely floored. read more

This is an amazing place! It's only been open a few months but it was already busy on the Friday night we went. First we were asked if the bar seating was ok and we choose the pizza bar. It was so fun having a front row seat to the pizzas assembly line. They have it down to an art. We ordered fried calamari. It was delicious. I also had a virgin drink that was yummy. But pricey. I got the pistachio pesto pizza which I cannot say enough about how delicious it was. The sausage was so good and there was plenty of cheese. The crust was probably the best part. So thin but also chewy at the same time. Some love went into that crust. Brock our server was awesome. He stopped to talk about the origin of the restaurant and was super personable. He talked is into the fresh peach pie and we were not disappointed. If you're in Moab check it out! read more
