We decided to take him home for the last few days of his life so we could be with him. We cried and so did the staff as we were going to miss them all so much. I do not recommend this facility because of that and because of the lack of care she received. Oh and she kept being found unresponsive because they were giving her insulin when she was barely eating. We asked them to stop giving her insulin but they never stopped. Her death could have been prevented from happening so soon had they taken better care of her.

The lack of communication, poor coordination, and unprofessional behavior from the supervisor made an already stressful situation unnecessarily difficult for our family. My mother had spoken directly with the doctor the day before and was told that my father could go home the following day. However, when it came time to discharge him, the supervisor repeatedly insisted that they could not release him. We were left dealing with conflicting information and unnecessary frustration, with little clear communication about what was actually happening. To make matters worse, we were told that if my father remained at the facility, he would no longer be receiving physical therapy or occupational therapy. Essentially, he would simply be sitting there without receiving the services that justified his stay. Despite this, the supervisor continued to ask whether he was going home or staying, rather than professionally working with the team to resolve the situation. To her credit, the case manager was genuinely trying to find a solution. Unfortunately, the supervisor's behavior was particularly troubling. She came across as rude, confrontational, and completely unprofessional, even appearing to argue with the case manager in front of our family. This was unacceptable conduct, especially in a healthcare environment where patients and their families are already under considerable stress. The one positive aspect of our experience was Julio, the physical therapist. He demonstrated genuine care and compassion toward our father and our family. Although his ability to resolve the administrative problems was limited, his empathy and professionalism made a meaningful difference. He showed the kind of human decency and concern that we expected from the entire facility. A rehabilitation center should provide more than physical therapy. Families deserve clear communication, coordinated discharge planning, respectful treatment, and staff who work together in the patient's best interest. Unfortunately, our experience fell far short of these expectations. Management needs to take a serious look at how communication, discharge procedures, and staff professionalism are handled at this facility. No family should have to navigate confusion, conflicting messages, and confrontational behavior while trying to ensure their loved one receives appropriate care and gets home safely. I sincerely appreciate Julio for the compassion he showed, but one caring employee cannot make up for such a frustrating and disappointing overall experience.

My doctor recommended Sundance Creek Post Acute in Banning, so I went online to do a little research. Sundance Creek did not have the greatest reviews but then again the other facilities in this category did not fair that well either. So Sundance it was! It is established that online reviews for rehab centers can be mostly negative and grim. I decided that since my stay went as well as it did, it was incumbent upon me to make sure that my positive experiences deserved to be shared with others so that there is hope for those who are seeking a facility that offers promise. Following is my takeaway: EXCEPTIONAL CARE: All of the CNAs and LVNs were caring, helpful, and even funny, but I have a special shout-out to Louis Obioha. He has a truly caring CNA heart. If you had any anxiety before his shift started, his calming presence, while very caringly addressing your needs, leaves you with an "all is well" feeling throughout the rest of his shift. MEDICATION CART: "Singing Danny", Karla and Vanessa (LVNs) brought their knowledge of all the medications that you should be taking along with much cheer to the daily med passes. EXPERT THERAPY: The physical therapy team is top-tier and abundant. Their knowledge and encouragement were the primary drivers of my recovery. FACILITY & COMFORT: The facility is "clean as a whistle" thanks to Mr. Longie, who is hard at work by 4:00 AM., who takes great pride in his clean and shiny floors! The housekeeping staff were very thorough with their daily sweeping, dusting and mopping of our rooms. My room was very clean and bright, the bed was comfortable, and my linens were changed daily. LEADERSHIP: The Director, Mr. Brett Kendrick, is an empathetic leader who ensures every patient's needs are met. He wears many hats. Organizer. Peacemaker. Overseer. Facilitator and is able to pick up the slack of others when necessary. He truly is the anchor of this facility. SOCIAL LIFE: From bingo and crafts to church services, there was always something to do in the activity center. My only minor gripe is the food. For the Love of Benji. Can nothing be done to make institutional food palatable? That said, I have to tell you that my choice to stay at Sundance Creek was the absolute correct decision to make. I do not regret the option that I chose. If you need skilled nursing and rehab, Sundance Creek is absolutely worth your consideration. I am including a few pictures of what happiness looks like.

In my case, I was blatantly lied to by my social worker, 2 case managers, and even my insurance. I was looked in the eye and promised to be sent to a facility that will take care of my medical needs by a nursing staff. That was the farthest thing from the truth. I was sent to a shared care facility where there are NO nurses, only caregivers. I repeatedly do not get needed medications and insulin and have almost died here because of the lack of care. So, do not believe the Social Services person, Case management, the DON, the charge nurses here regarding anything about your health and care. They will all lie to you as they all did me. The only good thing I can say about this place is that they keep the CNA's in the dark about everything and they are nice at least. Again, fight to stay here, do not let these creeps lie to you and send you to purgatory as they did me.

But in the short time they were there and on the three visits prior to their moving, I knew this was the place for them.  Everyone was so friendly, kind and more than willing to help in any way.  By the time they moved in the staff was already calling them by their first names and making them feel welcome.  The facility is well kept, very clean and has great food. The homemade cookies at the front desk were always delicious. The smaller size of the facility made it much more inviting.  While my parents were in hospice care in Sunny Rose Glen, the staff was wonderful to my parents, my brother, and myself.  They helped so much and after my parents passed it was evident how much the staff cared.  Thank you Sunny Rose Glen for making my parents' last days more comfortable for them and our family. After a fall with a broken hip, my elderly father who has dementia was transferred here from the hospital. When something like this happens, it's a whirlwind of activity. The hospital is working to get him out, the hospice company was working to find something, and The Bradley Gardens was the first place that had a bed available. So, he was transferred there site unseen. When my mother and I arrived, I took a deep breath as I pulled into the parking lot, and thought.... okay, this is only temporary. We can move him somewhere else. We walked in, and in the front lobby they had a bunch of patients sitting comatose in wheelchairs watching TV. I guess they have the TV close to the nurse's station so they can keep an eye on them. I'll get to the nurses in a bit. I went into the manager's office to sign all the paperwork and was surprised how dirty and broken down even her office was. I signed the paperwork reminding myself this was only going to be temporary until I could find another place. We then took the long hallway, all the way to the end where they had my dad. The entire place was old, broken down, in dis-repair and dirty. As we walked past the nurses station, they were all engaged in chit-chat that did not sound like anything to do with taking care of the patients. The nurses were dirty too, one of them looked like she hadn't washed her hair in at least a week. Ugh... if those are your personal hygiene standards, how do I trust you are going to keep my father clean? We walked past rooms that reeked of urine. And then the final kicker.... we walk into my father's room. It has 5 beds in it. A room no larger than a 2-car garage. The beds and other furniture looked like something that had come from a war time infirmary 50 years ago. But what really sealed the deal for me was the fly paper! FLY PAPER! Hanging from the ceiling, above the beds, with DEAD FLIES ON IT! I immediately called the hospice company and told them under no circumstances would I leave my father here. We got him out that night, and he came to my home temporarily until we found a decent place for him to spend his last days on earth. The Bradley Gardens is a place where poor people are dumped to die. It's disgusting. They should be ashamed of themselves, and the County of Riverside should revoke their licence.

What happened over the next several days was completely unacceptable. On his first day, he was not given a shower or an examination. His pain medication was not given in a timely manner and was sometimes hours late. Even worse, throughout his entire stay, no one came to assist him with using the restroom. For someone recovering from surgery and sent to a rehabilitation facility specifically because they need additional care and assistance, I find that absolutely inexcusable. I spoke with a supervisor, Jojo, about the problems with his pain medication and the lack of assistance. When I called my grandfather on August 12, the same problems were still happening, and he STILL had not started physical therapy. At this point, my grandfather was already fed up with the facility. I convinced him to give me an opportunity to go there myself and speak with staff in person before we made the decision to remove him. On August 13, I went to the facility and again spoke with Jojo because, quite frankly, she seemed like one of the only people there actually doing work. I specifically explained that my grandfather needed to start physical therapy, needed a shower, and needed his pain medication on time. She appeared surprised when I told her that he had received neither physical therapy nor a shower since arriving. I was assured these things would be addressed. While I was there, I was also extremely unimpressed with the condition of the grounds. The property looked dirty and poorly maintained, with dying plants and an overall neglected appearance. I learned that the property apparently used to be an orphanage, and unfortunately the entire place gave me the feeling of a place where forgotten people are sent and left behind. After watching how my grandfather was treated, that feeling only became stronger. On August 14, he FINALLY received a physical therapy session, three days after arriving. However, he still had not received a shower, still was not receiving assistance with the restroom, and was still having problems receiving his pain medication on time. I texted Jojo and told her that I would be removing him from the facility the following day. At that point I was no longer willing to keep giving the facility chances to provide the level of basic care that should have been happening from the beginning. Only after I complained again by text was he offered a shower, close to 6 p.m. He declined because of how late it was. His pain medication issues were also continuing. Once I made it clear that we were removing him, I suddenly began receiving calls from higher-ups. By then, it was too late. I had already raised these concerns repeatedly and had also contacted his insurance case manager. The final experience on August 15 made an already terrible situation even worse. My husband went to pick up my grandfather as we had already told the facility we were going to do. According to my husband, the head nurse (a tall woman with brown hair and glasses) yelled at him, berated and belittled him, and even accused him of stealing a wheelchair THAT WE BROUGHT WITH US. She then loudly talked about my husband to another nurse in the hallway. The behavior he described was shockingly unprofessional and completely inappropriate for someone working in healthcare. Maybe she was possessed by a ghost of one of the bratty children from the orphanage. I was later told that she behaved this way because we were taking my grandfather out against medical advice. That does not excuse her behavior whatsoever. If the facility was so concerned about his medical well-being, that concern should have been demonstrated through timely medication, assistance with basic needs, hygiene, rehabilitation, and attentive care from DAY ONE. You cannot repeatedly fail to meet someone's needs and then suddenly act outraged when their family decides enough is enough. What makes this even more frustrating is that this nurse reportedly claimed she did not know about my grandfather's condition, yet suddenly had plenty to say and had plenty of questions when we were removing him. My husband asked when my grandfather had last received his pain medication and was not given an answer. He also requested aftercare paperwork and was not provided with it. Thankfully, my grandfather's case manager through his insurance helped us get everything sorted out afterward. We gave this facility multiple opportunities to correct these problems. We spoke with staff, raised concerns repeatedly, went there in person, followed up again, and involved his case manager. Removing him was not some impulsive decision. It happened after several days of watching the same basic care issues continue despite repeated complaints and assurances that they would be addressed.
