My doctor recommended Sundance Creek Post Acute in Banning, so I went online to do a little research. Sundance Creek did not have the greatest reviews but then again the other facilities in this category did not fair that well either. So Sundance it was! It is established that online reviews for rehab centers can be mostly negative and grim. I decided that since my stay went as well as it did, it was incumbent upon me to make sure that my positive experiences deserved to be shared with others so that there is hope for those who are seeking a facility that offers promise. Following is my takeaway: EXCEPTIONAL CARE: All of the CNAs and LVNs were caring, helpful, and even funny, but I have a special shout-out to Louis Obioha. He has a truly caring CNA heart. If you had any anxiety before his shift started, his calming presence, while very caringly addressing your needs, leaves you with an "all is well" feeling throughout the rest of his shift. MEDICATION CART: "Singing Danny", Karla and Vanessa (LVNs) brought their knowledge of all the medications that you should be taking along with much cheer to the daily med passes. EXPERT THERAPY: The physical therapy team is top-tier and abundant. Their knowledge and encouragement were the primary drivers of my recovery. FACILITY & COMFORT: The facility is "clean as a whistle" thanks to Mr. Longie, who is hard at work by 4:00 AM., who takes great pride in his clean and shiny floors! The housekeeping staff were very thorough with their daily sweeping, dusting and mopping of our rooms. My room was very clean and bright, the bed was comfortable, and my linens were changed daily. LEADERSHIP: The Director, Mr. Brett Kendrick, is an empathetic leader who ensures every patient's needs are met. He wears many hats. Organizer. Peacemaker. Overseer. Facilitator and is able to pick up the slack of others when necessary. He truly is the anchor of this facility. SOCIAL LIFE: From bingo and crafts to church services, there was always something to do in the activity center. My only minor gripe is the food. For the Love of Benji. Can nothing be done to make institutional food palatable? That said, I have to tell you that my choice to stay at Sundance Creek was the absolute correct decision to make. I do not regret the option that I chose. If you need skilled nursing and rehab, Sundance Creek is absolutely worth your consideration. I am including a few pictures of what happiness looks like.

    I am writing to formally express deep concern and outrage regarding the treatment my aunt received…read moreduring her stay at your facility. What my family and I witnessed was not just disappointing, it was inexcusable! From the moment she was admitted, there were immediate red flags. Despite the presence of numerous staff members, basic standards of care were not met. My aunt was repeatedly left in soiled garments for extended periods, and critical medications were delayed or entirely missed. The neglect was not subtle....it was blatant. Angelica, who was assigned to her floor during several shifts, exhibited a level of indifference that was both shocking and dangerous. Rather than tending to my aunt's needs, she seemed more concerned with avoiding responsibility. On one occasion, I personally witnessed Angelica ignore multiple patient call lights, including my aunt's, for nearly an hour. Melissa, another staff member, displayed an alarming lack of compassion and professionalism. When asked direct questions about my aunt's care plan and physical therapy schedule, she responded dismissively and without any clear knowledge. It took over a week for any form of physical therapy to be initiated. And by then my aunt's mobility had severely deteriorated rapidly. Amanda's conduct was perhaps the most disturbing. She was observed speaking rudely to several patients and was especially careless during basic hygiene care. There were several days where my aunt was not bathed, and her wounds which should have been routinely cleaned and monitored were visibly worsening. Amanda never once acknowledged this or reported concerns, despite it being her direct responsibility. The facility itself was in disarray. It smelled strongly of urine and feces. Patients were heard crying out for help, and some were even seen wandering unattended. This is not a safe or healing environment. When I brought these issues up with the director of the facility, the social services director I met with passive indifference and redirected me to a case manager, Melinda, who seemed more interested in placating us as a family than taking action. Every conversation felt like a bother rather than genuine concern. To add insult to injury, when we decided to transfer my aunt to hospice care due to the rapid decline in her condition they still were completely unaware of what was going on and of her case. She was treated worse than a dog. There may have been a few nurses who tried their best, but isolated acts of kindness cannot undo the widespread and consistent negligence that led to my aunt's decline. This experience has been nothing short of traumatic for me and my family. I urge anyone considering this facility to think twice. The level of carelessness, unprofessionalism, and outright neglect we encountered has had lasting consequences. No one deserves to be treated this way especially not our vulnerable loved ones... there needs to be a better standard of code for this facility and they need to be held accountable for their actions! In other words if you have a family member in need of care I would steer clear from this facility. Hell you'd probably be better off taking care of them at home with no experience.

This building may be a bit older, but it is very clean. Everyone here is super nice and helpful. I am so sad that it only a short term care facility, as I would love be to keep my mother here long term as my mother seems happier and healthier. If you are looking for a caring facility for your parent/parents, I highly recommend Vista Real Post Acute.

She did better with her physical therapy in the new room too. They got her to walk again after her fall last January she stayed til March The staff did their best but was under staffed at times it took awhile to answer the call button. My mom said she got tired of waiting and used her wheelchair to get to the toilet. The girls never came to help some of the time. It was better than some places I've seen and worked in.     I've had enough...This place is a joke. It's been several months of nonstop bull. There are some…read morestaff members who are great like the two ladies I've met at the front desk. The PT staff is great but the nurses/cnas leave much to be desired. There is maybe a third of the staff that's great and on it but the others need to be held accountable for their actions. Weekend and night staff will leave patients sitting in their own urine or feces for long periods of time. The social worker onsite is rude and only professional in front of other people. She's very passive aggressive with comments. They come and take stuff out of patients closets and give it to other patients to use twice I've had to retrieve my family members jacket that has their name on it from another patient or the staff will use the patients personal items and steal their personal drinks and drink them. When the upper management leaves on the weekend and at night the staff gets really loud shouting to each other and laughing up and down the halls when patients are sleeping. Counted several times over the last several months where they leave patient lights on and don't respond for close to 30-40 minutes. The response given to me was they are all on break.(everytime?) it's not just been the one night or day. Wonder how many patients have had medical emergencies that they don't address right away. They feed diabetic patients nothing but sugar when their restrictions/preferences are listed at least from what I was told from other patient's family members and from my own observations, are they trying to send them back to the ER. Or the patient whose daughter told me has diverticulitis and they feed her items that can cause flare ups. The cnas come and take pull ups and wipes from other patients because according to them there is not enough supplies. They do this while in the middle of cleaning others up. That's a nice way to cause cross contamination and cause other patients to develop rashes/possibly catch diseases. I'm sure the CDC, OSHA and the state would have a field day. I'll be sure to be filing a concern with the state.

    Came here from an Assisted Living facility in Hemet that had caregiver problems as well as letting…read moreirresponsible ones remain employees, while firing others for doing marijuana on the job. and neglecting patient care needs, incontinence, giving adequate nutrition in meals as well as avoiding patient food allergies and dislikes, making sure oxygen tanks and concentrators remained in perfect working order, oxygen flow was not impeded. no falls or injuries. Place clean, etc. and patients feelingbalert and content, an important thing. And the Law, as are all these things for Elders in California. This hospital provided Emergency Care that was reasonably fast for a holiday weekend, my gurney only waited an hour , but I was having breathing and cardiac problems caused by someone at that assisted living facility in 92545 and so got some priority in being seen, and was taken to a small room, parked, and a nurse virtually moved in to watch me. very calming and polite. The lady Dr showed within minutes and got tests off to the labs ,EKG set up, blood draws ordered and me examined physically asking and answering questions, then Admitting me, and I moved there quietly and soon put in a more appropriate bed for my larger than life size ( caused by lymphedema an incurable body altering disease l). In the Ward most of the Drs were very knowledgeable and efficient and I felt confident with them, most nurses and lvn's were more than capable and most were friendly and understanding that they needed to be ultra careful of other non involved but still painful body areas in treating my current one and most complied. A few however apparently feel their job and ego is more important than a patients long term safety and comfort, even leaving pranks such as cold water poured on one foot uncovered as I slept , but really, if an aide feels her ego is more important than reactivating old spinal injuries in a elderly patient , who am I to complain? A Human Being that's who and a California Elder Citizen protected from such displays of childness. No job as a nurses side is more important than the overall health of the patient. If you don't know a patients limitations in movement ASK. Don't assume high weight is because we Big People eat too much or are lazybor druggies. Oh, and you saying ":sorry" out of the side of your mouth and arguing with us when we comment about you ignoring us and increasing our pain by your refusal to listen doesn't cut it. One thing I expected from any treating Dr in a hospital treating me is that they know why I am in a Ward bed. Hopefully they also can speak knowledgeablybabout my condition or tests they are running. What I do Not expect nor want is said Dr handing me religious " donate here" prayer cards and suggesting we talk about religion...doesn't really fill you with confidence about his intent to help you live longer ..but one Dr here ..well, he didn't three times want to discuss my case or even know it. Thankfully Dr Cox did in a later Dr visit and I am grateful for his help. Overall 4 stars, other problems as noted. Recommend you come here? Um... maybe.

    My partner walked in there at 2am feeling faint after not being able to hold in any food or liquids…read morefor the past week just to be taken to the back be given an iv and not seen by any doctor until 11am where the doctor then just dismissed his concerns was extremely condescending provided no information to him and just sent him home now he's at home feeling the same exact way because he was not helped at all! definitely don't recommend this place at all unless you want your time wasted and not be taken seriously
