With the help of Johann from Home Health, my mother was placed at Medical Center Convalescent Hospital. Before her admission, I read several Yelp reviews and honestly, many of them were not very encouraging. However, this facility was close to both my home and workplace, which allowed me to visit my mom every day, so we decided to give it a chance. We arrived at the facility around 4 PM on a Saturday. To my surprise, Ana, the Director of Nursing, was there waiting for us. What immediately impressed me was that it was the weekend, yet she greeted us with a warm smile and welcomed us like family. She personally showed us to my mom's room. Although the building itself is older, it was very clean, there was no urine smell in the hallways, which was a huge relief. My mom's room was spotless, with freshly changed lines and the housekeeper was even hanging newly washed curtains. Anna, then introduced us to my mom's CNA, nurse, and the entire admitting team, including PT,OT, dietitian and admitting staff etc. Everyone was friendly, smiling, and genuinely welcoming. At that moment, I felt reassured and at peace knowing I was leaving my mother in such caring hands. Around 5PM, my mom was already served dinner with the correct diet. I was truly impressed by how efficient and organized the staff, completely the opposite of what I had read in some reviews. The very next morning, I arrived around 8AM and found Glenda, my mom's CNA, already giving her a bed bath. Shortly after, Christy from PT came in to assess my mom. Again, I was amazed, this was Sunday, and services had already begun. The PT team was extremely patient with my mom, who was initially very scared to walk. She tended to tiptoe instead of placing her feet flat on the ground, and her gait was unsteady. With time, encouragement, and consistency, she improved significantly.Throughout her stay, all the nurses and CNAs were patient, loving, and kind. Every time I visited, my mom had nothing but good things to say. She remembered everyone by name, and even the housekeeping staff became her friends. My mom preferred staying in her room, so the staff would come in, sit with her, chat, and keep her company. That meant so much to us. One thing that truly stood out to me was Ana's leadership. When the facility was short-staffed, Ana didn't just supervise, she worked alongside her staff, even doing skin treatments herself. During the Palisades fire, when patients were transferred to this facility, I personally witnessed how the entire team worked together seamlessly to accommodate everyone. Their teamwork, dedication, and compassion were remarkable. After 11 months, my mom fully recovered and is now able to walk safely with a walker. We decided it was time to bring her home. When the staff found out, they were genuinely sad, the best way. They expressed how much they would miss her. There were so many staff members involved in my mom's care that I can't name everyone, but they truly became her second family. I am honestly teary-eyed while writing this review because I witnessed firsthand how dedicated, hardworking, and compassionate these people are. They loved my mother as if she were their own family. My family is deeply grateful for the care and compassion this nursing home showed my mother. From the very beginning, the staff treated her with dignity, patience, and genuine kindness. They cared for her not just as a patient, but as a person.The nurses, CNAs,housekeepers, and support staff consistently went above and beyond to ensure my mom was comfortable, safe, and well cared for. They listened to her, respected her, and treated her with warmth. Knowing she was in such loving and capable hands gave our family great peace of mind. Kudos to all the CNAs! Joyce became one of my mom's friends-she never missed giving my mom a shower. She moisturized my mom's skin with lotion-I could tell coz she does not have scaly skin! I have to mentioned Zoraya, Tata, Linda, Stephanie, Jose, and so many more (sorry I can't remember everyone's names but I know your faces) thank you to all of you! To all the nurse you're all heroes! Thank you Jocelyn for preparing all the discharge papers today. To Jenelyn-thank you! If you are looking for a place for your loved one, this is the place. I highly recommend Medical Center Convalescent Hospital. When we left, my mom was crying-not because she didn't want to go home, but because she would miss everyone so much. They truly became her second family. We took pictures before leaving, and you could clearly see the love-staff hugging her, crying! As my mom goes home, we leave with nothing but appreciation and gratitude. Thank you for treating my mother like family and for the compassion you showed her every single day. I agree with other comments the place stinks and the staff is not much better. They left my mom sitting in soil for 2 hours. They knew she had an infection when she was transferred but used that as an excuse to not provide therapy. I know it's not easy dealing with sick people, not enough help and never enough pay, if you don't like helping others find a different job. We got her out as soon as she finished her meds. And her roommate was so grumpy.

My 1st impression was: Omg the smell is atrocious! The smell hits you smack it the face! My mom laughed because she can't smell that well...I told her omg you're so lucky! It smells like Feces, urine, and bleach. The bleach doesn't cover up the overwhelming scent of feces or urine. You can tell they don't take pride or clean correctly. Most of the employees seem nice but some of them seem like they're just going through the motions. Elderly patients yelling for help while employees just walk right by. Didn't even acknowledged them. Trust me I so understand that old people can be very difficult but you signed up for this job! Thank goodness my mom isn't as old as some of elderly patients here. So she could do some stuff herself. As they don't check on patients very often. Leaving them in feces & urine for long durations. Or not changing their potty chairs or bedpans promptly. Mom said the food is nasty and even though she's diabetic they still kept giving her desserts with sugar and other high carbs in here meals. You'd think they'd have a special plate for those with diabetes. She even told them but they continued to bring it. So she'd just pick at stuff and I'd bring here meals and stuff to snack on. Unfortunately this place isn't family friendly. As they have no seats in the room fir visiting and the rooms are very small for two occupants. I honestly feel bad that anyone's loved one has to come here. Mom said they're were only a few kind hardworking nurses /employees and the rest didn't care. That the doctors that she had at this facility were hack jobs. Overall I'd steer clear of this facility if you can. They get 2 stars for the few nice people that took care of my mom. This place has so much room for improvement! Do better Arrowhead Springs don't you have parents and grandparents. You'd never want them treated this way!

Other than those things she does all her own dressing, personal care, eating and is active in the community. The owner was Dan and the director was Tommi when she moved in. It was wonderful then. It was sold to Oxford Health Corp and the current director of the facility acts like he cares but then is unkind. The Director acts good to the residents when corporate comes to visit. He is very rude, disrespectful and condescending to my mother. The Villas is a less desirable place to live now compared to how it was. Making money and cutting programs, staff and the quality of the food is now the Corporations #1 priority, not the residents. The Receptionists, Activity Directors, Med Techs, direct care givers and Admin Assistant are wonderful people and really care about the residents. Although many of the caregivers are under trained and often short staffed so answering call buttons takes a long time. But that is the fault of their directors. Most of them are very kind, just not properly instructed. The food is not gourmet as they claim. They say they offer transportation but the bus is often broke down. The building, rooms and grounds are nice but if you have to share a room, due to financial reasons, be prepared for sharing a bathroom, refrigerator and to have competing noise with TVs, tablets, phone conversations and visitor conversations. The cost of the room is reasonable for all the amenities but if you need any kind of care the cost is ridiculous! So, why do we keep Mom there? Partly because she's 90 but mostly because she has built community with some of staff and residents and is willing to take the bad with the good!

They both started out good but had a constant turnover in management which meant there was no stability of supervision of the staff working below them. There was many times the kitchens would run out of food for their residents, which was unbelievable. There would be no managers on call at night if a problem came up that I needed addressed. The Villas of San Bernardino is the shining star of Assisted Living facilities. Most of the management have been here many years, some over 18 years. With that stability in their management, the entire facility runs extremely smooth. There is always a manager on duty after hours that will answer your call. I cannot say enough good things about the Executive Director Kenya, Office Manager Yvette, Sales Manager Gladys, Activity Directors Janet and Courtney, Chef Ismael, Maintenance manager Juan, and all the caregivers and office support. My Mom was on hospice her last year here and was extremely well cared for by her caregivers. My Father in Law was here 15 years ago and many members of my church were here as well. Don't make the mistake that I did by looking at any other facilities, the Villas IS THE BEST hands down for your loved ones, you won't be disappointed.

It can be difficult to find a place where you feel truly confident leaving someone you love, but the Palm Center has given us that peace of mind. The facility is clean and well-maintained, the food is very good, and there are always plenty of nurses available. What really stands out is how quickly they respond -- whenever the call button is pressed, someone always comes right away. You can tell they genuinely care about their residents. Because of their kindness, professionalism, and the level of comfort they provide to both residents and families, I feel they truly deserve a full five-star rating.

    My family's experience at University Post Acute has been extremely concerning…read more My grandmother is a high-risk resident with impaired orientation, hearing impairment, dependence for transfers, inability to safely toilet independently, and known fall risk. Following a fall at the facility, the incident initially appeared to be downplayed and we were told she was only being sent to the ER "as a precaution" because she was a new resident. We were later informed that a male CNA reportedly provided her "privacy" because he was male. In my opinion, this demonstrated a serious lack of critical thinking and judgment given that my grandmother is not oriented, unaware of her surroundings, requires transfers, wears diapers, and is not safe to be left unattended. Despite visible bruising and ongoing pain, additional imaging was not immediately pursued. Days later, we were informed that she suffered a fractured clavicle. What has been most upsetting is the lack of proactive communication, delayed reassessment despite continued pain, and the fact that family members have had to continuously push for further evaluation and answers. I have repeatedly requested reassessment, pain management, and communication from leadership, including the Director of Nursing. Families place trust in skilled nursing facilities to protect vulnerable residents and respond appropriately when injuries occur. I hope the facility takes these concerns seriously and improves resident safety, communication, accountability, and staff judgment moving forward.
