We were referred to Jolene Nelson who was extremely compassionate, professional and informative. She answered all of our questions including the difficult insurance ones with certainty and clear options. She also followed up while my father was still in the hospital. My brave stepmother decided to aid my father back to health at home - which I am not sure she would do again. With this recent health scare it has become a frequently shared story of my father's. It is amazing how many of his friends have recovered at Apache Junction Healthcare and gave rave reviews. If in the future my family needs help, we know what facility we are using.

Unfortunately, our experience was deeply concerning. A formal complaint has been made with ADHS and is under investigation. Despite providing all of his medications upon admission (including detailed instructions), we later discovered that one of his prescribed antidepressants (Sertraline) was never administered during his stay. Multiple staff members contacted us about his severe depression and uncontrollable crying, even suggesting adding new medications--without us realizing he wasn't receiving the one he was already prescribed. Communication was poor. We were told we'd receive follow-up calls from nursing staff and the facility physician, but those calls never came. This left us making critical decisions without accurate information. By the time he was discharged, his mental state had significantly declined and he is still not recovered from this. He was far more withdrawn and cognitively worse than when he arrived. To make matters worse, several of his personal belongings--including hearing aids, eyeglasses, and clothing--were lost. His discharge papers indicated he had no hearing loss/aids, yet the staff admitted they could not locate them. Incredibly frustrating for my dad. The facility was unresponsive in addressing our concerns about my father's medication after discharge. While I was able to get my dad to an audiologist in early February, the VA was not able to provide new hearing aids until mid-March, so my father was affected for about 10 weeks. If you know anything about dementia and hearing, you understand how traumatic this was for all of us. We would strongly urge other families to carefully consider their options before placing a loved one here. I would NEVER recommend this facility. The guilt of placing a loved one with someone beyond family is tough enough, so to have endured the lack of care with this VA-recommended facility is heartbreaking.

She had prior medical issues, so we were told that her care would be managed by a medical team which included an M.D., only to find out that an M.D. was let go and no longer on staff the day my mom passed away. We don't believe she was ever under the care of a medical doctor any of the days she was at this facility, which is medical negligence and fraud! Kevin Kreisler, PT. DPT.CERT. DN is my go-to person, who is taking care of all my issues very thankful. They all work together as a team, listen to all your needs, take the time to work on your issues to keep you informed. They do many different Pt. before, after surgery, dry needling, other needs. They help each patient, to make sure doing well, and explain if you are not sure why this exercise is good for you. They also give you a plan if you can do them at home. Easy to make appointments. I was told they have the lowest patient/nurse ratio of 10-12 patients per nurse and that's not true. They were at 17 patients per nurse, and the nurses were very tired and run down. Some would get rude and make comments about not having the time, and how short staffed they are. Unfortunately, that's probably the case everywhere. We had an incident, and they had a very dramatic response to the incident. They talk very condescending to patients. The case manager told my mom that she sounds like a "child". They are missing compassion for their patients. My mom was given a note from Dr Monday morning that she's good to go home if she can walk 200 feet. They would not discharge her until Wednesday. The only reason they got any stars is because their physical therapy team is absolutely amazing.

I had visited a friend there several years ago and was impressed with the facility at that time. My mom fell in March, and after a week in the hospital, we had to go to a different rehab facility at first because Santé wasn't on her insurance. However, that place didn't take good care of her, and she ended up with a Stage 4 pressure sore and sepsis, resulting in another, much longer, hospital stay that included three weeks in intensive care. When she was ready for discharge to rehab that second time, Santé was covered by her insurance, so I insisted on going there. We were at Santé for about six weeks, beginning in June. Some have said in other reviews that the décor is "dated," but I disagree. The style is Southwestern, which is quite appropriate for Arizona, and it never goes out of style. The photographs on the walls are of beautiful Arizona scenery, and the rooms are set up like hotel rooms. Santé also has a Starbuck's café and some beautiful outdoor patios where guests can get some fresh air. The food here is much better than most hospitals and rehab facilities provide, and guests can order just about anything they can think of, even if it's not on the regular menu. Each room has its own refrigerator and microwave. Most of the rooms are private, so most guests don't have roommates, but the shared rooms are really semi-private, with each guest in his or her own separate space, and the kitchenette being the only shared area. The physical therapy gym is wonderful, with many different types of equipment for different needs, and the therapists encourage family to get involved in the therapy, which takes place six days per week -- far more and far better than the first place my mom stayed. The wound care nurses took excellent care of my mom's pressure sore. By the time we left, it had filled in far more than I had been told to expect, all thanks to their superior wound care. Everyone was happy to answer questions and help however they could. My mom's case manager, Angie, was very kind and patient, and she gave me lots of information about post-rehab care possibilities. With her help, we were able to get post-rehab assistance that worked best for our situation. I would also like to give special thanks to Nurse May, Physical and Occupational Therapists Cisco and the two Lisas, and especially Speech Therapist Aaron, who went above and beyond for us. I was happy to take my mom home when she was discharged, but I was also sad to leave because the staff all seemed like friends. Most of my mom's allotted rehab time had passed before she was really able to start getting back on her feet, but I'm happy to say that, in continuing at home what I learned while participating in therapy with her at Santé, she's now improved by leaps and bounds and is well on her way to resuming the activities she enjoyed prior to this misadventure. I look forward to bringing her back to see everyone after her recovery is complete. I can't say that there weren't ANY problems, however. One female CNA was changing my mom's brief one morning, and some fecal matter had gotten into her wound. I asked the CNA to make sure she cleaned it out thoroughly, and she tried to tell me that the fecal matter had originated IN the wound, which was impossible in this case. When she finally left the room, I finished cleaning out the wound and asked the wound nurse to double-check that it was clean. This same CNA also used my mom's phone charger to charge her phone without asking, which I found rather presumptuous. The only other problem I experienced was that I always took my mom's laundry home to wash, but I was gone for one full day and night, and during that time, they managed to lose one of her nightgowns. I provided the price of a comparable nightgown for reimbursement, but to date, we have received nothing. Those minor problems aside, the gist of the matter is this: if you or a loved one needs to stay at a rehab facility for physical therapy, I cannot recommend any facility higher than Santé of Mesa.

Daily physical therapy was instrumental in my 'at home' recovery. I had several nurse visits and physical therapy sessions in my home, which was another layer of recovery care!
