As long as you bring your ID and your insurance card and hand it over you're ready to go. An abundance of parking both outside @ ground level and in a lower structure as well, the entrance might be hard to see since it's kind of tucked away in the back but I posted it in one of the pics in my review. What's especially frustrating is that their posted hours are until 3:30 PM, and they even had a 3:20 PM appointment slot available. I completely understand wanting to wrap up at the end of the day, but if you're open until 3:30 PM and accepting appointments at 3:20 PM, you should actually be open and available during that time. It really felt like the staff simply didn't want to do the work and decided to close up early. If you don't want to serve patients until your posted closing time, then don't offer appointments that late. Very disappointing and unprofessional.

I called OPTUM who is our provider and they said to decline treatment from this company and that to reschedule after this debacle.
