Think twice before you contact this unethical egoistic dentist. This matter is escalated to Dental Board of California because of Leonard Lem's dishonest, manipulative and unethical conduct.
Lem overbooked patients (mostly non-English speaking) before his vacation. The office ripped off patient and exposed me to unnecessary radiation since I had CBCT done in another office weeks ago. they insisted I repeat the CBCT. Lem was not paying attention to what I informed him and put down the wrong post-implant frequency of pain and wrong tooth number. He refused to revise the appalling errors in his report to my neurologist and the general dentist David Koo who caused me permanent brain nerve damage from an implant because of his connection to Koo who paid for the consultation. Not only he lied to cover the errors in his report in terms of my post-implant frequency of pain, he also produced a phantom witness (his front desk staff) who was not in the room during the discussion of my pain. He lied, twisted facts and put words in my mouth. When I refused to be manipulated, he hung up on me.
Initially, I thought he might have mixed up the pre-implant frequency of pain with post-implant one. However, his lies and perjury on the phone made me believe it was done intentionally to down play to the general dentist's advantage. Below please find my email to him after his call.

During the consultation, I informed you explicitly about my post-implant multiple ER visits, the constant pain I am suffering every day, to the extent that my helpers need to hold me while walking and the lack of sleep every night due to pain. I mentioned to you I only had three hours' sleep the night before because of pain. How does that translate to pain occurrence of only three to five times a week in your report?!
When I called your office regarding the errors in your report, Susan mentioned she was not in the room when we discussed the related issues. In fact, she ushered me in and left the room with my helper. When I was talking about my ability in handing stress, she came in and interrupted me to ask you about other patients. Other than that, she not present during our consultation. To state Susan was in the room and heard my post-implant frequency of pain is despicable, manipulative and unethical.
You are not being impartial since David Koo paid for the consultation. This is the reason you refused to make any kind of revision. You asked me if I want you to be an expert witness which is ridiculous given your association with my Koo and the lack of credibility reflected by the deviations in your report I was reluctant to have a consul with you because of this connection and poor yelp reviews. You are not the first oral surgeon who diagnosed me with neuropathic pain which I concurred. I only came to you based on my neurologist's recommendation on biopsy to rule out the possibility of infection.
With the same training as or if not better than Warren Buffett, I am meticulous and responsible. I myself made several revisions in my reports even though they were not technical errors such as yours before they were circulated in Federal Reserved system. I am still wearing different hats in my life now. Losing a sharp mind and having to rely on harmful medication around the clock for the rest of my life is devastating. You mentioned that in your practice, no one has asked you to revise your report. Do you believe you are God? Even media makes corrections. I caught a crucial error in my orthopedics notes and he revised it without any issues. Like other reviews had mentioned, most of your patients are non-English speaking ones. How many of them know how to fully understand your report ? How many of them have my caliber to be able to detect the major deviations? You admitted that you have never come across any patient with my kind of symptom. So how can you deny and lie when patient tells you mixed up the pain frequency pre and post implant?
Post-implant frequency of pain and tooth number in your report is major deviation from what I presented to you. You put down the pre-implant frequency of residual pain (post extraction and bone graft) as post-implant pain occurrence either intentionally or by mistake.
As a dental provider, it is your fundamental responsibilities to get the facts right and ensure they are truly reflected in your notes or report. Your report causes confusion and damages if this case goes to court since you made up evidence to the defendant's advantage. The reason you chose to call vs. reply via email demonstrated it is a matter of ego vs. ethics. Is it professional conduct to refuse any kind of amendment and ask the patient to staple her email to your report?
