My husband was transferred here from a rehab facility to receive rehabilitation for a below knee amputation. He was at deaths door, practically, when he was admitted. Incredibly weak, out of it, at times lethargic from the care he had just escaped from. At American River Center, it's lively, clean, the staff is exceptional, and above all the physical therapy has been the best my husband has received the whole year of this newly experienced amputation ordeal. Who ever oversees the management of this facility, my hats off to you! It smells GOOD in here, they take care with organizing the menu, they provide weekly newsletters with the menu to their patients, and you can make requests to the menu. They allow family or visitors to come and go. The staff is very friendly and very attentive. The facility is always being maintained whether it's inside or out. I really just don't have enough nice things to say about it here. By the time my husband was discharged he was able to walk with assistance of a rolling walker. Compared to when he arrived needing 3 people to help him out of the car, there was literally no need for help upon discharge. Physical therapy helped him reach his goals. I am so grateful as his caregiver! Above all, I felt comfortable knowing he was in good hands and never worried about his cared. This is saying alot for SNF's!!! I'd give more stars if I could, but 5 it is!! Thank you American River Center!

    It's certainly better than the three others my husband was unfortunate enough to have to endure in…read morethe past six years, but there were problems with a bed that injured him needlessly, and the food on his diet was very lacking. There is a clear disconnect between the kitchen and the other staff at the moment but the rehab. staff is mostly very good and as they don't have long term care they do seem make a genuine effort in that area at least. I'd recommend using it if the need arises, and keeping a close eye on it while you do.

    This place killed my mother. She was having a heart attack and tried calling the nurses for 3…read morehours. They ignored her. Thank goodness she had her cell phone and called 911. If the nurses would have responded to my mom sooner she would still be be alive. 2 days prior my moms roommate had the same issue. My mom and the patient tried for 5 hours to get in touch with the nurces. No one responded. My mom called 911 to get her help. Everytime she wanted to talk to the nurses about her care they were to busy. The staff does not care about their patients. My mom almost fell off the bed twice because they have no bed rails. They dont care. They also conveniently loose loose legal paperwork. Everyone that I know who has had love one there have died. Funny when my mom was in the hospital she could get up and walk than she goes to this place and she cant get up let alone walk. All this place does is want your money. They dont care one bit about their patients, your loved ones. If my mom could have gotten up we would have put her in a wheelchair and gotten her out of that hellhole.

The management is unwilling to risk their own career on trying to make this place better for patients. They have a high turnover which is attributed to crazy scedules and not enough staff to cover absences. The nurses who have left tell me there is no reason to stay. No loyalty either way. Even the therapy department has similar problems.

Sometimes, the food tray they serve for our meals lack stuff for the meal. But other than those 2 comments, I am satisfied with the therapy I'm receiving, as well. I feel stronger everyday. And most of the workers here are really nice and friendly.

    My husband was admitted to this facility following parathyroid surgery. He had hyper parathyroid…read moremyopathy, which left him without the use of his legs. He could not transfer alone. He fell out of his bed the day he was admitted at night. He had fallen again next to his bed. It was not seen by the CNA. The only reason I found out about the 2 unreported ( to me) falls. They stated that my husband was the point of contact. "Hey, sir, you've fallen"........point of contact bs. Got that changed. Call me. He never told me about the falls. I read his Kaiser chart and discovered it. They slapped a diaper on him and told him to pee in it. When he told me that, I went to the facility. A CNA, who spoke broken English stated that they didn't want him to "soil" the bed. He finally ripped them off as soon as they put them on him. It's humiliating to be told to soil yourself, instead of transferring to the commode. Which is why he fell the first time. The doctor assigned to my husband saw him maybe twice a week for under a minute. Every week they padded his ADL's. He is a fall risk, and yes, he fell there. PT thought that my husband walking down the facility hall and back ( while following behind him with a wheelchair to catch him when he couldn't walk anymore) as self walking with walker. They decide to discharge him. I had to appeal to Commence Health. I went to second level appeal, and won. All I wanted was my husband to be able to walk, and be strong enough to be discharged to a two story home. My husband called his Primary care physician at Kaiser. Primary doc wrote a letter to the facility that my husband has ataxic gait, and is a fall risk. To discharge would mean return to hospital. It was faxed immediately to facility. That was on a Monday. I thought "What a week of bs, and waste of time with them". Then came this week on Wednesday, they hand him another discharge notice of Medicare non payment if he isn't discharged by today, August 7th. They bullied me about the costs if I was to keep him there. $550.00 for bed. $1200.00 if it includes his PT---the only reason I wanted him to stay. CalPers, who stated that he is fully covered IF the PRIMARY doctor deems necessary, which he did. Nope, they take the facility doctor ( who is also a MD for Kaiser, and just floats to their Kaiser approved facilities ) There are three in Sac. We were disallowed to place him in a facility closer. Nope, have to use Sacramento services. I have called Licensing, Commence Health, CalPers. I submitted a complaint with Member services regarding the two Kaiser employees who bullied me about the money, and refused to take his Primary care's diagnosis and warning. I had to call Commence yesterday to allow him the PT his Primary care said he needed. I can't lift him or move him to transition from bed to walker, he can't shower by himself, he has a real difficulty eating. An ADL of 5 is a blatant lie. Oh! About the food. Always cold by the time it reached his room. Pizza, a small side salad, a dinner roll ( with pizza?) some kind of cookie thing, and milk. I have been bringing in food for him, as the food is generally mystery meat in the evening, and he's always hungry. . All they give them in between meals is ice water. That's it. I brought Gator Ade, sliced cucumbers, tomatoes and fresh fruit. A big bag of beef jerky. A new patient was brought in my husband's room. Such a nice man. He could not walk, and needed a lift to get in and out of bed. He wanted to go back to bed, and made the request.One lady left the room, leaving an employee who had no idea how to use the lift. The patient was explaining how to use it to her.I went in the hall to second woman and stated that the patient is telling gal #1 how to use it. I have never seen a more chilling transfer. They hurt him, he told them he needed a pillow under his hip. They had no idea what to do for him?!? I grabbed a pillow and had him roll to his side, and relieved his pain. I always came with cupcakes or cookies for the front desk staff. they are overworked, underpaid, and the ratio is typically 15 patients to one CNA.The ladies who were on the floor when I would be there were always kind. Except for the diaper slinger. Parking is a joke. Employees double park. I am asked every time I come the make and color of my car. I use the disabled spot, because I am) Yesterday afternoon I pulled into the parking lot. A black Tesla was completely blocking the Disabled parking spot. After paging the owner to come move their car, it belonged to one of the nurses. So today I lost the Commence appeal. I am so tired of fighting with the discharge with Melany, and being threatened by her. All the time it takes to fight for your loved one's best interest is exhausting. Never again, don't put your loved one here, unless you hang out all day and bring food.

We frantically began trying to find her a spot, but it was hard because of her somewhat unique care needs. When we toured Aegis, we were skeptical. Without exception, the recruiting/admissions staff roll out the red carpet no matter what facility you're considering. They engage with staff during your tour, hype up the longevity of their employees, brag about the extensive amenities provided, answer all of your emails and calls immediately. Unfortunately, as we discovered at Atria (review pending), that treatment can end abruptly once you sign the contract and write that first check. But not at Aegis. Here, that excellent sense of security and care you get during your first tour is something they obviously strive to maintain. And it shows! After a full month at Aegis, all I can say is that I'm more impressed every day with everything, from the grounds to the staff to the food to the activities. Even the admissions coordinator continues to assist us and answer every question we have. The staff is happy, even if you're not touring. Someone comes in to check on my mom no less frequently than once an hour whenever I'm there, usually twice. If it's not the pet therapy, medication management, snack cart, or other scheduled thing, it's someone popping in to ask mom if she'd like to go to dinner or an activity, or simply if she needs anything. It's clear Aegis takes pride in its facility. It's well-maintained and concerns are promptly addressed. If the person who'd normally take care of it is out, someone will be there to try to fill in. The difference is night and day from the last place. For example, at the prior facility, we were only able to arrange transportation successfully about three times. The remaining fifty or so times, they ignored us or gave excuses. At Aegis, I had a last minute request to get mom a ride to the dentist and their response was "Don't worry about it. We'll make it work!" The grounds are well-kept and the facility is clean. Plus there's an adorable fluffy white dog that lives there and lots of times is just cruising around greeting folks. Aegis feels more like a giant shared house than a hospital, like some other places do. Many residents leave their doors open. It feels safe and I feel good knowing my mom is in such capable hands. Finally, they make extraordinary efforts to get to know you and your whole family. Some staff actually take notes about conversations with residents and families in an effort to personalize every single experience. I don't know how, but Aegis seems to have perfected the art of residential care. I cannot thank them enough.
