Pros- The woman working the desk was probably the best receptionist I have ever found in any medical facility in Phx, "enough said." The rest of the staff were nice The facility was clean, relaxed, and organized. Cons It took three hours to get an X-Ray. That is way too long for a facility that only had two people waiting to be seen ahead of me. Granted, I did not know what was going on inside but only two people left in all that time. I can see if it was packed but it was not. I know I was a walk in, but still. Not their fault but an FYI, the construction on Bethany and 19th Ave going north is a nightmare and extremely dangerous. I am resolved to not go near Christown Mall area for a year. It is extremely difficult and borderline impossible getting into and out to this facility alive! I would have given it four stars but took off one for the wait time. I did not take off a star for the construction since that is not their fault. However, giving fair warning to others to be careful or chose another Simonmed facility in town.

I specifically asked for a female tech. I was assured that the appointment was scheduled with a female and after all this particular location is called the women's center. well, I drive and arrived and waited only to find out the tech was a male and they have no female techs there at all. I was very disappointed and upset because there is a reason I wanted a female. This appointment was rescheduled for another week and because of the nature of the appointmen, I don't think they should make me wait that long. I attempted to submit a complaint. They put me on eternal hold and finally hung up on me. The process of submitting a complaint is an additional burden on the patient and they don't want to hear you out. they will never learn how bad they really are. This is not the first time I have had scheduling issues. They've scheduled me at the wrong office for the wrong imaging service and gave me the run around. All in all, I wonder how our insurances tolerate this kind of service for their members.

Should there be any emergencies or questions, he would not address them in writing. He continued to talk over me. I asked the front desk to help us so we wouldn't have communication barriers. Deaf does not mean we all can read lips. Some can, some cannot. Some can hear better, some cannot hear well. Some can speak well; some cannot. This violated my ADA rights. This tracks the character of the center behave to the people of representation; no compassion for that. Seek elsewhere.

The front desk folks and super friendly, as well as patient and informed. Paperwork doesn't take too long. The radiology technicians are wonderful and respectful, gentle and patient. The ultrasound tech if that's the correct way to label him was amazing and comforting given I was getting a sensitive ultrasound done. He had classical music while conducting the ultrasound. I know others have had negative experiences, but mine was positive. I highly recommend that you complete all of your online documents and anything they ask for in advance to save time and choose an earlier appointment if applicable. That could be the sweat spot. I had a COVID Antigen test at Formula Diagnostics. I got an appointment the same day (good!)…read more Before the test I asked if they take insurance, but said "no, but we will provide everything you need to submit it to your insurance company". So, they did a good job with my test and my results within 20 minutes. When I was sent my results, but it was missing: 1) Type of service/Procedure code. 2) Health care professional name/credentials 3) Health care professional Tax ID #. I called twice, emailed them 2 times with no replies (bad!). Then and went to the facility in Scottsdale and they said they didn't have it (bad!). Bottom line, they provide a good service if you want to pay full price. If you want to submit it to your insurance, don't believe them when they say they'll provide you what's needed to submit it to your insurance.
