This is a copy of my formal complaint. I am writing to formally report multiple serious incidents at your center involving unsafe medical procedures, inaccurate documentation, and unprofessional behavior from several staff members. Several months ago, during my donation, the needle was not inserted correctly. It was partially hanging out of my arm and causing bleeding and significant pain. I told the technician that I was in pain, and I showed them the blood that had just begun to pool and drip from the insertion site. I was told this was "normal," even though the needle was visibly not positioned correctly. This dismissal of my pain and the visible bleeding was entirely inappropriate and unsafe. I have attached a photo from that donation which clearly shows the needle improperly inserted and blood leaking from the site. I had been feeling completely fine until this occurred. Instead of acknowledging the improper needle placement, I was told that the reaction happened because I "didn't eat or drink enough," which was not true. After removing me from the machine, I was given something to eat and drink, told to sit for 15 minutes, and then instructed to leave. I followed every direction I was given. Later, I learned that this donation was coded as "the donor wasn't feeling well," which is inaccurate and misleading. I was not "feeling unwell" -- I was reacting to a procedural error. I want to know what potential medical complications I was exposed to due to a needle being improperly inserted and partially hanging out of my arm. Based on standard plasma donation safety guidelines, I could have suffered: Hematoma / internal bleeding Infiltration or extravasation (blood or return fluids entering tissue) Nerve irritation or nerve injury Tissue damage Infection at the insertion site Arterial nicking, depending on angle and position A vasovagal reaction triggered by pain and blood loss These are real medical risks, and none of them were acknowledged or addressed by your staff. During my most recent visit, my fingerprint would not register. A technician tried multiple machines before checking the system. She found an error message stating that I had "left without being released" during that previous donation -- which is completely false. I was released by the nurse after being told to eat, drink, and wait. Your staff gave me permission to leave. When I asked why my fingerprint was missing and why inaccurate notes were in the system, the nurse became combative and dismissive. She argued with me, insisted nothing was wrong, and went so far as to call me a liar. This is unacceptable behavior, especially given the safety issues that occurred. At the front desk, I was met with more hostility. The staff member spoke to me in a confrontational tone that felt aggressive and unprofessional. Had a supervisor not walked over at that moment, the situation could have escalated. Additionally, when you ask staff for their names -- including supervisors -- they claim they do not have to provide them. This lack of transparency is inappropriate in a medical setting where accountability is essential. Because of all of this, I am requesting: A full investigation into the improper needle insertion and the staff's refusal to take my report of pain and bleeding seriously. A formal explanation of the medical risks associated with the needle being inserted incorrectly and partially hanging out of my arm. A correction of the inaccurate system record stating I left without being released, and the inaccurate coding of the incident as "donor didn't feel well." A review of the conduct of the nurse, the technician, and the front desk staff. Clarification of BioLife's policy on staff identifying themselves when a donor asks for their name. Assurance that appropriate retraining and corrective action will be taken. I take donation seriously and have always done my part to follow procedures. What I experienced instead was unsafe medical practice, inaccurate reporting, and openly hostile treatment. Just know that once you schedule you appointment and arrive you may have to wait 20 to 30 minutes before you get to a table and once you get to the table it could be another 20 minutes before anyone attends to you and then once you are done be prepared to sit there for 10 minutes before anyone will come and un hook you from the machine and the needle. Please also avoid the one they call Xena worrior princess with the black curly hair and freckles her real name starts with an X. If she is having a bad day she will be a bad stick with the needle and if you say hello and she is in a bad mood she may get you banded from donating. read more

I called 14 times over a course of an hour and half and left 2 voicemails trying to confirm that my renewed license would be approved even though it was a paper copy. The day before the lady on the phone said that if I renewed it I would be good. Drove 25 minutes to get there for them to be super unprofessional towards me. All they said was since it didn't have a picture they couldn't accept it, even tho I was told other wise. That is also why I called 14 times so I could verify that. But no one answers. even when i get their not a soul was at the front desk which makes me wonder why they couldnt answer 14 phone calls. I'd like to recognize Caroline @ BL in Lewisville, TX She was absolutely amazing today!! This young lady went above and beyond to figure out why the machine constantly beeped. In the past, other staff wld ignore the beep or stop it with attitude bc it's interrupting them frm other duties... I know it's aggravating, but I can't stop the beeping machine bc if I cld, I'd reach over every time and stop it! BL shld hire someone just to monitor the beeping... They tell you to let them know if you don't feel good and I did 3 times. 2 staff members also stated I had issues in the return process. I love that donating plasma helps people who need help and I can be compensated well for doing it. I don't like that this locations front desk staff had a bad attitude when I was late for an appointment that I had booked for a long time. I work 2 jobs and move my whole schedule around to be here and could not find parking near the building so I wouldn't have to walk so far in the heat after donating I was 11 min late and their system kicks after 10 and I know for a fact that they have given overrides to other people when they come in... it took 2 min for her to walk over and tell me I was 3 min past the allotted time and they were over booked already. Their over booking is not my fault. Her lack of customer service is her fault. I have been doing this for years and that's the way they want to treat long term customers is a horrible lack of customer service. I have yet to hear back from a manager... read more

Checking in is easy, some staff is friendly, but don't bother coming or wasting your time as a new donor, waited almost 2 hours for them to reschedule me. I already had to reschedule once before and still couldn't see me this time because the nurse was running behind. If this facility can't handle new donor/new patients then they definitely shouldn't be scheduling them to come in. Disgusted and violated doesn't begin with how I feel about the supervision of BioLife in Lewisville. A Lead there searched for my information through their system to find my phone number without any sort of consent and proceeded to text me saying he's been waiting for me to return to the center. Not only is this completely UNPROFESSIONAL, it's also ILLEGAL in so many ways according to HIPAA. I felt so uncomfortable I stopped going. I felt scared because my information isn't trusted with the workers. I contacted supervision and I was told he will still be working there and ONLY received a write up. When I spoke to the manager on duty about me being there for such a long time and seeing others leaving that arrived after me, and I had not been hooked up yet. He was very rude in my opinion saying thats why we get paid. because we would be waiting a while, totally ignoring or disregarding an appointment had been set and I had other things to do. The lack of communication here between the staff and the donors is ridiculous. I have always had a great experience here. The phlebotomists, for the most part, make the experience painless and pleasant. Except for one bad experience I've had where I got a huge hematoma after he dug around for my vein. The lead handled it well and made sure I was not with him again. Was literally 5 mins late to my appointment rude guy who was the" nurse " was up front saying he couldn't check me in because I was 5 mins late and explained to him I live in McKinney it's not my fault one day theirs traffic , then theirs not . I had a great experience until I had a medical emergency unrelated to donating and the staff handled my situation completely disrespectfully and unprofessionally. Instead of helping they sent me away to handle it on my own as well as ending my partner's coupon after the incident. I'm an NPC competitor/athlete currently in my off season. I know exactly my #'s even down to my protein levels every single visit (even at prior plasma donation centers). Daily Electrolytes along with other fitness/health measures. Everything passes to get through to the donation floor. I switched here because they had a good new donation promo going. I drive 35 min one way to get here. So far I've completed all donations and even the last time when the needle stick wasn't the best the phlebotomist even admitted they had challenges properly hitting my vein being on the smaller side. Then today we tried what they suggested last visit.. using the outside vein so we did. It got more than 1/2 completed and another girl that didn't even stick my vein come by and calls it. She says she's gonna remove & do my other arm. I explain everything to her while she's prepping and she clearly didn't even look at my chart because she went ahead and said it wouldn't count against me today or future donations (I only have 2 left after today). She sticks it and just turned around and took it back out saying she doesn't wanna chance it after all bc my vein is indeed tiny?! I'm clearly confused as she didn't tell me why... Then she actually pulled up my info and said oh actually you will get paid today but will be deferred for 52 days!!! I'm sorry but this is complete bogus. They kept reiterating the qualifications. I know those. I fully meet those every time. I even asked them direct questions about why can know one here seem to hit any veins even if they're on the smaller side? They said everyone's trained the same. I reminded that none of us are the exact same so how are they able to accommodate different types of sticks as needed? They just looked at me and kept insinuating that it's because my veins are defective or something. Seriously? If that's what they want to claim to deflect and avoid admitting that their employees are clearly a one type stick all regardless of what walks through the door then fine. I'm infuriated because the lack of training for various types of people is astonishing here. Everyone has been nice and I had been overall happy with switching to here until you basically call out their poor ability to stick properly or on the first time or two. I used to do hormone injections for clinics so I do understand that no one is perfect and sometimes it can fail. But this much with a patient that is so on top of health and wellness measures. Nope. Terrible service, especially if you are white. I made an appointment for 11:20, arrived at 11:15 and watched as 2 people who came in after me "jumped the line". The receptionist who controlled the flow set my ID aside rather than pulling my chart as is required for further processing. I was still awaiting processing (as a returning donor) as the person directly in front of me (a new donor) was quickly processed and sent on to give blood. The web site says to except to be here two hours for the entire process as a new patient. So I booked for 11:50. As I got there at 11:40 the place was packed! People standing everywhere. Not 6feet apart at all. I finally got to the desk to be told that it would be a 3hour wait...... Why are so many appointments avail online if you can't service the ones that do schedule. All I got was you can reschedule for another time or wait.. NO THANK YOU... Not having been to any other plasma collection business, I don't know how this one compares. I've been donating every week since October '20. So I can say I've been there when it's super busy and have to wait an hour before initial vitals, to walking in, checking in, and hooked up within 15-20 minutes. Employees have always been kind, considerate, and knowledgeable. They seem to enjoy working together and are respectful of each other. My only complaint (?) is that the music can sometimes be a bit frenetic in a place where some (1st time donors) may have a little anxiety. Easy solution is headphones. read more

I understand that they are very busy running promotions and such but I arrived 30 mins before my appointment time and gave myself a 3 hour window to at least get the physical out of the way. It took a very long time for them to just confirm that they had my paperwork transferred even though I called ahead of time to verify and I told them that. For some reason they only had one nurse on a Saturday with a $1000.00 promotion going on.?? I hate to be that guy but I was really looking forward to donating again and it's very disappointing to spend that amount of time just to get through a physical and eventually I ended up having to leave for work before I even saw the nurse. So hours of my life gone. Not worth my time so not trying again. Honestly I used to be a regular at another location and I feel like this experience could have been mitigated had they simply quoted accurate wait times or at least not overbook themselves to the point that you can't be serviced in a reasonable amount of time. I was good for 3 hours. That would have been okay but I firmly believe it would've ran over into the 5 hour mark if I had stayed through, donation and all. Maybe try the Denton center? The extra drive would probably be worth it in the long run. read more

Very disappointed with my 2nd donation. A bad stick on Tuesday evening by an inexperienced or tired phlebotomist caused a low donation, a swollen arm, and a horrific bruise. Showed up Sunday to donate from my right arm and was refused due to the bruising on my left arm. Perhaps the phlebotomist should remove the 2 inch false eyelashes so she can actually see what she's doing or receive more training. You'd think with as many people as they stick daily, she would know what she was doing. Now I can't donate for 2-3 weeks until the dark purple bruise fades which will put me out of time to donate 8 times to collect the $$ promised for 8 donations in my first month. I should have asked at the time they told me I couldn't donate again until the bruising faded if they would extend the timeframe due to the negligence of their employee. I'd love to continue donating since it helps people, but I can guarantee you, that particular phlebotomist will not be allowed to stick me again. Everyone else has been quite friendly, professional, and the lines move relatively fast (it takes about 15-20 minutes from the time you enter the building to be called to finally donate.) So give yourself about 1-1/2 hours to donate after your initial physical/donation appt (which took me nearly 3 hours). I have noticed people who have "cut" in line by bypassing the computer questionnaire (which I thought was required). Maybe signage of: Step 1: Complete questionnaire on computer. Step 2: Stand in line to have your BP, blood, weight, and arms checked. Step 3: Stand in next line until called to donate would be beneficial. read more

Many businesses are doing their best to serve our communities with the devastation of Covid-19 all around us. BIO Life assistant management Specifically Casey is not one of them. After completing the Hour long process of intake. Waiting paitenly in the Playroom area a male nurse approaches to call out a donor. He mispronounced the donor name several times. So we were unsure who he wanted. In an effort to lightened the situation. I stated "wow it's good to know my name isn't the only one people struggle with. The female donor eventually rose from her seat to let the male nurse know it was her he was attempting to find. They leave. As I'm reading emails on my phone 3 minutes later a Staff member comes in asking was I upset about something? Apparently the male nurse felt embarrassed or shamed by my comment. All the while I felt it was the thing people say to lighten the situation. Social skills 101. Apparently this guy had No clue I was throwing him a life float. So Casey his manager became upset that I was in fact not upset and even had a few questions about her ability to Guage whether a donor is upset or just has a dry sense of humor. Sad Assistant. Stay away if you have or don't have a sense of humor. It may be misinterpreted by Casey. Hate that I couldn't share the life saving gift of plasma. But this experience has changed my mind about risking entering these crowded facilities. read more

the lead technician is AWFUL. shes couldn't stick me and was extremely rude. she does not walk you through what shes doing at all AND got pissed that i wanted an arm she couldn't do herself. she readjusted me 4+ times before having another girl come over. THE LEAD TECHNICIAN HURT ME MORE THAN THE OTHER GIRL? the LEAD. after telling her my personal doctor said not to use my other arm she then proceeded to bring the head nurse over and say that i said he told me that.
