The wood fired pizza was excellent. The salad with iceberg lettuce, salami, capers and some slice cheese was too pricey. Our server/hostess (Sierra) was excellent and made great recommendations. When she saw the type credit card I was using asked if I was military and gave a discount. She didn't have to do this, what an added bonus. Great food. We started appetizers by trying ancinni-different but good and then garlic bread that was light and delicious. Main dish was from chef recommendation which had mild sausage that was delicious. Staff was great from Dino who seated us with great smile and Brian who served us and all other staff who checked on us. Definitely recommend this place. Be ready to take a gamble with your time. Was a very frustrating experience to watch nearly 8 open tables for a full hour. We were finally told our table was getting wiped down, but after 20 minutes more of waiting we finally gave up hope. Best advice would be to make a reservation beforehand!!! read more

We found this restaurant on Yelp and decided to follow our noses and the great reviews in for a meal. The restaurant did not disappoint!! They only opened 2 months ago but the food and service are amazing! We had Stephanie as our waitress and we loved her! Give that girl a raise, she was so fun and positive and we had a blast getting to know her. If you're in the area, you MUST eat here!! We have two young children and they have the restaurant 10 stars, I mean, if you're enjoying a meal with kids, it doesn't get better than that. We just had a very tasty woodfired pepperoni pizza, primavera salad, cheesy bread at Antica Forma. The pizza crust was worth it, chewy, flavorful and just the right thickness. Cheesy bread was very good, it appears to be extremely fresh, open-faced focaccia bread underneath. They make fresh mozzarella in-house, and you can tell, so wonderful! Service was friendly and attentive, dining room was clean, albeit basic. We visited at lunch and the food was quickly served, but not too quickly. Fully recommended. Cost was reasonable. read more

My husband and I were visiting Moab for 4 days, and this was our favorite restaurant of all that we tried. The service by our waiter Brian was fabulous and the decor was awesome. They have only been open a month, and aren't quite finished with the outside seating, but looks like it will be a great place to sit. The food was delicious... we both had terrific salads that were well balanced with ingredients and dressing and we enjoyed a wood fired pizza (tre fungi) that was awesome! I ordered the GF Vegetariana pizza without cheese. When I took one bite I realized that would most likely be the best bite of anything I would eat this entire summer. We placed a phone order and were informed that our order would be ready in 20 minutes. We arrived at the restaurant 30 minutes later, and were informed that it would "just take a few more minutes". 20 minutes later, we asked them for the status of our order as it had been 50 minutes at that point. Each time we asked about our order and expressed frustration with being give an extremely inaccurate preparation time, no employees seemed apologetic or concerned. One employee just stated that all customers had been waiting a long time. By the time we got our order, we asked if they could throw in something complimentary for having us waste over 50 minutes for an order that was said to be ready in 20 minutes. They said since the other customers were waiting long as well, it wouldn't be fair for them to give us something complimentary. This is the first restaurant that refused to throw in a little something while refusing to acknowledge that they made an error in giving an order's prep time. The food was good, but nothing special. It is truly unfortunate that the employees there barely acknowledged our valid frustrations. read more

If you like thin-crust, wood-fired pizza, Antica Forma is a must! We had the salciccia & fungi and it was delicious! Our server Carmen was friendly and helpful. I had a nice cold Park City Pilsner with our pizza and it was a perfect pairing. Upon paying our bill, I left my credit card behind and her co-worker Anna ran out and caught us in the parking lot before we left. Bad experience .. we wait for over 10 min sitting , there and we got not attention from waitress , not even water . We just want to try some hot soup from the menu as had been traveling all day a long distance .. after all that wait they don't have soup but if we want pizza or pasta they have that but we told they are understaffed so we would have to wait awhile for that. Finally Moab has good pizza. They serve one size, a 12 inch pie, wood fired, with all fresh ingredients as it should be. It's pricey, between $15-$20, what you will experience in Moab, but so much better than the other pizza restaurants which charge a similar price for frozen and canned toppings on their pizzas and dough which is not even made at the restaurant. read more

We were so excited to try this restaurant and have some good pizza but unfortunately our experience wasn't great. Our server was one of the worst we have ever had. He came to our table and just looked at us and said are you ready to order. We asked for some drinks first and he asked us twice to repeat the order, which was a kids orange juice and lemonade and a Diet Coke, nothing crazy. He came back with a lemonade only. Then after asking again we got the Diet Coke, then about 10 minutes later we got orange soda not orange juice. Finally another server brought us water but not without sneezing and not covering his nose steps away from us. We also asked about 3 times to 3 different people for an extra plates so our kids could share pizza and finally a server came back and said we only have one, were super limited. We then ordered our food, I asked about a pizza and if it was good and his response was I don't know I've never had it and then just stared at me, so I then ordered a salad with chicken and my husband chicken parm. I got the salad, with no chicken and he never came back for me to say anything so I just ate it and about 25min later my husband got his meal and with that they brought us out a full pizza we never ordered and said we did. He apologized and told him I never got the chicken on my salad and he said oh sorry! We then got the bill and the pizza we never ordered, orange juice we never got and chicken I never got was all on the bill. It took 15 min for him to come back to our table and he said he would take it off. Then another 10min and he finally brought the right bill. This was just such a bummer. I'm not one to write a review and would rather go right to the manager so they know how they can improve but there wasn't one to be found and there is no direct contact info on their website. We were hungry after hiking in Arches NP, it was getting late, and this place looked like it had potential. Our server, Linda was exceptionally helpful in pointing out what the popular and tastiest items on the menu were. She certainly knew her food there! We started with the Girella which was very fresh and flavorful with the roasted peppers and marinated artichokes starring inside the mozzarella wrapping - delicious! Then we consumed the pear with gorgonzola salad, but added our own variation by going with the roasted tomato dressing. This was very fresh with wonderful flavors. We followed that with the Pizza Primavera and were extremely happy with the results - great crust and delicious roasted balsamic tomatoes, spinach and very tasty prosciutto di parma. We managed to squeeze in a very tasty strawberry shortcake to end our visit. The whipped cream and fresh strawberries were my favorite part, and the cake wasn't overly sweet. The service was attentive and the food was served promptly. We definitely recommend this place! read more

Back a month or two from Moab, Utah, and while my thoughts still wander back to Arches and Canyonlands National Parks and kayaking on the Colorado, I'm still thinking about my pizza at Antica Forma. My wife and I are on a constant quest to find the perfect pizza and being from Brooklyn, we thought we would find nothing to fit this description anywhere west of the Delaware. Wrong. Antica Forma's pizza matches up to anything we have had in New York and we've had some of the best pizza going. (The pizzaiolo was trained in a New York pizzeria). Fresh ingredients, well cooked, great sauce good char on the crust --- all combined to make a great pizza. You'll find Antica a bit down on Main Street a little past Canyon Voyages, another great place for kayaking, rafting, exploring, etc. Combined the two --- kayak in the day and eat pizza afterwards and you can't go wrong. Who would have thought -- New York style pizza two thousand miles west of Brooklyn!! When in Moab, get thee to Antic Forma for a great dining experience. read more

While in Moab, UT we ate at a this restaurant as we were looking for Italian and their sign indicated wood fired Neapolitan pizza and pasta. We noticed an outdoor patio eating area, but found out it was still under construction so we sat inside, all tables no booths which gives it an airy and community feel. There are several large screen TVs on the wall and three of them picture the chefs live, preparing the pizzas and cooking with the wood fired oven. Our primary server Sierra was knowledgable, personable and very quick. My wife had a a glass of wine and a penne prima vera which she described as flavorful and delicious. I had a never empty glass of iced tea and the Margherita pizza, which had a thin crust and wonderfully prepared. Exploring Dead Horse Point State Park and Canyonlands National Park made us hungrier than normal and we both finished generous portions. Then came a desert menu we could not resist. My wife had the cheese cake with berries and I had the strawberry short cake, all made in house with fresh ingredients. Both were outstanding. We learned during our dinner the restaurant has only been opened eight weeks, hence the patio area still under construction. Before we left the place had filled up and people were eating at the counter set up to overlook the pizza preparation. Also our server got very busy with several additional tables but we were checked on frequently by other staff and our server. Overall a wonderful dining experience in a place with wonderful food and service eager to please. read more
