Where will I go after this hospitalization? Think about the anxiety and tension not knowing where I would be going. As an 84 year old senior citizen, this predicament could be considered filled with debilitating uncertainty. The social worker and case manager both offered information and possible dispositions. One of the referrals was to Giselle Placement Help. In fact the workers gave me a tip to start off with Giselle. I called her first and she gave me three referrals one in Lynnwood and two in the San ​ Fernando Valley. One never called back , one said I was not eligible , and the third was to an administrator who arrived at the hospital at 12:30 p.m. . After extensively interviewing myself he spoke with all the nurses and doctors in the unit and then inform me that I would be eligible to come to his facility and showed me pictures of the private room where I would be . He was extraordinarily kind and made sure that every need that I had was going to be met . In addition he actually helped me move some of my belongings . So you can see that this referral took care of enormous uncertainty and perhaps Karma is involved in making such a successful transition . ​

He listened carefully to my mom's needs and found a place that balanced her considerable needs with more opportunities to socialize than your typical board and care. Within two week of my first call with Jason, my mom was preparing to move! Jason's support didn't stop there. He regularly checked in with me during the first few months of my mom's adjustment, advising me through the inevitable bumps that come with the transition to assisted living, validating my concerns, and guiding me to a place of candid conversation with my mom's new support team. In the past year her utilities and cable services were being turned off, but it wasn't until I received a call from apartment management that rent had not been paid that I suspected things were not as should be. Shortly after clearing up issue with rent we discovered mom was speaking with magazine issues as if real. I was completely clueless on steps needed to get help for my mom. I started googling agency's related to elder care and placement and contacted Connections Care Home Referral. I spoke with Jason Bloom who patiently listened to me and gave me the most important piece of information that helped me through this journey. Do you have a Power of attorney for mom? Obtaining that document opened the doors I needed to doctors, social worker, and banking. Once doctors became involved, tests were done and mom was officially diagnosed with progressive dementia. The doctors declared mom as not being competent to handle personal or financial affairs and I needed to find a suitable placement for her. Jason once again gave me the great advice on what type of placement mom, needed. I admit at first I was hesitant about a board/care facility I wanted mom to have a placement as close to living independently as possible, so I was leaning more towards Assisted Living facilities. I also was in process of learning about the disease and was ignorant of how much it robs you of the person you love. Jason was on point the progression of my mom's dementia was at stage where traditional Assisted Living would not have worked for her. Jason and I had many discussions over about a 60 day period, and he sent me lots of lists of possible placements and he always checked status of any contacts I may have made. As I went through this process I began to have a better idea of the type of home that I would want for mom. Jason said give him a couple of days to look for me. Within that timeframe Nina contacted me saying Jason referred her to me. After weeks of visiting different homes and facilities I knew within 15 minutes that her place was the board/care I was looking for. I can't express my gratitude to Jason and Connections Care Referrals for helping me through this process. He was able to take into account location and price points to find what my family and I would be comfortable with for mom.

Hard to believe a medical service can be as attentive as a fine dining restaurant, but that is what…read morethis was. We used Five Star for about five days. Medicare and Optum covered the hospice side. I paid separately for 24/7 caregivers through Sincere Care Services, which I chose because I wanted Cantonese and Mandarin speakers so my mother would be more comfortable. Five Star was recommended by the Optum physician on staff. Separately, a family member recommended them too, through a friend whose father had passed the week before. I checked their ratings on Yelp and Google before moving forward, and I am extremely happy with the service we got from start to finish. This is the point where you decide between more hospital intervention, which can be painful, and accepting the situation and moving to hospice. Hospice is the wedding coordinator for a peaceful death. Things can still go wrong even when you plan everything perfectly, but Five Star was there at every step to handle whatever came up. The rough sequence for anyone about to do this: 1. Decide on hospice. 2. Line up caregivers. They are separate from Five Star, but Five Star will point you to good ones. I wanted Chinese-speaking caregivers and Mark made it happen. 3. Coordinate with the hospital on transportation. 4. Be home to receive the supplies. 5. Be home when your loved one arrives. Have a bottle of distilled water ready for the oxygen machine. 6. Have the nurse and the caregiver both there so you can learn the comfort meds. 7. I used 24/7 caregivers on 12-hour shifts. I made a point of being there whenever a new caregiver came on, so I could walk them through the medications and get their phone number. Nurses came once a day to check on my mother since she was close. You do not have to be there the whole time, but I tried to be there for shift changes. One thing I wish I had watched more closely was bowel movements. My mother had been constipated since the hospital because she was not eating. Another tip: put together a playlist of beautiful music to play in the room. Otherwise it is silence, or your loved one is listening to caregivers talk and scroll on their phones. Even when your loved one is not responsive, they are still listening and drifting in and out. Tell them everything will be okay. Thank them for everything they did for you. I think it helps them go peacefully. Anytime I needed something, Mark was a text or a call away and he always answered. Again, they are the wedding coordinators for a loved one's passing, and they will do everything within their power to help you through it.

As soon as we decided to switch to hospice care, we've been in touch with a few places but Maximum Care Hospice felt most transparent and comprehensive. Other places were a bit unclear about who is coming from where and what exactly is being provided with hospice care. Switching to hospice care was a difficult decision, but we are so happy to have chosen Maximum Care. We had a wonderful nurse Victoria. It was a short seven days but she provided all the support that we needed. There was no judgement whatsoever (my father was difficult) and she was very respectful and kind. We had a somewhat unusual circumstance, where the police had to be in touch with them (due to my father's passing as a result of a crime), but they handled it all very graciously. Equipment delivery and pickup was excellent. They are contracted with a place called St Joseph's and every staff from there was respectful and friendly. We also had a wonderful woman come to bathe my father before he passed away She was very thorough and efficient. I believe she isn't an employee but someone who is contracted to provide personal care to several hospices, but she had told me that she tries to take every job requested by Maximum Care because they treat her very well. When my father passed, Victoria came immediately and connected us with their recommended mortuary. My cousin, who is experienced with contacting mortuaries at her church community, was very impressed how seamless and "easy" Maximum Care made the process.
