I have to pat to get my medical records sent to me so I can submit them. First my cell phone was stolen but they didn't document that I even had one so $700 out of my pocket. Second I got covid which I was vaccinated for. Third (I was 51) admitted after attempting suicide (and had had hypoxya) and when I was well enough to go home the staff wouldn't let me leave to try to sell my condo. They barricaded the doors and put several workers in my way so I couldn't leave then was left with a stern warning that I HAD TO COME BACK. I DID NOT GET MY MS DRUGS FOR 5 MONTHS, WASN'T GIVEN MY NORMAL ROYND OF MEDS. My roommates, of coarse, we're all Demented because I was the youngest one there! Be careful! read more

I have reported everything I could when I could in regard to my father's care. It still haunts me to this day what types of abuse that goes on in that facility. I am posting screenshots of the medicare website for this facility's rating. If you live your family DONOT take them here. Patient care is minimal, office staff unprofessional. At the time of my complaint their executive director either or fired or resigned and so did their social services director. Within 4 months two executive directors were replaced because NO one wanted to take responsibility for facility's poor performance to the point they abused their patients. The RED hand next to their name is Medicare's recognition that the facility has proven history of abuse. My dad was transferred here after spending a few days at Northridge hospital and I am so thankful for this facility. Beginning with the Director of Nursing, Duke, followed by the Admissions Director Yezarik and including Jackie alongside their exceptional staff, we extend our deepest appreciation for the remarkable accommodations provided by this facility. Roubina at the front desk went out of her way and consistently provided assistance whenever my father required a respite and expressed a desire to step outside. The CNA's-Luz, Hermine and Solo were so attentive to my dad's needs and always offered a helping hand. The charge nurses Jaime, Suzy and Malik are exceptional. Their professionalism, dedication, and compassionate care make them an invaluable asset to the entire team. Joan and Rhoda-the RN supervisors we had the pleasure of meeting are absolutely outstanding. Their leadership skills, knowledge, and attention to detail ensured that my dad received top-notch care. They are a true asset to the team! Adrine the activities coordinator was very friendly and great at creating and engaging in meaningful experiences for all the residents. The collective efforts of the rehabilitation department team were instrumental in facilitating my father's recovery journey. Through their expertise in physical and occupational therapy, Briccio, Carlo, Steven and Terry not only restored his strength but also bolstered his confidence, contributing significantly to his swift recuperation. Last but certainly not least, Dr. Bassuk has been incredibly amazing. His proficiency, attentive care, and meticulous approach have made a significant impact on my father's well-being. I wanted to leave a good review for this facility but after what happened? I unfortunately have to leave a one star review. If you have a loved one that is staying at this facility, please take anything of value with you. There is someone who works at that facility who is stealing from patients who are long-term residents. My father's debit card was stolen and used by someone in that facility and his watch went missing. I was already someone who was uneasy because I don't like the idea of nursing homes and it wet from a positive experience to a literal nightmare. When my father was transferred to Glendale Adventist, I was then told that my father had open sores on his backside and from what I can guess? They probably didn't change my father's diaper as often as they needed too, not to mention he also had a wound on his foot. My father recently passed and I am just livid that they are allowing someone in their facility to steal from terminally ill patients. read more

This is 100% NOT a place to put your loved one. Very unsafe and unsanitary. Very unskilled staff. Employees come and go very quickly because of the heavy responsibilities that are place on them they get burnt out very very quickly!! Especially the CNA's they over work them double shifts until they don't perform the duties well because it's so short staffed. Patients have been severely neglected as far as even being sexually abused by one of their male CNA's. Don't take my word for it go on the states website and review the complaints. Feb 26, 2025 If you love your loved one you won't even think about sending them to this facility. Find all complaints on this facility on Medicare.gov review the one dated on 02-26-2025 and get the full report on sexual abuse. This facility has had 80 complaints in the past 3 year. The Administrator Luis Pages there is trash and heartless. has NO REAL concern about the residence/patients there. It's just a job and a paycheck for him. read more

Mom was taken by ambulance in a pre arranged transport from Kaiser. When she arrived, the ambulance staff had to make her bed, as it was not ready. She was assigned to a triple ( I think all rooms there may be triples-- probably formerly doubles) in the middle bed. During her first five hours there, she had her bland food delivered and removed, and an LVN changed her once. When we arrived during posted visiting hours, the door was locked and no one was at the front desk. After about 10 minutes, one of the patients flagged down a nurse who let us in. We arrived to a darkened room with the curtain pulled back. One of her roommates get migraines from light and is claustrophobic,so needed to have window open and see the door. It is illegal for a hospital to accommodate the needs of one patient to the detriment of another. We have had pretty good experience with Kaiser so far, but this place should not be on their list of acceptable facilities! Now, how do we get her out and maintain her Medicare coverage? read more

My grandpa is being cared for at this rehab. We are happy with the place as it is clean, has good food, has activities to keep him occupied, they have good staff which always communicates well and when i have a concern or question they are available to answer. I like that they answer my call on days when I am not able to visit him, and they update me with everything like if he has ate, or showered or has done his daily physical therapy. I like that I can have a peace of mind while he is there and being cared for. The staff are very friendly and helpful. I also know he feels comfortable and safe there. This place is disgusting and the employees should be ashamed of their selves! They leave patients in soiled diapers for hours! If they are asked to help them to the restroom they are told no, we PREFER you to go in your diaper because the plumbing is bad. There are patients screaming for help because they are hungry! There are patients minding the entrance and buzz people in on their own. They are supposed to be a rehabilitation facility but all of the patients are either in a bed or wheel chair. This is horrible. I was visiting one night and there were no nurses to be found I had to go looking for one. One of the nurses was playing on one of the rehab machines for over an hour! This place is very unprofessional. I have requested many things to be done in regards to helping my grandfather and his care. These people are either lazy or just don't care at all, and I have spent many hours there when I've gone to visit my grandfather to fix the same constant issue of him not having a TV and I'm just fed up and I called several weeks ago to speak to the Director of this facility. He has not called me back in regards to $150 that were stolen from my grandfather and I called multiple times to speak with Jessica in social services and she chooses to not call me back purposely knowing that is the reason why I'm calling and I don't know what to say here anymore. I recently spent a little over a week at GPAC and thanks to their care I am now at home...healthier than ever. The facility is very clean and thenurses are very skilled and attentive. The food is good and varied. (I actually had Gumbo and Rice for dinner once).It isn't 4-Star, but well balanced and health conscious. The physical and occupational therapists really help improve strength and balance. I read the lower star reviews and must say they do not represent my experience. The facility is meticulous about recording any personal property, but frankly there is no need to have money or cards there. Yes the nurse call system can have a wait time, but not usually beyond 15 min. And one must remember that every nurse has many patients.. some of which require additional help. It seems to me that there are three types of patients at GPAC. Those who want to improve and those who have given-up. I had a roommate there who was able to walk to the bathroom, but preferred using a diaper... he would also scream for help randomly. The third group of patients are residents who are aging-out, or need 24 hr. care. I saw them receive caring, loving and professional support. The daily activities and the therapy offer a well-rounded curriculum. Overall, I found GPAC to be a top-notch facility and I am grateful to the entire staff. read more

As we have brought our loved one to the rehab for healing in August I noticed many things about this facility. First and foremost the Register nurses are very helpful and kind, the supervisor for the RN and RNN and CNN is extremely knowledgeable, on top of all her tasks and genuinely puts time and effort to care for the patients needs and listen to the family's concerns. The social worker here is very helpful, she answers your call and sees you in persons to answer questions and acknowledges your concerns. I also am happy with the administration staff, very happy how they help and answer questions. I am not adding the staff name for their privacy but we have thanked them many times and are great-full that they are in this facility and are part of the team for helping our loved one heal. The place is clean and has round of clock service for activity, food and overall safety and hygiene of the patients. No one likes to see their loved once at a rehab but if you know they are being cared for and are in good hands then at least you have a peace of mind. read more

As we were visiting from Vegas, my Grandma fell and broke her hip. After the ER we were sent to Glendale Post Acute Center. I must say Everyone was amazing and extremely helpful. We were treated with respect and the Admissions girl Kristine (i believe her name was) explained everything step by step, which made the whole process easy to understand and smooth. I hardly ever bother to give a review but I had to in this case. It's hard finding a place where you can trust your families health and well-being with. Where do I start? Today is day 1 of me here & this place has left a horrible impression so far. I agreed to come here on Wednesday or Thursday after they painted a pretty picture & even took flowers to my hospital bedside. However they had to work on getting me a air mattress & pretty much getting ready for my arrival. Friday I'm told They are ready for me to arrive. I was suppose to arrive yesterday however there was a delay on my behalf. Today however when I do NOTHING is ready!!!!! The air mattress was "not was not working" they did not have medication orders from my Dr therefore not having my medication. Nothing was ready!!!! The person "in charge" Ryan was only seen in the morning when I arrived after that he went M.I.A Nobody here I can talk to. My family had to call several times in order them to change me after using my necesites. Please don't sent your family here. read more

I beg you--not bring your loved one here. My grandmother was not taken care of adequately to the point of her having extreme bed sores near her lower back, thus resulting in a wound that has caused her to go to the ER in Glendale Adventist. All that needed to be done to her was to be rotated routinely so that she doesn't get any sores in the first place, and even my mother has voiced her concerns about repositioning her MULTIPLE times. Not only that, but a former patient that shared a room with my grandmother while she was still there would purposely raise the volume on their TV and cause other surrounding patients to complain. What the employees did as a result was close the door to my grandmother's room, and that is the OPPOSITE of what should be done because what if something had happened to her? Barely anyone would've heard my grandmother if the door was closed. I tried to be understanding and forgiving of the employees here, but unfortunately it has led to otherwise. As I've said before, do NOT send your loved ones here. This facility does not care for its residents. read more

Please read before you might leave negative comments/feedback. Hi, I'm 65, I've been a medical patient of GPAC since 28 December 2018, transferred from Dignity Health. Yeah, that's just before Covid19 largely infested our world killing hundreds of thousands of Earth's inhabitants. As of this writing, 31 May 2024, I'm still in this facility, sadly. I became captive of this place starting with/because of Covid19. Had the coronavirus lockdown begun Tuesday, not Sunday, I would have been out of this place because I had secured a disability apartment, signed a lease for it and was moving in. My biggest mistake was to acquiesce to ending my lease, late 2019, before the pandemic lockdown ended. My error, mine. It's pretty hard to find another apartment when the only help that I get from Social Services *here* is for Assisted Living that I'd have to share a room with 1-2 people, when I don't need that type of accommodation. They'd take all of my SSDI but for $100. So, I'm screwed. Moving on... I have a lot of experience with this place from the inside, through 4 administrators (yes, 4 administrators in 4 years), many nursing care providers, CNAs, RNs, LVNs, and others. Yes, medical staff here aren't perfect, who is, they're human, so am I. The staff CNAs working 7am-3pm often care for 15+ patients, 1/3rd of that get showers/bathed each day, 3pm-11pm 20+ patients, and, 11pm-7am 25+ patients, that's how it works in these places, as more people are available for the primary work shifts. Bathing is 7am-3pm shift only. Some residents are bedridden and hand-bathed in bed and linens changed. The first two administrators, I had a lot of interaction with them beginning with the one that was here when coronavirus hit. After a couple of conversations together, he requested my opinion on what his plan was because of coronavirus. We found a quiet spot to talk and he laid out his plan as he wanted an independent sounding board to listen and give their feedback on, as the situation of some New York nursing facilities was devastating for patients and staff alike, near 100% infections and deaths of vulnerable persons. It was a very good plan, part was to section off the building, and staff, which was soon implemented and brought Covid19 under control in this facility, as nearly 50% of patients and staff had already become infected. Not me. His plan halted it there and soon began reducing in numbers, until there were only a few cases. Later, hospitals were transferring milder cases here so they had room for the severe cases. We had a designated Covid-Care-Wing for quite a while. Staff entered from other doors, disposable items were used for meals and snacks (so no infected items returned to the kitchen), the doors were locked off to the smoking patio, the only thing to come out was laundry, which was managed. This, also, isolated staff. It worked! About the care of patients/residents: Yes, there are times where a patient needs their care person and that person is not immediately available, they are tied up giving care to other patient/s, giving a shower, changing a Pull-up/Diaper, doing cleanup for medical treatment, feeding someone who can't feed themselves, or other patient services, & a Call-light doesn't get answered for a time. Remember that: CNA staff may have 20-40+ residents under their care each shift. Yes, I've had to wait over an hour to get my light answered but, that's the exception rather than the rule. Most often they answer within 10-20 minutes. Yes, a few residents often vocally call-out that don't get answered, and visitors don't understand this. Here's the viewpoint you should look at this from: every bed has a call-light switch that lights up a light at the room door AND on several light boards near nursing stations to notify staff of that resident needing tended to. This "call-light system" is the correct means for getting staff attention/care. Beyond this: some patients have comprehension disabilities, they will call out for help (even when it's not needed), or maybe they're just lonely. If staff answered that way, they'd never get to those that correctly use the call-light method, and, the call-light system then becomes ineffectual/useless. So, patients calling out without also using the call-light system are ignored. I learned this early on being here. Yes: a patient might need emergency assistance, call-light and vocal (this has happened to me). By extension, a patient sitting in a soiled diaper, without alerting someone that they need cleanup and changed, is NOT that person being neglected. Staff can't read minds, yet, they're human, Is this a bad place? Not at all. Good staff! The food's good, big variety, ethnic, changes seasonally, very low salt for everyone, and they are allergen aware. I have Dairy Milk & Gluten allergies, they're accommodated well enough. I also get food delivered & I get microwave prep quick food. Dang Microwaves kill food value. Out of writing room. Nuff Said. read more

After two and a half years as a resident of GPAC, I cannot in all conscience remain quiet any longer. In my opinion the administration is working with a nonexistent budget. One prime example is the lack of adequate experienced kitchen staff. The number of 'cooks' in the 'kitchen' most likely more than one, considering there are over a hundred patients/residents. In all the time spent, I can easily count the meals; which are presented on a weekly menu, that did not make me ill, or turn me off completely. I have had many trays removed as to not become nauseated. I believe there is one cook that actually knows what they are doing. The others have no idea how basic ingredients can be turned into lovely appetizing dishes. Technique, knowledge of pairings that work well together, herbs, spices, are all keys to running a well organized kitchen. Also, a thoughtful dietician, that actually has an active imagination, that does not lean too far in any ethnic direction would be a great relief. Food names which are written in a foreign language, in an english speaking country. If there is multi ethnic diversity, then it should be written to accommodate all people. Armenian, Filipino, Hispanic, Anglo-Saxon, Western European people. This being an overly exaggerated task, let it be kept to the language of this country, english. In this day and age, not having proper TV reception is a joke. Antennas are attached in a 'scotch tape and bubble gum' fashion. If the simple act of a nurse entering a room, across the corridor, adjusting a bed, disrupt and cause tiling, is absurd. But I assure you, it is a constant. The noise levels can be deafening. At times, it seems the confused dementia, Alzheimer's, patients; not in their right minds, are let loose to roam. The facility is terrorized by these individuals screaming, having outbursts of frustration due to feelings of abandonment by their families. In many ways it's a fine line between a care center and an insane asylum. From day to day. read more

My stay here has been more than exceptional. I was admitted for rehab in this 24 hour care facility for about two weeks and I did not know what to expect. If you are going to send your loved ones here I would highly recommend this place. The staff are very welcoming, caring, attend to your needs and always come to work with infectious smiles on their faces. The rehab OT/PT/ST team do not play and they work hard to make sure you're able to function independently when you leave this place. I am fully satisfied with the care here and I wish the best for this place. In late August 2019, I stayed at Glendale Post Acute Center, for 10 days, after a total knee replacement. This facility was one selected by my insurance. I took a tour with Yera the administrator, and was pleasantly surprised. The center was clean, no bad odors (as in other places that I worked in). Here as a patient, I was treated with respect and excellent care. Lovely staff, CNA, nursing, ancillary, and excellent Physical and Occupational staff. As a retired P.T.Assistant, I had experience working in a variety of settings. Now as a senior citizen, I can honestly recommend Glendale Post Acute Center to my friends. I like to thank the entire staff taking good care of my mom who was a patient at Glendale Post Acute Center for almost a year until she passed away. I understand, there are areas to be improved, however, the attention my mom was getting, the incredible work the nurses and supporting people provided made my mom's life much easier while she was in pain and away from the family. I like to mention few names which my mom was thankful all the time: Justin - her charge nurse, Linda - CNA, Hasmik Avetisyan - the night charge nurse who I talked to when my mom's pulse stopped beeping, the supporting people who were next to her most of the time - Adrine, Anahit, Rubina. This place was petty bad, the employees were good when they came around, but the problems is with the overall level of service, cleaning and commitment! Also, the food would be a ZERO in Star Ratings and should be looked into by the Medical Board! My spouse was transferred here after spending almost 10 days in the Hospital with Covid. Unfortunately, a Doctor never once saw him while being there for almost 10 days! The food was barely edible! He is Diabetic and his blood Sugar tested over 300 on several days because everyone eats the same thing, NO special Diets! Carbs and sugar galore here ! Most people that he saw routinely returned their food as none edible! Every day the hot food was served COLD and once they brought it to you they quickly disappeared only to return to pick up your plate! The shower facilities were dirty with missing tiles and mold in places! I clearly heard the cleaning lady in very broken English belittle him harshly for even "letting her know that the bathroom was dirty". He only asked her if it was cleaned everyday or every other day because there was dirt on the shower floor and mold on the shower tiles! She didn't know he was on his Cell Phone with me when she went into his room to throw away trash, at this time he put down his cell phone next to his leg to ask her the question after only being there for 1 day! Her comment to him was "I clean Mr. don't tell me my job or the bathroom is dirty, go back to sleep and be quite, OK". His Physical Therapy which was suppose to take place everyday was not routinely scheduled or follow through on! I called numerous times and Voice Mail Boxes were full or no one ever returned my calls! After much stress and disappointment in his care and services we made the decision to get him out ASAP and move him home! Look at some of these food pictures, most of the time it was mystery food! We had to bring him outside food to eat because his weight was dropping and his Sugar levels were spiking high being in there! We could NEVER recommended this place to anyone! My parents are in long term section for almost 4 years. Of course no where is home but this facility has employees like Adrineh , Anahit , Roobina and other caring nurses who help my parents to feel comfortable and happy. They are always helpful when me or my parents need them. They always helped me for visitation because they believe visitation is the best medication for elderly people. I've been coming here for about a year now after my car accident, I was referred by a family friend and I'm extremely happy with my decision to attend... the staff is wonderful and tailors to every patient's specific needs. I feel comfortable coming here which is extremely important given the circumstances. Not to mention the location is very convenient.
