    I wrote an earlier review of this establishment in February of this year and gave it 2 stars and was talked into raising it to 4 stars by staff members, but now after being a patient here for 10 months I am re-reviewing NVNC from my experiences. Please note that all the "sterling" reviews listed here are generally from relatives or friends of patients, NOT people who live here, I have been an inmate/patient here since 11.05.2019 and what I have to say I am relating from actual personal experience 24/7 at this undesirable facility. 1. Nursing: There is great nursing staff turn over rate here which is a telltale sign of bad management and low pay. Daily treatments are often not given more that 3-4 times a week. My insulin shots are often prepared in wrong doses, I recieve a set amount of insulin plus a sliding scale according to my glucose levels and yet MANY times nurses have tried to administer either the set dosage without the sliding scale or just the sliding scale minus the set dosage. Often when I am administered my medication regiment the attending nurse needs to be reminded that I also need an insulin shot - one nurse argued with me that I did not require a shot until a second nurse pointed out to her that my shot was clearly prescibed in the medication logbook. There even was an incident where a nurse "forgot" my nightly shot, but falsely recorded in the treatment log that he had administered the injection! On one ocassion a nurse tried to administer pills to my roommate and because the patient spoken only Spanish and the nurse did not speak any Spanish I had to step in when the nurse called the patient by the WRONG name...my roommate did not require any pills and the nurse was trying to give him someone else's pills!!! 2. The doctor assigned to me by NVNC was utterly incapable of properly treating my diabetes and my daily glucose levels were between 300 and 450 - my vision is still slightly blurry thanks to his incompetence, and thanks to the ineffective aministration of NYNC it took me almost 3 months to change doctors...then their first suggestion was that I select the business partner of the original incompetent doctor! 3. Food Services: Let me start off with this factual statement, for 5 years I was the Chaplain at a Maximum Security Jail in NYC and the food served to our inmates was better tasting and of a higher quality that what is served at NVNC! In my ten months they have gone through 3 or 4 dietary managers and the quality of the menu has declined as time passes. As a diabetic I am served high carbohydrate foods at every meal. Pasta, rice, potatoes, breads (all high carb, but low cost foods) plus often I am served (but don't eat) sweet rolls, sugar pudding, pies and canned fruit in heavy syrup. Fresh fruit is sometimes served, like grapes, melon and bananas, all high carbs. Sometimes they are unripe or in the case of bananas actually nearly rotten with blacken peels. The taste of the meals is usually, but not always horrid. It is hit or miss with them missing the mark most of the time! 4. Population: Many patients here are mentally ill and scream on regular intervals and sometimes all night long! They have threatened Nurses' Aides and even other patients, there is NO seperation between such patients and the general population. One former patient walked the halls with his pants and shorts below his knees exposing himself totally. When asked to cover himself, he shouted in the hallway "When I was younger I molested little kids, do think I care if anyone sees my penis?" He was here for months before being transfered elsewhere! When people, or the State Inspectors visit things dramatically improve, but return to a steady decline when such persons leave. I am 60 years old and no matter how much longer I live the experience of being at NVNC will be a permenant scar on my psyche.

    Excellent staff! Very clean and helpful nursing home. A family member is here and is very happy. Highly recommend.

I was expecting a hospital environment, but to my pleasant surprise, it was just a big house the place is incredibly beautiful and clean. in the back yard, It was big and overlooking a golf course. there are 6 single bedrooms with their own private entrances. The staff was very professional and friendly, and willing to help you with all your needs the food was exceptionally good mostly fresh. I would recommend this place to anybody who needs skilled nursing for their recovery.

    FYI: They are also known as Montrose Springs, their former name was Verdugo Valley…read more My 86 yo Mother-in-law fell and broke her hip and wrist, so after her hospital stay was transferred here for continued care. The staff here have really been amazing. Their Covid protocols and reasonable: they allow 2 visitors you just have to make an appointment (okay, np). You check in and show your vaccine card or get a rapid covid test right then and there, and you're in, no problem. The case managers, Jennifer and Maylee (sp?) & the social worker, Winifred, have been SO helpful with everything that my MIL needed. I was able to email and text them and they always got back to me right away. They were always there to help when I needed, and thoroughly explained everything to me very well, so I always knew what was going on and what needed to be done. Winifred went out of her way to set up an amazing assisted living place for my MIL to go to at discharge and we couldn't be happier with how it all worked out. When this happened with my mom and she went somewhere else, the level of care wasn't as good as this place!

They have, at the bottom a 4.9/5 (see photo) but when reviewing further... yelp was 3.6/5 and google was 3.7/5. That's pretty far off! LIERS! What a horrible experience! Terrible service and help. They act as if they don't receive report from accepting facilities and don't seem to know anything. They can't answer any of my questions right and it appears as if they don't know anything. My mother is at end of life and still, they try to keep her going despite the fact that she's in hospice care. I was told that she was a "full-code" when she's at "end-of-life" and her wishes was to go when the time came. They never bother to verify this information. They shoved my dying mother in a room with 5 other individuals, robbing my mother and family the dignity she deserves and requested. They kept trying to feed her over and over again despite the fact that there's a doctor's order for "nothing by mouth" (or NPO). She's unable to swallow, eat or drink and they were shocked when we stopped them. According to them, they never got report (of course they did because that's how the ambulance EMTs work). According to them they didn't have notes (which they did because the EMT hands it to them and I also received the same copy). Should they have read the notes, they would have seen the (in all caps) "DO NOT RESUSCITATE / DO NOT INTUBATE". When I started asking them questions about my mother... they stated that she was a "full code" which was just not true. According to the DON (director of nursing) my mother is not on NPO and tolerated 25% of her diet (despite the fact that she has dysphasia and hasn't swallowed or eaten in 10 days). Not only did the DON tell me this, he stopped me mid conversation (just after I introduced myself and told him who/were my mom is) to 'correct' me, stating that she was on a liquid diet and not NPO... he was rude and off putting. This shocked me because I wasn't rude nor did I come at him in a rude manner. I only walked in and sat down (and only after introducing myself and asking if I could). He stated that I was robbing my mother of her dignity when I advocate on her behalf... as she has been nonverbal 9+ months. The only one 'robbing' anyone of anything was this facility, the nursing staff and this DON. They almost robbed my mother of her wishes and robbed my family and I of my mother having the death she requested. As we were walking to my mothers bedside to do an "assessment", I stated to him once more that she's not able to swallow and it shocked me that I'm told otherwise, to which he replied "if you don't like the services here, then you can leave to another facility. Unprofessional and rude. Horrible experience at my mother's end of life. Oh... and BTW, I reached out to her hospice doctor, who also was surprised to hear that a liquid diet was provided to her... despite her being NPO. The doctor ended up clearing it up and informed (the then uninformed DON) that she was, indeed, NPO. Before anyone wonders if I'm just a grieving family member... I am... but I am also a registered nurse and have a pretty good understanding of what's happening and going on. I might not know how a SNF functions 100%, but I certainly know how to show compassion to a family member when their loved ones die. Am I angry? Hell yeah I am. But I am also level headed and compassionate. I'm looking to move my mother out of here before her death to not only bring peace to her when her time comes, but to also provide a caring and compassionate hospice for my family members to grieve. This place does not do this. HORRIBLE!!! HORRIBLE!!! HORRIBLE!!! Also, why did the Director of Nursing (DON), Augustine Akioya, place a 5 star review for this facility and not indicate that he is currently the administrator for this facility anywhere? He made it sound like someone who used this facility commented on it (see image below). Why did he do this?? To inflate their horrible star review. Skip this place. Don't put your family through this.

    I deeply regret ever putting my father in this facility. He experienced theft my father had named…read morebrand clothing and crystal items we wanted for him to feel comfortable, his personal property was stolen and money we took for him to purchase goodies was taken as well. Meds that were given were not needed. He had soars under his waist from lying around with no help to move around. He fell from his bed and didn't like their food. So we took his favorites on a daily basis. His food was thrown away. After the COVID-19 7days restrictions were completed he was never given a health plan the family was not called to let them know of Physical therapy was needed to help him move around. No special diet was needed. Then why? were they given my dad purred food. He lost a lot of weight and in 1 month time got sicker. He was a active dyeing patient but still deserved respect and well treatment. I am filling a small claim for damages, stolen property, and neglections. Everyone loved ones' deserve to be happy in there resting place of home away from home, unfortunately my dad wasn't.

Covid restrictions gives them an excuse to be more lazy. Went there twice to visit my grandpa (from behind the window) had to ring the bell 8-9 times for someone just come by the door while I could hear them talk. The lady came to the door seemed annoyed as if we were asking her to do us a favor lol. She finally went to his room pretended to open the window so we could see him from the outside, she told us the window was "locked" and to call him instead or to come back on Monday... Tried calling him 5 times, the operator connected me to his room but the phone just kept ringing.... none of my family members were able to see or speak with him during the time that he was there. They feed them garbage food and give them sleeping pills. I'm not sure how they are still in business. It's very questionable. We asked them to pack his things so they can release him, they told us they can't lol. We had to speak with their supervisor. This place is a joke. Please reconsider if you are planning on taking anyone here.
