Ok so I'm finally going to share my experience with you all…read more My mom was in their care for just a couple of months. They won't work with you if you have Kaiser Permanente Insurance so we had to switch to Cigna. They transferred my mom to Culver City Hospital after I was told she would go to Cedars-Sinai for a few days to be evaluated. Do Not let them send your loved one to Culver City Hospital ever! My mom was starting to walk with her walker before they sent her there and now she still can't walk. She had a broken ankle from a fall but was 100% healed. Physical therapy at North Valley Nursing were the only one's caring and listening to my mom. They don't record or track the patients bowel movements from constipated to diarrhea, their pain or pain when urinating and they don't track when they aren't eating. The business office asked why I didn't come to them with my concerns...what the F! Why didn't your staff pay attention and say anything to the Head Nurse for my mom! Oh, I know why...because even when I expressed to the Head Nurse that I thought mom had "another" UTI she did contact the Dr. like I asked but just told me thy were giving her Imdiom for her diarrhea. Within 4 days I get a call that they have called 911 and are sending her to the ER, she's not eating, drinking and can't speak. They even told me the wrong Hospital that the ambulance was taking her to. We visited my mom about 5 days a week as my parents live in Sunland. I hope to God that Nurse isn't still employed there. She would never look up or engage me. It was like she just wanted me to walk away. Honestly this place was recommended to us but I'm telling you be careful. My mom was severely dehydrated and in kidney failure with a bad UTI. She spent 6 days in the hospital and was more alert and herself then when she was at North Valley Nursing. Trust me when I say I have seriously thought of reporting this place. I could continue with more but I hope this gives you an idea of how poorly they cared for her. Physical Therapy cared about my mom and would always update me, love them but the people who spend the most time with our loved one's need to pay better attention to them. I can honestly say at the new facility my mom has had no UTI's or health issues. Good luck if you use this Nursing Facility.

Pleasant accommodations, pleasant staff, Food was good, diverse activities for all that wanted to participate. In comparison to the ones in Palm Springs this was by far the best. Any issues, were taken care right away by administration (Thank You Denise ). Grounds were calm and relaxing atmosphere. Only drawback were the bathrooms (not wheel chair accessible) and transportation not provided for residents. Yes I would you twelve oaks again. This is not just true of the professional staff but includes servers in the dining room, housekeeping, maintenance, etc. Employees are not only supportive of residents, friends, and family members like myself, but they support each other which creates a palpable familial atmosphere throughout. Whatever one's quirks and needs, they are addressed both lovingly and respectfully. They never miss an opportunity to ask after residents' well being and take the time not just to administer individual care but to talk to the residents as if they matter to them. The hallways are wide and bright, and the rooms include closets and bathrooms, not unlike a hotel room. The dining area and common area has high ceilings and very roomy, a nice piano and book shelves and water/fruit stations, a large TV. There is a game room and outdoor patio on 2nd floor. STAFF I've had nothing but good interactions with staff. Importantly, it appears they have a full staff because one of the problems with SNF or the like are that they are understaffed. I've never seen that be an issue here. Always cheery and attentive. Part of the benefit of a place like this is keeping a tight routine, and they do so very consistently, with respect to meds, treatments, food and activities, FOOD Pretty standard but good. The dining area is kept well. The servers are very nice and attentive. I ate with my father-in-law once. Not bad. PARKING Street parking, no lot, but I've always found a spot. COST Reasonable. I've seen higher costs for lesser care, that's for sure. ACTIVITES They seem to really keep it going. Like any institution this can get a bit repetitive and perfunctory, but they seem to keep on top of this. My father-in-law participates and enjoys some of the happenings. Origami, I believe. Do your own due diligence, of course. Anybody making the placement decision for someone should contact the applicable regulatory bodies--City, County and State--to find out about licensing status and any public violations or other concerns. I've been to a lot of these places over the years. It doesn't take much thought to realize that it's not easy to work at, let alone manage a place like this, for obvious reasons. Following a Dental Surgeon's removal of one of my husband's teeth and the requirement for soft foods, they created a week's worth of meals for him including delicious smoothies to enjoy after his appointments. The home is also very clean and well organized. I know my husband is safe and treated with well. The owners are also very clear about what they need to do to help him progress. It is a joy to work with such professional and good people. They see a lot good friends.
