My grandmother did come home from her previous stay here...but unfortunately returned a few months later...and she passed away while staying here. The staff was wonderful to us during her last few weeks here and were very loving after she passed. I feel that the staff truly cares about the patients who live here. read more

My mother was suppose to have a bed in your facility- Encinitas Nursing and Rehabilitation Center. Scripps was ready to transfer her the evening of 9/7/2022 but we were told by Scripps that Encinitas Rehabilitation said it was too late to process her. The following morning 9/8, her nurse told me she she would be transferred soon. I called your facility and got details on what to bring her etc. Soon after that I was informed by Scripps there was no longer a bed available and you would not accept her. I was understandablely concerned. I spoke with Tiffany in Admissions.I explained I was trying to figure out where/ why the placement fell thru. She accused me of being hostile. I was concerned and direct but I was not hostile. I was treated in a dismissive manner by Tiffany. She spoke over me and was not willing to listen. All she had by way of explanation were excuses and blamed the situation entirely on Scripps. It is my thought that there are usually issues on both ends of a problem and a professional would have accepted part of the responsibility. I called back to speak with an administrator to express my concerns. I was told the administrator was not there today. I asked to speak with whoever was in charge. I got Tiffany! In my view, your rehab facility is not being run professionally or with concern for the clients/their families. read more

I applaud the staff who cared for my Mom, Esther during her three week stay after open heart surgery. The Filipino staff were stellar and even called her Nanay so lovingly. DO NOT BRING YOUR LOVED ONE HERE. My grandfather was brought here and the facility in itself is a dump. The nurses would take almost an hour to respond to the button we pushed for my grandfather. They do not have rails on the bed therefore my grandfather fell out and none of the staff knew how long he had been on the floor. They also do not have heart monitors so your loved one could pass away and be dead for HOURS without anyone knowing. Thankfully my family was with my grandfather at the time of his passing. But, during that time the nurse came in checked his heart and said nothing to us, to condolences or anything. Please please do not bring your loved one here. read more

Everyone from the administration to the maintenance members treated my loved one with respect and dignity. I could not have asked for a better place to bring my family member to help recover and come home after. Recently my father was transferred into this facility with a broken rib. Last week he contracted Covid. My family and I called many times leaving messages for the nursing staff to give us a call with an update on his health. We never did receive a call back. On the night of 1/14/22 I called the facility around 7 pm and a lady answered the phone. When I expressed my concern about not hearing from my dad or any of the staff, she told me if I didn't get a call from them it meant he was fine. I told her that wasn't acceptable and I needed to talk to my dad. He was put in isolation without a phone. She began yelling at me and eventually hung up on me. I tried to call back multiple times and they would never pick up the phone. I eventually called the Sheriff's Office and had them do a "welfare check" for me, as I live in Colorado and he lives out there in CA. The Sheriff went to the facility and the staff told him my dad was fine. I asked that the deputy go back into the facility and lay eyes on him since the staff's behavior was so odd. He did and my dad was fine, but wow, all of this over the staff not wanting to return calls. I get they are probably short handed but we gave them a week to make a 5 minute phone call. read more

I was transferred from Scripps memorial hospital to Encinitas Nursing and Rehab Center (ENRC) on March 29th. The director Scott manages here very very well. My case manager would like to solve my problems with insurance company. The therapy team here is the best and supportive, and Director Jessica always make my perfect schedules for me. Certified Nursing Assistants (CNA) are very friendly and responsive for my three meals and helping me take a shower while I need. LVN/LPNs, nurses, and doctors care about my health very much. My room housekeepers are very responsible to keep my room and bathroom clean. I feel lucky to stay in ENRC. read more

I recently visited this skilled nursing facility to visit a friend and was very impressed by the COVID protocols in place. I am a nurse and administrator in the industry and am well aware of the challenges we all face. The building is quite old and not fancy but it was clean and there were no foul odors which is more important in my opinion. It was easy to schedule a visit. It was a simple text to an employee by the name of Lisa. I text her on a Saturday and she got right back to me with an appointment for the next day. She was clear about the COVID protocols and answered all my questions. My friend is quite satisfied with the care she has been receiving. She told me the Physical Therapy department has been motivating her to work hard and reach her goals. I asked her about the food and she said it was very good. If you are looking for good quality care, a strong therapy department, good tasting food and a clean facility then I would recommend you check them out. I'd like to add there is no perfect place but it is the effort to execute a good plan that counts and they definitely do that at this facility. read more

This is the DEFINITIVE review of the Encinitas Nursing and Rehabilitation Center as I WAS THE PATIENT who actually stayed there for 1 full week. I could have left after 3 days, but I chose to stay longer. I liked the regimentation of knowing I had to do Physical Therapy in the morning and Occupational Therapy in the afternoon. And, I loved my caregivers at "ENRC". The facility is dated in small cosmetic ways but very clean and well run. The second day I was there, the Director, Sandra, came to my room with all of her supervisors who introduced themselves to me. They asked if I needed anything, but I told them everything was fine. How many places do you think that happens? Zero. Even the supervisor of the kitchen was there to see if I had any special requirements. The staff, I can't say enough positive things. They were always there when needed them and with an infectious happy personality. Not bad when you are in physical rehab to have everyone around you up and positive. You felt like these people really cared about YOU. Most of the food was good. But, if you were concerned you might not like the evening entree, simply order a sandwich of your choice to fall back on. They will deliver both and then you can decide or eat them both. The dietitian came to my room on the second day and said I should be on a 2400 calorie per day diet because of my size. Their standard menu is based on 2200 calories per day. So, she offered me some simple options like adding a small bowl of cottage cheese or yogurt to each of my meals. Everyone that works there seemed really genuinely interested in me. They have 2 gyms. One with a spectacular unobstructed view of the ocean and the other with about a 50" flat screen TV. Sometimes I would just pull up a chair and enjoying the sun setting over the ocean. Very peaceful and relaxing. You will find this on their website, but at "ENRC" they live these words every day: Treat Every Person with Dignity, Respect and Kindness Attract and Retain Skilled, Compassionate & Dedicated Employees Provide High Quality Medical Care and Exceptional Ancillary Services Maintain a safe, Clean and Comfortable Environment Act as a Responsible, Accountable Steward of Financial Resources Improve the Quality of Life through Adaptive, Innovative and Progressive Strategies Again, I did not see this facility from the outside in. I was the patient that lived there. I don't think you can get a stronger endorsement from anyone. Finally, this should not even be an issue worth bringing up, but it may the single biggest difference between "ENRC" and other nursing and rehab centers. 80% of their employees are Filipinos with 2 Vietnamese and the remaining are Caucasian, Hispanic, etc. That turned out to be the hugest plus of all for me. At least 80-90% of my care and interaction was with the Filipino staff and 2 Vietnamese. They are awesome, caring, loving, concerned, well trained, professionals with great attitudes. They are always happy. It is infectious and adds to a positive rehab experience. read more

Terrible facility! They forgot to order my sister's medications, would not keep to the medication schedule, and dismissed her pain and nausea (caused by missing medication times). The staff were uncaring and more concerned with their schedule than getting my sister her medication on time. I call many times and got no answer. read more

My father stayed here for just over a month as he recovered from multiple fractures that occurred as a result of a car accident. We chose this facility because it was COVID-free and had a great reputation for PT/OT. The PT was excellent. Often twice per day and wonderful staff. The nursing staff was friendly (my dad especially liked his night nurse Ruby) and I was able to get the info and updates I needed from MD/nurses overseeing his care. His only complaint was that the food was bad. Even though the facility is not as fancy as some in the area, I would definitely choose it again if PT is a priority. read more

I toured the facility prior to sending my mother here for post-acute rehab following back surgery. I was told a lovely story by the administrator, and the place seemed okay for a brief stay. After she arrived, the nightmare began. She was slumped at the bottom of her bed and unable to pull herself up, and also had not gotten her pain medication. No amount of requesting assistance mattered, as the aide said "You will just have to wait until I can get there. We had 5 admissions and I am busy!" (Clearly, they are extremely understaffed, and that is part of the problem). Eventually the aide arrived and repositioned her, meanwhile calling her "sweetie", "honey", etc in a childish tone. Made me shudder. Next, the nurse supervisor came in and gave an orientation to the call light,etc- however, she spoke so quickly that no one could follow her. Then, and here is the real clincher, she looked at her med sheet and said "we will get the insulin right away". What???? I told her that my mom is not diabetic, and she insisted it was on the sheet. I looked at the sheet, and it was THE WRONG PATIENT. She was going to give her the wrong meds! I got her out the next morning, after she spent a night with the demented roommate hovering over her, saying "I need to take care of you." When I called in the morning, I heard another aide yelling at my mom...yes, yelling. My mom said "yep, she has been yelling at everybody all morning." So happy she got out, but feel so bad for the remaining patients. My advice is DO NOT take your family member there. read more

My father has been in this facility for three weeks. I recently met Peter the administrator. I found him to be warm, caring and professional. He takes care of any problems that have come up and is quick to solve any issues with staff. I believe he will take this hospital to new levels of care. I am very fond of the cleaning staff, especially Chris who always has a smile and keeps this hospital as clean as a whistle. All of the cleaning staff is the same. I remember Angelic and all the others are kind and friendly. I will always love Maintee, one of our favorite nurses who loves my dad and is always in a great mood. This could be a great place, but it isn't. Problems, if you or your loved one have serious medical issues like diabetes that should be checked every few hours, be afraid. The lack of urgency and mentality of the CNA'S and RN'S is evident. Many rooms have an ocean view. The entry remains unmanned and unlocked all times, big safety issue. This is not highly skilled nursing, it's mediocre. The food was OK. The staff was accommodating to most requests and willing to correct deficiencies. read more

PLEASE READ BEFORE YOU PUT YOUR LOVED ONE HERE! This facility was sub-standard in ALL WAYS in the care of my mother who was brought here after a fall with multiple fractures. When she arrived, she was ambulatory (limited), was eating and working on getting stronger. When she returned to the hospital (yes, this facility has a terrible rating in hospital re-admits), she had lost her ability to walk, had the beginnings of bed-sores, had a catheter placed (without our consent or knowledge), and basically had stopped eating. Bottom line: the staff is unskilled, lazy and the lack of communication with family members is abhorrent. They talk a good game until you leave...then - nothing. STAY AWAY. read more

My mom is having to wait an hour for her medication to be delivered when she asks for some to help her they just ignore her and say they are on break or lunch and are not able to find someone to help the nurses here are really rude at times when I came here to drop her off the first dropped her off there was no direction or guidance of where we were supposed to go the main lady that works in the office was not helpful at all read more

I was in Encinitas Skilled Nursing Facility twice and there are not enough words to say how wonderful this facility is. I have been in two prior facilities (one where I had broken my back), Encinitas Skilled nursing Facility however was by far the friendliest, happiest, most caring facility I have ever expected to be at. The nursing staff, PT&OT, the CNA's, even the laundry and janitorial staff are all so very friendly. Everybody knew my name, every morning when I was rolling around in my wheelchair every staff member would say good morning even the administrative staff (even the staff tha didn't know who I was). I would attend the activities every day and the activity Director Lisa Makes every day fun for patients of all ages. I cannot express how hard this woman works to make every day positive, enjoyable and leaves you wanting more. The two times that I have been at this facility have changed my life. I have made friends that I hope to continue to have outside of the facility and I would highly recommend this facility to anyone who has to go to a Skilled Nursing Facility or who has a loved one that they are thinking of putting in the Skilled Nursing Facility. If I ever get sick again or am unable to care for myself, this is the only facility that I want to be at!!! read more

Updat to my review. I contacted the California State Licensing Dept. After a lengthy investigation Encinitas Rehab. was found guilty of complete neglect. It was confirmed that the staff stood around my dad bed and just watched as he choked to death. It was also confirmed that all that was needed was to have his air way cleared and his chances of survival would have been greater then his chances of dying. Please do not believe the giant sign hanging from their building. The survey that determined how many stars they receive has nothing to do with the state of California's guide lines and laws. read more

I had major surgery and after a week and a half stay in the hospital I was sent to Encinitas Nursing and Rehab for wound care, physical therapy and dietary. Upon my arrival I was just thrown in a room by a health aid. She was bar far the rudest person I have ever had contact with. This woman did not plug the bed in so it could be adjusted. The tv remote did not have batteries so I could not turn the tv on. There was no call switch. I had to use my cell phone to google and call the front desk for assistance. When the health aid came back she had a salad and pretty much threw it on the table. I had then had enough of her and gave her a piece of my mind. I demanded to see a supervisor who came in and I requested another room because the elderly woman they had placed me with was in agony. The next room I was placed in was a little better a distant view of the ocean. My roomate this time was a elderly sweetie however in the middle of the night she was ringing her bell for assistance and the workers completely ignored her. Again I used my cell phone to get someone in to help her. I had to use my cell phone so many times to get assistance. The staff would forget to bring my pain meds... My dietary needs were not met... therapy was great with the therapist. I was supposed to be in Encinitas Nursing and Rehab for 6 weeks I was there 6 days. I stayed long enough to go to my first follow up appointment with my doctor and I had my ride waiting on me at my appointment to go home. I told my doctor I wasnt returning to Encinitas and I showed him pictures of the filth and I wish I could have bottled the smell from that place...Upon my refusal to return, home health was set up for me at home. I would not suggest this place to anyone unless you have no other choice. If you do place your loved one here you will have to watch this place like a hawk! Alot of the patients here are elderly and cannot speak for themselves. I am young and I could not walk but I could communicate and see the things that this place hides. The workers are used to controlling and yelling at patients. When they seen I still have my God given sense I was neglected as a patient because I was more of a threat to them . I will say it again ONLY USE THIS PLACE IF YOU HAVE NO OTHER OPTIONS and you ARE WILLING TO BABYSIT YOUR LOVED ONE. read more

This place is appalling. It has been renovated fairly recently, but new flooring, drapes & paint can't conceal the depressive state it is in. My mother died there years ago after a lengthy stay. It was a continual battle for good service. They hated to see me coming in, that is for sure! Now a very close friend has the sad misfortune to be stuck there. I have been visiting her & have witnessed once again an uncaring & sullen staff. I am sick to death of this common problem with so many of our health care facilities & it is definitely a major issue in this place. I have a friend who was staying there, luckily for a brief time. She is not elderly or hallucinating, so when she says she is getting lousy care I believe her (doctors' orders not followed, dietary restrictions not followed). I visited to make sure she was okay. Upon entering, I was overwhelmed by an industrial chemical scent - bad sign. In the event I ever have to chose a place like this for myself or a family member, I'll know what to avoid. read more

My close friend called me today hysterical. The nursing staff completely neglected her father at the facility. We went together and with my medical background I was able to give insight to the staff. Her father has Parkinson's and diabetes and is in a subcritical state of care however this facility was not treating properly. For instance, his medication was not distributed at the the proper time or AT ALL. I would not recommend for anyone to admit their family to this facility. read more

I am a Healthcare Liaison for Home Health and Hospice Services and have been fortunate enough to work with this facility and their wonderful staff. Whenever there is someone we are helping with our services that is in need of Nursing or Rehabilitation I always recommend Encinitas Nursing and Rehab. It is an extremely clean, professional, and welcoming environment. The staff is very warm, gentle, and great at what they do. They have really stepped up their game as far as the food is concerned, my mom is so happy this time around. Mom has been here about 4 times so far. Earlier this year the food was really lacking in all areas as usual. Huge shift for the better. The nurses have been very attentive and helpful. There was only one person I did not get along with and I could tell she was sleep deprived. I would trust them for further rehab and am grateful they take mom's insurance. I am only writing about my wonderful experience as a rehab patient. I found the nursing and administrative staff to be courteous, competent, friendly, and professional. The facility is clean with great views of the ocean in most rehab rooms. They have some single rooms (I had one of those), but most of the rooms are doubles. The food was good with several choices and great dietary staff to advise me. I had very positive experiences with physical therapy, occupational therapy, speech therapy, and dietary. The attending physician (Anand Patel, MD) is very competent and has a wonderful bedside manner. I progressed from a very devastating illness and was discharged ambulatory in three weeks which is a small miracle in itself. I recommend this facility and its wonderful staff to anyone who wants to cooperate enthusiastically with their rehab. read more

Total lack of compassion from the nursing staff. Mother-in-law placed in a semi-private room. Roommate had total control of phone, played TV loud, constantly hollering for staff. Meanwhile, my mother-in-law could not even get staff to help her to the toilet. Other had fan, none for her. Food poorly prepared/presented = uneatable. She developed bed sores from lack of hygiene. Finally, sent home, Sunday. On death bed same day. Staff simply did not care. read more

Fortunately, I've not had to experience any nursing facilities on a personal level.... until now. I finally found my father (after 18 years).... He has been a resident @ this facility for 3 years. I'm very unhappy at what I've seen & heard of my Dad's care here. There is no attentiveness to his hygiene needs. There is no schedule of any sort. Unfortunately, the lack of care is obvious when I visit. He has on 2 different shoes, unmatched clothing, bad breath, unshaven and long fingernails. This is unacceptable. read more

Would NEVER recommend this facility. My father in law waited for 2 hours for a diaper change twice.....screaming in pain and was told to stop!! He had been waiting for an MD to adjust this pain meds for days. Was told there was no MD available....I had to track down the MD myself to get help. When I found the MD on staff he was very kind and helpful. He is dying of cancer and needed some relief. HIs nurse and LVN has no compassion and rude. He was afraid to call for another blanket when he was cold. read more
