I just completed a 20 day day stay at Bayshire Carlsbad after total knee replacement surgery at Tri-City. I'm giving my strongest recommendation for Bayshire Carlsbad. Of all the positive things I can say, the thing that is the most meaningful to me is how much each and all of them cared for me both professionally professionally and personally. I felt cared for. I will always be grateful to the people at Bayshire Carlsbad, for how they cared for me while I was recovering from a painful recovery that was fraught with obstacles. Now I am home and I feel confident that I will complete my recovery and have the best possible outcome I will always remember with fondness the many people who helped me at Bayshire Carlsbad. read more

I know this place has good reviews. but so far that's not been my full experience. I'm sure the PT part of the Rehab wing is good. my mom is currently staying there to transition from a hospital back to home and getting rehab. They have had trouble adjusting to her vegetarian diet and they left her sitting in a chair for 6 hours one day and another day they didn't get her up til 11 AM. once they forgot to bring her dinner. She is perfectly capable of advocating for herself but also is adjusting from a hospital stint and lots of drugs so she needs some support to be moving more. That's why she's there. I get that she gets a PT person there, but the rest of the time she's not moving much. she should not be left to sit in a chair for prolonged periods and should be allowed to transition back tot he bed after 2 hours and then back again To keep repositioning and get some stimulation. But to sit 6 hours in one spot just because she is dozing is not good. im Not saying the staff isn't good quality. There have been many fine CNAs who have been really great and nurturing, I don't want to paint a whole negative picture. But when you have a lot to do and people to serve it can be challenging. I feel it's management's real to be on top of this and making sure everything is getting down. It seems like good care might be sporadic here. I recommend staying on top of them and keeping a watchful eye. read more

I just want to let everyone know a very close friend was at Bayshire My friend suffers from Parkinson's disease and took a fall at his assisted living facility Unfortunately he injured his hip and needed a replacement During his recovery at Bayshire he was yelled at by a nurse Left to sit in his BM for quite some time after multiple attempts to get a nurse for some help. He dislocated his hip and sent to the hospital with no communication to the family. I could continue with more examples of the lack of care at this facility but will just finish this review urging everyone to look elsewhere for their loved ones needs read more

My mom has been a memory care resident at Bayshire for just about three years now. My sister and I are very happy with the care that she is given and appreciate all the incredible staff and their patience. Dealing with residents who suffer from dementia is not easy, yet all the staff are always so gracious and kind. I really appreciate that they take the time get to know my mom and engage with her on a personal level. They listen to her stories and have effective ways of redirecting her when she gets frustrated. She also loves the food, activities and outdoor garden area. I also, appreciate that they allow us to bring her little dog in for weekly visits. My sister was a recent patient and received substandard care. While there are many fine staff, the care was sporadic. Wrong food often was given that would seriously affect a patient with abdominal surgery. Critical supplies were unavailable and on backorder. Shower was dirty. Improvements are needed. read more

Clean, friendly, a little understaffed and for some reason they moved my mom to 5 different rooms while she was their recovering from a broken femur and some health issues. she lost several of her belongings while their which was frustrating for me during her stay. I liked the staff as people but they need to get their facility properly staffed w enough help and stop moving patients all over the facility while there. Good care. Good communication in general. My mom liked the food and said she liked the facility so that was nice to know. She felt like she was cared for properly while she was there for 6 weeks. read more

My mother has had to be at Bayshire twice. She has been treated with professionalism and kindness. The staff is well trained for the jobs they must perform. Whenever my mother needs anything, it is given or completed with careful consideration. The nurses and caregivers provide the best support and quality care . I ,generally, speak with Katie one of the head nurses and she provides me with specific information about my mother's condition and care. The food is good and snacks are provided. The Activities Director,Cecilia, is very knowledgeable concerning activities for the clients. The activities involve all the patients and include thought provoking items. The clients have to recall past events and experiences. I feel my mom has had to activate her brain to participate. Overall, our experience with Bayshire and all of the staff has been positive. Can you believe a SNF where the people who worked there were actually HAPPY !!! This was a devastating time in my family's life watching our husband / father go thru 7 Spine Surgeries . Each surgery created more permanent cognitive sundowners issues . BAYSHIRE SKILLED NURSING WAS THERE FOR ALL OF US !! Beginning with Kate at the front desk a true gem she was patient and explained all papers I needed to sign The P.T. was understanding with , his limitations all while encouraging his success Extremely pleased is an understatement . Bayshire SNF provided high quality of service, delivered excellent clinical care the entire staff from director of nursing , (Denise) the fabulous case manager (Lani) supportive nurses & CNA' s housekeeping ,food services , activities director actually cared about my husband and I The staff always conducted themselves in a professional manner and showed a lot of love for all of the patients they care for daily. Very disappointed in the administration of this facility. In an otherwise clean and decent environment, the administration was defensive and borderline abusive of our family. Any input was received with disrespect and rancor. After our family member went into the ER for the 3rd time in his 2 week stay, they refused to hold a room and basically kicked him out. I absolutely loved working here. The staff genuinely cares about the residents. Everyone from the maintenance team, like Shane, to the marketing team, like Karinna, shows up to work daily with an attitude of service. This is by far the absolute best work culture I have ever experienced! Each and every member is so focused on resident care first. I will miss this team, and I will miss the residents. Broke my hip and femur and was lucky to end up in Bayshire for rehab. The doctor and staff were first rate. And not just with me. I witnessed them treat the truly in-need with endless kindness and compassion -- I felt that I'd stumbled into a nest of angels. Special shout out to Dr. Mark Kyle who's knowledge and bedside manner calmed me though my worst days. And Grace, dedicated, loving and always smiling. Nealani from Social Services who made the overwhelming paperwork make sense. I would give Bayshire Skilled Nursing 10 stars if I could. They demonstrated how exceptional they are when they helped my mom when she was in a dire health and safety situation at a Board & Care. Nichole and Alexis were working to get my mom a room when I had to take her to an ER with a waiting room swarmed with 60 people. Nichole called at that moment saying they had a room and to bring mom in immediately. Honestly, caring for a loved one with urgent health needs and feeling helpless about it, having a capable, compassionate team swoop in feels like a miracle. I asked Nichole if they wear capes, and if not I'd get them some because they were super heroes in that time of need. Mom had been at Bayshire once before, and just like before the staff was attentive, professional, friendly and skilled. Within two weeks, mom's deteriorating condition had reversed, and case management helped us find and facilitate the transfer to a great assisted living facility. read more

Bayshire Carlsbad has been the perfect place for my mom. From our first tour, we were explained all of the options and opportunities that are afforded to the residents. Since my mom does not drive, we were looking for a place that provided ample activities for socialization and enrichment. every day there are in-house activities and on many days there are also opportunities to explore outside of the community. The activity is Director, Oleh, is a lovely human being. He is full of joy and so kind to every resident. The dining options are wonderful as well. Residents are given the opportunity to dying restaurant style in the main dining room or to receive meals in their rooms. The food is varied and quite good. After a short time, the staff members all knew my mom by name and looked out for her as if she was a family member. We joke with my mom that she lives in a community that is the equivalent of being on a cruise every day! I am so grateful for the staff of Bayshire for their genuine care for someone who means so much to me. read more

I cannot say enough about this skilled nursing facility. My 94 year old mom has been there several times over the last few years and received the very best of care each time. The staff were compassionate and caring and stayed on top of her care needs during her time there, which was about a month each stay. When it was time for her to be discharged, she did not want to leave and she's pretty picky so that says something about this facility. They also have an assisted living facility attached to the skilled nursing and I noted several skilled nursing patients ended up moving over to assisted living when discharged so they must have also been impressed in order to make the decision to permanently live there. The patients are really treated like family. I highly recommend Bayshire Carlsbad! read more

I love Bayshire ! They took such great care of my mother. There was never a shortage of nurses. They are so well staffed. I love the fact that they would get the patients out of their rooms and get them all together in the common room to play games, listen to music, sing , watch movies. She loved the food and made friends. She stayed in a five star skilled nursing facility on the beach a couple years ago and they just left her in her room. Was here for 3 weeks. Here's my take. Cleanliness 5 Cna's and Nurses 4.5 Phys and Occ therapy 4.5 Staff and employee communication 2.5 Food 3.5 Overall it was an good experience. The nurses are great as well. Food is above avg considering where you are. Kitchen works hard to satisfy your desires. I would definitely recommend this place for your loved ones. I was injured by Physical Therapists at Bayshite through neglect and lack of care. This is not a 5 star facility. I've spent thousands of $$ trying to avoid the surgery that my shoulder needed after they let me fall and then refused to report It. Do Not go there! read more

My mother has been a SNF patient for a few weeks and I could not give a higher rating. I am a PT who works in the Hospital setting so I did not want to have Mum go into a SNF. However the nurses, Rehab therapists, Activities director, CNAs and DR Kyle are top notch. They communicate well and really make the patients welcome. Oh and the Hairstylist was amazing as well. As someone who sees and hears a lot about different SNF in my area- this place was a very nice surprise! read more

My wife was transferred to Bayshire last year after having major back surgery. The rooms were fairly clean, but cramped as she shared a room. The check-in went smoothly and the staff seemed friendly. However, that's where the skilled nursing ended. After 2 days of sub-par care from the doctor and nurses, my wife had developed a severe cough and her lungs were starting to fill up with fluid. She needed better treatment than what she was being given. I asked the charge nurse how her vitals looked and she said they looked fine. After considering our options, I decided to get her out of Bayshire asap and back into the hospital. I couldn't risk taking her in my car because she was hooked up to an IV etc, so I asked if they would call 911 for me. They refused, so I called them myself and the paramedics showed up within 5-10 minutes. They determined that she needed to go to the ER immediately. Long story short, the nurses and doctors in the ER told me that she had pneumonia and she had become severely dehydrated. They also said that my wife was in very bad shape and it was a good thing we got her there when we did. Looking back on the whole experience, Bayshire (formerly called Brookdale) is not the same skilled nursing facility it used to be because it had changed owners a year or 2 earlier. No one we met at Bayshire last year had been employed there more than 6 months. We can reliably compare the care my wife received at Bayshire last year to Brookdale in years past because my wife was a former patient at Brookdale and they really treated her well. The care was excellent back then. If we ever need a skilled nursing facility in the future, we would never consider going here again. read more

Our mother Barbara lives at Bayshire Assisted living and they are wonderful, she loves it there. She did get a UTI and it progressed before she was able to get the medicine and she ended up going into the hospital. Once we got her out she was sent to Bayshire skilled nursing. We did receive very good care, sometimes slow to arrive when call button is pressed. The overall care and concern was good. Where we had trouble was the management of our mother. Our caseworker was on vacation when our mother came in. We had to ask lots of questions and try to figure out what was going on. We did feel as though we were bothering the administrative staff on a regular basis. When our case manager, Nichole, came back from her vacation we asked to have a meeting with her. She said she could schedule a call for 2 of us to talk with her but there are 5 children and we all wanted to be a part of the solution. She told us we would have to organize how to do that cause she could only organize to speak to 2. We did have a zoom meeting we set up. It was frustrating that Nichole never turned on her camera. The lack of seeing her seemed unprofessional since we are dealing with very personal situation and seeing a face would have been much more personal. It all worked out with us asking for our mother to be transferred back to Assisted Living which was a better situation for our mother. The overall experience was good but we felt as though we were lost in the shuffle due to our case manager being gone. When Nichole came back we felt as though she was very put off with us and not too helpful at the beginning. read more

June 2023 My Dad has Parkinsons and was brought to Bayshire for physical therapy. They are severaly short staffed, and it shows in the level of care provided/not provided. My Dad went seven full days without Parkinsons medication. You could tell something was wrong by his demeanor and inability to fully participate in PT. When we asked about his meds, we were told each time that his meds had been administered, when in reality they had not been given. When we checked out - we were able to confirm by counting the pills. We were there full days and noticed he was not getting meds -- only to be told "he already got them". How? We were here all day?? Lack of staff and training. If your loved one has incontinence issues, plan on them staying unchanged for hours at a time. They are so short staffed that the patients simply sit, unchanged, asking for help. This causes sores to develop and is extremely dangerous for elderly patients. The case worker and doctor were never in sync. Communication is lacking and not consistent. Be prepared to question everything and you will have to fight for your loved ones care, and rights. The management team should be ashamed of themselves. We were not expecting miracles. We were, however, expecting basic care, such as meds being administered and timely help with bathroom needs. I took a tour of the assisted living side of the facilityas well, and would never put a loved one here. Maybe it's lack of staff, but this place did not live up to the reviews or even basic expectations. read more

My 100 yr. old Mom was in skilled nursing at Bayshire for 3 weeks. She, Mom, was recovering from a hospital stay due to severe dehydration. Mom returned to her own home today -- I can't imagine any facility giving Mom better care than the care she received at Bayshire Carlsbad! read more

There are a few concerns. They double dosed my mom her medications because the prior shift forgot to sign off . I understand nurses makes mistakes however a lack of communication between shift changes. The staff were nice and pleasant . My mom's Christmas dinner that was brought in from her family went missing:( . She was looking forward to it! Some of her things went missing but ended up showing up a few days later. The staff came into my mom's room with loud voices . My mom asked daily for a cup of coffee each morning to have with her breakfast but never showed up until hours after she ate her breakfast. Some mornings they were just completely forget it all together. Forget about using the call button sometimes they would come and others times they wouldn't. I am praying for the staff to have a better bed side manner . Some staff were exceptional. The facility was clean ! read more

My Dad stayed at the SNF. They were attentive to him, they answered all my questions, and were generally great. I think because of covid it was especially hard to attend to every need. I was not allowed in and so had to depend upon staff to do things they don't normally do and in that respect sometimes it was hard to get requests completed,but in general I have only good things to say about their attention, care, and cheerfulness. read more
