first day- Power outage at night with NO backup generator, no staff on duty to assist. While doing PT, the old equipment I was told to utilize (parraell bars) broke at the junction where the bar was slotted into. I regret not submitting this review sooner. My mother stayed at this facility in mid November while recovering from surgery, and the care she received was appalling. The staff barely attended to her--it felt like they just tossed a sandwich at her and walked away. The doctor never once saw her in person, despite claims to the contrary. I was there the very night the records say he made his rounds, and he was nowhere to be found. Thankfully, her surgeon was so concerned that she personally came to check on my mom. Alarmed by what she saw, she immediately arranged for her to be transferred back to the hospital the next morning. I spent a sleepless night fearing something might go terribly wrong before the transfer could happen. This facility should be shut down. I truly feel for the patients who are still there--honestly, prisoners probably receive better care. Please do not place a loved on here no matter what. read more

Most horrible place I had to go to. I caught covid and the idiots told me they make a big deal about covid. None of nurses at night wore masks and I wound up with covid. And they never cleaned my room. I had fell and broke my hip and femur and a person that came in my room I asked for her help to get go to the bathroom and she she grabbed my leg that I was in there for. I cannot believe the horrendous experience my grandmother had at this place. She was only here for a few days before my father and his brother complained so many times to the point where they just let her go. My grandmother broke her hip, was in the hospital for about a week, and went here to recover with the promise for physical therapy and a nice environment to help her. The place is disgusting and severely understaffed. There was not enough staff to provide her the therapy she needed to get better. She rang the button to go to the bathroom at night, since she LITERALLY CANNOT GET UP BY HERSELF. The nurse's response was to "use your diaper and we will clean you in the morning." Absolutely despicable and evil. Zero empathy. These people should not be in healthcare. My grandmother is also recovering from a UTI and this is how she is treated? In a place that is supposed to support her getting better??? What a joke. I swear these people don't want to do their job. She is way better off being home and having family take care of her. An absolute joke! My mom was sent there after leaving Scripps Encinitas Hospital for rehab. She was there two days... no rehab, no shower, completely ignored when she pushed her button to go to the bathroom. They "found her" on the floor at 7am on the morning of day 3. Black eye and three broken ribs. She pushed the button to go the bathroom... no one came and she took herself and fell. Staff will tell you they are understaffed. Staff does not follow through on requests. Staff don't have access to records, medications... that's what they told me which I find hard to believe since Scripps ALWAYS has discharge paperwork. Moved my mom to another facility. I didn't trust them to take care of her or follow through on anything. The place is depressing... narrow hallways, drabby decor, ripped sheets, and no views of anything pleasant outside of the window. I'm reading some of these reviews and these people must've been at another facility or "paid" to write a great review. And yes, I'm contacting the higher ups to make sure they documented her fall / injuries at their site. I don't believe the "Five star" rating at all... must be very low standards for them to have a five. read more

physical and Occupational therapy was outstanding, but the CNA staff are completely clueless about medical care. Food is inedible, I took my 86 yr old husband out of this facility after 7 days, the air conditioning is nonexistent, combined with the lousy food and inept nursing assistants added up to a facility not at all contributory to rehabilitation or post surgery recovery. My family and I decided we could do better at home and we have! The building is very old, and the rooms are too small to allow for visitors chairs. This facility is not deserving of 4 stars or even 3. The nurses and therapists were excellent, but the CNAs were rough in handling patients and downright rude. My 85 year old Aunt Edie had a bad fall & broke 8 ribs . Some were fractured but the others were broken ribs . This is a big risk because the broken ribs could go into an organ . My Aunt ( by marriage) is very petite and small . Unfortunately November 15 she had surgery for cancer in her mouth after healing and getting on her feet I believe about four months this fall in the middle of March happened . After cancer she was lucky enough to be at the Rancho Bernardo Care Facility for rehab & that was pretty fancy this time she was at the Encinitas skilled nursing facility and I have to admit that the people that work at both of these facilities have their hands full ! All the staff does deserve to be recognized at both facilities( there are RN's, LVN's , CNA's & housekeeping staff , a kitchen staff , maintenance staff , clerical staff and management staff..It is a lot of work to take care of these people .I told someone that my aunt did not care for the rehab facilities their answer was no one likes rehab. I wanted to recognize a Krystal for checking us out and also letting me return for copies of paperwork . I am grateful to the special attention that we received from Krystal ! If u love your family and friends? Stay away from this place. The place was dirty. The majority of the workers were extremely rude. They treat you as if to don't exist. My sister went there for recovery after back surgery, All she did all day was lay in bed. Only once did she receive physical therapy to help her walk, and that lasted for 15 minutes, then back to bed. When I went into her joined restroom to rinse a glass, a tear came down my check because of how filthy it was. How a place like this, can take care of ANYONE is beyond me. PLEASE BEWARE. What a disappointment. My mother is currently at Scripps and we were looking for a rehab facility. We showed up to the facility to see if we could get a tour. The secretary in front after checking with the administrator advised they were at lunch and gave us a card to call back later. Imagine driving 30 minutes just to be given a card. Where do I start? Let's start with the GOOD: my elderly mother was here for a week after a stay at Scripps Encinitas. Although my mom is 94, she is sharp as a tack. In other words, she knows what is up. The goal was to get her stronger to go home. I would say that the physical therapy helped her accomplish that. The "gym" was nicely equipped and had an ocean view. My mom loved that. She also liked the physical therapists as they motivated her towards her goal. They did their jobs and were attentive to my mother's needs. The other thing, my mom really enjoyed was the chili relleno, so props to whomever made that chili relleno. Muy sabrosa! She didn't mind the food so much. I thought it looked gross, personally, but it wasn't for me, so there you have it. The BAD: the communication is seriously lacking. Not only does the staff and nurses need to communicate with families, but also with their patients. There seemed to be a lot of miscommunication, unanswered phones and omission of information. For example, one night my mom needed her medication and after waiting for two hours (her roommate, as well), I called to find out that the "computers were down." I felt that should have been communicated as a courtesy to waiting patients. Medication is pretty critical for care. Additionally, my mom was there five days before anyone helped her shower. When I asked the nurse about it, she told me my mom was "skipped over" in the schedule. I wonder if the nurse goes five days without a shower? I'm guessing no. I get the feeling this facility is used to not communicating with families. My mom felt ignored several times and there was always a "reason." I feel badly for the patients who are there with no family close by to give them a voice. Ugh. The other thing, the facility needs to be updated. It does not seem hygienic or maintained. My mom's shared bathroom was nasty. It smelled like a rest stop. It looks as though the roof is being fixed, I sure hope so (see photo). I know it's not a Four Seasons, but a lot needs to be fixed up in here. The equipment is super antiquated, e.g., beds, curtains, medical equipment (you should see the scale to weigh the patients. Omg. What.) Okay, so the UGLY: remember that one time when I said I don't think some of the staff are used to communicating with families and/or the patients regularly? I had the medication "nurse," Denise actually tell me "your family asks a lot of questions." That was legitimately the FIRST thing out of her mouth when I inquired about my mom's medication (on behalf of my 94 year old mom). When I tell you this person is rude, that is an understatement. This nurse, as I told her, is in the wrong profession. Having your elderly parent in a rehabilitation center is difficult enough without having a nurse with zero empathy and a repugnant attitude. All in all, I'm glad my mom got some rehab, we are grateful to the nurses that did look after my mom during her short stay, namely Judith, who seemed to know what was going on, but I will do everything in my power to keep her from ever coming back here again. read more

My wife was a patient at ENR for 35 days ( July 2nd thru Sept 1st ). She needs further rehab and has been transferred to an Assisted Living facility for care and possible future living full time. While at ENR, she received expert care and attention. While the facility is a bit old and needs some remodeling here and there, the staff is anything but in need of a thing. They are super friendly and very professional. Attentive and helpful at each step of every day. They operate in groups of separated skills and duties which keeps the care centered, efficient and special to the patient. Updated review with more photos. My husband stayed here in February 2018. Tip: Don't check in on a Friday afternoon. Administration only works Monday through Friday. We were never welcomed or given any kind of orientation. Facility doctor was curt, uncaring, and saw my spouse only once during his 5 day stay. There is a huge gap between the unseen administration of this facility and the daily, kind nurses aids and therapists. Personally, I would never ever have a loved one at this facility. Speaking to the staff and observing the quality of care here it's my personal opinion that this center should be shut down. Lacks cleanliness (from the peeling paint to the unkept floors, and the yellow toilets falling apart) Lacks attention to patients (I watched as a patient pushed her button and wait 45 min for care while some nurses were with patients the others were sitting at the front desk talking, and patients sitting in filth for an hour after knowing they went to the bathroom) Lack of equipment such as bed sensors, side rails if needed, protective gear, and many other appropriate items needed for the elderly. Nurses don't read patients charts properly. A nurse mocked and guilted her patient with confusion. Medication not distributed on time to most patients. After inspection I would not send my family member here and I don't recommend it to anyone. In fact, if you have someone that stays here, get them out they are not cared for how they should be. read more

My mom spent 7 weeks here and was very well cared for. She arrived in a weakened state and exited close to how she was before she got ill. There was helpful PT and OT, with a nice view of the ocean. The nursing staff was kind and kept a close watch on her. The entire staff put on a great dinner and music event for Valentine's Day that mom really enjoyed. They judged when she was ready to go home and discussed with us to ensure we were all comfortable. We appreciate the entire staff that got her back to better health. read more

My mom was in Hospice care at this facility. Although she was only there for little over a month, they took very good care of her. The place is clean and well staffed. They seemed to know what mom needed and when. They even sat with her while she ate because she was having a hard time swallowing. All of the nurses were so sweet and pleasant to work with. I am giving one star for the ENR Physical Therapy team as they are the only positive aspect in addition to a few of the CNA's at what is laughingly listed as an "Award Winning" Rehab Facility. This facility is a shabby, rundown and very understaffed patient care place that needs a complete overhaul of its beds, meals and "trained" CNA staff. My Mom as a new amputee had to stay here and her entire experience was a nightmare from beginning to end. 1. The Scripps Doctor who is assigned to this place, Dr. Clifford, is very incompetent and appears incapable of reading new patient's medical charts after the patient has been admitted. He reduced my Mom's pain medication of his own volition and failed to read on her incoming medical orders that the pain medication was to be administered in 1/2 pill dosages. So for weeks, she was receiving 1/2 of a 2.5 mg. of Hydrocodone which did NOTHING for her intense pain in her stump. He also did NOT read that she is Hypoglycemic and she was tested 3-4 times a day for DIABETES w/blood samples taken from her fingers. This left all of her fingers very sore and bruised. The med techs also tried to give her Insulin every day! IF she had taken the insulin, she could have become very ill. The med techs obviously need more training so that they do not endanger patient's health w/administering insulin to NON-Diabetic people. I called the Director of Nursing, Dennis Carter, and had to leave messages but was successful in having these medication errors stopped. 2. CNA Training To Handle Amputee Patients: We quickly discovered that all of the CNA staff was NOT trained in how to transfer amputee patients from the bed or wheelchair to the bedside commode. Many of them had no idea of how to assist/support my Mom so she could easily transfer to use the commode. Some of them had to get help as they were not strong enough to transfer her safely. A. She often waited 30-45 minutes to be transferred to the commode. Completely Unacceptable for a senior patient to wait longer than 15 minutes. B. She did fall one time when the CNA said to her, "I haven't had much training in how to do this." So, the wheelchair was in the wrong position and my Mom fell onto the floor directly on her new stump. I was furious and wanted to file a report w/the ombudsman for the facility. But my Mom would not let me -- as she was afraid of reprisal from the ENR staff. 3. The "Nutritional" Menu -- is very subpar and horribly undercooked/overcooked every day. Most of the lunch/dinner meals consist of unrecognizable "meat" smothered in salty or spicy gravy/sauce, undercooked vegetables that are very spicy and a bread roll. Breakfasts were usually palatable unless spicy sausage was served as the main meal. We know that the ENR staff never eat the facility food because we asked them. The common response was a visible shudder and a definitive, "NO!" I do not know how the long-term patients survive on the horrible food choices. 4. The Med Techs: Not well trained and negligent with the administration of medications for patients. My Mom was brought her roommate's medications at least 2 times with the med tech saying, "Here are your medications for today". If she was not a coherent patient, she would have taken the wrong medications and possibly had serious complications. These same med techs also tried to get her to take Insulin even though her medical chart listed her as Hypoglycemic. A definite lawsuit situation. 5. Sexual Harassment From CNA Staff: There was a CNA, Jose, who acted in a very inappropriate manner towards my Mom on numerous occasions. She had requested to have female CNA's to assist her w/her bathroom needs. Jose was assigned to her on 3rd shift and one night told her that "He had needs too and had seen it all (referring to seeing female patients naked)." He treated her roughly when assisting her in getting back into bed and was angry that she reported his behavior to the CNA staff. ENR should have fired him after the report was filed but instead sent him to a different hall for a few nights so that he did not have access to my Mom. When he was allowed to work on her hall again, he was not allowed to assist her. ENR is grossly negligent in not following legal protocol for sexual harassment guidelines in this situation. 6. Room Telephones, Roommate TV Noise & Patient Noise Levels: A. My Mom's room had ONE phone that had to be shared w/her roommate because ENR refused to repair/replace the phone jack on her side of the room. B. Roommate TV Noise and overall Patient Noise Levels are not monitored so that TV's can blare and Patient's can scream/yell ALL night long! Very difficult for any patients to get a good night's sleep for optimum healing. DO NOT allow your loved ones to stay at ENR EVER! This facility is a total failure and could potentially cause your loved ones irreparable harm and severe complications from errors in medication administration or lack of proper treatment by the staff. read more

I recently visited this skilled nursing facility to visit a friend and was very impressed by the COVID protocols in place. I am a nurse and administrator in the industry and am well aware of the challenges we all face. The building is quite old and not fancy but it was clean and there were no foul odors which is more important in my opinion. It was easy to schedule a visit. It was a simple text to an employee by the name of Lisa. I text her on a Saturday and she got right back to me with an appointment for the next day. She was clear about the COVID protocols and answered all my questions. My friend is quite satisfied with the care she has been receiving. She told me the Physical Therapy department has been motivating her to work hard and reach her goals. I asked her about the food and she said it was very good. If you are looking for good quality care, a strong therapy department, good tasting food and a clean facility then I would recommend you check them out. I'd like to add there is no perfect place but it is the effort to execute a good plan that counts and they definitely do that at this facility. read more

This would have been a 4.5 rating if not for the Social Services department. But more on that later. We felt that our loved one was well cared for during their 5 weeks of care. Nursing, therapy, food services, admissions, reception desk all seemed to take pride in their work and showed sincere concern for the patients. Our loved one progressed well in therapy and was adequately prepared for discharge. Food was OK, edible except for the eggs (in our loved one's opinion). I visited at least 4 times per week and the visitation coordinator and reception made check-in and COVID testing as easy as possible. Social Services was incompetent in my opinion, at least the person that we dealt with. Luckily, they don't directly assist in patient care. They did not always return calls or show up for scheduled meetings, delayed processing of important paperwork, failed to advise of appointment changes, failed to have the patient ready to leave for outside doctor appointments and even mis-scheduled the patient's discharge date. Other staff also expressed frustration with Social Services. My suggestion is to double check and follow up on everything, checking with other departments as much as possible. Be prepared to assist your patient with scheduled departures. In conclusion, ENRC did a good job with the stuff that really mattered. read more

My mother was suppose to have a bed in your facility- Encinitas Nursing and Rehabilitation Center. Scripps was ready to transfer her the evening of 9/7/2022 but we were told by Scripps that Encinitas Rehabilitation said it was too late to process her. The following morning 9/8, her nurse told me she she would be transferred soon. I called your facility and got details on what to bring her etc. Soon after that I was informed by Scripps there was no longer a bed available and you would not accept her. I was understandablely concerned. I spoke with Tiffany in Admissions.I explained I was trying to figure out where/ why the placement fell thru. She accused me of being hostile. I was concerned and direct but I was not hostile. I was treated in a dismissive manner by Tiffany. She spoke over me and was not willing to listen. All she had by way of explanation were excuses and blamed the situation entirely on Scripps. It is my thought that there are usually issues on both ends of a problem and a professional would have accepted part of the responsibility. I called back to speak with an administrator to express my concerns. I was told the administrator was not there today. I asked to speak with whoever was in charge. I got Tiffany! In my view, your rehab facility is not being run professionally or with concern for the clients/their families. read more
