The nurses and the nurses aids are all top drawer if you're unlucky enough to end up in a rehab facility it will change your luck if you end up here I highly recommend Palomar health rehab rehabilitation out of five stars. I wouldn't recommend this facility for a patient that requires significant help or has mild cognitive impairment. Post stroke she was wheel chair bound and required assistance for everything. In summary: Dr. B was excellent. Thoughtful, problem solving mindset and treated patient and family with respect, and some humor when appropriate. Rehab Team: Competent and caring. Speech. OT AND PT good therapists. Management: Poor. All patients receive 2 consecutive days with no therapy. That is 2 days isolated in your hospital room with no activities. This is truly detrimental to the healing process. In my experience this exasperated anxiety and feelings of isolation, reversing progress made during the week, disturbing sleep cycles and recovery. It's very important if you are considering this location for a loved one that you reflect on this. Your loved one will be sitting in a hospital bed for two consecutive days with no interaction except a nurse popping in to check on them briefly. Of course family can visit and we did, however it is not a substitute for professional engagement. This workaround that administrators have finagled to ensure patients hit the certain hours required while limiting weekend staffing is absolutely horrible and very transparent in their motives. Nursing Staff: Very inconsistent and overall not good. Didn't appear in alignment with goals of therapy. I didn't feel safe most of the time with the quality of care my mom was receiving. Not all the time, there were a few standouts that were excellent, but overall very lacking. I share this info. so if you're considering this facility you know what to expect. And with hopes that administrators will make changes for future patients. When searching for a rehab facility I admit I was influenced by the the gold star ratings from outside organizations, but after my loved one's experience, I realize this is more a reflection of savvy administrators working to get accolades from governing bodies vs. true gold star care.

Worst experience! I…read morehad an emergency surgery & was too weak to go home from the hospital 1) I was told Dr. Novac was my doctor while at the facility. Dr. Novac visited me once after I was admitted & barked questions at me. A couple of days later, there was confusion on my new prescriptions & I was concerned about my abdominal surgical site. I requested to talk to him again he refused to meet with me in the 11 days I was there. Husband called facility asking to speak to him & was told it wasn't permitted. (Red Flag) 2) Not once but twice, I had to correct 2 different CNA's? Nurses? about my meds being changed. In fact, I was already given meds that I wasn't supposed to take & I brought it up but was told I was mistaken. The young man who dispensed the meds, double checked & later apologized. The 2nd time it was a nurse, she argued with me, went out & grabbed the blister pack of pills to prove she was right & I had to point the name of meds on package & show her she was calling the meds by the wrong name! That was where the confusion was, both meds started with the same 2 letters. Scary! These are supposed to be professionals. 3) In 11 days, I was moved THREE times to 3 different rooms! I'm supposed to be resting to get stronger to go home. In all fairness, I initially requested to be moved to a quieter spot. The door to the rooms are generally left open & I had a light shining in my eyes all night long plus was in a very noisy busy area & they did move me in a day or 2. I was so grateful. This room was more quiet But every time the bathroom door was opened the stench of MOLD was super strong. I noticed there was a hole in the drywall under the sink & towels wrapped around pipes under the sink. One day, I had to let a cna know there was water on the bathroom floor which was mopped. Safety Issue! A few days later!! I was moved yet again. 3) In each room there are 3 beds. 1 bathroom. WARNING! It is not unusual for your roommate to be a sundowner. Be prepared for screaming and disruptive behavior all night. Nurses also come in the room at all hours to change their diaper and of course the lights go on & disrupt any sleep you might get. They are not quiet, either. I can't begin to tell you how anxious & stressed I became. Even with earplugs lights being shined all hours was so disruptive to sleeping. 4) Physical therapist & occupational therapists were very nice. However, I was super weak, had to use a walker, just had abdominal surgery & after a couple of sessions, I was so sore that I had to take tylenol and place ice on my abdomen for a few hours for 2 days. Knowing this, they continued to nudge me to do the exercises until I mentioned it to the head of the department in passing, then the exercises did become lighter. I was told by both therapists at different times I should have told them I was in pain during the session. But I tried to to tell them the discomfort did not happen during the sessions. Knowing how weak I was plus just having abdominal surgery possibly from the beginning warranted a gentler approach. 5) I had noticed that some "problem patients" would use the call button repeatedly for the nurses with stupid questions, etc. They were eventually ignored. But there is something so disturbing about hearing someone yell "NURSE" sometimes for hours. I was so stressed out. I was determined to be a good patient while here. It was clear to me that this facility was extremely understaffed. But ONE day I really needed to speak to a nurse. ONE DAY out of 11 days I pushed the nurse button only to be told the nurse would be right in. An hour later, I pushed the button & was told the nurse just had her break but would be right in. An hour later I tried again and was told yet another excuse. Nurse never came, I gave up and the next morning she came in to see what i wanted the night before without much interest. Very disappointing & frustrating. 6) Not always, but for the most part the food was less than cafeteria food. Some meals were ok. You fill out a meal form when you get there. I tried cold cereal for breakfast & it upset my stomach. I told 2 different people and also filled out another form but continued to get the food I couldn't eat. Sometimes, my roommate got my food by mistake and I got theirs. That's my experience. I hope it helps someone. In my case I was discharged from the hospital unexpectedly & my husband was contacted by a social worker or insurance agent (?) & was told I was going to La Paloma. This all happened within a couple of hours. Knowing what we know now, check with your insurance & see which facilities are covered. And if possible check the reviews. This was our first experience with a nursing facility and I'm not sure if they are all this bad, but it was a horrible experience.

During that time, our family has experienced our share of hospitals and rehabilitation facilities. In our experience, Ocean View Post Acute stands apart. Never before have we seen such a strong partnership between Physical Therapy and Nursing. They make something incredibly difficult look easy. And we know firsthand that it is anything but. Our mom is a vulnerable 88-year-old navigating a difficult journey. The staff are responsive, attentive, and compassionate. They visibly slow down for her when she needs it, speak to her with dignity and respect, and encourage her while continuing to push her toward her goals. They do this even on the busiest days, when it is clear they are working hard to meet the needs of everyone in their care. No group of people is perfect, and neither is Ocean View. But what we see every day is the striving. The teamwork. The professionalism. The humanity. We don't know whether Mom will ultimately recover from her trauma. But what we do know is that Ocean View is giving her the best opportunity to do so. And in the end, that's all any family can ask of the people entrusted with the care of someone they love.

NO ACCOUNTABILITY FOR ANY OF THEIR NEGLIGENCE/MISTAKES!!!…read more First night my mother was transferred over she was woken up by a CNA named Arturo attempting to inject her with insulin, my mother spoke up and told him the she had already been given her nightly dose of insulin at the hospital before she was transferred over, he ended up injecting her with 15 units anyway. My mother called me right away scared and while he was still in the room put me on speaker and told me what had happened. When I attempted to tell Arturo over speaker that he had given her a double dosage he cut me off and wouldn't let me speak, I had to ask him to please let me finish, he didn't, he continued to cut me off. When I finally had the chance to speak and tell him what he had done he proceeded to tell me that I was wrong and that "it was impossible" for that to happen. Also note that because of allergies my mom needs a specific type of insulin which was provided to them but they gave her another type of insulin. STRIKE 1! The following day I was visiting my Mother. I asked to speak to a supervisor regarding the insulin incident, I was introduced to the floor supervisor Luz. As I was telling her about the insulin incident that happened the night before she seemed unconcerned and not bothered. I also provided her with a very important appointment my mother had. I provided the date, the time, the name of location and the address. Because of the type of care and procedure my Mother had she recommended that the facility take care of transportation and told me not to worry about it, they would handle it and make sure she gets there safely and on time. THEY DID NOT PROVIDE TRANSPORTATION AND DIDN'T BOTHER TO TELL US ON TIME. We had to scramble to get my mom there and back. STRIKE 2! My Mother needed to use the restroom and requested help from multiple staff members, every time she asked, each one replied "let me get someone else to help you". Her nurse that day Rose told her to go to the bathroom in her adult diaper instead of helping her get to the restroom. Eventually my mother could not hold her urine anymore and urinated, it overflowed to her sheets and she was left to sit in her urine for over 3 hours! When we brought this to their attention the response I was given by her nurse Rose was that they didn't think that she could put weight on her leg. I told them that it did not excuse them leaving her in her own urine for over 3 hours and they should have contacted the Dr for weight bearing concerns. STRIKE 3! My Sister and I were bombarded with calls asking about her medical history, medication questions, instead of contacting the hospital or her PCP. It went as far as contacting us for a phone number to a pharmacy that could have simply been googled! Every time we spoke to someone they had a different story or excuse. There is absolutely no communication amongst their staff, directors, nurse practitioners, or Dr's. It is complete crap show! I decided enough was enough and called to speak to the facility director Jason. When I asked for him, I was given the run around. I was 1st told that I could leave a message for him with the receptionist, when I told them I wanted to leave a voicemail they told me that he did not have one it would have to be a written message, then I was told to call back in a week, then they told me to call back tomorrow on 9.23.26, and finally got told that Jason would be calling me back tomorrow. After a long day of back and forth phone calls that both my Sister and I had, I went in person and demanded to speak to a higher up "Director of Nursing". She was well aware that I was on my way. When we sat down instead of discussing the issues that I listed above and that they were made aware of, she decided to sit down and discuss power of attorney instructions as well as advance directives. I had to stop her and remind her why I was there in the 1st place. Again they showed that as a whole THEY LACK COMMUNICATION, ACCOUNTABILITY, AND EMPATHY FOR THEIR PATIENTS AND THEIR FAMILIES. THIS PLACE IS HORRIBLE, GETTING MY MOM OUT ASAP!!!

Before I came to this facility, the nurse liaison, Sarah, sold me on this place. What a phenomenal salesman. She knew I was eager to get Good physical therapy. I've been diagnosed as a partial quadriplegic. I've held the deep belief that I could improve if I could get the right physical therapy. She convinced me that would be the case here; I would receive the best physical therapy, with the best facilities, with the best trainers. I'm on my seventh week without physical therapy. Why? The same Sarah is also somehow the one responsible for getting my letter of agreement to the insurance company. For seven weeks she simply hasn't done this. Why would she? I'm not a person after all, I am an insurance payment. If you happen to run into Sarah, she is a great spinner of stories; none of the stories are true. I was getting RNA until they head-trainer Frank, canceled the program. In a phone conversation with him I tried to explain that I thought there was a miscommunication in regards to the cancellation. He repeatedly talked over me. Not letting me get a word in He repeatedly said, "I don't care what you have to say, I canceled the program." Frank's favorite things to do are moping around and watching TV (not training patients). Recently after coming back from PT I had a new roommate. The roommate I recognized from a previous facility. He assaulted numerous employees: kicking, punching, even throwing his own feces. He is not sane. Imagine the shocking horror I experienced when walking into my own room and finding him feet from my bed separated by a small curtain. It took less than an hour before he threatened me telling me he would **** me up. k*ll me. These aren't empty threats. He has further assaulted at least two employees here. A nurse heard this and he was moved to the room next to mine. We now share a bathroom and a door that links our rooms together. So he can now walk into my room and carry out his threats whenever he's so decides. Imagine the fear and anxiety that are my constant companions in this godforsaken facility. This man can walk, i am partial quadriplegic. I am not safe here. It's hot on my side of the facility because they keep the windows open to outside. Why? Because the stench of urine and defecation is so strong and overpowering coming from this patient Room that the windows have to be open to mitigate the smell. He urinates and defecates in his bed. He refuses to be washed or cleaned -and with the threat of assault who would want to clean him anyways. The administration regularly has 5 CNA's on the floor for 74 beds. That is 14.8 patients per CNA. That is budgeting patient-care out of the building. "They got it", says Jay the administrator when made aware of this predicament. No Jay, they don't. Jay is the third administrator in the year-plus I've been here. Patient- care (or a lack there of) is at an all-time low. The collective morale contained within this facility is at an all-time low. Experienced employees are being fired or leaving and being replaced by 22-year-olds with zero experience (why pay an experienced CNA when you can hire a one with no experience for much cheaper). One of these was put on the floor without finishing orientation in a section he had never had. The incompetence and carelessness of this administration is concerning, it's frightening. My quality of life in this facility is one of constant anxiety, sleepless nights, and complete the dehumanization. I can go on and on with the truth about this place. About how, for instance, I was pushed out of bed by a CNA who said I fell out of bed because of my "bad attitude" (does that mean she did it on purpose?). A new administrator who didn't introduce himself (Jay)came in my room and told me he wanted me to "feel safe" (not to be safe). The fact is: I'm not safe. Like a street preacher proselytizing, here I am trying to keep one person out of hell. Will I be further punished and denied physical therapy because I write this. I very well may be. So be it. If you read this, believe me: to lie in a bed here is to forgo your humanity. Don't come here.

Others when you ask for them for something will say they will be back in a minute which turns out to be 30 minutes or more or they forget what you want. Suggest they write down things to remember better. They also need to be in the dining areas and stay when meals are served unless an emergency occurs. The food for the most part is deplorable due to a lack of supervision. Especially weekends. They need constant supervision wherein the company including the board of directors need to invest in another head chef in addition to what they have. They don't pay attention to detail most of the time. I would have only 1 to 2 meals a week that would be good. I would give the employees a language test before hiring. The administrators including admit and discharge areas are outstanding. I would suggest that instead of having the discharge review board on just Wednesday's they do this on all weekdays like the hospitals and other rehabs do. It would help if they told all the patients their disposition and intentions to clear up a lot of misunderstandings as it is the patients rights to know that as long as they are lucid.. The Doctors are mostly hospitalists and they do take the time to discuss their intentions with the patients. I had Dr. Bertani and he did a fantastic job getting me back to health on several occasions. The physical therapists are outstanding. They see all kind of injuries and are well trained to help each situation. I don't believe they need any improvement from what they do. Helpful and caring is their motto. Occupational Therapy also helpful and excellent. I believe once a patient displays they can successfully do things on their own they don't need to go over and over the same things and waist time spend it on patients that need it the most.
