I had major surgery and after a week and a half stay in the hospital I was sent to Encinitas Nursing and Rehab for wound care, physical therapy and dietary. Upon my arrival I was just thrown in a room by a health aid. She was bar far the rudest person I have ever had contact with. This woman did not plug the bed in so it could be adjusted. The tv remote did not have batteries so I could not turn the tv on. There was no call switch. I had to use my cell phone to google and call the front desk for assistance. When the health aid came back she had a salad and pretty much threw it on the table. I had then had enough of her and gave her a piece of my mind. I demanded to see a supervisor who came in and I requested another room because the elderly woman they had placed me with was in agony. The next room I was placed in was a little better a distant view of the ocean. My roomate this time was a elderly sweetie however in the middle of the night she was ringing her bell for assistance and the workers completely ignored her. Again I used my cell phone to get someone in to help her. I had to use my cell phone so many times to get assistance. The staff would forget to bring my pain meds... My dietary needs were not met... therapy was great with the therapist. I was supposed to be in Encinitas Nursing and Rehab for 6 weeks I was there 6 days. I stayed long enough to go to my first follow up appointment with my doctor and I had my ride waiting on me at my appointment to go home. I told my doctor I wasnt returning to Encinitas and I showed him pictures of the filth and I wish I could have bottled the smell from that place...Upon my refusal to return, home health was set up for me at home. I would not suggest this place to anyone unless you have no other choice. If you do place your loved one here you will have to watch this place like a hawk! Alot of the patients here are elderly and cannot speak for themselves. I am young and I could not walk but I could communicate and see the things that this place hides. The workers are used to controlling and yelling at patients. When they seen I still have my God given sense I was neglected as a patient because I was more of a threat to them . I will say it again ONLY USE THIS PLACE IF YOU HAVE NO OTHER OPTIONS and you ARE WILLING TO BABYSIT YOUR LOVED ONE. read more

Our daughter was treated there after a horrendous accident..hit by a car. She spent three weeks there rehabilitating enough so that she would be able to fly home to New York on March 1, 2017 . Jill after I had a full knee revision advised me this place was a 5 star rehab facility. I wanted to go only to get a head start in my recovery using their PT Department. When I arrived, entering the front door a peppermint mist smell engulfed me, used to cover up the smell of urine. Used as an acute nursing facility primarily I stayed for 5 days because I signed the agreement with Jill the sales person for this nursing facility. I was in bad pain from the surgery when I arrived and needing pain management it took more than 14 hours for them to get my medications onboard. Dr Vu, the PT, and the occupational therapist were kind, and deserve recognition for it. The food was horrible like feeding prisoners in jail. You get a shower every 2 days in a community shower. Several staff members were helpful but reeked of cigarette smoke. It was noisy at night so sleep was hit and miss at best. The nursing staff for the most part were kind but the front desk crew left a lot to be desired. I sure hope I never have to be placed in a facility like this later in life. 1 star is my rating because you can't have 0. read more

When you listen to their telephone recording they make laughable claims. They claim to be a five-star nursing home and have an ocean view, the only one in San Diego County. The Ocean view is kept only for people in specific rooms. There is no Ocean view for the other residents. My husband has been there three years, the family/visiting room has not materialized yet. Consequently, you have nowhere to go to be alone with your loved one. This place is cold, and unwelcoming. This is the best of the information I can give you. I have contacted the department of public health three times for assistance. The first time was when my husband walked out (he has dementia) and was found by an employee who works in the laundry walking down Santa Fe. No one knew he was missing. Recently he was taken by ambulance between 4 and 5 AM with a terrific gash on his head. No explanation for it. The nursing home did not know where the ambulance took my husband. I had to make multiple phone calls to get this information. People are afraid to make complaints about abuses to their loved ones at nursing homes. They think the nursing home will treat their loved ones worse. Had it not been for me and my frequent visits, my husband would not have gotten half the bad care he gets now. read more

Having had two experiences at Encinitas Nursing and Rehab Center. I can honestly say I would rather drain my retirement savings to have home care rather than go back there. My first experience, in 2003, I was there about three hours; I left after getting no dinner, no help with bathroom when I called, and finally having medications brought to me that were for the patient in the room prior to me, and actually being yelled out by a non-nursing staff member that they were mine, even though I didn't recognize any of them and he didn't know what they were. The second time I was there was in January 2017. I was put in a bed that literally had about three inches between the bed and the wall on one side, and about three inches between the bed and the curtain on the other. I did not know I had a table with typical hospital toiletries until the day I left, when I noticed it on the other side of the curtain. The above bed light was controlled by a chain that was broken, which I could not reach, and was not repaired. The pictures of the rooms on their website were apparently taken of rooms hidden away somewhere. I was purposely, quite literally repeatedly jammed with an index finger by an LVN on my back incision. Snacks were thrown to me across the room. I had to plead to be taken to the shower on my fourth day there. It was three days before I received physical therapy, the sole reason I was there. I was not provided any information about the facility until I was ready to leave, and received "admission" forms - about 50 pages - at home about two weeks later. I usually had to ask repeatedly for medication. The staff gossiped loudly in the halls during the night; some even had their children with them who were running up and down the halls. Staff who were assigned to me didn't know what I was there for. It was the most unprofessional environment I have ever been in. I returned there the second time ONLY because an outside banner proudly stated they were one of the nation's 100 best such facilities, and they had new management. After an internet search when I was home, I found the facility is still OWNED by Covenant Care. Finally, I started receiving statements and phone calls saying I owned money to them. This was totally incorrect, as I had NO co-payments, which even THEIR admission forms stated. They even threatened to send my account to a collection agency, which I thwarted by sending paperwork copies to the agency before that happened. Almost five months later, I am still receiving bills, even after the insurance company contacted them. The only positive comment I can make about Encinitas Nursing and Rehab Center is that the food was pretty good for a medical facility. read more

I see that my review of last year was relegated to the Not Recommended section. So why bother writing again? My brother died in December. I have great memories of the people who tried hard to care for him, and bitter memories of those who didn't. Encinitas Nursing and Rehab is at the top of my list of those who did NOT care. They lied, neglected him, endangered his life, then dumped him back at the hospital under false pretenses. The guilty parties at this facility are the people running it--there were some good CNA's and RN's who tried to be conscientious and helpful. The DON and facility director were liars and have hearts of stone. read more

Excellent Thanksgiving dinner for residence and family last night. Penny, the Activity Director did a great job bringing families together in a very festive and warm environment. The prime rib dinner was absolutely delicious. Thank you to the chef also to prepare such a delicious meal for everyone. read more

I am currently a patient at Encinitas nursing and rehabilitation center getting rehab. The staff are very caring and attentive and takes care of all of my needs. They are also very nice and I definitely recommend this facility to anyone who has rehab needs. read more

I was so impressed by the hard work that the nursing put into their job, every time I come to visit I see them practically running to assist their patents. As an R.N. (retired) for 50 years I feel humbly qualified to assess my total care and all the extras provided at this facility. My medical treatment and coverage for my issues couldn't have been better even in an acute setting. Dr. Given visited me multiple times a week and spent time discussing my current treatment plan, etc. (a lot longer than anyone, anywhere these days). Attention like this just doesn't exist in medicine anymore and I'm very thankful for it. The physical and occupational therapists got me up and going everyday (even when I didn't think I could) and I was able to walk again. Everyone, down to the nurse's assistants were very serious and well trained. I guess you'd say I highly recommend this facility. read more

If neglect is what you want for your loved one, send them here. There's a reason they can't fill the beds - the place is half empty and half staffed. On the weekends, there is no staff. If your patient can't feed themselves, you had better be there to do it for them. If your patient has any kind of "episode" don't expect the nursing staff to notice. The food is atrocious, and they provide very little in the area of personal grooming. Send your loved one to Aviara. At least you'll know they'll live through it. read more

Over the years I have taken my piano students to perform for the residents at the Encinitas Nursing and Rehab. Center many times. It was recently remodeled and is a nice building, clean, well-decorated, smells nice (not foul like most nursing homes). The staff are friendly and so gentle with the patients. I wouldn't hesitate choosing this facility for my aging parents. read more

My 76 yr old mother was transferred from Scripps hospital to Encinitas nursing and Rehab Center to finish her recovery after a terrible fall on a treadmill at the gym. She spent 10 days there doing Physical therapy and rehab. The whole experience for her and are family was absaloutly wonderful. The staff was very attentive to my mother and caring. We were pleased to know that a lot of the CNA's and nurses had been there , some for more than 15 and 20 yrs. I cant say enough about the Activities Director Penny, that was amazing. She made sure my mom went to social events and even brought things to my moms room for her to read occupy some time while healing. I would greatly recommend this place to anyone who needs care for a family member, short or long term. My mother was a patient at Encinitas Nursing and Rehabilitation and received excellent care. The staff, when asked, communicate that they are happy in their jobs and that is reflected in their care. They treat even the most infirm and elderly with respect. My mother was in a wheel chair and unable to speak yet the staff spoke to her with warmth and were able to successfully communicate. She received PT, OT and speech therapy and was given the freedom to scoot her wheelchair to activities, the dining room and to visit others. The staff displayed an excellent attention to patient hygeine and facility cleanliness. . Though the physical facility is old, the paint and flooring are fresh. I am a physician and have experience with a number of facilities in north county and was impressed with ENR. The director of nursing, Sandra and the administrator should be commended for their leadership as this skilled nursing facility is excellently run. read more
