The lady who brought out our food was the one who finally noticed that we were out of water and brought a pitcher over. She and the server then proceeded to check on us 4 different times, seemingly out of guilt, but never acknowledged what happened. I fully understand that mistakes and breakdown in communication happen, but at least have the decency to acknowledge it and apologize.
