My mother was 89 when she was there, and we're about to celebrate her 91st birthday she had done a job on her femur bone, they helped her learn to walk and strengthen her leg. Thank you so much. Again, it's awful when you're given a sheet of paper and told your insurance covers these facilities pick...     Recently, our 86-year old mother was a patient at this facility for 3-weeks. She had no idea that…read moreshe would be checking into a rehab after having shoulder surgery. To say that she did not want to be there is an understatement. The day we checked our mother in we met with many of the team members who would be in charge of our mother. They were all very nice. The facility itself was nice on the inside and outside. There is one team member that I would especially like to thank. His name is Joshua, and he is the Patient Advocate. He met with my siblings and myself along with our mother, the day she was checked in. During the meeting, he was very attentive to my mother which helped make her feel at ease. He listened to every concern we had for our mother. He took notes and followed through with all of the details and concerns that were discussed. Joshua is amazing at his job! He is professional and has a perfect personality to be an advocate. He is patient, understanding, kind, efficient, and very helpful. One day after a really hard physical therapy appointment, no staff members were able to help my mother be put back in her bed. She was miserable while she waited 15 minutes in a wheelchair. I text messaged Joshua and he was there within 5 minutes. I don't believe this is part of his job description. That is how helpful he is. He sweetly helped her back to her bed (they had to use a hoya to lift her in and out of bed). Next, he helped make sure she was clean and comfortable. This meant a lot to us as we watched him truly care for our mother. This is just one example of his kindness. Joshua was truly a life saver when our mother was there. If we could rate Joshua alone, he would receive 5 stars. He truly was wonderful. The problem we had was that the nurses and aides were understaffed. Our mother was fortunate because we were able to take turns to stay with her. The week I stayed with her, hours would pass with no nurse or aide to check on her. Even when the nurse call light was on, it would take at least an hour for someone to come into her room. One morning she was throwing up for over an hour. The aide was made aware of it. She said she would let the nurse know. No one ever came in that day to address the problem. There were other problems as well. It is our belief, that more nurses and staff need to be hired, if they want their patients to receive better care.

The biggest problem they have is the nursing staff - they were unresponsive and sat around at the desks the majority of the time. My mom was only showered once in two weeks (they say it's twice a week, it isn't.) Because many of the staff is primarily from staffing agencies, they don't know the patients. The nursing staff that is employed there also is moved around from floor to floor and sections so you rarely get consistent nursing - they too don't know the patient. Since my mom couldn't use her hands to feed herself, get water, or even call for help (still waiting for that "pancake button" which never arrived) , she needed all meals and water to be given to her. For two days the nursing staff did not communicate this during handoff (and apparently charts are a thing of the past.) On the first day I arrived to find her sitting with her meals on the tray in front of her so she could stare at them hungrily, so I fed her. The next day I was working and luckily the CNA figured it out after noticing my mom starting at the food for two meals straight - she confirmed that she had not been updated by the previous nursing staff that feeding and water help was required for this patient. Two days later I found blood in my mom's catheter tube and bag - the nurse shook the bag and sent a photo to a provider who apparently diagnosed through a photo via phone that the urine was concentrated. When I insisted on testing, it took one day to find out it was in fact a UTI and an additional 4 days to figure out what type so it went untreated for 5 days! Additional things: No doctor was on site Food was inedible at times (chicken wings, chicken tenders, salad are examples) TV didn't work without an able bodied person to change the input Clock didn't work Room didn't get cleaned for two weeks - never saw a housekeeper. I wiped the tray table and surfaces, and the patient care coordinator had to sweep and mop the floor when I complained about it. Just know that there are better skilled nursing facilities out there, and this is not it! I'm glad I found one for my mom that truly cares about her.

The caregivers really do care about each and every resident. Every resident was well cared for, making sure they were always clean and dry and pain free. The caregivers made sure all residents were brought out to the living room area, if not bed bound, to enjoy activities, if they cared to. These caregivers truly care about THEIR residents. They do activities with the residents, aside from having a weekly activity director come weekly. You can really feel the love in this home! The owners are very attentive to the residents and caregivers. They make sure they visit with each resident. And they always answer any concerns and all texts from family. My biggest issue, NO ONE and I do mean NEVER EVER does the skilled facility actually answer the phone, so there is no way to call and check in on your loved one if they aren't capable of answering/ calling you. Second biggest issue was medications, I was not told to bring the meds from home (I place that error on the hospice) but after he was there they requested the meds, so I drove there after work with the meds. The nurse on only took 2 or 3 stating they received the rest delivered from the pharmacy. I good, right? Wrong. The night shift nurse in day 3 calls me (Bless her) and says, hey did you know we only have some of his meds? I drive over and bring his meds again. When they sent him home, didn't send all the meds home, lol had to drive over to pick up and 2 of the med bottles were lost forever. At this point i want to mention it's a 45 min drive from my home one way, blah. He did have a fall at the facility, while using the restroom alone. No real injuries and they did notify me (The nurse used her personal cell so she did answer when I returned that call)! Facility was clean and the room spacious, but the respite stay was not restful for me.

The daytime nurse (Mary 'Jersey') sat down and talked a long time with me and gave me the courage and determination to try my best to turn a bad situation around. This woman is an inspiration and angel. Very hands on and genuinely cares. She seems to have a great relationship with other patients too. Had to pick a place off a list the hospital gave me before discharge and went by yelp ratings. Never been in a rehab before and was soooo scared. But when i got here, the staff were amazingly kind and professional. The PT and OT staff are knowledgeable, friendly and encouraging. All staff, cleaning lady, nurses, everyone has been more than kind and sweet. The building is clean, no smell, cheerful and full of activities. Was ready to throw in the towel but now am excited to work with this wonderful staff moving forward. Highly recommend from a patient's point of view.

Physical therapy room is spacious & well equipped. PT/OT staff is pretty good too. Only downside overall is communication. Very hard to get ahold of Nurse's or Dr's or PT via phone. Sometimes I've called 3 times waiting for staff to answer. In person is much better thankfully. The social worker, Leslie Robinson is fabulous though & has helped me tremendously with coordination of care. Front desk is also helpful during the day until you get transferred. Admissions coordinator Liv is also fabulous & very friendly. He picked out the best room for my dad & made the transfer from hospital seamless, arranging ambulance transportation & everything. The food is pretty good too with their dining room very pleasant & bright. Staff is there to help patients every step of the way. They even give patients a Choice of food too which is nice. Overall, this place is great. My only gripe is communication. If your loved one needs rehabilitation, rest assured they're in good hands here.

    We spent a great deal of time here in the past few months. My dad was sent here for hospice care…read more He had Alzheimer's. For the most part the staff was amazing. He was frequently called the cutest old man and told he had a great smile from multiple staff. There were wait times for certain treatments, but the staff was busy. The treatment was great, but what impressed me more was how my mom was treated. The job of the staff is to take care of my dad. But they were so incredibly caring towards my mom and family. Big shout out to Alexia. She comforted my mom all the time with words and hugs. When he passed, so many staff went out their way to hug my mom. Not generic "sorry for loss," but with love and tears in their eyes too. They took a horrible time for our family, made it just a little bit easier knowing we could trust them with my precious dad's care.

    If I could give negative stars I would. I'm posting this review for my 70 year old mother who…read morearrived at this facility post-injury and after leaving Banner Thunderbird. I would not recommend this facility. The response time for the call button for anything my mom needed was between 10-25 minutes, IF anyone ever came to help. The facility left a wheelchair in her room and allowed my mom to use it to wheel herself, unassisted, into the bathroom when she needed assistance with a walker to get strength in her leg after a hip surgery. I requested it be removed so my mom would have to use the walker to strengthen her leg and the wheelchair was never got removed. My mother could have been seriously injured. She said she needed to do it herself because no one ever came after she would click the assistance button. This is unacceptable for a "rehabilitation facility" and I use that term loosely. My mother is diabetic and I told 3 different people this information, including the social services dept., her direct nurse and the liaison before we arrived. She was given sugary food the entire time and high carb meals. We had to beg for a shower for her when she arrived and the employee that cared at all to ever help my mom was Susie, an aide. The nurses never smiled and spent more time joking and laughing at the nurses station than they did assisting patients. On any given visit when I arrived, there would be several (more than 5 lights lit up) patients needing assistance. Additionally, they were NEVER quiet at the station, even late into the night. The overall morale from the good employees was obviously very bad due to work load. We were told by one of these good employees that showers for patients are put off until she arrives. Upon leaving, I had to ask several times to get help getting her to the car, when Susie (the best employee there) offered to help. She was awesome. The other staff - AWFUL. In fact, when I left, paperwork with HIPPA info was inadvertently included in my mom's discharge papers. Wow. Do NOT allow your loved ones to be left alone at this facility. One reviewer said an investigation needs to be done on this place and I AGREE 100%.

Since the community was purchased by a corporation, the atmosphere has changed dramatically. Residents have been pressured to either pay a large buy-in fee or face significant rent increases, leaving many feeling like they have little choice. Instead of focusing on the well-being of seniors, the emphasis now seems to be on generating more revenue. Services have also declined. Apartment cleaning, which residents relied upon, has been discontinued (they won't admit to that), while many of the activities that once brought the community together have been replaced with fundraising events. It feels as though residents are constantly being asked to spend more money rather than enjoying the amenities they were promised. Another concerning practice is the frequent serving of wine at community events. While there is nothing inherently wrong with offering alcohol occasionally, making it a regular feature in a retirement community where some residents may have some cognitive concerns, recovering alcoholics, or take medications that interact with alcohol seems irresponsible. The staff has also become increasingly unfriendly. The site manager, in particular, has shown very little empathy or compassion when addressing resident concerns and seems to find joy in being vile. Instead of feeling heard and respected, residents are often made to feel like they are an inconvenience. A retirement community should provide safety, dignity, compassion, and peace of mind. Unfortunately, this community has become more focused on corporate profits than on the people who call it home. I hope management takes resident concerns seriously and remembers that seniors deserve respect, honesty, and quality care--not pressure and indifference.
