I would have rated a five stars, but there was a small concern with discharge planning. The facility social worker volunteered to obtain a referral for someone to declutter my family member's house before she was discharged. She even told me that she would have it done by the next day. Three days later I had not heard from her. I sent her a letter asking if she had that referral for me and what was my aunt discharge date. Another five days went by and I heard from my aunt. She said she was being discharged in three days. I understand that the social worker's father had died. I felt terrible for her. I know that the guidelines state that if someone is unavailable to work, they have to have someone covering their job. When I called to talk about the issue, I expected service recovery. What I got was the disgruntled employee. There were some amazing staff members. My aunt got much better under their care. I really enjoyed the physical therapy staff. They helped my aunt immensely. Overall, this is a fabulous facility. They are going through some difficult times right now because they have just been sold. I can't imagine the turmoil being sold to a private corporation exacts on its staff members. They have a wonderful mental health provider there. If you have any concerns about your loved one. If they may be depressed or feeling anxious, please request a consultation on admission.

I spent 20 days here after a 2 day stay at a local hospital following a total knee replacement. It…read morewas not planned, nor did I partake in choosing where I went. I got excellent care and occupational therapy (OT) and physical therapy (PT) 5 days a week. The staff treated me very well and I learned a tremendous amount in PT & OT. There will always be times where a wait feels a bit long no matter where you are, but you must realize the nurses know how to prioritize the severity of the requests. I feel that if my other knee ever needs replacing, I will certainly plan to stay at Evergreen Windsor the next time to recuperate as well. Thank you Windsor Staff, for the great care! My first OHSU evaluation therapy was yesterday and the therapist said the progress I made from the therapy at Windsor was excellent, and this is after having no formal therapy for 9 days. I have been exercising at home the way Windsor PT and OT taught me, but wouldn't have scored so well at OHSU had it not been for my Windsor stay. Thank you Windsor staff. Nurse said, "OH, we forgot." They did the blood draw and collected urine sample. Then she heard nothing after several days, so she asked about the results. Nurse checked, and said, "the blood & urine samples expired because the lab didn't pick them up." Evidently, NO ONE CALLED the lab. So they had to redo the blood draw and collect another urine sample. Make sure this is the type of care that you want for your loved one BEFORE checking in.

The long-term care industry typically experiences a great deal of turnover, given the nature of the business, but once the upper management stabilized, there was more consistency with regard to staffing and care. We couldn't have been happier. My mom loved living there, she loved her apartment, she loved the staff, and she enjoyed all of the activities that kept her busy. My one suggestion is that the corporate dietitian be more involved in overseeing that the food is prepared and presented well. I witnessed over the past 1 1/2 years multiple resident complaints about the food. So this is an area I feel that could use some work. But overall, it was a great place for my mom, and feel blessed that Bonaventure was her home towards the end of her life.

Yes, two days and they were charged for a full month. The care here is pathetic. My grandmother on my Mother's side worked here when they first opened. I have no memory of it because I wasn't born yet. This last August my Grandma from Dad's side moved in. I stopped in a few times to see her with my doggie's, and my Son's. I thought her apartment was really cute, updated, and clean. When you walk in you smell the food. It smelled pretty good. I was hoping to get a chance to eat a meal with my Grandma. The last time I stopped was mid September. I am glad I stopped. That was the last time I was able to spend with her. My Dad said the meals were good, he went to eat with her, and so did my Aunt, and Uncle. If I lived up there I would have been there a lot. I am just glad she was able to spend her final days some what close to home, and some what familiar.

Our search…read morefor a new assisted living facility took us all over the Salem area with extensive interviews at every site. The Keizer Village was far superior in what they offer. First off, the administrator, Nancy, is excellent, and she is what directs this finely tuned assisted living home. She has communicated to us every step of the way and responded promptly to any additional care or changing need our dad requires. The facility is spotless! The maintenance staff is continually working keeping everything in order. When our dad moved in, they put together furniture and hung artwork. The front desk staff knows each resident and is extremely respectful of all coming and going. They always have the paperwork ready for doctor's appointments. Nurse Joyce comes with years of professional experience and the calmness to listen and help problem solve. Nurse Joyce follows through on what needs to be done. There are many additional nursing supports coming through the doors as in Home Health, OT, ST, PT, and VA. Keizer Village welcomes them and is respectful of their professional skills and responds to the findings and reports. The care staff is very supportive and patient with our dad following through with the written care plan. With limited mobility, he requires additional support in transfers, dressing and bathing. They have always be respectful and compassionate. The med aides have been extremely careful and timely. Meal time is definitely a time the residents interact. I really appreciate the interaction between the residents, and appreciate the many family members that feel welcome to enjoy a meal there. Keizer Village offers many activities for the residents to engage in relationships with others while doing meaningful activities. From singing, movies, flower arrangements and special events, there is always something residents can participate in. We moved our dad from another assisted living resident in the Salem area because lack of follow through on care plans and the failure of the administration to respond to our concerns. So, when I mentioned we researched to find the number one place for dad, I mean number one. Keizer Village is by far the best assisted living facility in the Salem area, and the staff, beginning with the administrator to the part time high school students, are all a team making those golden years the best possible.
