This doesn't even deserve any stars. The nurses wouldn't come for about 30 minutes after she pushed the EMERGENCY BUTTON and this is NOT OK. These people deserve to be fired and treated the same way. You try having a wet diaper for hours. NEVER AGAIN AND DONT TRUST IT FOR YOUR FAMILY!!! This was torture and supposedly for any person with dietary restrictions it is "DISCRIMINATION" to aide to those restrictions. Not good for aftercare or for a home facility. They overdosed the patient with drugs in order to fall asleep and "forget about them". This is not a safe place whatsoever. My grandmothers patient next door to her even pressed her button and it took more than 30 minutes for the both of them. This place is DISGUSTING, DISRESPECTFUL, and INHUMANE. As a business owner myself I am hurt by the fact that anyone could be treated in such ways.

The lady forced herself between me and the patient and proceeded to abscond with him. I informed her that it was my wheelchair that the patient was in and I would like to operate my own wheelchair to which she abruptly said "no" and proceeded to tell me that she needed to do some of her own duties before I could take my chair with me. I informed her that I was merely transport and I was short on time and did not have time to run around the facility with her to, she attempted to proceed anyway. There are further considerations to mention regarding liability in the event of incident. The longer the patient is in my equipment likelihood I could be held liable for any incident. As a courtesy, I try and accommodate them as much as I can, but if I am short on time, they need to stop trying to intimidate me into accommodating their schedule. It would be different if they had requested my services and were paying for them, but they are not my customer.

I got excellent care and occupational therapy (OT) and physical therapy (PT) 5 days a week. The staff treated me very well and I learned a tremendous amount in PT & OT. There will always be times where a wait feels a bit long no matter where you are, but you must realize the nurses know how to prioritize the severity of the requests. I feel that if my other knee ever needs replacing, I will certainly plan to stay at Evergreen Windsor the next time to recuperate as well. Thank you Windsor Staff, for the great care! My first OHSU evaluation therapy was yesterday and the therapist said the progress I made from the therapy at Windsor was excellent, and this is after having no formal therapy for 9 days. I have been exercising at home the way Windsor PT and OT taught me, but wouldn't have scored so well at OHSU had it not been for my Windsor stay. Thank you Windsor staff. My new knee and I will always be grateful!

The long-term care industry typically experiences a great deal of turnover, given the nature of the business, but once the upper management stabilized, there was more consistency with regard to staffing and care. We couldn't have been happier. My mom loved living there, she loved her apartment, she loved the staff, and she enjoyed all of the activities that kept her busy. My one suggestion is that the corporate dietitian be more involved in overseeing that the food is prepared and presented well. I witnessed over the past 1 1/2 years multiple resident complaints about the food. So this is an area I feel that could use some work. But overall, it was a great place for my mom, and feel blessed that Bonaventure was her home towards the end of her life.

The individuals would bring her food sit on the counter come back and pick it up when it had not even been touched. Nobody asked her if she needed help eating. they didn't have all the meds she needed and she was there for two days before she died and they charge the family a full month. Yes, two days and they were charged for a full month. The care here is pathetic. I can remember him driving people to go shopping down town Salem. I loved riding along. Not sure a person could get away with that now. My grandmother on my Mother's side worked here when they first opened. I have no memory of it because I wasn't born yet. This last August my Grandma from Dad's side moved in. I stopped in a few times to see her with my doggie's, and my Son's. I thought her apartment was really cute, updated, and clean. When you walk in you smell the food. It smelled pretty good. I was hoping to get a chance to eat a meal with my Grandma. The last time I stopped was mid September. I am glad I stopped. That was the last time I was able to spend with her. My Dad said the meals were good, he went to eat with her, and so did my Aunt, and Uncle. If I lived up there I would have been there a lot. I am just glad she was able to spend her final days some what close to home, and some what familiar.

Lots of positive change was promised, as of yet we haven't seen an abundance of positive change. I've seen a few promises that have not been brought to fruition. The laundry facilities on our floor have been inoperable for nearly a month and a half. The floor above me is about the same. I understand everyone is flocking to use the few machines on the first floor. With my wheel chair, that's not possible. Laundry staff are absolutely overloaded. Care givers are willing to help, but due to lack of available machines, it has been a horrible situation. Management have been telling residents commercial machines have been ordered. It will be wonderful when they arrive and are installed. At the moment, we are stuck. I purchased more towels and clothing. Also had to buy larger laundry hampers to hold all the soiled clothing until I can wash it all. I had a Very bad experience with vaccinations. Their greeting on the answering machine states they are doing their part in helping to make sure we have access to vaccinations. They WILL NOT provide Covid vaccinations. Actually, after my doctor prescribed a few vaccinations, they made it very clear the nursing staff is not allowed to administer them. Very little contact exists between nursing staff and residents. And now they just informed they will be charging a fee to pay our rent. $50 a month if we do not give them access to our accounts. I have seen errors on my statements in which my account would be horribly withdrawn had they taken what the statement reflected. I am on social security and can't afford $50 a month. So I am forced to give them access to my account. Be wary if looking for a home here.
