They qualified her the next morning and within the same day, we had THREE different team members reaching out to make appointments with us to guide us through the process, coverage, shower/bath assistance, meds, medical equipment, social worker assignee, and our AMAZING AND FABULOUS Hospice Nurse, Chantel. I don't know how we would have managed all that was happening so fast with my mom's decline without the whole team, but again, particularly the love, compassion, care, and time that Chantel gave us. Sadly, my mom passed within 2 weeks of setting up Hospice...and there again...they took over handling everything from calling the funeral home, arranging the transportation of respectfully and carefully taking my mom to her eternal home, picking up all the medical equipment, offering bereavement support for a continuing 13 months to my family, and of course....our beautiful Hospice Nurse was right there for us at the most painful moment in the end. Thank you WVH for ALL that you did for my family. xxxooo

When I call it is not a warm greeting. It doesn't make me feel safe and I just don't think that they have the right people working the counter & the desk answering phones, going on hospice or palliative care is a very stressful situation as is and whenever you're not greeted with kindness and warmth when you call a company, it definitely makes you not want to have them involved with your care at all!

They sent Victoria, who has been an awesome caregiver. Both my parents enjoy having Victoria come weekly, and she does housework so my mother is able to do things she enjoys. She is willing to undertake other tasks, too, and that may be needed in the future. It will be easier to do so since both my parents love Victoria. Going through New Horizons takes away the bookkeeping burden of hiring an independent worker. My parents and I just hope Victoria gets enough benefits from working with the company to keep serving us.

But when our loved one began transitioning Tues/Wed the team didn't seem to recognize it and didn't provide additional nurse visits. Then the aide Friday declined to reposition or clean our loved one and left us without any plan for weekend care. It took 4 calls Saturday morning to get help. The weekend on call nurse was wonderful, but when we called early Monday to let her know our loved one had passed she didn't have access to the funeral home info, so when we called the funeral home 3 hours later they had not heard from anyone from Traditions. Our takeaway is that in the last few days when it was most needed, the communication and support were not what we needed or were led to believe would be provided. Suggest looking elsewhere.

Recently brought a gentleman to this facility And was very surprised by the lack of professionalism…read moredemonstrated by one of their employees. The lady forced herself between me and the patient and proceeded to abscond with him. I informed her that it was my wheelchair that the patient was in and I would like to operate my own wheelchair to which she abruptly said "no" and proceeded to tell me that she needed to do some of her own duties before I could take my chair with me. I informed her that I was merely transport and I was short on time and did not have time to run around the facility with her to, she attempted to proceed anyway. There are further considerations to mention regarding liability in the event of incident. The longer the patient is in my equipment likelihood I could be held liable for any incident. As a courtesy, I try and accommodate them as much as I can, but if I am short on time, they need to stop trying to intimidate me into accommodating their schedule. It would be different if they had requested my services and were paying for them, but they are not my customer.

I got excellent care and occupational therapy (OT) and physical therapy (PT) 5 days a week. The staff treated me very well and I learned a tremendous amount in PT & OT. There will always be times where a wait feels a bit long no matter where you are, but you must realize the nurses know how to prioritize the severity of the requests. I feel that if my other knee ever needs replacing, I will certainly plan to stay at Evergreen Windsor the next time to recuperate as well. Thank you Windsor Staff, for the great care! (Colorful Resident from room 28) 9/23/16 Insurance only covered 20 days of inpatient rehab, so I have 6 more PTs scheduled at OHSU-as outpatient. My first OHSU evaluation therapy was yesterday and the therapist said the progress I made from the therapy at Windsor was excellent, and this is after having no formal therapy for 9 days. I have been exercising at home the way Windsor PT and OT taught me, but wouldn't have scored so well at OHSU had it not been for my Windsor stay. Thank you Windsor staff. My new knee and I will always be grateful!

I was really looking forward to added support as I knew our journey ahead was going to be challenging. Unfortunately, not even Lumina knows what is meant by palliative care. Each time we had a need, I would contact them and they'd say, "oh we don't do that. But call us we are here to help 24/7." Problem was they could not define what it was they were there to help with. They also kept referring to my mom as being in "hospice" and we are not there yet, that's why we signed up for "Palliative" care. Honestly during the month we worked with them trying to figure out what they could do for us- the only thing I figured out was that they were supposed to keep us from using the ER for services. So at this point "palliative " care must mean, "make sure we keep that person out of the hospital/ER" aka cut costs so they get paid. Very disappointed!
