We couldn't have been happier. My mom loved living there, she loved her apartment, she loved the staff, and she enjoyed all of the activities that kept her busy. My one suggestion is that the corporate dietitian be more involved in overseeing that the food is prepared and presented well. I witnessed over the past 1 1/2 years multiple resident complaints about the food. So this is an area I feel that could use some work. But overall, it was a great place for my mom, and feel blessed that Bonaventure was her home towards the end of her life.

First off, the administrator, Nancy, is excellent, and she is what directs this finely tuned assisted living home. She has communicated to us every step of the way and responded promptly to any additional care or changing need our dad requires. The facility is spotless! The maintenance staff is continually working keeping everything in order. When our dad moved in, they put together furniture and hung artwork. The front desk staff knows each resident and is extremely respectful of all coming and going. They always have the paperwork ready for doctor's appointments. Nurse Joyce comes with years of professional experience and the calmness to listen and help problem solve. Nurse Joyce follows through on what needs to be done. There are many additional nursing supports coming through the doors as in Home Health, OT, ST, PT, and VA. Keizer Village welcomes them and is respectful of their professional skills and responds to the findings and reports. The care staff is very supportive and patient with our dad following through with the written care plan. With limited mobility, he requires additional support in transfers, dressing and bathing. They have always be respectful and compassionate. The med aides have been extremely careful and timely. Meal time is definitely a time the residents interact. I really appreciate the interaction between the residents, and appreciate the many family members that feel welcome to enjoy a meal there. Keizer Village offers many activities for the residents to engage in relationships with others while doing meaningful activities. From singing, movies, flower arrangements and special events, there is always something residents can participate in. We moved our dad from another assisted living resident in the Salem area because lack of follow through on care plans and the failure of the administration to respond to our concerns. So, when I mentioned we researched to find the number one place for dad, I mean number one. Keizer Village is by far the best assisted living facility in the Salem area, and the staff, beginning with the administrator to the part time high school students, are all a team making those golden years the best possible.

The facility social worker volunteered to obtain a referral for someone to declutter my family member's house before she was discharged. She even told me that she would have it done by the next day. Three days later I had not heard from her. I sent her a letter asking if she had that referral for me and what was my aunt discharge date. Another five days went by and I heard from my aunt. She said she was being discharged in three days. I understand that the social worker's father had died. I felt terrible for her. I know that the guidelines state that if someone is unavailable to work, they have to have someone covering their job. When I called to talk about the issue, I expected service recovery. What I got was the disgruntled employee. There were some amazing staff members. My aunt got much better under their care. I really enjoyed the physical therapy staff. They helped my aunt immensely. Overall, this is a fabulous facility. They are going through some difficult times right now because they have just been sold. I can't imagine the turmoil being sold to a private corporation exacts on its staff members. They have a wonderful mental health provider there. If you have any concerns about your loved one. If they may be depressed or feeling anxious, please request a consultation on admission.

We visited a number but quickly made a decision for the Woods at Willow Creek following a tour of their facility and after consulting with their staff. My wife's aunt has now been at the Woods for over 6 years and we couldn't be more pleased with the care she has received. I have been so impressed not only with the care but also the staff that when my 95 year old mother needed care in the Sherwood, Oregon area we immediately visited The Springs at Sherwood. My mother has now been at The Springs at Sherwood since December of 2015 and my decision to place my mother there has been validated innumerable times. A secondary blessing to having great care provided for both my wife's aunt and my mother is the interaction with all levels of staffing at both facilities. My MIL lived with us for 4 years before having a stroke. She was placed in a group home but after a second stroke she needed a facility. I called many facilities but when I called GC I spoke with with Mackenzie she was so nice, understanding and compassionate. I told her all the issues with my MIL: dementia, stroke, macular degeneration, two-person toileting, wandering, and potential sundowner that Salem Transitional Care relating she had developed. She visited MIL at Salem Transitional Care twice. Mackenzie called and stated MIL would be a perfect fit for GC. She assured us that it would take her some time to adjust to moving again but that this was a normal process. She asked us to leave a down payment of $500 to reserve a room. We went over verbally on the phone the cost which was $4250 a month including showering, toileting, laundry, assisting to bed and wheelchair, assistance to dining room. She stated that the great thing about GC was that they never charged over $4500 a month. She knew that MIL was in a spend down period to then transition to Oregon Health Plan. A week before the move Mackenzie called said we needed to come down today to sign paperwork to hold MIL's room. I said "I gave you a deposit". She stated corporate wanted the paperwork signed before the end of the month. I presented that day with my 15-year-old daughter and 10-year-old daughter. I reminded Mackenzie that we did not have power of attorney to sign paperwork for MIL. I could write a check since I was on her bank account, but my signature would not be legal on the contract. She said they would have me sign and have MIL resign once she moved in. Mackenzie reviewed the contract with me, left blanks as she need to figure the pro-rated rent for February based on the day she arrived. She spent most meeting reviewing the section of the contract stating if we had an issue to consider mediation over hiring an attorney. Mackenzie also stated that if for any reason they felt MIL wasn't a good match once she moved in, GC would be responsible to find a new facility for her to be transferred to. We would not have deal with the stress of moving MIL again. I gave her a check for the amount she figured and a week later MIL moved in. Within a week of MIL being at GC I received a call from a team member early in the morning stating Mackenzie had been let go and we were behind on rent. I said "she just moved MIL in and we gave you the rent that we were told". The staff member said the charge was for January 2020. I said "she didn't move in until February". She asked me if I had a contract that I could provide them. I told the staff member "No, Mackenzie didn't give me a copy of what I signed because she was going to have MIL resign since she was her own legal guardian and what I signed isn't legal as we are not power of attorney". She stated they couldn't find a copy of the contract. I told her everything Mackenzie told me at our meeting and phone calls prior to moving in. The staff member contacted corporate and after a few phone calls back and forth everything was straightened around. One morning we arrived to GC to visit with mom. The day shift CNA took us aside and informed us that MIL had not been checked on the whole night by night shift and was covered head to toe in her urine in the am. She said she reported it to the new manager but since Mackenzie was no longer there, she wasn't sure what would be done since the management was changing. In the early part of March when COVID was starting to be an issue, I was called into a meeting with Jordyn to review her care plan. I had never heard about a care plan. Jordyn started informing me about how much they had to do for my MIL. Mind you, we were visiting her daily to every other day and were cleaning her appt, helping her with laundry, toileting etc. He informed me that we would need to pay an additional $3507.00 a month on top of $4250.00. I told him we were not going to be paying more as we were told by Mackenzie that $4500.00 was the max out of pocket. I told him he might want to get with corporate and get this figured out. He then told me that MIL hair needed to be cut because they need the residence to look put together and kept. If the credentialing board comes and walks through their facility and sees residences looking unkept they can get in trouble and it lowers their scores. I asked him if he had made her an appt at their hair salon since we were not able to take her from the facility to get her hair cut. I never heard from Jordyn again and then COVID hit and everything shutting down and we were unable to go to the facility. I assumed that he would be getting back to me so we could reach and agreement on the fees but I never heard from him.

The staff is very friendly and accommodating, there's a computer lab and printer for residents and their guests, and there are plenty of senior-friendly activities. There is a ceramics shop complete with two kilns, a woodworking room, a mini-gym complete with a pool, bus tours, a chapel, lobby areas to watch TV, a library on each floor, gardening plots for residents, quilting and sewing. The only drawback is that it is a huge place and is easy to get lost in the first few times. Even now I still get lost from time to time or end up exiting the building into the wrong parking lot. I probably know more about the guest rooms than I do anything else since I don't actually live here. Since I'm from out-of-state, I stay in the guest rooms every time I visit Mimmie. They are just like hotel rooms only better because you don't have to deal with seasonal or weekend rates and it's cheaper than many hotels anyways. Tip: stay in a room with two beds, even if it's just you, because you pay per person not bed, and because the double rooms have a fridge. In other words, free fridge. Free breakfast in the main dining room on the first floor, but I always go up to Mimmie's room and eat with her. There are several levels of residence depending on the level of the senior's need. Mimmie and Poppie used to live on the eighth floor, but after Poppie passed away and Mimmie's health began to decline more rapidly, she moved to Manor Care where she can get monitored more regularly. Back in 2009 when she happened to have a medical emergency while I was visiting, I pulled the emergency cord and the nurse was in their room in a flash. It seems to be a close-knit community: everytime I rent a room or sign into the computer lab and identify myself as Mimmie's guest, everyone already knows who she is. I recommend checking this place out if you're looking for places for your senior citizen.
