Not a small feat to support me all day and night but The Pearl was up to the task. I've made friends with many of the staff and I look forward to returning to do some volunteer work if possible. After all of the surgeries my husband spent a month in the hospital and he was kind of at square one when discharged. When he started at the Pearl he was unable to stand. After his month long stay, he was in good enough shape to slide transfer from a wheelchair to his bed or shower/commode. He was also able to stand for brief periods of time. He progressed enough to join the intensive Rio Rehabilitation Program at Legacy Health. During his stay he received top notch care from Rio staff. They were very caring and met him where he was physically. The therapists pushed him to get where he needed for additional rehabilitation. Thank you Colin and Debra! The nursing staff were caring and as timely as they could be and several nurses were standouts (like Carol and Sherry). The weekends seemed to have less staff so things could take longer. That said, they were upfront about other work they needed to do before being able to help. And when they were helping, they took all the time and care that was needed. The food was very good with several alternatives if the main menu didn't sound good. Bambi was great in helping set up wheelchair transport for appointments, Sondra helped coordinate lots of appointments and helped make Rio happen. Tawnya was amazing at coordinating transition to Rio and backup plans. Penny was the nicest therapy pooch. She lifts everyone's spirits. I highly recommend the Pearl as a skilled nursing facility. Mary and Julie were very accommodating when I asked if we could have a family dinner welcoming my mom back to Portland from Arizona. The room was all set up and when I thought we were running out of food Julie made another dish for us. Also when I had delays with Medicare delivering my moms medical bed Mary allowed us to use one of the beds at the facility. Novi the nurse is very nice and when I came to her with concerns she took care of them. The front desk staff and maintenance staff are great Thank you for taking care of my mom Jeannie Deas

I was immediately surprised at the building architecture and surroundings and upon entering my room, I was greeted by sunlight, clean, fresh, and a great room, kitchenette and bedroom, large roll in shower and bathroom. I was greeted by name and even one of the residents was holding me a place for lunch. The executive staff is very professional but warm and kind. The support staff is equally amenable. I have never had anyone be short with me upon a request. The activities director is amazing in the number of activities she organizes. If you are looking for a place for a family member or yourself, this facility is a nice size. Not too small, not too big. I would definitely return here in the future if necessary for my well being. One of my best friends just retired as an ombudsman for retirement centers. Oswego Place did not know who she was of course and she said to me "the atmosphere here is positive and welcoming."

Following a stroke, my grandmother was transferred from OHSU to West Hills, where she stayed for two months. Last week, when she was facing death, rather than going home, she chose to stay with her incredible team at West Hills. We got to know nurses, CNAs, case workers, therapists and more on a first-name basis. They all were so kind and gave such skilled and compassionate care not just to her body, but also her mind and her spirit -- she discussed her Zen Buddhism with one of her favorite nurses, talked endlessly with her physical therapists and wanted one of her CNAs to move in with her. Throughout a painful process, West Hills was a true partner and comfort to my grandmother and the entire family. We would have care conferences that lasted hours and involved upwards of six people, everyone working hard together to find the best choice. When we were down to the final hours, they brought food and water and reminded us all to eat. So many caregivers throughout the building came to say goodbye to her, came to give us hugs and cry with us. We could not ask for more from them. They came on good recommendation from a family friend, and she was so right. We are so grateful to all they have done for us (and tolerating when we all piled in her room for pizza parties). Thank you, West Hills.

Reviews here are consistent with our experience!! Unprofessional, poor communication never listen…read morenor listen to understand or mindful. Transactional, stories/excuses not accountable nor integrity based with no regard for well being or psychological aspects of the patient. A couple caring, but overall rudeness and bullies that neglect via ignoring, minimum 30 minute to 45 response times would call me crying. No dignity or reverence provided diminishing and falsely profiling patients/family. Despite concerns for overmedication potentially harming cognitive and kidneys, causing weakness/dizziness,, added more including narcan and 3000 mg daily, ordered equipment delivered to room at 9pm after medicated asking 95 yr old with cognitive to sign. Use upholstered chair for storage and closets wadded up clothing saturated in urine 2 weeks because only PT can take them to potty necessitating urinating in bed with long wait times causing bed sores to never heal. Got backache and developed congestive cough while PT on week vacation without substitute felt neglected. Don't address inflammation through basic icing even after provided cryo and voltarin ourselves had to remind, not following surgeons orders, bypass primary care/cariologist specialists, not getting patients out of bed through strength building early on prolonging rehabilitation then blaming patient. Their ratings are from Medicare whom give frequent cutoff notices that add to stress loads of families. Only one shower/shampoo in 2 months and no modesty draping with two men changing mother. HVAC inadequate and rooms uncomfortably hot even for visits, miss important doctors appts, food not heart healthy nutritious, and poor quality causing 25lb wt loss to 124lbs, Belongings, Flowers Food out of view/reach,. one actual "care" meeting in over 2 months stay (used for attempt signing off rights hearing aids later lost one between facility/hospital refused 1/2 cost i/e 150 for 7g aids) or 2nd medicare notice only limited 20min vs. care, poor record keeping and never provided when requested. I could go on, but you get the picture. sigh. Care in this country is a broken bureaucracy.

Farmington's highly skilled and compassionate caregivers and staff provided outstanding care for my…read moremom until the end of her life. She came to live at Farmington after a difficult period. She had been in the hospital and then a rehabilitation center after suffering a hip fracture at a retirement community's memory unit in Portland's West Hills. That community looked like a high end hotel and was considered the best in the Portland area. It was expensive but I assumed it meant my mom would be receiving high quality care and that was extremely important to me. I quickly learned that a beautiful facility and high fees do not translate to good care. The staff were pleasant but did not seem connected to the residents and took care of their needs in a perfunctory manner. They were also unskilled when it came to working with memory impaired individuals. When I called to check in, they seemed to have no idea how my mom was doing and my mom was rarely bathed. This concerned me and so I ended up paying extra for personal caregivers several times a week. It was at this facility that my mom was intentionally pushed over by an 18 year-old unskilled caregiver and suffered a extremely painful and debilitating hip fracture. After rehabilitation, my mom arrived at Farmington Square. She was crying and clinging on to me as this was her third transition within a span of 2 months. I was not able to accompany her inside as Farmington was abiding by strict COVID protocols. The staff there went out of their way to help my mom feel at ease and helped her get to know other residents. They also reassured me. Whenever I called to check in on her, they clearly seemed to know and care about her and provided detailed updates. When I spoke with her on the phone, she seemed happy. Once limited visits were allowed, they were very accommodating. In June of 2021, my mom's health declined and she was placed in hospice. Initially, I was going to bring her to my home, but I realized her needs exceeded my capabilities. I felt like I was letting her down but after spending time at Farmington with her, I quickly realized that she had wonderful relationships with the residents and caregivers and it was home to her. My sisters and I stayed with my mom 24/7 during the last weeks of her life and we were able to observe the consistent high quality care that my mom as well as all the other residents received. These outstanding caregivers were all emotionally mature individuals who were naturally compassionate people. They were attuned to my mom's needs and she was receptive to their caregiving because they were well-trained and had developed a trusting relationship with her. The staff and administrators even took care of me and my sisters. They brought in trays with coffee, ice water, and snacks and sat beside us as we struggled to say goodbye to our mom. Through these experiences I have seen the very worst and the very best of care for a loved one. Farmington exemplifies the very best by focusing their efforts on hiring extraordinary caregivers, which meant that my mom was always clean and comfortable and experienced the joy of meaningful relationships and feeling loved at the end of her life.

While a few individuals on the care team truly made a difference--Damalesh, Shantay, Louie, and a handful of others were kind, attentive, and absolutely wonderful--the overall experience left me shocked and disappointed. The social services director, Sam, is the main reason this facility gets a 1-star review. He was dismissive, condescending, and ultimately responsible for the stressful, manipulative mess that surrounded my attempt to leave--saying that I was "denying care" when in reality I was taking my "care" elsewhere... Had to fight to get basic discharge paperwork (that I asked a half dozen times to get for hours on end), was pressured to sign documents without being given full information, and felt completely stonewalled when I asked for reasonable medical equipment and referrals for in-home rehab. Sam made what should have been a smooth and supportive process incredibly frustrating. As for the food? I'm vegan, and while they made an attempt to accommodate that, I can't live off bread and tofu forever. Toward the end, I could barely eat anything at all--it got that bad I understand care facilities aren't luxury resorts, but respect, communication, and basic follow-through shouldn't be optional. If it weren't for the handful of standout staff members I mentioned earlier, I would've lost my mind here. If you have a choice--keep looking. You deserve better.

What can I say about this place? The fact that my concerns were not met when I felt completely…read moreviolated, disrespected, and shamed while having to take a shower, while a male staff member sat just inches away from where I had to take a shower. I was told that if I didn't take a shower at that moment, I would not be able to take one the next day. There were enough female staff on duty that one of them could have come to the shower instead. But yet, being humiliated. I still have trauma from that night. Only one staff member approached me about what happened. I was not apologized to by Hussain nor by anyone else. I felt absolutely outraged and degraded. Yet, no one seemed to care. The only reason I was here was to rehab from cancer treatment It was much more banal at best. A complete lack of communication between staff members. Often told "oh, I forgot, or no one told me," which is absurd when dealing with people you are there to care for. On more than one occasion, I was brought the wrong tray of food while the staff jokingly said, "oh, it happens all the time", like that's ok. Another issue.. having to put a sign on my door asking people to knock before just walking into my room. It is a common courtesy to knock on someone's door before walking in... it does not matter where you are. Yet, even with the sign, some just walked in without knocking. As far as rehab.. I did better PT in my room. A couple of old bikes and a few weights. It was a waste of my time and dignity. I stayed there two weeks too long. However, that being said.. some of the nicest people you ever wanna meet. All seem pretty stressed most of the time, definitely overworked, probably underpaid. From my personal observation along with asking the R.N's CNA's, LPN's, the rehab staff and others. Each caring for patients with different needs and specifics. The length of time spent that particular resident varies, leaving others under their care basically screwed until they can get to you personally. That's why the one star, because they do try. But ,things definitely need changing as far as respect, patient care, effective communication, the fundamentals and most valuable not treating your patient as you did your last. Each patient are different in their own right, they should be respected as so. As an individual, which we are all and not just another bed filled.
