I sat down and waited and waited until they would finish their conversation but they kept talking. I tried to check in on a computer but it couldn't find my name. I finally got mad and said my appointment was at 2:00 and it's 2:15! The therapist came out and got me. He did a brief exam, gave me two exercises and said he'd see me next week. It took me so long to get in I was no longer experiencing any pain. The woman at the desk apologized and gave me a $5 Starbucks card. I was scheduled to go back but I could never bring myself to go back there. I was really hopeful going in too.

Left me without my pain meds for a week. Still made yet another mistake with one of my meds. I'm so glad to be getting away from the medical side of this clinic. I will say their mental health side is awesome. I love my therapist Mac. She has been simply amazing and has helped me through so much.

After the past six months of attending on and off bi-weekly sessions, I am finally starting to be able to walk again! A simple everyday task that I took for granted in my past that now has taken me over a year to be able to do again! I owe this success in large part to my Physical Therapist Jake! Jake created a specific personalized treatment plan just for me. He keeps our sessions fresh and exciting by always pushing me harder, so I continue to get better and reach my goals. He is extremely knowledgeable and explains why we're doing each exercise and how it relates to my walking goals. He also has a background working with athletes and uses that education to skillfully add-in exercises that are known to help speed up recovery. In addition, he teaches me stretches that help reduce inflammation and decrease my chance of being sore later. Jake starts our sessions out with a targeted massage and the moment I tense up or show pain he backs off that spot, works the area around the spot for a few minutes and then goes back to that painful spot, and it usually no longer hurts. After the massage, we start the workout which varies each time. Powered By Motion is fitted with lots of fancy equipment and I've had the pleasure of getting to play with quite a few! Some of these exciting exercise equipment items include resistance bands, vestibular discs, balance board, foam roller, stretch strap, aerobic step, staircase, stability trainer, weight bars, etc. During our sessions Jake allows me to pick the music - which I love! It makes working out easier when there are good tunes on! During the difficult, but necessary exercises Jake is good at distracting me, so I can keep going. But don't ever forget your count or you're starting over lol! The majority of my appointments have been with Jake; however, I have also worked with Grant a few times. Both therapists are friendly, professional, encouraging, and respectful. Grant also provides an in-depth and enjoyable massage that wakes up your muscles and prepares them for the second half of the session - strengthening. Grant fills the session with several exercises that feel beneficial and geared towards my recovery. In addition, since he's not my main physical therapist he also checks Jake's notes and makes sure to follow the plan he set so we stay on track with my rehab schedule. Stefanie and Miranda are always so friendly and welcoming as you arrive. They quickly check me in and often offer a beverage and snack while I wait. In the lobby area, there is also a Keurig with coffee and coffee condiments, tea, bottled water, crackers, fruit snacks, and more. I have also had the privilege of recently meeting the owner, Roland, at a deliciously catered event at Powered By Motion. He was insightful, kind, and thoughtful. It's nice when even the owner seems to be appreciative and considerate about his customer's well-being and hold his employee's in high regard. Rehab can be hard and painful, but Jake makes our sessions fun and not feel so agonizing like I had originally feared they'd be. Since I've been working with Jake, I've noticed that my balance, flexibility, stability, strength, coordination, and posture have all improved! In addition, I've had increased energy, more confidence, higher self-esteem, and a better attitude. I can finally feel the anger, fear, and depression of my handicap melt away as I become more in control of my body and get back to walking again. All in all, I have enjoyed attending Powered By Motion and I am pleased with my progress as I feel I am on the right track to recovery. I would absolutely recommend Powered By Motion for all your physical therapy needs and more specifically Physical Therapists Jake and Grant!

Heidi Thomas MD is by far the most caring, patient and thorough physician I've ever had. She's been with me through thick and thin and always has an encouraging word or hug for me. Her newest assistant Faith is a sweetheart and a step up from the previous assistants. Yes, the office staff has seen some turnover and has made mistakes, but I see a definite turn around in how things are being handled. My husband and I don't take lightly writing this review but we want to help others be aware of what…read moreyou might face when you move your loved one into a Bonaventure property or any other assisted living facility. My MIL lived with us for 4 years before having a stroke. She was placed in a group home but after a second stroke she needed a facility. I called many facilities but when I called GC I spoke with with Mackenzie she was so nice, understanding and compassionate. I told her all the issues with my MIL: dementia, stroke, macular degeneration, two-person toileting, wandering, and potential sundowner that Salem Transitional Care relating she had developed. She visited MIL at Salem Transitional Care twice. Mackenzie called and stated MIL would be a perfect fit for GC. She assured us that it would take her some time to adjust to moving again but that this was a normal process. She asked us to leave a down payment of $500 to reserve a room. We went over verbally on the phone the cost which was $4250 a month including showering, toileting, laundry, assisting to bed and wheelchair, assistance to dining room. She stated that the great thing about GC was that they never charged over $4500 a month. She knew that MIL was in a spend down period to then transition to Oregon Health Plan. A week before the move Mackenzie called said we needed to come down today to sign paperwork to hold MIL's room. I said "I gave you a deposit". She stated corporate wanted the paperwork signed before the end of the month. I presented that day with my 15-year-old daughter and 10-year-old daughter. I reminded Mackenzie that we did not have power of attorney to sign paperwork for MIL. I could write a check since I was on her bank account, but my signature would not be legal on the contract. She said they would have me sign and have MIL resign once she moved in. Mackenzie reviewed the contract with me, left blanks as she need to figure the pro-rated rent for February based on the day she arrived. She spent most meeting reviewing the section of the contract stating if we had an issue to consider mediation over hiring an attorney. Mackenzie also stated that if for any reason they felt MIL wasn't a good match once she moved in, GC would be responsible to find a new facility for her to be transferred to. We would not have deal with the stress of moving MIL again. I gave her a check for the amount she figured and a week later MIL moved in. Within a week of MIL being at GC I received a call from a team member early in the morning stating Mackenzie had been let go and we were behind on rent. I said "she just moved MIL in and we gave you the rent that we were told". The staff member said the charge was for January 2020. I said "she didn't move in until February". She asked me if I had a contract that I could provide them. I told the staff member "No, Mackenzie didn't give me a copy of what I signed because she was going to have MIL resign since she was her own legal guardian and what I signed isn't legal as we are not power of attorney". She stated they couldn't find a copy of the contract. I told her everything Mackenzie told me at our meeting and phone calls prior to moving in. The staff member contacted corporate and after a few phone calls back and forth everything was straightened around. One morning we arrived to GC to visit with mom. The day shift CNA took us aside and informed us that MIL had not been checked on the whole night by night shift and was covered head to toe in her urine in the am. She said she reported it to the new manager but since Mackenzie was no longer there, she wasn't sure what would be done since the management was changing. In the early part of March when COVID was starting to be an issue, I was called into a meeting with Jordyn to review her care plan. I had never heard about a care plan. Jordyn started informing me about how much they had to do for my MIL. Mind you, we were visiting her daily to every other day and were cleaning her appt, helping her with laundry, toileting etc. He informed me that we would need to pay an additional $3507.00 a month on top of $4250.00. I told him we were not going to be paying more as we were told by Mackenzie that $4500.00 was the max out of pocket. I told him he might want to get with corporate and get this figured out. He then told me that MIL hair needed to be cut because they need the residence to look put together and kept. If the credentialing board comes and walks through their facility and sees residences looking unkept they can get in trouble and it lowers their scores. I asked him if he had made her an appt at their hair salon since we were not able to take her from the facility to get her hair cut. I never heard from Jordyn again and then COVID hit and everything shutting down and we were unable to go to the facility. I assumed that he would be getting back to me so we could reach and agreement on the fees but I never heard from him.
