It felt like a prison, where you're kept in a confined space. My wife had to come and take me out of there covertly. That's why I said the policies and practices suck.

Our family had serious safety concerns during my mother's stay at Rossmoor Post Acute. She was…read moreextremely weak and a known fall risk, yet at one point she was placed in a wheelchair without proper leg support. When staff attempted to move her, our family had to physically hold her legs up because she was unable to do so herself. We eventually had to ask staff to locate the proper leg rests. We also had concerns about fall prevention. My mother had been trying to get out of bed, yet when we asked about an alarm-equipped bed, we were told one would need to be ordered. When we asked what could be done in the meantime, staff had to locate a floor mat. There were also delays in responding to changes in her condition. She was complaining of feeling sick and being in pain, and it took more than 24 hours for a chest X-ray to be obtained. Our family repeatedly advocated for imaging and bloodwork to be made STAT. Her condition ultimately worsened and she had to be transferred back to the hospital. I understand that skilled nursing facilities care for medically complex patients, and a hospital transfer does not by itself mean a facility did something wrong. My concern is specifically about what our family personally witnessed leading up to that transfer: significant weakness, complaints of feeling sick, wheelchair safety concerns, fall-prevention measures that did not appear readily available, delays in diagnostic testing, and how much family advocacy it took to get attention to those concerns. I shared these concerns on another review platform. The facility responded, but the response did not address the specific incidents we described or explain what corrective action was taken. Families entrust facilities with people they love during some of their most vulnerable moments. Based on our experience, I strongly encourage families to be present, ask questions, understand the care plan, and advocate closely whenever something does not seem right. Our experience at Rossmoor Post Acute is why I am giving this facility one star.

My mom was in this facility for a month in spring 2026 after she fell and broke her hip. I was…read morereally nervous at first but the staff was outstanding. Everyone was so kind and caring and responsive to my many questions and concerns. The CNAs were great, really helpful with even the smallest request. Every person on the PT and OT staff is incredible! They all really care about each patient and work hard to help them progress. My heart swells with gratitude to them all, especially Ian who was always smiling and patient and made the sessions fun for my mom even when it really hurt to do the work. Ariel was wonderful as well on the therapy team! We were welcome to be at the facility any time we wanted during visiting hours and often went to my mom's therapy sessions or sat in the dining hall during meals or participated in activities with her, which was a good sign because it shows they have nothing to hide. The staff is very attentive and they treat everyone with respect. Nancy at the front desk was another highlight of every visit. When I toured the facility before my mom was admitted, her smiling welcome at the front calmed my nerves and made me realize it was going to be OK. The liaisons at the facility made my mom's transition home easy. I am so grateful to WC Skilled Nursing and Rehabilitation Center.

I was coming in twice a week for 6 months and not only was my surgeon confused with my progress, he said my hip would have worn itself out within 2 years... the new hip that supposed to last 20. Put yourself in my shoes thinking you're going to live at 60% forever. I'm finally feeling normal. How many sessions would it have taken for you to jump and intervene? 100? $10k later? I came to Moraga post acute care because I broke my pelvis in January 2024. I found that the care…read morewas exceptional. Breaking my pelvis was no joke and very difficult for anybody especially the CNA's who attended me for all my needs and toiletries. There was always someone to help me and not leave me alone during my recovery. I found the food satisfactory and always delivered to our rooms on time. The rooms were cleaned every day by a very very efficient lady. She was also very entertaining. I stayed for six weeks and had three different roommates which the last one was very nice and we seem to have mutual understandings. I would recommend Moraga post acute care. The physical therapy sessions were seven days a week for me and the therapist were very thorough and supportive of me to walk again and have the equipment that I needed to support myself without falling. I have been in two rehabs in Nevada. The first after I had a stroke ending and left side deficits and the other for collapse vertebrae in my lumbar region, and neither of these facilities had daily cleaning Moraga post acute care made me feel very comfortable facility took cleaning very seriously .

The nursing team communicated daily with their counterparts (i.e., patient needs/preferences) when shifts were ending/starting. My CNA's were also wonderful. Employees always had a smile on their face, no matter what their areas of responsibility were, a testament to the positive work environment. Cleanliness was a top priority. Every team member took his/her job seriously and with pride. The positive team dynamics I experienced are a direct reflection of the leadership skills of Judith Gasmin, Director of Nursing. Both she & Herna Mellizo, Assistant Director of Nursing, appeared in my doorway daily to check in and be sure everything was going smoothly for me. I appreciated the diversity of the therapy (PT & OT) teams as well. They tried to accommodate me into their a.m. schedule whenever possible. Shadelands has continued to follow up with me. I stopped by to tour this facility looking for consistent in house care for acute rehab for my…read moremother who was discharged after suffering a stroke. 1. No one acknowledged me outside of patients in the lobby. They looked happy which is why I gave three stars. I'm assuming these were patients waiting to go home. 2. I found a nurses station with a nurse sitting at the desk at the very end of the hallway and boy was she unhappy at work. She was very rude. That was a huge red flag. When families come here they want to see the staff as happy as they can be because for some families this is their loved one's last stop. 3. I received a small tour which was very informative. *they don't have a contract with Kaiser. *they do have a contract with contra costa health plan *OT, PT, SLP don't work on weekends *Rehab is based on insurance coverage from what I was told. They will not give your loved one five days a week rehab although they're paid for everyday your loved one is there. My loved one is there for rehab everyday they're there not to enjoy the scene. I decided to move forward with an acute outpatient rehab and they're doing amazing with much more equipment.

The dining hall isn't quite large enough with all the patients and wheel chairs. I wanted to get him up and an orderly helped right away and placed him in his wheelchair ( no easy task) He's now able to walk the length of the hall with assistance but shuffling along I intend to see him tomorrow. Yay!
