Moraga post acute is taking care of my mother for a few years now. she was referred to this facility from John Muir hospital for recovery a number of times before, and was able to go back home after physical and medical therapy. Unfortunately with time passing, her physical and mental health degraded to a point when she had to stay there for longer time and then permanently. This is not the only recovery facility our greater family came across in the past years, and believe me, we have the compare done... nightmare to safe heaven I will give the Moraga Post acute 5 stars, and many many thanks to staff. my mother is still alive and stable because of care they provide, and she's been through a lot of illness, especially in the beginning, after her hospital stays. The atmosphere is loving and friendly, we get contacted often, any time we visit at random times, I see my mom in good condition and taken care of. Some members of the nursing and physical assistance staff we know for a long time and formed the relationship with over the years, and it helps to feel better, my mom is not just in the care of the system, but persons who care. special thanks to Jit Samra, nursing assistant for her support))) I see a lot of new faces as well, and all great so far. No "bad shift" I was writing this long due review, and came across some negative reviews. Noone is protected from the negative experiences, and huge part of it is the system process flow itself. we had it in hospitals as well but the other part that balances it, is the effort of the people working in facility. And they do all they can here at Moraga. Sometimes they just cant overcome the guidelines, even if it makes sence. For example, when my mom has her "episodes", the best for her is to stay in bed, and to be restricted from getting out to prevent fall. But any kind of restrain of free movement is not allowed, the rails on the bed or the belt on wheel chair. Same was in the hospital, it took couple days and special permission to install safety net over the bed. So they do all they can, like putting the floor mat before the bad, or keep her in wheelchair near the nursing post, and going extra mile doing that. 5.0 star rating 10/9/2018 Hi, my name is Anthony Hill. I wanted to express my appreciation and care I received during my stay at Moraga Post Acute therapy. The entire staff is friendly and they showed they really cared about me getting well. The Nurses and CNA's were fantastic and did everything to make me feel more comfortable in a place that is hard to feel comfortable. They really did well. As far as my Physical Therapy went, it was absolutely the best I've every been to. Unfortunately I had a severe gram mal seizure and that affects the way I can walk sometimes. Some day's I couldn't even walk at all, but with the two scheduled therapy sessions I had each day I progressed to standing up with the walker and was able to safely go use the restroom when I needed too. I had to ask the nurses at first so they could keep an eye out for a fall, but that never happened. Eventually I was able to stand and walk sturdy enough I no longer had to ask during the end of my stay. The Physical Therapy Staff and department deserve all 5 stars is how I rate them. I'm a bit of a grouch so when I say 5 stars I mean it and it's for real. The Dr's were friendly and of course knowledgeable too. The front host and administrator Victoria was always on top on things as Vicroria is the perfect person to greet visitors when entering. She makes a great first impression. Abe the G.M. was extremely nice and approachable. Most G.M's don't hardly even say hello to you or acknowledge your presence , but Abe took time out to speak with me and always had a positive smile on his face.. Great guy. Letecia the activities director was great too. Wonderful interaction with her patients and always tried to make a dull day at the nursing home/ short term rehab facility positive and a little more fun. I can not believe how this place has gone straight down since the arrival of their new administrator. What a fake. He is rarely on-site, is so unqualified (don't believe his story about working at several places in several states, I checked his past and he was an admin at one place in Texas a State which is known for abuse of patients because of the lack of laws to protect them.) Mr. O put up cameras all over the place which also record sound which is against both Federal AND State law and violates HIPPA. He had to take pictures of his staff because he does not know any of their names. He works for a Mormon owned company (Providence) and the only reason he has his job is because he is Mormon too just like almost ALL of their other Admins, regional directors, etc. This man got rid of the employees that made Moraga Post-Acute GREAT and now has the nerve to brag about how great they are? Ask the overworked, under paid employees, see if they agree. The new DON is rude, under-qualified, and also frequently absent from the building too as she leaves right after Abe does. She can't use their computer system so she forces other to do her work while they certainly don't get her pay. Social services is a wreck and a half and always trying to find a way to cheat a patient for services when she isn't walking up the street smoking every five minutes. The only employees left there that are worth their weight in gold are Anna the business office manager, and Emily the unit manager. I had been a fan of this place for so many years, and insisted that this be the place to put my loved one when they needed therapy and pain management, but when "Abe" who loves to hear himself talk, arrived and then I moved my loved one out of that place ASAP! This review is long overdo. In February of this year, my father had a bad fall resulting in multiple fractures. After hip and elbow surgeries resulting in no weight bearing for 6 weeks, we decided to take my father to Moraga Post Acute for his rehab. He was visiting from Portland, OR so our main goal was to find a facility that provided the perfect balance of physical therapy and care so that we could get him home quickly. Moraga Post Acute was just that. My father's physical and occupational therapists, Lauren, Nick & Steve were the reason that he recovered so quickly and so well. They gave him the personal attention that his case needed and worked on gaining back strength within his limitations. They all really worked very hard and diligently to get my father up and mobile. The facility was also great. Beautiful grounds, well maintained, clean and spacious accommodations. Great food selection and a very attentive staff. Everything runs very smoothly under the great management of Lisa. Bringing my father here was the best choice. The attentiveness and quality care that he received at Moraga Post Acute is the reason that he is back to his usual activities after such a serious fall. read more

Moraga Post Acute nursing home is an incredible place, and we feel so lucky to have had my mom under the care of the nurses and other staff members there. They were attentive, friendly, skilled, and always willing and available to answer questions at any time of day. This has been an especially challenging time as we are all now under the shelter-in-place orders due to the COVID-19 pandemic. As a result, we weren't able to visit my mom during the last two weeks of her life. The staff told us that because my mom was on hospice, we could come visit her if we wanted to. After a family discussion, we made the difficult decision not to go as we didn't want to put anyone else at risk, especially considering the age and fragility of the people staying there. Of course, we also didn't want to put any staff members at risk since they are the ones taking care of everyone! I would also like to mention Bridge Hospice since they provided support to our family during this challenging time. What an incredibly dedicated group indeed. We feel blessed to have had the care provided by both Moraga Post Acute and Bridge Hospice, especially during these uncertain and challenging times. read more

My mother was admitted to Moraga Post-Acute following a brief stay at John Muir Hospital. She has been at this center for a few weeks. I have been very impressed with the quality of care from the entire staff from the staff that clean her room daily, the CNAs, the LVN, the nurses, and the therapists. They all have been very attentive, frequently checking in on her. They are patient in explaining what is happening such as the medications that they give her, every new process like taking a shower, activities or things on her schedule for the day. She is hard of hearing (and won't wear her hearing aids) and they all speak louder and slower for her. My mom loves the food. I tried to bring in food from her favorite restaurants and she prefers to eat the food from the center (?!). Special shout out to the rehab therapists - occupational and physical therapists have been stellar. Her own goals for therapy before she leaves are to walk, bath and toilet independently and to be able to do light meal prep. With her determination and expertise from the therapists, I have no doubt that she'll accomplish her goals. Did I mention that she is 96 years young? read more

As short-term skilled nursing facilities go, this one is top-tier. My elderly mom was a patient at this facility a year or two ago when it was under other ownership and management, and frankly we were not impressed. The place was dingy and woefully understaffed and unresponsive. She's just leaving there after a couple of weeks post-hip-surgery, and I can say that it's changed so much for the better under the current management. It's bright and clean, the staff is involved and very caring and attentive to my mom's needs, and knew how to coax her to do her physical therapy even when she didn't feel like it. I want to give a shout-out to all the staff: to Nick and Lauren, the PT and OT specialists, to the nurses, the kind CNAs, the servers and cleaning crew -- all are super. Special thanks to Lisa the Director of Social Services who has helped me coordinate my mom's care and answered tons of questions patiently and with humor. read more

Amazing! The moment I walked in the facility, I couldn't help but notice the cleanliness of this place. It didn't look like those old, gruesome nursing homes that I have visited before either. Instead, Moraga Post Acute has that modern, cozy feel that assures families that we can trust them from the very start. What makes this place unique is their exceptional hospitality and overall ambiance. From the nurses and therapy team to the housekeeping and kitchen staff, the people here are extremely welcoming which is why this place deserves more than 5 stars. The staff members here will always greet you without a fail and will provide a warm sense of family for everyone! For a skilled nursing home, I was surprised that it wasn't loud nor hectic during the numerous times I've came here. My Husband was at Moraga Post Acute for 5 weeks in the late spring of 2023. The nurses and certified nursing assistants were very caring and prompt when he lit the light to call for assistance. Since we live close to the facility, I was able to visit him every day. I was very impressed by the physical and occupational therapists who managed my husbands safe walking exercises up and down the hallways, taught him the proper way to get out of a chair and the safe way to get up out of bed. I was able to meet regularly with the social worker and the case manager, who kept me up to date with my husband's progress and goals. I am particularly indebted to Karen Coane, the facility's administrator, who popped in every day with a friendly greeting and helped my husband get set up with his laptop. My mom spent 3 weeks in a hospital in San Diego following a stroke and then 4 weeks at Moraga Post-Acute. Her improvement while at Moraga Post-Acute was leaps and bounds above what she experienced while she was in the hospital. Overall, the staff at Moraga Post-Acute are kind and caring, which made a lot of difference in my mom's recovery. She was barely able to walk when she arrived and with the help of the physical therapists at Moraga Post-Acute, she was walking 100+ yards with a walker and almost ready to switch to just a cane. Reviews for a place like this are hard as I don't think anyone wants to find themselves needing skilled nursing. However, with the kind and knowledgable staff, the beautiful outdoor spaces and the activities they put on to engage those in their care, Moraga Post-Acute does a great job. read more

I have survived 4 strokes, 2 knee surgeries, and 1 TIA with the help Moraga Post Acute's amazing nursing and rehabilitation team. I am currently in for severe osteoporosis and they are helping me gain back my mobility day by day. Everyone is incredibly friendly and knowledgeable and most importantly, they really care. The facility is nestled up against a hillside with a sunny backyard patio that you can use to relax or spend time with family and visiting friends. The activity center is incredible and offers live music, pet therapy, resident birthday parties, movies, bingo and more which really helps lift everyones spirits. The dietician and kitchen makes sure that I get my gluten free meals and my needs are always addressed in a timely and caring manner. This review is long overdue. In January my 98 year old grandmother needed SNF care after being hospitalized with a flu. Moraga Post Acute is outstanding facility! All staff is very friendly and professional. Great, caring CNAs, nurses, therapists. All of patient's needs, and beyond, were met not only by medical staff, but also by other workers- admin, kitchen, activities. I also have to thank all housekeeping staff, they are doing an outstanding job by keeping patients room and the whole facility clean. The discharge home was smooth and well organized by the Social worker Lisa. Thank you everyone for taking such great care of our mother, grandmother and great grandmother! Our 99 year old Mom has been at Moraga Post Acute for three months now after a fall at home that resulted in a broken femur. She has been very well taken care of by the staff and administration. It is a major transition to go from living independently (with help from her kids) to being bedridden and totally dependent on others for your every need. She is a very private person and in the past was very particular in establishing friendships. I'm happy to say that she now has many friends at Moraga Post Acute! We try and visit her every day and are constantly told by staff how much they enjoy her stories from a long life well lived. I was in a car crash and broke my femur. The rehabilitation team worked tirelessly with me to help me heal and start standing and walking again. Everyone was incredibly friendly and helpful, from Mia and Arlene at the front desk, to all the nurses (especially Michelle) and Steve, Patricia and D on the rehab team. Leticia the Activities Director helped get me out of my room and participating in social activities and Jacky and Lisa helped answer all my discharge questions and concerns.
