I want to express my gratitude for the excellent care at Pleasant Hill Post-Acute Care. I was admitted with a fractured pelvis and could not walk. Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but the following are a few. I'd like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients' well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient's needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfasts always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs. read more

Pleasant Hill Post Acute was an amazing rehabilitation program, and I'm so glad I chose it. I really appreciate everything they did to help me recover and regain my mobility after surgery. I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. I especially want to thank Marth and Arryan--they are so sweet and genuinely care about their patients. The night nurses were excellent as well. I'd also like to thank the CNAs Melissa, Beth, and of course Dalia. They were helpful, kind, and attentive. They took such great care of me and made me feel safe during my time away from home. Physical therapy and occupational therapy were also great throughout my recovery. They helped me from the very beginning to regain my mobility and strength. The therapists Jordan and Justin were fun, funny, and kind throughout my sessions. They made each visit engaging and always took the time to listen to my needs. They consistently found ways to challenge me while keeping the sessions enjoyable, and they helped me grow stronger every day. I truly appreciated their support and encouragement. My case manager, Bianca, was amazing throughout the whole process. She was always available whenever I had questions or needed someone to talk to. We definitely had our moments--lol--but she was truly supportive and helpful every step of the way. I also want to thank Claire, the activity director. She was so lovely and caring, and she truly tried to make every day fun and different for all the patients. She recognized each patient's unique qualities and worked hard to make their time at the facility as enjoyable as possible. My husband spent 10 days at Post Acute and was very impressed with the care he received. The CNA's were very attentive and pleasant. They treated him with dignity. He liked all the physical therapists. I found tfw staff very helpful. He said the food was ok but he was on a soft diet. Highly recommended. read more

This was our third time dealing with PHPA! Back in 2020 during Covid my mother in law ended up there and then again in 2022. Both times she received excellent care and great pt and ot. The staff is very professional and care deeply about their patient care. Recently my 92 year old mother fell at home and after having a partial hip replacement was able to get into PHPA. Since my mother in law had great experiences there prior, we knew we wanted to get my mother in there, which luckily she did! Unfortunately a week later my 95 year old father in-law was not able to get a bed there because facility was too full! Otherwise we would have had three family members with great experiences there! Their staff efficiency is to be recommended! Any time I had questions about anything they responded within a timely manner. They took care of everything! There were a few downsides to this place. Since it's an older building it's a little too dark and not very light. It also did not smell as pleasant as the other facility where my father in-law stayed at. One thing that most places all have in common is the ratio of staff to each patient. It seemed at times they were short staffed and cnas were overwhelmed. After reading the reviews on Yelp, I decided to place my father here for rehabilitation after his 2-week stay at John Muir. Neither my father nor I wanted him to go into a nursing facility, but John Muir insisted he spend at least two weeks regaining the strength he had lost. During the first week, my father was not given his prescribed heart medication. I visited daily and quickly noticed he was agitated and confused--clear signs that something was wrong. When I asked to speak with the doctor, it turned into a big ordeal. Eventually, they began administering his medication, and I saw him return to his normal self. A week and a half into his stay, he was showing no progress in regaining strength. I began calling and emailing the social worker to arrange his discharge, only to be ignored repeatedly. I had to take time off work and physically interrupt a staff "meeting" just to speak to someone about getting my father released. What I saw during visits was alarming. One day, I arrived to find my father hanging off the bed sideways, naked, and covered in blood. When I asked the nurse what had happened, she casually said, "He was like that when I came in." I had to pull him back onto the bed myself, cover him, and request wipes so I could clean him up. Later, when I went to speak with the physical therapists about his progress, my father refused to get back into bed, so they left him in his wheelchair in the hallway with his cell phone. The next day, his phone was gone. It has now been six weeks of me calling for answers about the missing phone, and not a single person has returned my calls. This experience has been heartbreaking, frustrating, and unacceptable. If you love your family member, do not place them here. read more

This facility has an amazing staff. The nurses like Martha and CNAs  - Olivia, Dalia - were always on top of everything and truly attentive to my needs. The therapists - Jordan, Sara, i cannot remember their names but - were highly motivated and consistently encouraged me to stay active and keep moving, which made a big difference in my recovery. Pleasant Hill Post Acute is a great facility that provided our family with support , kindness and care during a difficult time. We are deeply grateful to the Intake Specialist Zymond, the Nursing Director and Mary, the Operations Manager, for the extra time the took to support us. Geraldine and Monique are fantastic. Their warm smiles brightened our daily visits. The nursing team, Lesley and Matthew, along with Kevyn, Julie, and Daria, went above and beyond in showing both care and kindness for my father. Marilyn and the physical therapy staff were phenomenal. Their positivity and motivation helped tremendously and got dad home much sooner than we ever expected. Lastly, we'd really like to thank Nurse Vicki. She was truly exceptional. Nurse Vicki's kindness and dedication made a world of difference for my father. She went above and beyond by creating a routine for him that encouraged him to get up and engage each day, giving him a sense of purpose and positivity. This facility is UNBELIEVABLY POORLY MANAGED. There are no bathrooms in the rooms. Patients cannot get response from call buttons. The receptionist is PART TIME and noone can reach any staff or patient after the part time receptionist leaves in the afternoon. NOBODY answers the phone late afternoon or evenings. This is just scratching the surface. My friend has been a patient for only 3 days and the above are already apparent. If you have read this far - read the negative reviews. I was admitted to help heal for a broken femur and knee revision. I found this place to be very helpful and very gracious. They were there to help me with everything that I needed and then some. You didn't have to chase them down for anything. They're always available to help you with whatever you need. I found them to be very, very personable and just a very family oriented type of facility. I would recommend this place for anyone who is looking for long or short term stay. It has taken me a while to write this as the death of my mother was so disturbing and devastating. My mother was a patient at PHPA for four months, from October 2022 to February 2023. Between me and my three siblings, we were there every single day to make sure she was given the proper medication; that she was eating; that she was clean; and that she was provided her required physical therapy. From the doctor prescribing the wrong medication to the nursing staff forgetting or neglecting to administer it, we had to be her advocate every day. It was a constant battle to get them to get her out of bed to try to walk. She had spine surgery and the physical therapy was why she was there and we couldn't get anyone to provide it AS PRESCRIBED. We understood that the staff was busy and we did our best to be good partners in our mom's care. However, the longer she stayed there, the more she declined until she contracted RSV from one of their staff. Mom died from RSV within 4 days of being rushed to John Muir! And to make matters horrifically worse, as soon as PHPA heard of her passing, they cleaned out her room and left all of her things in a pile in the hallway for us to pick up. We will always feel the guilt of leaving her there. I don't know if things have changed in the last year, but it was our experience that they were severely understaffed and many of the staff were clearly in the wrong profession. I became very familiar with many of the elderly patients who were left alone screaming for water or for help.... and ignored! My mom was one of the lucky ones who had family to advocate for her. Many others were not. PHPA is part of a systemic issue with our elderly care and it is not the place for anyone who requires real medical attention and definitely not the place for our precious elderly. read more

I have been in PHPA twice and I can definitely give them a 5 star rating. They are by far the best rehab center in the area, and that is why I chose them again. The staff is very professional, and they are all so friendly. I recommend them highly. I would particularly like to lift up Olivia (CNA). She is by far the most outstanding employee. She is always cheerful and ready to help in any way. She goes beyond to make sure your needs are met. Everyone loves her, she is like family. Another one I would like to lift up is Josefa (CNA). She is very friendly and always waves when she walks by. She will stop to see if you need anything when your CNA is not available. Jordan is an outstanding PT. I had him when I was here before and he got me walking with a walker when I was told I may never walk. I had him again after I broke my hip. He once again worked with me until I was strong enough to walk again and go home. He is very patient and kind. And finally, I want to lift up Justin, my OT. He would come in every morning to say hello and ask if the work out time was okay. He is very humble and kind. He trained me to reach in the cupboards, use the kitchen safely, get to the bathroom, and many other things I would need at home. He made therapy fun. read more

My husband was at Pleasant Hill Post Acute for 6 weeks recovering from a left side stroke. PHPA was a bridge to getting back to John Muir Acute Rehabilitation. We found the nursing care and physical therapy exceptional. Our son recommended them. After touring several facilities, he thought PHPA would be the most up lifting for his Dad because the staff was so friendly and nice. He was 100% accurate. The CNA's take very good care of their patients. I would mention all of them if I could starting with Olivia & Julie. The same with the nurses. Martha and all her colleagues were very professional and skilled. My understanding is the PT Department is one of the best out of all the local nursing facilities. Candi has put together an amazing team. With a lot of hard work, they helped my husband achieve his goal of transferring to John Muir for intensive rehabilitation. My mother came here to Rehab her Hip from surgery and ended up passing away from multiple serious life threatening infections and Covid! my mother was neglected from care from the facility after experiencing mulitple (2x) exposures to COVID from their staff. The first outbreak, she didn't contract the virus until the 2nd outbreak that took place a month after..this is when she caught the virus. I have seen staff not utilizing proper PPE after constantly telling them . Also, they do not isolate their patients that were exposed. Julie was the director and she was telling me that "my staff are also exposed and we are doing the best we can" with no accountability or sorry for putting the patients there at risk. Another episode is her blood sugar was not tracked appropriately where she was unresponsive and was comatose. They didnt sent her to the hospital yet only gave her apple juice and monitored her as she slowly came back to awareness. We asked she should go to the hospital and was told by the director that sending her to the hospital will risk her to contracting COVID and she is better off staying at their faciity. (yet she eventually caught it at their 2nd outbreak in their own facility) I get medicare pays these facilities and they risk losing money if patients leave. What most concerned me was that she was discharged when she was in need of medical attention right away. She came home Dec 12, 2020 not herself. She was very confused and extremely weak. She had diarrhea and couldnt eat. She slept all day and was in pain. We decided to call 911 after she wasn't responsive and confused the next day Dec 13,2020 and they took her to John Muir Hospital. She was admitted for Septic shock, C-Diff, Kidney Failure, dehydration, and the list goes on. the hospital didnt understand how she was in this condition and the nursing facility didnt send her to the hospital after weeks of diarrhea. My mother was hospitalized for months and her quality of life declined drastically. She ended up passing away April 7, 2020 on hospice care as she was no longer able to thrive. Tiffanie @ Medical records made it very difficult to get my mom's records as we had proper power of attorney and paperwork to recieve them. Alot of excuses via email and they sent the usb in the mail and LOST IT and claim no tracking number. Not realizing this is HIPAA and they basically allowed the public to have my moms records. we got her medical records back and notice her lab work before discharged had high spikes of white blood cell counts = serious infections!!! Not only that, her records were tampered with. Her birthday and social security number was alllll wrong. They claimed it was a mistake. That this may have caused things to be overseen! why would a facility send her home claiming she was fine and end up admitted at the hospital the next day?!?! I recently spent (53) days here, and can honestly say that I miss them. They helped me celebrate my 52nd Birthday, and they spoiled me like a King. I miss my chocolate chip cookies and chocolate ice cream. No matter what, it sucks being away from your own home, but the staff here really was fantastic, and I hope they know how much I truly appreciated each of them. My elderly mother was in this post acute for over a month after her total knee replacement surgery and she had a good experience. All of the nurses, PT, OT, and the staffs are helpful, friendly, and caring. They serve good food and the environment is clean. Would recommend this to whoever need post acute care. read more

This place provided every need that my brother needed. He had physical therapy, occ therapy , medical , and wound care. Everything anyone that would need assistance. They did it at 110%. My husband was in Post Acute for two plus weeks after heart surgery. The staff was professional and clearly dedicated to helping with patient needs. several staff members were more than outstanding in their care, compassion and kindness.(Julie and Olivia!) The food was overall much better than "hospital food" and care was given to what was appropriate for my husband. The facility was a bit care worn in some areas but was very clean and generally inviting. every concern was attended to quickly. management was responsive and helpful. i would definitely use Post Acute again if the situation required post hospital care! read more

It's been a while since I had left a review anywhere, but I just got to say I had been in this facility multiple times before and during pandemic becuase of our families and friends that stayed here. We had loved it here in the past and had left rave reviews since. I had just visited a week ago and I don't know what happened. It has been more chaotic than how it was during pandemic. I can't believe they let go of Marge and her happy welcoming self. The staff that replaced her was just- different. No welcome or anything and just cold and barely in the front. I have not seen the usually staff that would have happy cheerier vibe. I'm leaving this review for the management to know how different this facility is now. I understand pandemic happened and all, but it's sad to come back to a familiar place that's not as happy as it was before. read more

I had a teriffic encounter with their LVN, Kate F., this morning. She was extremely sweet and helpful while offering her assistance with one of their residents and his family member. The facility is still closed to family, and outside visitors and guests, but she facilitated a meeting with the patient from inside the building, while we were outside his closed window. The parking lot is extremely small, with lots of no parking signs. Note: their sign says Oak Park Post-acute, rather than Pleasant hill post-acute. read more

The best home care out there! Stayed here for 3 weeks and had the best time here, staff is amazing and nice.

Nurses are nice but grossly understaffed. Patients are left waiting way too long after hitting the call bottom. When you push the button a light comes on and I watched as most times they would just walk past the room. The Dr on site is incompetent and don't bother trying to reach out for answers or explanations because they won't get back to you. My mom's dressings were left on her leg completely drenched which then caused another wound. Strict orders to keep her legs elevated and when I would arrive they often weren't. I watched as she was left on the commode for over 20 min after pushing the call button. I raised concerns about her behavior that only one other nurse actually seemed concerned about. My mom was there for 2 weeks, ultimately ended up back in the hospital and not once did I meet or speak to the Dr. I was told he was doing a blood test..never got results. He believed she had a stroke which EMT personnel promptly ruled out when they came to transport her. Turned out she had a severe blood infection...which I would assume would have showed up in the supposed blood test they took. So although the facility itself is clean and nice and the nurses overall are kind I would steer clear. read more

This is a update on my past fabulous review for this place. The care staff hv been good but the higher management team, social workers are not ok. The administrator had my mother's bed removed and her belongings were gone through and some things were thrown away when she was at dialysis. There has been no action getting her transport set up for her firstula to be checked because it's not working correctly . They have harassed her since she became long term due to her residence not being finished until Jun (they are not getting paid extra through Medicare and no pt) after 3months, but has medi-cal) She was living at home with me but unable to walk stairs now. A plan was set and accepted but was it? My mom toes turned black within days, Im healing from a surgery and was not present for few weeks. I'm currently trying to get a hold of someone and phone had been ringing over ten mins and still no answer. Is it because she ended up in sudden heart failure last Thurs and they illegally gave her bed away and where are her belongings? No phone call to me or her. Ombudsman is already representing her but has not updated us past few days. It sucks because care staff are really nice and this place is supposed to be one of the better locations . What is becoming of this system?? My mother was walking up stairs last Oct and driving last Summer. Check on your loved ones and stay strong advocating for them! Pleasant Hill Post Acute - Still no answer - Skilled Nursing Near Me - Pleasant Hill, California

My husband was in for rehab at PHPA, after a stroke. All the staff were welcoming and cheerful, and that's where the positives end. The negatives are many. Call lights were on for hours, to the point that he would get himself up and go to the bathroom, which by the way was in the hallway next to his room. The bathroom in his room was out of order the entire 30 days that he was there even though it had been pointed out to the Social Worker on the first day there. There was a noticeable draft from the window that was inadequately covered by a too short blind. We were told to stuff some blankets in the window sill, beds were cramped into a room meant for one, it was crowded with no privacy for either patient. The therapy was what seemed like an afterthought, and intermittent and brief, at best. The meals were such a joke that even the CNAs would laugh as they brought the food in and took off the cover. As a Vegan, my husband has specific requirements for meals which were not adequately met. Most meals consisted of a black bean burger, or a scoop of hummus, or a piece of tofu, accompanied by an occasional salad or vegetable. One meal consisted of a peanut butter and jelly sandwich, some potatoes and rice. Fortunately, our friends and I provided food for him regularly. He rose early every morning as his custom and washed himself and dressed for the day. The floor was not properly cleaned after spills and they often dried leaving a mark which was wiped over several times but not thoroughly cleaned. We were not notified that his insurance had ended coverage on February 29, until March 7, when we were presented with a bill that included his stay to March 11. After discovering that the doctor had signed his release the day before, March 6, we exited the facility. read more

Post-8 pm, I am very uneasy about this facility. My father needed to be changed into his PJ's, put to bed, his colostomy bag changed, and that evening's dinner thrown away but for 45 minutes we waited for anyone to acknowledge his red light and received nothing. I put him to bed myself. Got him out of his clothes myself. My dad was exhausted and his full bag was beginning to hurt but he went to sleep. This is unacceptable to me and this little write up on Yelp is going to be least of their worries if these issues aren't addressed. If you're understaffed, hire more staff or don't accept as many patients. I'm kicking myself for leaving, but it was after visiting hours and my dad insisted I just go because he hate worrying us. I drove to my mother's 20 minutes away and tried calling the facility multiple times. At one point I just sat on the phone and let it ring for 15 minutes and again, no answer. It's been over an hour since I left. My dad could on the floor. Is anyone there caring for him? read more
