I am quite grateful for the good care I received from the nurses and Certified Nursing Assistants (CNAs), and the outdoor grounds are beautifully maintained. However, there were notable areas that need improvement. There were times when the care staff felt stretched thin, leading to slow response times when calling for assistance. Furthermore, communication between the administrative team and family members could be improved. A major concern was the doctors, who visit the facility sporadically; my wife was never able to speak with them. The biggest issue was the performance of the physical therapy team. There was no set schedule whatsoever, and they often rushed through sessions in just five minutes, combining physical and occupational therapy together. I tracked each visit to the gym, and we never reached the expected five sessions per week for either physical or occupational therapy. While it is a nice facility with a caring nursing staff, closely monitoring the therapy department, staffing levels, and doctor communication would make the experience much better.

Canned vegetables, over processed meats and delivered cold to boot. Tell me how can you make scrambled eggs taste bad? They managed. Their physical therapist are the only bright and shinning area in this place. Kudos to them. Nursing staff is rude, inconsiderate, lack an ounce of compassion, care or concern. He was frequently called the cutest old man and told he had a great smile from multiple staff. There were wait times for certain treatments, but the staff was busy. The treatment was great, but what impressed me more was how my mom was treated. The job of the staff is to take care of my dad. But they were so incredibly caring towards my mom and family. Big shout out to Alexia. She comforted my mom all the time with words and hugs. When he passed, so many staff went out their way to hug my mom. Not generic "sorry for loss," but with love and tears in their eyes too. They took a horrible time for our family, made it just a little bit easier knowing we could trust them with my precious dad's care.

My mom was admitted here without my knowledge, by Banner Desert Medical (she broke her femur and had surgery) She gets here around 6 pm on Christmas Eve. Receptionist was not friendly first of all and yeah, definitely felt like a post acute facility. They did bring her a dinner tray that night so that was good. (Food is bland but I see why it is which is fine. It wasn't bad honestly) Her Care: Not too good , especially because she has dementia/Alzheimers. This is not the place for ppl with dementia. They didn't seem to understand that she is non verbal and cannot express her concerns , so that was a huge problem. We visited all the time and I stayed until at least 2 am daily because they didn't do much for her as far as helping her with physical therapy or anything for that matter. My daughter also noticed the extension cord that was secured to the wall, to connect the tv , had two live prongs barely sticking out from one of the sockets. If someone would have touched both of those at the same time , they would have been electrocuted. Not okay. Long story short, I pulled her out of there like five days later because I just couldn't keep her there any longer. She was getting delirious and it didn't phase this place. Couldn't even talk to an actual doctor either. Even the one nurse said she had never seen the doctor herself. That afternoon when I told them I was removing her , my mom's roommates family pulled their loved one out of there too. She said they were no good. And she was right , they weren't any good, at least from my experience. And definitely not for someone with dementia/Alzheimers. I hope this helps someone and if they do have to stay here , always visit everyday to make sure they are being taken care of. =^^=

Response time from the call button to use the bathroom was at least a 30min wait. I timed it!! The hallways smelled horrible. Very rarely do you receive eye contact from anyone sitting at nurse station and if you do and ask for assistance they seem very put out like their job is a chore. My mother claims an orderly climbed in bed with her roommate! He said he was her husband?? I will never suggest this place to anyone! I got my mother the hell out of that place as quickly as I could. If there was a negative star sign me up!

My Mother is currently a patient at this facility, I chose it because it appears to be a very well maintained facility, new looking, clean, private rooms, physical therapy room, activities etc...unfortunately they are dangerously understaffed where it counts the most! My mother is recovering from West Nile Virus and was moved here to receive rehab. Every single time I visit we have to hunt down a CNA to get her to the bathroom, she is 75 years old holding it is not an option, when a CNA finally appears they have put her in the chair and take her to use the toilet, this is what should always happen but on other occasions the CNA doesn't even attempt to take her to the toilet, she lays her in the bed to pee in her pull ups then cleans and changes her, this is absolutely humiliating for her, she knows she's capable to use the toilet with help, the CNA that finally shows up is either incapable or too lazy to do it right! Today we arrived to find her feeding tube dangling from the pole disconnected from her peg and the liquid food all over the floor, there were foot prints from someone that had walked thru the mess and dirty towels on her bathroom counter from someone that made a half assed attempt to clean the mess up but didn't finish the job, the dirty towels were almost dry so they had been there for a long time. A physical therapist came and helped me get mom to the toilet, we couldn't find any right sized pull-ups to put on her after she used the toilet we could only find Junior size, the therapist told me to go buy her some and be sure to write her name all over them so no one else takes them! We got house keeping to come clean the floor and remove the soiled towels. The house keeper that showed up dumped a liquid cleanser on the mess that had been there long enough to dry and ran a mop over it and started to leave with the mess still on the floor, I said "this isn't clean you're not done are you?!" She said "it won't come all the way clean!" I literally took the mop from her and cleaned the floor myself while she stood there! I got the floor totally clean, I told the housekeeper it will come clean if you actually make an effort and take ANY PRIDE in what you do! I've voiced my complaint to the charge nurse and director about my mother having to pee in her pants earlier this week, they both assured me that is not the norm and they would make sure the CNA's get her to the toilet. I am extremely disappointed! While there are some staff there that seem to be great there just aren't enough and there's absolutely no excuse for housekeeping to leave her room in the state we found it! Shame shame shame! My husband was discharged here because of the medication he was on and the type of care he still needed, the moment I got to this place and I saw his room I knew this place was not going to take care of him, he had been suffering from the worst case of SE that Banner University had ever seen, the 1st 24 hours they didn't give him any of this seizure medication because Banner didn't send him with any. I couldn't believe what I hearing, he had just survived from Status Epilepticus and they had not given him ANY seizure medication because of Banner?? They were sitting there waiting....they were lucky my husband didn't go back into status because of that. Then after a week a gets a UTI, and then he gets C-diff, then after another week he falls and they find him face down on the floor, my husband is 40yrs old and could barely get himself to sit up let alone fall off the bed, no one could tell me how, when, only that he fell. Another week goes by he gets another UTI, I kept telling the nurses and the people there that his urine looked so dark and bloody and all I would hear is that its normal after changing his foley. Then a few days later he falls again, this time on a mat they left on the side of his bed. He then is sent to Banner University where they admitted him back because of another UTI but this time he ended up getting sepsis. My husband did not deserve this treatment, I called the nurse that day on Aug 11th to see when they changed his catheter/foley, the nurse looks it up and tells me that they have no notes as to this ever being done!! Now my husband is fighting another bout of C-Dif because of the antibiotics he had to be on for the sepsis he had gotten from this damn place. Now he cannot be discharged after going through so much already, this is the type of care this place offered my husband. I'm so angry, the state covered my husband but failed to place him a facility that would actually be able to take care of him, they couldn't even take care of the RNS devices that they were instructed to interrogate daily, I had several nurses tell me that seizures were out of their area of expertise!!! WHY WAS MY HUSBAND SENT TO THIS DAMN PLACE! Please do not send your loved ones here, not to mention when he was being cleaned they would leave his ointment that they used when cleaning his bottom next to his trachea cleaning area, the risk if cross contamination was everywhere, these little cups of used ointment just left by the window and all around his room, this is the stuff they spread all over his rash area when they would change him! Disgusting and so terrible! I hate to say such awful things about a place like this because this doesn't reflect the few people that we friended and found peace knowing that they were around taking care of my husband when the majority were not, this place has some golden gems that make the daunting experience less daunting, you few are too good for a place like this, I appreciated those who I knew actually loved their job and wanted my husband to get better (RT) you guys know who you are, I thank you for your kindness through all of this. But please do your research, do not send your loved ones to this place....Now my husband is fighting these infections again because of the lack of care these nurses gave him. I Beg to differ. Things are not always what they seem. The place is inviting, the atmosphere is calm, and people seem like they really enjoy what they do. I was skeptical of what I was seeing. My sister required skilled nursing services several times in the last few years and her experiences with other facilities in the Valley were not pleasant. She was sent to facilities in West Phoenix, Scottsdale and South Phoenix that did not provide the attentive care that she received at Suncrest Healthcare Center. I know this because I received many calls from her with concerns about her care in other places. When she was at Suncrest Health Care Center she received Iv antibiotic therapy in addition to Physical therapy. The staff at Suncrest Healthcare Center were attentive and engaging. They are very knowledgeable and professional. They showed that they genuinely care about their Residents. The Residents and the staff really care about each other. They are like one big family. This is unusual in these days and time. Every day may be different but the care that my sister received was consistent. My sister, I, and my family were pleased with the care that she received at Suncrest. I want to give a shout out to all the Staff members at Suncrest Healthcare Center you made a difference for her, and I saw you make a difference for other Residents, you are appreciated! Today's visit from CNA Cordny and supervisor Leslie went very poorly…read more First, they told me that I could not double-diaper a patient and said it was illegal. It may be against their agency's policy, but it is not illegal for our company. I offered to call my supervisor so I could put the diaper on myself. Leslie made a call to confirm, and she said she spoke with her administrator, Kathy, who stated that I could only do it after they finished their visit. So they will allow it, but not while they are present. It seems they are unwilling to cooperate with what I believe is in the resident's best interest. Both Cordny and Leslie said they would lose their license, which I found questionable. They were also very strict and not generous with the supplies provided, with no allowance for extra needs. This was only one department. I would like to challenge Kathy and Leslie on why I cannot provide what I believe is better care for my resident. I feel they may be putting on a show rather than focusing on the resident's needs. I had also asked to bring the supplies I needed. Cordny said she passed the message to Leslie, but I still did not receive the correct supplies. As a result, I was left to work without proper supplies. After the CNA and Leslie left, they contacted the resident's family and told them I was unprofessional and had a hot temper. I did not feel that I lost my temper; I believe they escalated the situation by reporting it to the family. I addressed it directly and explained my side.

She was sure that this was the end of "life as she knew it", and wanted us to start looking into an assisted living facility. She was quite depressed due to her weakness and inability to get out of the house. As this was her first time requiring in home care, she was a bit leery of letting "strangers" in her home as she has always been so independent. After her first visit, she immediately called us and reported how kind and wonderful the nurse was. What a relief! He was very thorough, took time explaining her new medications, and assured her that she would be back to playing Bingo soon. She had hope, and was overjoyed! The therapist came a few days later, and she was again... impressed on how gentle he was, while still giving her a workout! Sure enough, after a few weeks of coordinated care, she was feeling confident enough to get out and about, and was back to playing with her bingo buddies. We will definitely use Palmeira in the future if needed. It's not every day that you find a cohesive care team that puts the patient's needs over the insurance company's desires.

While the initial intake call was reassuring--I was promised that a single dedicated nurse (Ashley) would handle both an introductory visit and a crucial upcoming neurology appointment to ensure continuity--everything following that call was complete operational chaos. Dangerous Lack of Clinical Preparation: The primary reason for the neurology visit was to manage a safe medication titration schedule and handle lab work for my father's epilepsy. Despite emailing a medical summary in advance, speaking to Allie on the phone for the better part of an hour, and texting Chi two days before the appointment, the neurologist's office emailed me the following day without the faintest idea about the reason for the appointment. According to the neurologist, the person sent by Navi Nurses brought zero medical context and simply told the doctor that she had only met my father 30 minutes prior and knew nothing about him. Broken Continuity & Lack of Planning and Communication: Despite explaining my time difference and the critical need for consistency for a patient with severe vision loss and dementia, Navi Nurses swapped the assigned staff at the last minute. The replacement texted me minutes before the visit at 10:00 PM my time. After I had already prepared my father for one specific person, this sudden swap added unnecessary confusion and distress for him. Extortionate Cost for Zero Value: In total, this single appointment cost me $760--a $250 initial setup fee, $350 for the nurse to accompany my father to an appointment just 10 minutes away, and $160 for the transport vehicle. Paying nearly $800 for what amounted to an unprepared transit chaperone rather than professional care navigation is completely indefensible. Administrative & Billing Disarray: Basic onboarding failed due to date-of-birth errors on their platform. Worse, after paying the $250 fee, my saved credit card was recharged $350 without explicit consent. Despite paying the remaining balance in full on July 23rd, billing emailed me again two weeks later asking for another $350 payment. Navi Nurses charges premium prices while delivering disjointed communication, zero clinical preparation to physicians, and sloppy financial oversight. If you need dependable, prepared, and accountable care navigation for a vulnerable loved one--especially from a distance--do not trust this company. I will not be using them again.
