I'm a seriously ill patient with severe, progressive, intractable pain due to advanced chronic pancreatitis and related complications. I reached out to Sage Hospice, Primary & Palliative Care hoping to receive the comfort-focused, home-based support they publicly claim to provide. Instead, I was met with deceit, slander from the medical director, and a cruel emotional bait-and-switch -- not once, but twice. My first interaction with the Sage location in Tucson involved a care specialist who seemed to express genuine compassion. When I explained that I live in Sells, AZ -- a rural tribal area -- he assured me we could work around the geographic issue if I had a Tucson address for appointments. I provided one, a relative's address, and he acted as though I was a good candidate for palliative care. But behind the scenes, their medical director -- who had never evaluated me, never spoken to me, and had not even reviewed my case thoroughly -- labeled me a "drug seeker," claimed I was "seeking for high doses of opiates," and refused to provide care. This reckless accusation came without a single clinical conversation. I have extensive medical documentation confirming the severity of my condition. I was seeking palliative care, not addiction services. Furthermore, I have NO history of addiction, it does NOT run in my family, and I've never abused pain meds. I was and am seeking RELIEF for my uncontrolled severe pain, pain that has utterly debilitated me and destroyed my life. But instead of Sage helping me, I was cast aside and stigmatized. That alone would have been enough to earn this review. But then, several weeks later, Sage reached out to me again, asking if I still wanted services. Desperate for help, I said yes -- because I'm suffering every day with no relief and no one else will help me. They gave me hope again. Only to betray me again. After this second round of false hope, I was finally told the truth: Sage "refers out" for pain management -- meaning they don't actually offer palliative care at all. For crying out loud, the verb "palliate" means TO RELIEVE!! Pain management is the cornerstone of palliative care. I used to be a hospice nurse in the past, so I am very well-versed on palliative care, both end-of-life palliative care (hospice) and palliative care for a serious, life-altering disease (which is what I need badly). To claim to offer palliative care but refuse to actually treat pain is misleading at best and fraud at worst. Refer out to WHERE? In southern Arizona, pain clinics are almost exclusively interventional-only, which means they reject patients with complex, high-level chronic pain unless they agree to invasive procedures or stop taking all pain medication. I am far too sick to endure their hoops and unethical demands. What Sage is really saying is: "You're on your own." Sage Hospice, Primary & Palliative Care falsely advertises compassionate care and symptom relief, but they do not treat severe chronic pain and they will stigmatize you behind your back. They will waste your time, raise your hopes, and disappear when you need them most. If you're suffering and seeking real palliative support, avoid this company. Their actions left me more isolated, demoralized, and abandoned than ever -- and I am still in excruciating pain. read more

Beware of using this Hospice for your loved ones final days! I DO NOT RECOMMEND using this company. It was a series of total frustrations from the lack of supplies that were supposed to be delivered -and it took over 2 days, to uncompassionate care by the Admitting Nurse, no "Welcome to Hospice Care Book/ binder- that explains what to expect when your loved one is dying," meds that took over 4 hours to be started and a lack of communication with these medications. We were told to call the 24 hr phone number for everything, yet no pamphlets, written materials or business cards were given to us. We wrote down the 24hr phone on a piece of scratch paper. They continued to drop the ball with everything. After 24 hours with no incontinence supplies, we called the 24 hour line and was told they can't tell us when the supplies would be delivered (because it was up to Mckesson), or when meds would be delivered. The admit Nurse failed to leave the Book about what to expect when your Loved one is dying (she said she was under the impression we received the digitized 95 page book via email) I'm not sure why she thought this since no one in our family was asked, and to be honest, what grieving family wants to refer to an email that contains a book with 95 pages. As for supplies, when the RN did the initial hospice admit, she left our incontinent 95 yr old mother, with one pull-on adult Depend, 5 mouth swabs and 3 incontinence pads. (Apparently this was supposed to last us for the next 2 days) until the box of ordered supplies was delivered via Fedex or UPS. When the Nurses Aide showed up the next morning to give a bath, (she didn't arrive with any supplies either) I asked how are you going to give a bath when we don't have supplies. It was at this point we called the 24hr line and asked to speak with a Director as we are going to "lodge a formal complaint" It took another 2 hours for a Director to call back to address our issues. She did say she was "heartsick" about how the ball was dropped on our Hospice experience. We called Sage Hospice the "Unhospice." I wished I had read the Google reviews before we chose them, as I now see a pattern of the same frustrations we experienced, after the fact. They obviously need to revamp their policies and processes. Not the Hospice experience we had envisioned. Unfortunately, you don't get a do-over when it comes to to the death of your Mother. read more

I am new to this whole hospice care thing, since I am now dealing with an elderly mom who has all sorts of medical needs, including COPD, dementia, emphezema, and possibly cancer, so this has been a very daunting experience. Sage was recommended by the acute care facility she was recently at, and Sage Hospice stepped right in and made the process of dealing with everything such a breeze. They are a fraud, they have NOs who show up to appointments with out a pencil never mind bp cup or laptop. I later asked about my recent BPs in my chart, he had made them up, all made up vitals, all not even close to my norm so they look unusual. Staff disappeared for 2 days no response to the office, as if he had 2 jobs, then when he responded promised to make my care difficult. Which he has done, he slandered me to other staff, who all were taken in by his sociopathic ways. The last visit by V was so uncomfortable from minute she walked through door, she once again spent 10 minutes reviewing ever med, which is her main gig, forget it if you r sick, recommend to go to ER and their never following up. Beware if your family has dementia, I can only imagine their lack of assessment and then CARE that goes w the assessment is missing. read more

Sage Home Healthcare is very unprofessional. The nurse who sees my mom, Valerie, spends as little time as possible with her, shows up for visits at inappropriate times, today she showed up at 7am. Valerie's reporting back to family members is nonexistent and info we do get makes no sense. Sage ordered a motorized scooter for my mom who neither needs or want one and never checked with family members before placing order.I can only imagine what Sage is billing Medicare for my moms nonexistent care. To date no one at Sage has addressed family concerns with my moms care. No one at Sage cares about anything but what they can milk Medicare for. This company appears to be a Medicare billing machine with little or no actual care provided to its patients. read more

There Home health aid are unreliable and not providing consistent care. In the short one month with Sage, the Aid has failed to show. In the 2 situations, the replacement were not much better When requesting suppliers they have ordered either the wrong size or sent items not needed. They answered all my questions and addressed whatever concerned I had. Someone was always available by phone. This hospice agency failed to render appropriate comfort measures in a timely manner to my dear friend. We pleaded with this company for over eight hours and multiple phone calls to expedite her pain/comfort medications to her home. Unfortunately by the time one of her meds arrived, she passed only 10 minutes later. This behavior is nothing short of gross negligence. Be advised. read more

Words can't begin to express how wonderful Sage Hospice was during Tom's illness. From the first visit with their coordinator to all the nurses, especially Stephanie and our admission nurse and all call nurse and the nurse who came at the end -chaplain Fred, social worker and aide Hans. They were always very attentive to every detail- making sure Tom and I were both having every need met. The medical equipment people were always very quick with delivery and making sure I knew how to run the equipment. Their pharmacy also was very fast with great fast home delivery. It's rare to find a company that offers such excellent customer service especially in the medical field. I have end stage copd and lung cancer. I have been to very many medical facilities and these guys are the most helpful. They are reliable, courteous and a person always answers the phone. The people who work here are very cordial. My Dad was attended by Sage hospice care for about two months before passing in 6/2020. I can honestly say his care, the professionalism and the unbelievable way I was treated was beyond any service I have ever had. I am able to compare because my mom passed three years earlier and she had hospice care too by another company. First and foremost his nurse was the best. She kept me in the loop, called me with every detail. Asked my opinion and I was included in every decision. When he passed, within 30 minutes the regular nurse called me to say how sorry she was for my loss. She was the fantastic. There was an on call nurse when he died who was also great. The Chaplin conducted a prayer outside my dads window (due to Covid). I was treated with dignity and respect at such a horrible time. So... here it is a year later. In the morning of my Dads anniversary of his passing, first thing I received a text from the Chaplin giving me a kind word on this day. I was so touched. Then later I received a really nice letter in the mail from Sage. This somber day into a beautiful one. Thank you Sage, what an incredible service at the worse possible time in ones life read more

This is the worst organization ever!! My mom is 90 years old she has been hospitalized several times she is growing weaker by the day they promised us care then denied her we have been told lies by a man named Zach that works for them he actually came to the hospital and told us my mom would be taken care of they even set an appointment came to her house and after coming said she had no medical coverage with them through her health plan misinformed people who really don't care please don't send your elderly parent to them for care this would be possibly a deadly mistake if they can't take care of the paperwork upfront and make mistakes continually could you imagine what it would be like putting your sick elderly loved ones in their hands. read more

Comedy of Errors - Sage was not able to provide much needed services for my family member. They provided more stress and anxiety than anything else. Refuse to provide high quality equipment but rather received low quality rusted equipment and equipment which did not work or was old. We had to consistently ask them for supplies over and over again. The staff were not able to follow simple directions and caused anxiety and stress on the entire family. Their entire staff didn't seem at all concerned for the well being of my family. It was a week of hideous care. We fired them after 6 days. The owner is a doll but her staff needs much improvement. Please be weary, as we do not desire for another family to go through what we went through with them. They made promises and refused to keep them. read more

We weren't really prepared for what was coming, but they walked us through it, answered our every call, came whenever we needed them. I will start by saying this: Abish came to my home and gave my wife and I one hell of a sales pitch! My wife suffers from late stage ALS. Abish and I spoke, and a day or two later, he came to my home, met my wife, he sat at her bedside espousing about all of the things Sage, and he was going to do for my wife and our family. He held her hand, he cried with her, he laughed with her. By the end of the visit, we were confident we were heading in the right direction to find her some comfort and help in this time of suffering. I received a call a short time later from him saying that there was a problem; she would have to stop the use of the bipap breathing machine she is on. they would put her on oxygen. (The purpose of the bipap is to help removed the Co2 that builds up in her lungs from breathing in natural oxygen. By putting her on oxygen will only exacerbate the situation, and hasten her death.) I digress; I told him no, she was going to stay on the bipap. He all but assured me he would find a way to get her into hospice, WITH the bipap. His last words to me were on Friday, March 17th, and he said: I will call you back first thing Monday (the 20th of March).. well, its Thursday,March 23rd, and I haven't received a single word (call, text, email) from him. Because it was not an easy sell, he obviously moved on to the next potential customer. My problem with all of this: do not make promises you cannot make; do not prey on a family facing the impending death of a loved one, be HONEST in your dealings, AND when you cannot do what you said you could do, TELL ME YOU CANT. First of all, think about it. Why does Sage Hospice have all 5 star or 1 star reviews? Because the 5 stars are false that's why. Even Hospice of the Valley, the number 1 hospice in the valley doesn't have anywhere near this many reviews. This is the most horrible hospice ever. From intake, to nurse, to pathetic social worker showing up in high heels like she's going on a date, to Carol the CNA that manhandled my mom and bruised her and even the owner who has nothing to say except well..... My mom is 94 and been in and out of hospice for years including Hospice of the Valley, Hospice of the West, Hospice Care and now Americare Hospice and none come close to the horrible care, unprofessionalism and disregard for safety of the elderly. After two weeks I complained about the bruises on my mom and they blamed me. When I actually watched how Carol, CNA handled my mom in the shower I was very angry and basically threw her out. She retaliated by telling Social worker, Leslie Beck that I caused bruising and other things. Leslie Beck, a woman that has no business being a social worker, reacted by not investigating but calling Adult Protective Services. Of course, I was cleared having the Area Agency of Aging Social Worker, previous Hospice's and 3 different caregivers, 2 neighbors vehemently stating these were false claims. If after reading this review and the other truthful reviews with 1 star ratings you still sign up with Sage Hospice then it should be on your conscience if anything bad happens to your loved one. If you are currently with Sage Hospice or decide to sign up and realize you made a mistake as I did, you can file a report online at AZ Department of Health Services, Licensing Division. And for your own piece of mind, NEVER allow anyone, especially a CNA to close a bathroom door and be alone with your loved one. You must supervise! I am sure the owner will respond and deny, lie or whatever but just read the other 1 star reviews. We are not all lying! read more

When Sage first started caring for my dad they were friendly, attentive to his needs and seemed to know what they were doing. My 92 yr old dad wasn't your average hospice patient. He had Parkinson's Disease so his decline was slow. He was bed bed bound and a little crabby at times. Sage promised all kinds of great stuff----volunteers to visit him, a physical therapist to come evaluate him and they even said they would give me a break (respite). Well fist of all they never sent a PT. The nurse said it wouldn't do him any good. Dad was very lonely and no volunteer every showed up. The aides that came to bathe him (not FRANK he was great), but the two females would show up and move way too fast for a bed bound guy with a neurological problem. I printed a pamphlet for them abound how to handle a Parkinson's patient--i do not think they ever even gave it a second glance. One day while using the hoist, one of the aides nearly castrated my dad who was screaming the his testicles were caught, but she kept pumping that lift higher. I finally heard him yelling and was ready to punch her she just said i know i was hoping they'd come loose. WITH? wow---the pamphlet also in very larger letters said that exercise is the best thing for someone in dads condition. he was depressed and sad and so lonely and afraid.....mind you my father fought in WWI and Korea. He should have been given the absolute best care possible. Well he passed away on April 4 and aside from the nurse coming that night to pronounce him----I have not received 1 phone call or visit or anything. I'm grieving over here and nobody has even called. These people were billing medicare $6000 per month for 8 baths and 4 nurse visits. They were in and out so fast they didn't even spend 8 hours a month between them all. $6000 they should be ashamed I am shocked at the lack of sympathy and service. They didn't even call me to set up the time for the equipment to be picked up s i was told they would. The sun valley guy just showed up. read more

I'm compelled to share my experience with Sage Hospice. I have worked in the senior care business for the last 12 years, over this period of time seeing many hospice operations from the inside out. For the most part, I do not have a negative thing to say about any of them. All of the hospice companies that I have dealt with have great and caring staffs that do a good job of taking care of families and patients. What sets Sage Hospice apart is extra effort and support that they provide to all of their patients. This starts with the ownership and management they set the example to treat all of the patients as your own family. The rest of the staff is nothing short of extraordinary with their care for the patients and their families. The staff at Sage Hospice exceeds the one requirement that I have to do business with a company; the whole staff has a genuine smile on their faces. These smiles have gone a long way to providing comfort to my clients that I have sent to Sage Hospice. When it comes time to put one of my family members on hospice I will only need one number to call. Until that day comes for my family, I will continue to send my clients to Sage Hospice; I know they will be treated like family there. Keep up the Great work! read more

My Mom passed on 07/03/2017. I want to "Thank" Sage for the care that she received from them. They were very respective. A CNA and a Nurse came while I was there. I forgot the Nurse's name I Remember she had the Christian Fish on her necklace or somewhere. Mom would have loved that! Ya know because of another past appt , I was apprehensive of the Company. My "BIG" mistake. They came thru 100%. They did what they could do for my Mom. I am so grateful for their care and understanding. So would I suggest "Sage" for Hospice & Palitive care ? Very disappointed in Sage Hospice. The love of my life was admitted for head and neck cancer. His nurse and bath aide were wonderful. The rest of the team not so much. Chaplain spent 15 mins with us and then talked about Trump. SW said she would look into getting an emergency food box for us since I had to quit my job to take care of my love. Never got the food box and was suppose to get a wipe board so he could see the day, month and year. Never got that either! A different SW gave me a bunch of phone numbers to call for help but where was the time to do that when I was taking care of my love 24/7? When he passed, I called hospice and they sent an rn out. He never pronounced him. Rn destroyed some of the meds, called the appropriate people to come get his body and left. Left ME alone with the body. I stared at my dead honey for 1 1/2 hours with NO SUPPORT! I would NEVER recommend Sage to anyone needing hospice services. I even wrote a letter to the program director and never got a response back. How professional! read more

Sage hospice employees are fantastic. My Mother had the best care from Sage Hospice. My thanks to Her nurse Sherrie. She took such excellent care of my Mother. Hospice is what it is but Sage Hospice ended up to be part of my family. Sherrie's sole purpose was to make sure my Mother was comfortable. She didn't sugar coat the process but helped me get through it. Sounds selfish on my part but it wasn't. My Mom was comfortable but the end part is so so hard on the family. Sage was there for me and my family. Juanita the hospice supervisor visited once a week, the Chaplin was a great comfort. The aide twice a week to shower and in the end bed bathe my Mother. I can't say enough good things about Sage Hospice, they cared for my grandma during her last couple months with Dementia and Alzheimer's and they were quick to respond when I called about my good friend needing care for his cancer. I called just to get general questions and Tiana returned my call that day, but I missed her call, so she then text me letting me know that I can reach her day or night and even weekends if I needed. She reached out to my friend that same week after telling her about his diagnoses and she visited him at his home. He has been having depression and anxiety and for once in months they were able to give him reassurance and allow him to feel he is in good hands. They aren't just there for people in there last days of life, they are available whenever needed. I so appreciate Tiana and Sage Hospice, they are true hero's and angels. read more

Extraordinary level of care! My mother is 96 and has been with Sage Hospice for four months. Tiana Zang, the President, is always accessible, responsive, and caring. Molly, my mother's nurse, is attentive to every detail, and it's clear she loves her job. She calls me almost every week to update me on my mother's care. While my mother's health was very rocky at the start, she's stabilized and now doing very well. When Molly calls to update me, she sounds as happy as I am in sharing how well my mom is doing! Julie and Kelly are two more exceptional women who have been involved in my mother's care. When my mom wanted a milkshake and particular sandwich, a substitute nurse (can't remember her name) drove out of her way to buy them for her and would not let me reimburse her. Sage provides and maintain her oxygen equipment, wheelchair, meds, and whatever else we've needed. They send in chaplains and social workers to visit. As my mother's mind is sound, they've brought in warm and enthusiastic volunteers to visit my mother socially. The volunteers take her for drives and walks and offer conversation and normality. I live in nearby, and I see my mother regularly. I have personally seen her extraordinary level of care. She's happy and positive. I recommend Sage highly. read more
