Yes, you can say that I should read the form 100%, but typically, if there is to be a fee assessed, the staff should disclose this verbally, and not hide behind a disclosure that a majority of their patients breeze through. If its a new fee, it should be VERBALLY mentioned, and the option to not have the test done be given as an option. I'll NOT be buying my glasses at Target again.
