We love Dr. Gujrathi! My kids and I saw her at her last practice and were so fortunate to find out that she was now at 4C. She is very knowledgable and friendly and always takes the time to talk and find out what's really wrong. Her medical assistant, Diane, is fabulous as well! I'm really happy that they now have a phlebotomist in their office! It's so nice if there is blood work to be done that they can do it there at the same time. Wow, I'm stunned. After my doctor's office closed unexpectedly and my medication started to run low, I needed to find a primary-care provider ASAP. Luckily this place was close to my house and took my insurance so I decided to try it out. Simply put, Dr. Katayoun Baniriah is amazing. After waiting in the lobby and having my vitals checked, she came in, heard me out, what I needed, and was extremely professional the entire duration of my visit. I'm so happy to call her my new doctor, as she's clearly very knowledgeable and knew exactly what I was going through and what I needed just minutes into seeing me. I really love my doctor, and followed her from her former practice to the 4C Medical Group. When I finally get in to see her, she is excellent. However, I am now considering changing doctors, because there is no way to to contact my doctor, except to mail her a letter, or drive there and drop off a letter. The front office never answers the phone, and if you leave a message, as the recording requests, they do NOT call you back. You might be put on hold for hours, only to be hung up on. This is not professional. There is also NO patient portal, and NO email to contact your doctor, as you would find at other clinics. I am elderly, and have increasingly serious problems, and I need to be able to contact my primary care physician for referrals. I worry that if something serious happens, like falling or a stroke, I will have NO WAY to contact my doctor! You know that if you go to any urgent care, they always say: "call your doctor tomorrow". It takes about 3 weeks to get a referral from here. I am reading some of the earlier comments here. Dating back to 2019, comments say they "do not answer the phone". What is wrong with 4C? There is no excuse for not having a communication system in place. read more

I gave up and left, dr torres was ok but it seems she isn't at the practice anymore and no one is doing rx refills, which leaves me w/o important medication. medication i've been taking for 15+ years, trying to call the office to get any info is a disaster, i wait on hold for hours to be disconnected, i get told i need to make an appointment when i just saw dr torres weeks ago (maybe she quit? maybe she realized what a dumpster fire this office is?). I had labs done, no one forwarded those to me, am I dying, do i need medication adjusted, no idea!! i needed a referral that dr torres was going to set up, didnt get that either. what i did get was my EOB from insurance showing $350 dollars was billed for the office visit. ehhhh i dont think 6 minutes is worth $350 when NOTHING GETS DONE. The practice doesn't have a patient portal, the practice does not answer the phone, I only know dr torres left because someone in my family received a letter, i didnt receive one. what i received was numerous bossy phone calls that my rx's wont be refilled until i make an appt with some dr iv'e never heard of and an employee who i could not understand. maybe if someone over there could read the chart they would see i've had the same meds for over a decade and kindly respond to the pharmacy. maybe someone could send me a copy of the lab results so my insurance is not billed again when i go see a new pcp this week? I would love nothing more than to toss this entire company infront of the state medical board. suggestion to the practice- answer your phone, answer the pharmacy, don't abandon patients when your dr's leave, communicate lab results and have some staff who can speak clearly on the phone. your patients are running from this place, you need to fix something! read more

I had an appointment with my physician and discussed several things. She had her "Scribe" there to take down notes of everything that occurred. I asked for very specific drugs to be refilled because I was almost out. One of these drugs I must take every day. it is extremely important. A week passed which I thought was abnormal. I hadn't received a call from Frys to say my prescription was filled. I called Fry's and they said they never heard from them. This is the SECOND time that has happened. Of course, this was on a Friday and their office was closed. I had to wait until Monday. I called Monday and was 13th in line on hold. I called at 9 am. I was on hold for HOURS. I did not leave a message because last time I did NO ONE got the message. Then I was hung up on hold because it was their lunchtime. I called back at 1 PM and got put on hold again #8 in line this time. Waited an hour until I finally got to talk to someone. Explained the situation... There were no notes about the prescriptions in their "system" AT ALL. After a lengthy explanation of why I needed my prescription, the girl on the phone assured me my prescriptions would be filled including a new one. 2 days later Frys calls to tell me I have ONE prescription waiting for pickup. I went to Frys to pick it up and it was the new one NOT the ONE I REALLY NEEDED that I said was VERY IMPORTANT. Yes, I did go to the office but was greeted with COMPLACENCY and RED TAPE which really pissed me off (no medication get it?). I left with NO RESOLUTION. This is what happens when someone doesn't get the medication that keeps them pleasant What if I my prescription was insulin or something life-threatening? would you trust this Doctor and this Facility with your health care needs? Please think TWICE before you do. There is simply no excuse for something like this to happen in any medical facility. I have now been without the medication for 5 days and counting. I followed Dr. Dwvedi here. It looks like they're renovating this building. They were super friendly. I'm not sure that I would have chose this office had I not been following Dr. Dwvedi. But with that said I am giving five stars to Dr. Dwvedi. She is thorough, she listens and discuss his treatment plans. She is truly amazing and everyone should see her. I like the fact that there is a lab in the building which makes it easy to get blood work. Unfortunately the weight was kind of long. I have been going to Dr. Taylor for years and could not be happier. He and his Nurse Sarah set a new bar for patient care. They always remember me in and details of me as a person in a surprising way and have never made me feel like just one of the files in their system. They're exceedingly thorough and actually care to make my health something that is important to them. Which is especially important to me who is somewhat of a worrier about any issues I have. But once I let them know what is going on, they're quick to help me solve it and put my mind at ease. Dr. Fruchtman and Dr. Baniriah are amazing. I love them and trust their judgment and expertise. They both have excellent bedside manners and are understanding and compassionate. I am leaving 5 STARS simply because I would feel bad leaving something less given the superb level of medical service these doctors provide. HOWEVER, as stated in virtually every other complaint, it is very frustrating to call into the office and get ahold of someone. It usually always takes at least 24 hours for them to return the call. If you happen to miss it, well you are back at square one and must call and leave another message and wait another 24 hours for return calls. I'm patients would be much better served if they had a full time dedicated person to answering phones AND calling people IMMEDIATELY back. read more

They will just transfer you to a third party billing call center for even worse service. I still haven't spoke to a competent person who can resolve the error they created! My last appointment was at the end of January 2022. The entire process, from making my appointment to now dealing with multiple insurance issues, has been incredibly frustrating. Here are issues I would like to bring to your attention; I have called the office and was left on hold so long I was actually able to get in my car and drive to the office, only to walk in and see three receptionists behind the front desk and not one was on the phone. I left the call going on my cell phone, and was still on hold upon walking in. When I arrived for my scheduled appointment, I was told that my appointment had been canceled due to the Dr not being in the office, and would need to be rescheduled for another day. When asked why I was not notified prior to arriving in the office it was only then that I was told there was no phone number listed in my chart. This is confusing seeing as I have given my information multiple times to office staff both in person and over the phone, have had the same phone number for the past 10 years, and have been called by the office before. When I was able to return for the rescheduled appointment, all of my previous paperwork needed to be filled out again. I completed all required paperwork, including providing my updated insurance information (again, this was given prior to the appointment). During my appointment, I was asked to fill out the paperwork AGAIN by the nurse (this would be the third time) as the paperwork I completed and had given to the front desk minutes before being called back could not be found. After much confusion, the paperwork was found under a box of food delivery at the reception area. Now, this bring us to where we are currently. For the past month, I have been receiving bills from your office for my appointment that far exceed my copay amount. When I have called for more information I have been told multiple different things; (1) that the bill was sent in error (2) that my insurance was never billed, and (3) that your office is actually not even in network with my insurance. If you are in fact not in network with my insurance, I am extremely confused as to why today (4/19/22) is the first time I am hearing this after providing my insurance to multiple staff members prior to my appointment, during my appointment, and calling for information about my bill after my appointment. Due to the inattention of the office staff, I am out both time and money. I feel completely disregarded as a patient, and I do not feel that 4C Medical respects my time or can be counted on to provide accurate and timely information. As much as I have enjoyed seeing Dr. Torres over the past few years, I will not be returning. read more

I have a very rare autoimmune disease and am on a lot of medications. Needless to say, my doctor plays a huge role in my healthcare and well being. One day, after 5-years of care, my doctor told me that my disease took up too much of his time and he couldn't treat me anymore. I was shocked, very afraid and I needed a Primary Care Physician fast! After having a very hard time finding a doctor that would be willing to treat me with all of my "problems", a friend of mine told me to try Dr. Matin at Care MD. I set up and appointment immediately.... and it was the best thing I ever did for myself. The staff is like family to me, and Dr. Matin is not only one of the greatest Doctors I've ever had, he's like family and treats me like his. He is the kind of doctor who will spend the time with you and not make you feel rushed. I HIGHLY recommend Dr. Matin and Care MD if you're looking for a family practice doctor. I was not happy due to the long wait just to see the doctor, nobody answering phone calls or returning messages. My mother loves Dr. Matin and refused to find another doctor. Before last week I would of gave no stars. My mom needed a prescription and I had questions so on Thursday 10/18 with huge hesitation because the front ladies are full of false information I called. Wow! Phone answered " Good Morning, thank you for calling CareMD this is Tonia." Usually if answered " CAREMD". So to hear a friendly voice was refreshing. My questions were answered very professionally. I decided to voice my concerns and express how upset I am. I explained about the long waits and a situation with being given the wrong information by Mariam, calls never being answered and messages never returned. Tonia listened never talking over me or interrupting. She apologized for everything, explained my billing question in complete detail and how a referral works. Tonia explained why Dr. Matin could be running behind and she is right. I am going to have more patience. Tonia- thank you for everything. The people who work in this office are complete fucking idiots. Every time I would call to ask them if she would fill out my paperwork they said she would do it. I CALLED 5 DIFFERENT TIMES and each time they said yes. They also said they never received the fax from my work that requested medical records and to fill out the FMLA form. Once I went to my appointment, they magically found the paper work that was faxed over to them. These people love to lie just to get out of going out of their way to help you. They are complete garbage people. Idk what happened to DR DWIVEDI, she used to be great. Now she's a total piece of shit, judgmental bitch. She doesn't even give a fuck about her patients. She Just gives a fuck about getting paid. If you prefer being more natural than to take meds she will talk to you with such a judgmental tone, it's disgusting. Cassandra the receptionist at the front desk was rude and condescending to me when I explained that I attempted on two different occasions to make an appointment. I called during business hrs and when no one answered the phone I received a recording to state my name, telephone number and somebody would return my call. No one did. When I brought this to her attention, she clearly did not like it, and her attitude and demeanor towards me was completely unprofessional and unpleasant. The doctor and the facility seems OK. But be careful when you consider making an appoint to see a doctor. This office will charge you at least the office visit fee even when you did not have any treatment or checkup done or even when they ask you to go to see the doctor by mistake. On my last visit, the office charge me $100 for the doctor to tell me that she looked at the wrong blood test result and this office visit should not happen. I've learnt my lesson that I must check my own blood test before making an appointment to see a doctor at least to make sure that the doctor did not look at the wrong test result or blood test of somebody else. In my case, it seems to be OK (except that I had to skip work and pay $100 for nothing). But it may not be OK if the situation change. If the doctor has patient's wrong information, the doctor will definitely give you the wrong treatment and it can be dangerous. read more

I wish I could leave this office five stars, because my physician Dr. Fruchtman is amazing. He spends a generous amount of time with me and always is genuinely concerned and no matter what I come in for if I have further complaints he always addresses them during the same visit rather than make me have another appointment like other physicians have me do.. The only challenge I have with this office is the front office. They have certain times that they supposedly answer the phone so when I call during those hours and I get a voicemail telling me to leave a message why am I leaving a message? If you're supposed to answer the phone during the time that I call ,why am I leaving a message and having to wait 24 to 48 hours for a return call? I never get an answer. If you say you answer your phone between the hours of 9 AM and 1130 if I called at 10:30 why do I have to leave a voicemail? Why do you never answer the phone? I work very busy hours and do not have time to wait for a callback. And if and when I do get a call back I cannot always be available to answer that call. And when I actually do you get a call back and make an appointment when I check in for my appointment and there always seems to be confusion. read more

Well i really wanted to like this place as it's so close to my house and the office is actually pretty nice inside. I've been here twice now and have lost so much time because of it. The first time I came here I was brought into the actual exam room over a half hour past my appointment time then sat for another 45 minutes before the doctor came in to see me. I just was back in today, I had an appointment at 3. I arrived early as you should but was still not seen by 3:30. Everyone that came in after me was seen before me, my guess is they were late and took up my scheduled time. I approached the front desk and let them know I would be leaving if I wasn't seen soon. They asked the doctor who told them it would be even longer. I finally left. WHEN WILL DOCTORS OF ANY KIND RESPECT MY TIME???? This is one thing I absolutely hate about doctors, their time seems to be the only time that matters. All these offices say they'll charge you for missed appointments or for being late but what do I get when I always have to wait at least a half hour past my appointment time? NOTHING! Except time lost from work or my life! If you want a doctor that sees you on time, THIS IS NOT THE PLACE! Zero respect for my time!!!!!!!!!!! I was involved in a mal-practice lawsuit and have no trust for doctors WHATSOEVER! Unfortunately we can only treat ourselves as best possible until we need a doctor. I've seen 6-7 different doctors since my malpractice case and can't even finish the appointments in some cases because I'm disgusted with the level of care . If I could, I would give this practice 0 stars. This office is the worst Dr.'s office I have ever been in! There are practically no patients in the waiting room but they always take you to the back late. I was there early and people who came in after me were taken back before me. I was in for a pre surgery exam where I needed blood to be drawn. When I made the appointment I was told that blood would be taken at that time. Because the Dr. was late, of course, the girl who takes the blood was gone for the day. I had to return the next day to have it taken. When I returned a week later for the results, same thing, everyone going in before me. My surgeon was calling for the surgery clearance and the receptionist, Cassandra, was complaining and laughing about all the phone calls for my surgery, right in front of me! DO NOT go to this office unless you have all the time in the world to wait and you want sub par medical care. read more

Dr. Matin is both my doctor and my husband's doctor. He is also my 17 year old son's doctor. I have suffered from chronic migraines and daily headaches for about 25 years, and as a result have seen more than my fair share of doctors. I've ridden what I call the "hamster wheel" of referrals to try and determine the cause of my headaches and migraines, so many times that I was not willing to jump back on the wheel again. Well, Dr. Matin accepted that resolution and worked around if for a while, then he wiggled under my stubbornness and sent me to a very, very good neurologist. I'm still getting the headaches, etc., but I can actually see the light at the end of a very long tunnel. I'm not the world's best patient, I sometimes forget to get the blood work done or to go to the specialist if my symptoms resolve. Dr. Matin doesn't get mad at me but he does keep me in line. Just the right touch. And to add the final bit, Dr. Banerrhia in his office takes care of my adult daughter and she loves her. And my daughter is quite picky! And finally, we love the staff. They are professional while still treating us like friends. I highly recommend this office, take it from someone who's been to way too many! read more
