Was actually sad to leave the staff. In just 6 wks I learned to walk all over again, even line dancing with Courtney. All of the physical therapists were great to work with each had their own specialty and focused on different muscle groups. Last Christmas, my father had to stay here post stroke. As soon as his coverage was up, they moved him into a shared, dingy room with 3 others while he waited for MediCal to come through. Four patients crammed into a room no bigger than a prison cell, separated only by thin curtains. The call buttons? Useless. You're left waiting indefinitely. My grandfather had an accident--wet himself--and staff actually told me he'd only pushed the button ten minutes earlier. Left him sitting there in urine, tossed a napkin over his lap like that fixes anything, then vanished for another ten while he lay exposed. I thought maybe he was exaggerating to guilt me into springing him... until I walked those halls myself. Smells hit you everywhere--bedpans, sweat, decay--you can't tell if it's furniture or flesh rotting. And everyone working there? Hollow. Dead inside. Okay, now this is only my SECOND review of this place within the last 24 hours. Yep. That's a fact. And why am I not allowed to leave a -1 review here? Doesn't make any sense. My fiancé needs dialysis because he's got severe kidney disease. At present he's bed bound and quite helpless. However, today he did NOT get any dialysis at all ! What???!!! Oh, but Lone Tree Convalescent Hospital (they call themselves a hospital?) they can come up with excuse after excuse after excuse. The fact is, I was there with my fiancé this morning at 8 am when my fiance explicity told the staff exactly what needed to happen and when it needed to happen in order to get him to the off site dialysis clinic on time for his 12:45 pm appointment today. As it was, the excuse the (polite) staff person who just called me came up with is this (eye roll) the transport vehicle had a flat tire! OMG! I was born yesterday but NOT last night, okay? So there's that. Thankfully, my fiancé is a Viet Nam Veteran so we have demanded now that he be transferred RIGHT AWAY to the V.A. Hospital on Clement Street in San Francisco where they have an onsite dialysis clinic. This place, in my honest opinion, is a death factory. Do NOT take your loved ones here at any time. You may live to regret it, but your loved ones may no longer be alive. And, in reading other online reviews, I do not think I'm being extraordinarily harsh in my estimation of what transpires here at this facility. You decide at your own peril. read more

My 90 yo dad was just admitted yesterday. He was moved to a different room today and at 1:00 had to go to the bathroom (#2). He pushed his call bell which doesn't work and my mom told one of the staff at 1:45. They said they would tell his nurse. My mom left and found out that no one helped until 3:00. They cleaned him and left so quickly he didn't have time to tell them that he had to urinate. His call light still doesn't work, and my dad is left embarrassed and upset! This is elder abuse!! My father fell at home. After going to the hospital he had to go to a recovery center. His overall care was good. The activities team was friendly and very caring. Overall I was happy. My only concerns was his sugar levels were higher then they had been at home and what Covid-19 vaccine they gave hI'm. My father had not been vaccinated for COVID-19. They called me because the county was coming to give feee shots. I spoke with their staff and said my father cou;d have either the Pfizer or Moderna vaccine, but under no circumstances should he have the Johnson and Johnson vaccine. So on 6/23 he was vaccinate. Later that night I asked what vaccine did he get. I was told by the nurse in charge she could not tell, but his vaccine card would say. So speed ahead to today my Father was released. The discharge person was going through the myriad of pills I asked for the vaccine card and the guy was like they didn't give it to me. So he brings me the card and it says Jansen. To say the least I was very upset, infuriated. I asked for someone in charge. The guy said of no one is here it's the weekend. I said I don't care who you have to call but I want to talk to someone now. he called Dusty. She proceeded to tell him James is not J&J, which they are and she was the person I specifically told no J&J vaccine. So either she is incompetent or a liar. I would like to put 00000 They don't care about your love Ones!!! my fiancé had a mini stroke! And the hospital assigned him to LoneTree, I based myself on a brochure that was provided from the hospital ! It seem nice, and comfortable, It was a nightmare!!! For my fiancé he was put in a one room with three patients, to crowed !! Went home in tears !!feeling so bad for my fiancé went there the nxt day my fiancé said he didn't sleep at all bcz one of the patients yelling, day and night! Went to the front desk and notified them, lied through their teeth, lies telling me they were going to move him to a room with only one patient, lied again saying that they were moving the patient yelling, my fiancé couldn't handle anymore 7 days without sleeping!! He was going to Dialyses couldn't handle it bcz he was tired he would go to sleep in a old couch they had in the corridor!! So finally I was so upset!! I went to speak to the supervisor and I asked for discharge bcz that was 7 days of nightmares!! Finally !! On the last day there. he was transferred to a room, poor guy when he returned from dialysis around 1:45 pm he was so tired that once he went in to the new room didn't wake up until nxt morning, ready to go home! They were so cheap that they didn't bother to give him breakfast!!! All it is if you don't have good insurance! That's the nightmare expected !!! His sugar always high !! read more

This is a first experience for myself and for my sister (the patient). She had a fall almost two weeks ago and broke 9 bones in her left hand and left leg and foot. She needed surgery, but it couldn't take place until a lot of swelling went down. One week after being admitted, the surgery finally took place. On my first visit, before surgery, I had to physically get up and go to the front desk, since no one had answered the "Red Light" signal, and ask for assistance for her, since she couldn't walk or get out of bed without help. Because of this, she had to wear diapers. The first day I went to visit, I had to go find someone to come in and change a very soaked and dripping diaper she had had on for more than two hours, because no one answered her "Red Light" signaling that she needed help. She has constantly had to lay and wait for someone to bring her ice for her warm water that has been sitting all day. The workers are up and down the corridors a lot, and won't stop and look in on her unless they see that I am in the room with her, then they "might stop in" and ask if she needs anything. I have had to find the attending nurse or person in charge more than once, to get her pain medications to her when she has been in "moaning worthy" pain of them! They keep it so hot in the rooms that I have even had to start shedding or loosening clothing to stay comfortable. I, to say the least, am quite appalled with the conditions, and treatment of the patients at this facility. I wonder, would they treat their own family like this? Think about it!! read more

My aunt stayed here a couple months ago to receive therapy. The therapy staff, activity staff, and cnas were very patient with her. We were very hesitant about sending her here because of all the bad reviews. But the staff was amazing!! & facility was very clean. I thank everyone who helped my aunt! read more

If I could make this -0000000 I would. My Mother is in there right now 3-16-21 and is not being cared for when it comes to PT, OT and more importantly THE FOLLOWING OF COVID-19 GUIDELINES!!!!!!!! I was told all packages coming into the facility were to be left in the Recreational Center for 24hrs before patients could receive them, but no they were given to her within 20mins of me dropping them off. If they cannot adhere to this simple task what else are they not doing???? Oh, let me tell you .... she was told after using the commode for a BM (TWICE) and called for it to be dumped that they were trying to conserve unnecessary usage of supplies in order to save money!!!!! She sat with this in her room until 8:30 the next morning finally saying, "Could you please remove this before I have my breakfast at least!!!" She then asked for a pillow that evening before too, after being told the above (which was brought to her with no pillowcase) the same thing about the pillowcase. They are trying to save on laundry services so she should be ok without one!!!!!! Well .... they are plastic covered pillows are they trying TO SAVE ON THE SPRAY THAT THEY ARE TO BE WIPED DOWN WITH TOO????? IF YOU HAVE FAMILY IN THERE KEEP ON THESE PEOPLE AS THEY DON'T GIVE A (S**T) ABOUT WHAT THEY ARE OR ARE NOT DOING!!!!!! THERE IS NO CONSISTENCY FROM NUSRSE TO NURSE OR AIDE TO AIDE ESPECIALLY AT NIGHT WHEN NO ONE IS AROUND!!!!!!!! We treat animal at the zoo better than they are caring for our fragile elderly loved ones!!! BE AWARE OF THIS FACILITY! In terms of Convalescent Hospital locations in the area, this is one of the better ones but as you can tell by my 2 stars, that doesn't say much. This place is rather clean and most of the staff are overly qualified, but just because you have those two things doesn't mean this place is going to be great. What Lone Tree suffers from is poor attention to deal and genuinely caring about their patients. From what I've observed everything is done like it's a procedure or a chore. You rarely see the nurses be friendly with the patients or treat them with any respect and everything is rather hurried. I understand that sometimes one nurse or a nursing assistant may have 10 patients to take care of, but that's no excuse for shitty care. read more

My mom is fairly young (she's 61) to be in a Convalescent Hospital - she had a major accident 10 years ago. She's been at LTCH for about 5 years now - and I am so grateful. Look past the older building - the staff are wonderful, I still see many of the same staff that were there when my mom was first admitted. I get a call if there is ever an issue, she is always well groomed and clean. I never have a problem reaching anyone and the social workers and supervisors are very supportive and kind. I was recently there for 46 days, worse place ever, i needed therapy for hip replacement, therapy is good once it gets started wasted 2-3 weeks because of confusion on getting me up before therapy since therapy only 40 minutes, aides didn't want to help, some our good others pure lazy, some don't change gloves between patients in same room, don't answer lights or come in and shut them off and say they will come back and dont, nurses poor care, lie about times they give meds, a few are very good, place is run by a 26 year old guy only cares about the money, food is terrible especially night meals, afternoon meals ok, but night meals discusting, overall care is horrible.. i left earlier than they thought I should couldn't stand it anymore. They need inservices for aides regarding bathing, don't strip patients of every piece of clothing when bathing, how to care for different diagnoses, they don't know difference between hip fracture and hip replacement, therapy could do inservices with them, it would help alot, higher ups do not follow through on complaints, I suggest you don't go there, until they make changes for the residents. read more

Awful experience. Mom had horrible care. She's diabetic. They fed her sugar water and insisted she drink it. Her blood sugar, always well controlled, went to 441. They missed her thyroid medication, and her thyroid dropped to dangerous levels. They failed to control her sodium. As to quality of care, they did not answer her buzzer and her NEIGHBOR - in the next bed - had to help her to the bathroom. They lost her clothes and wrapped her in a blanket. Within a week Mom's health went from good - but needing good care to regain physical strength - to a trip to the ER by ambulance where she almost died. If you care about your loved ones, do NOT use Lone Tree. Mom said, and we agree, "If I'd have stayed there one more day, I'd have died." She's right, and the Docs agree. Awful place. My experience with lone tree is awful, the staff is nice and that is it......no one seem to attend to the patients needs and on the surface they play nice when the patient family is around and treat the patient with no care when no one is around. My father had several falls since his stay at Lone Tree convalescent which is unacceptable, I do understand things occur, however repeat incident is unacceptable. I feel the staffing to ratio of patient is in great need of change to provide better care. It stands to reason that a convalescent would give patient care when the family is not around as while. I would go on but I feel it is extremely important that you monitor the facility closely to ensure proper care. I will be taking my Parent from the facility ASAP. read more

I never thought that I would be giving a review for a convalescent home. Well as fate has it, my dad ended up having to stay here for 4 weeks to recover from surgery and a fall. The staff was very friendly and helpful and all the doctors and nurses were amazing. My 87 year old dad also feels that his care was top notch. The place was very clean and friendly and we never had a problem with parking. The physical therapist Brian worked with my dad daily to help him recover and get back on his feet. When you have a loved one that you care about, it is a good feeling to know that they are in caring and competent hands. Thank you Lone Tree Convalescent Hospital!. read more

The Lone tree Convalescent home is based on the corner of Lone tree and Ridgerock drive, the facility, does not provide a parking area for there employees, and expect them to find parking, which results in a business using a residential area for there parking, cars are parked in front of my home everyday all day and weekends, and in front of every house on this block, I can't park in front of my own home or even have my family come over because there is no where to park, I have been down there several times and asked if they could leave me some space to park I was told that the street is public and they can park where they want, I then spoke to the Administrator who said he would take care of it but to no avail. Some of employees are coming to there cars on there lunch hour and eating in there cars, or sitting on the sidewalk in front of my house leaving there garbage, I think, if they aren't good neighbors and don't care about the community around them how are people being treated that live there? There is a grass area on the side of the Convalescent home, and we are in a drought, why not make some parking for there employees instead of making the neighbors carry the burden of cars parked in front of there house all day, 7 days a week. I was so amazed with our experience. Everyone was so friendly and beyond professional. I totally recommend anyone to pick this facility.

As you can see from my rating they do not even deserve 1/2 of a star my mom was taken there for what they said was rehabilation only and it would be for a few weeks only the first day she got there the nurse refused to check her sugar levels because she said that she didn't have anything about it on her chart mind you she had just come from John Muir hospital where her sugar levels were in the 30,s! When I asked to see her chart and why she was there in their opinion no one could tell me why exactly she was there I had to go in there everyday and monitor her care she developed breathing problems and they would not address it other than saying she had asthma I watched my mom detiorate before my eyes their physician only came 1 day a week and along with the nurses and the whole facility's incompetence and concern or rather lack of and as a result of constantly complaining she still received poor poor care I pulled her threatening with a law suit took her straight to John Muir emergency where she was immediately admitted and a couple of weeks later she died. My only saving grace is she died at home with people who loved her my ending plea is please please please DON'T. TAKE YOU LOVE ONE TO THIS FACILITY read more
