This place needs to be reported to the health inspector. Dirt all over the place, very filthy. Curtains being held up with duct tape. I pressed my help button, started a timer and waited 35 minutes for anyone to show up. When they did show up, they were unable to assist with the care that I required. Keep your family away if you want them to get better. read more

Update again: Had to move my grandma out of here and it was the best decision we made. The team's care here is inconsistent and with the exception of a very few people, most people here don't care about their jobs nor do they have the sympathy or mercy for the patients here. After a week or so of caring for my grandma, we saw drift where my grandma went back to being soiled for hours on end and being severely neglected. She developed bed sores (which are a med mal liability), developed diarrhea, and was sitting in her own feces for hours on end. So glad we're out of here and in a facility that actually cares for her. read more

Stopped by recently to visit a family member and was really impressed with the updates they've made. The new lobby renovation looks fantastic. It is clean, modern, and very welcoming. You can tell a lot of thought went into creating a comfortable space for residents and their families. It's great to see a facility that's investing in improvements and really taking pride in the environment they provide. The management team clearly cares about the quality of care and the overall experience for everyone who walks through the doors. If you're looking for a skilled nursing facility that's continuing to improve and invest in its community, this is definitely a place worth checking out. Terrible place. One night stay before we removed our family member. His room had a second person, which was fine, but there were pee jugs just sitting on the floor near their bed. A woman kept wandering into the room and we had to ask if they placed male and females together. She was escorted out but kept coming back in. Room was in terrible condition and the bathroom was worse. Do not recommend!!!! read more

Hello this my first time in a hospital or a skilled nurse faculty. I was in this faculty for multiple things. I have to say that this faculty was on top of their game never had a problem even they accommodated for food I didn't like or allergic too. There was a lot of people that helped me to my recovery and I'm very grateful cause I was very scared and ready to give up but an RN named Giselle talked to me which got me on track.. I have names i like to mention that helped me on my recovery and if I misspelled some my apologies. This names range from RN,LVN,CNA, physical therapy and other office staff, Vicente, Giselle, Aileen, Lorie,Patrick, Matthew, Don, Deja, Rowana, Rayna, Thad, and Alex. I have to give my appreciation to these to everyone helped but these names that I put stood out more because they went over and beyond what their job ask them to do. Recently had an older family member there and care was terrible! Staff were uninformed when we visited and asked for feedback. The unit smelled badly and rooms were crowded and appeared dirty. We suggest avoiding the facility if possible. read more

Diablo Valley Post Acute is fantastic! My sister in law went in for a two week rehab and the staff was wonderful. Special props to their social worker RJ who did so much to ensure we received the information we needed and got the paperwork done to ensure a smooth transfer. read more

I Had a great experience at this Diablo Valley Post Acute care facility.The Physical Therapist team was amazing very professional. They treat you as their own family. My stay lasted near a Month. The CNA, Nurses and Therapists team effort caused my complete recovery. A very painful wound was expertly treated by Nurses while CNAs made things as comfortable as possible. As pain subsided the team kept me busy most Morning hours and sometimes in the afternoon. I did not partake in any of the activities, I did hear patients having a good time. The Meals are the highlight of the day. A time where the Therapists would leave you alone. XD Three Meals a Day with Lunch being the larger meal. Tasty variety of meals mostly made scratch. Many thanks to the Staff. read more

I was sent here by Kaiser to recover from CHF and to walk again.. all my regular CNA's from morning to pm to NOC were all perfect examples of good workers/people. Johnrey morning, Carol/Caroline PM and Rosie NOC/Graveyard they are good examples of people who care and are professionals at what they do. Shoutout to Catherine and Meagan too who are morning CNA's. There was also a registry nurse named Sharmaine who worked mornings and pm at times she was also a good CNA. Nurse Mika is really great at what she does and is very nice nd genuine, also nurse Cathleen who was basically my nurse case manager was very helpful and stayed in touch with my father. RJ is a very good social worker and helped me a lot as well. And the nurses who always made sure I was given meds on time like Ralph(AM),Gurdeep(PM) & Juan(NOC). There was another in the PM too but I forgot the name. The PT's who regular saw and helped me were Alex and Rene, also OT was mostly Johnathan, they were all good therapists and very helpful. Overall my little over a month stay here was good.. there was also a very cute nurse I'd see roaming around sometimes named Kimmy, made my day every time I saw her. read more

DO NOT GO THERE! I waqs there for a month post surgery and they almost killed me. My surgery got infected and they did nothing. I had to call 911 myself and have the fire dept take me to the hospital. The nursing staff sucks. They never answered any of the calls. You'd lay there for over an hour before someone came. Wound care nurses left garbage and scissors in bed. The food is awful. It was like being in prison. People screaming all night long. There was an old lady howling like a dog in the hallway all hours of the night. People pounding on the walls. Other patients would just come in your room at night. The dude in the bed next to me was doing meth. If you want to go to hell go here. If you want to die a slow miserable death this is the place to go. read more

I also agree with the rest of 1 star review is do not send your loved one here. My father was there for 2.4 weeks after his foot surgery and I don't know why Kaiser recommended this skilled nursing facility. The nurses are rude and act like you owe them something. My fathers wound was infected thanks to them I don't know how they took care of it but if you're a profession nurse even with a degree you should know how to take care of the wound now he is on two antibiotics because of this. Before discharge I got a call from nurse and discharge coordinator and I specifically asked for a commode and a shower chair. The commode was delivered but there is no shower chair in site. When I called medical supply asking where is the shower chair I was told there was no order. How careless of them I even asked them twice before getting off the phone with them and they reassured me there will be one. I have called and left several messages to discharge coordinator and still no response it's been a week. Wtf! Also the phones in the room don't work, it was very stressful to reach your loved one when you called. The conditioner in the room is broke and they promised to fix it while he was there for 2 weeks and nothing. Whatever you do stay away from this place! TERRIBLE!!! ! read more

Significant neglect and harm; physical and psychological. Incompetent and neglectful doctor and rehab director. Injury to shoulder; not following rehab instructions of surgeon; hygiene concerns, INEDIBLE FOOD; and (often) unbearable stress caused by roommates / other patients wandering into the room; including 4:00 a.m. and waking patrient. An increasing number of dementia patients; the latest of which makes LOUD staccato type noises similar to 'ah-ha-ha.' This site has been a torture chamber for an elderly woman who has no family near by to advocate for her. She states: "I don't have the strength to do this any more. "I feel like I am in a psychiatric ward." "This place is F--N nuts." "It is worse than One Flew Over the Cuckoos Nest.'" Management and owner have created a severely understaffed site; and should be fined. Many staff members working close to the patient are heroic for working in such a depressing situation. BUT too many beds are filled for the staffing available. Most often; patients are given only 2 minutes of 1:1 time. AND Dementia Patients with significant needs; (wandering, constant loud utterences, a lot of time in the bathroom) need their own room and their own floor!!! So much more. State legislators, lawyers, licensing; investigative reporters... take a closer look at this site AND all others. Many patients are suffering; tremendously and needlessly. read more

Let's see . . . 3806 Clayton Road. GPS Google maps said we had arrived. But there was not address sign or business name. We made several passes signs on the street. Annoying ? Aggravating ? Especially when it's dark and you have a patient in the car who's not feeling well. Turns out you have to drive through an apartment building entrance driveway and parking lot to get to Diablo's parking lot. Too difficult to give advance notice ? Or just plain incompetent ? One of their employees told me they couldn't find the place on their first day of work there. We finally got to the patient's room and discovered the piece of s**t lumpy mattress. What an insult to the patients. Complained and the mattress was changed. Some of the food barely qualifies to be called food. After patient release they totally dropped the ball on ordering home physical therapy. A week went by before any return phone calls to schedule PT. Standard incompetent operating procedure ? There we broken slats on a closet door, paint chips off the wall and an obnoxious loud rickety bathroom fan. This is as good as it gets ? On the plus side we thought the staff was great. read more

I understand that these facilities are so inundated with patients and not enough workers...but unfortunately my brother suffered from lack of care in my opinion. You have to advocate for your family members because they can't do it for themselves. He was sick, I understand that but he had to go twice to the hospital while there. He got better at the hospital and had to come back here...then before he could go home he started declining again...He said he'd push the call button for help and no one came for a long time. Rehab was a joke. They worked with him 5-10 minutes and then... you're done. Needless to say when he went to the hospital for the 2nd time...he didn't come back. Just breaks my heart. I pray no one else I know ever has to go there. read more

This facility is very dark and depressing. My mother was there for several weeks while recovering from Covid. They were the only facility willing to admit her while testing positive for Covid. The facility was not very clean and had a bad smell. On a positive note, the staff were very polite and caring. read more

This place is fantastic. We've had nothing but positive experiences here. From the front desk, to the CNA's, nurses, behind the scenes, and PT team. Everyone has been so accommodating and kind, has a positive attitude, and is extremely helpful. I have been to several other rehabilitation centers. This place is the best. read more

DaI stayed there for less than 24 hours and in that time did I not only observe a horribly ran,and understaffed facility, but I was literally poisoned by the staff ,when they gave me a wound cleaning product to rinse my morning pills down with instead of water . Needless to say, I wouldn't let them baby sit my hamster,let alone a human being. read more

This place is horrible , the food they bring is horrible & has no flavor , my mom stayed here for less than a week and she already wants to leave , they pay no attention to there patients :\ my mom had an accident and they didn't come and help her clean her self until 1-2hours later . 100% don't recommend read more

MY husband was admitted to this rehab on November 26, 2024. He was 81 years old and had had surgery at Kaiser WC and went to rehab to help him get his walking and balance back. For two weeks he was doing well and improving his movement. Then he got SICK with Covid, double pneumonia, Sepsis and a bacterial infection in his left arm. A Kaiser Dr. came once a week but did not examine my husband. He stood there for 5 to 7 minutes and did not even examine or talk to my husband. I told the Dr. that my husband was in terrible pain, coughing, unable to move to do PT. He said he is fine...his lungs are clear. He said my husband was not cooperating. When his left arm swelled up 4 times normal, they said they did a scan and his arm was clear! My granddaughter who is a nurse, insisted he be taken back to Kaiser for treatment. The only person to try to help him was Alex, the PT associate. I blame this rehab for my husband's death because they did not treat him for his illnesses. What a FRAUD situation!!!! YES, I AM ANGRY! My husband was transferred back to Kaiser WC. He was so sick that they could not do surgery on his arm. They said he would not make it thru surgery. He went on Hospice Comfort Care and died a horrible painful death January 1, 2025. Had the rehab treated him for his issues, he would have had a chance to recover and return home. Something should be done but what? NOTHING WILL HAPPEN!!!!!!!!!!!! ANGRY ANGRY Angry!!!! read more

Though i would give 3 stars for the nurses and Cna's only, who are doing their best with what they got. Seems very few knew what was actually going on with my mom. I would call to check on my mom and often had to call back later. In talking to a few of the nurses, even after being on the floor for 3 to 6 hours, they had to look up who my mom was. No one knew if physical therapy ever came to her room. And the whole time my mom was there, I left them several messages to call back and never got one call from them. Tracy is on top of things when it comes to moving a patient in, after that never mind getting a return call from her. Again, after leaving several messages for her. Desk clerks are often hurried and seem at times bothered to get the nurse on the phone for you. Ending several times with I'll have the nurse call you back. Many of those times when finally reaching the nurse myself, they said they never got the message to call me. You can stand at station 2 and wait for 15-20 min, with workers in and out or even sitting there before anyone even offers to help. Courtney clearly needs some lessons on answering phones, multi tasking, masking her stress and communication. Requesting call backs from this place is a joke. I actually feel bad for the nurses and Cna's. It is literally like there is no leadership in this place at all. And the team there is not working together. Their voice message while waiting on hold, trust me, I heard it several times, claims that they strive to keep their patience and families comfortable and happy. Sorry, I didn't see a lot of that. My mom would have to call me to beg for pain meds. By the time I reached the nurse she was out of her mind in pain. Each time that happened she was way past due for her meds. They would put food on her tray knowing she could not sit up and or grab any of it and no one would help her. Do not let your loved one go to this place. My mom has been in a few and this is by far the worst! read more
