Excellent care by staff at Delta View Post Acute. I was a patient for 20 days in August of 2026, recovering from abdominal surgury. I required frequent care to empty my illeostomy pouch and general help. The care provided by the following personnel to accomodate my needs was greatly appriciated. CNA's - Ashley Aragon, Patricia, Paula & Saleem - During their shift assigned to me, were attentive to my needs, checked on me regularly and made me comfortable. Acctivities Coordinators - Maylon & Ana were very helpful and provided the enjoyable experience. Physical/Ocupational Therapist - Lampert, Jesus, Darius, Delaney were forcused on my needs and attended to my recovery. I had a comfortable recovery period and was able to walk with a walker when I left the facility. read more

I had no choice. Warned by the Kaiser placement person that I should not ask to be moved to another rehab/post-hospital facility because "they're all the same," I was glad that, at least, Delta View was IN Antioch (where I live). That made visits from my husband easier on him, at least. I had a roommate who snored loudly, watched loud TV all night, even when asleep, it seemed, and there was nothing that anyone could do. I get that. The actual rehab staff was great; they really did help me to literally get back on my feet. Most of the nurses, LPN and CNAs were good, kind, caring, and patient. But a few bad apples, as they say. The TWO who, when called to help with a bedpan overnight, said, just pee in your diaper appalled me. It could take forever to get someone to answer a call button, and they let you know that, pretty much, they would not respond to buttons during meal times. Not sure if the placement coordinator from Kaiser was right, but I would not choose to go there again. read more

Don't put yourself or any family member into this nursing home! Here is an example: My mother (93 yr.s old) asked someone to change her wet diaper. Staff member arrived to say the diaper was not wet enough in front of my daughter. I arrive, pushed the call button for both my mother and her roommate. We waited over an hour, so I went to the nurses station where they are all talking to each other (lights for help were on for several rooms). When the same staff member arrived, she YELLED at both my mother & her roommate for not calling sooner. Both ladies were cold from wet diapers, wet clothes and wet bedding. I complained at the office about this and her not getting daily PT (the reason mom was there). Next day, they released my mother who could not even use a walker to the bathroom. Kaiser said it was the only placement available. Shame on Kaiser for not offering better options. Shame on Delta View Post Acute (which has changed their name several times). Why are you still in business? This is not how to treat people in need! read more

I am a dispatcher with Bayline Medical Transport, and I have had a wonderful experience working with Delta View Post Acute. Their team is always professional, responsive, and dedicated to ensuring residents receive the care and services they need. I would especially like to recognize Ashley Arroyo and Brianna Gonzalez. Ashley is incredibly helpful with accounts payable and is always a pleasure to work with. Brianna consistently makes transportation coordination smooth and efficient, which is invaluable in our industry. Both Ashley and Brianna are knowledgeable, friendly, and always seem to have a smile on their faces. Their positive attitudes, professionalism, and willingness to help make Delta View Post Acute an outstanding partner. I truly appreciate the opportunity to work with them and look forward to continuing our partnership for years to come. read more

My grandmother recently stayed here for short-term recovery, and I am beyond grateful for the incredible care she received. I'll admit, when my family and I toured the facility a year ago, we weren't sure it would be the right fit. But after her most recent health situation, we decided to give it a try -- and we're so glad we did. From the moment she arrived, every single staff member was warm, welcoming, and attentive. You could truly feel how much they cared for the residents. I believe the compassion and dedication of the team played a huge role in her speedy recovery. Thank you to everyone who supported her healing journey with kindness and tenderness. I'd like to give a special thank you to: * CNAs: Ashley, Elvia, Satnam Kaur * Nurses: Leandro and Michelae * Physical Therapists: Darius and Delaney * Activity Coordinator: Maylon You all made a difference, and we'll never forget it. read more

0 stars! This place is awful and they will let your family members die here! My grandmother was in awful shape. With shit under her nails and everything ! She suffered a heart attack and no one even notice! Her sugar was high and d they were supposed to be checking it! Because of this place my grandmother is now in icu ! A lot could have been prevented if the staff here did their job! Even when the ambulance came to pick up my grandmother, the nurses there had no clue on information on her chart! If my uncle didn't show up I swear she would have died in this place ! AWFUL AWFUL AWFUL! The care is shitty, no one knows nothing. read more

My mother was admitted to Delta View Post Acute for a 3-month rehabilitation stay, and I cannot thank the staff enough for the care and support they provided. My mother does not speak English, which could have made her experience more difficult, but the entire team went above and beyond to accommodate her needs and make her feel safe and comfortable. Every staff member we encountered was kind, patient, and professional. From the nurses and caregivers to the physical therapists and administrative staff, everyone treated my mother with genuine compassion and respect. It brought our family so much peace knowing she was in such capable and caring hands. Thank you again to everyone at Delta View Post Acute who helped my mother on her journey to recovery--your dedication truly made a difference in her life and ours. A special thank you to all the amazing therapists--Nita, Marsha, Erica, Dana, and Makira. And to the wonderful nurses--Leandro, Tran, and Patricia--as well as all the dedicated CNAs, thank you for your care and support! read more

Please avoid taking your loved one to this place. My mother has been in constant struggle since being a patient there. Once healthy as a horse and having a minor pelvic injury, she is now suffering from her third bout of C.diff at a sister facility. The staff was negligent and stalled to get her to emergency when she was obviously having an issue. She contracted a staph infection (resulting in copious amounts of antibiotics) which then resulted in C.diff. I have had to live at the sister facility to make sure she is being cared for properly - at 101, the attitude is "well, she is old..." - BS! Much better facility, but it does not excuse what happened at the other one. Now I pray that she recovers, once again from C.diff, so I can take her home! read more

Communications. Availability of medication and nurse response time. Facility doesn't specialize in care of bladder infection and catheter and impotence of straps, copd and GERD. Have to advocate for yourself. Have to repeat same information to each caregiver many times. Food tastes good most of the times but no fresh vegetables or salads or fruit caused me constipation and pain that added to my uti inflammation and infection discomfort. Very attentive caregivers but don't listen to the patient. My pain medication took three days to arrive through Kaiser pharmacy as theirs doesn't carry it and anti constipation meds were too expensive. Have family bring from home as first solution. Mobility test is not taken upon arrival and a diaper is put on with BM checks which are unnecessary especially with catheter. I was weighted in a hoist which pushed pressure to my bladder and tubing that took two days to get better. A standing scale was implemented later. My iv plug the was put in at Kaiser emergency failed on and off with unnecessary pressure added to staff as infusions can't be skipped. My catheter was set up at Kaiser er but the holding taped came off tight while in weight host and not until talking to my Kaiser urologist told me that is has to be firmly secured to prevent internal movement as it causes pain if it's moving. On second day pt staff noticed blood streaks inside tubing and so did I. Urologist was upset with me for not advocating for myself. I do but it's creates unnecessary tension to have to remind everyone including myself not to be pulled out. I think slowing down and listening would eliminate staff patient friction. I also have side effects from entrepanem which staff has informed requiring continuous requests for cortisone cream to stop itching and sweat throughout the whole body. I also have emphysema and copd requiring at least twice daily nebulizer treatment. So far I only got two in 9 days. Sucrafatem med is to me made into water slurry to be drunk on empty gut as it coasts ulcers. It was administered together with other pills preventing full absorption of either one. I saw this at Kaiser hospital too which makes me apprehensive of when I receive my medications. Patients appear happy and I am glad to get an understanding cna and attentive nurse. Place is very busy. read more

4.8 is my rating. My husband arrived May 11th and is coming home tomorrow 6/6. He was there for physical therapy, occupational therapy rehab. The staff from the Social Worker Yanni, Director of Nursing Katherine, to all of the staff members who treated him so well and provided great care we are so grateful. Thank you for allowing my emotional roller coaster to be understood and always addressing and helping with my concerns. We are more prepared as we move into Hospice Care for Richard for all your kindness. read more

My stepfather stayed at Delta View Post Acute from May 16th through June 19th, 2024, and it was an utterly horrible experience. The nurses were extremely loud at all hours of the day and night, showing no regard for patients trying to recover. Despite being there for over 30 days, he made no improvement. Instead, he got weaker and lost over 30 pounds during his stay. They kept him heavily medicated in bed all the time, with no physical therapy (PT) or occupational therapy (OT) provided as promised. He even fell and got a large bruise on his head, yet the staff assured us over the phone that he hadn't hurt himself or hit his head. When we picked him up, after two separate appeals were declined, he was in a terrible state. Contrary to what we were told, he couldn't sit up, move, or even feed himself. My husband had to carry him to his wheelchair and then to the car. He looked completely malnourished and sickly. He was home for just a day and a half before he fell again and had to be readmitted to the hospital. From there, he was moved to Pleasanton Nursing and Rehabilitation, where he started to recover in less than two weeks thanks to excellent care and consistent PT and OT sessions. The nurses at Pleasanton were amazing, and their dedication made all the difference. He is now thankfully at home and on his way to a full recovery. I truly believe that if my stepfather had stayed at Delta View, he would have died from their lack of care. If you want your loved one NOT to be taken care of and to be left in bed to wither away, then send them to Delta View Post Acute. If you love them and care for them, send them elsewhere. I wouldn't wish this facility or experience on my worst enemy! I hope Kaiser who contracts with them actually visits the facility and hopefully rethinks their contract. If I could give negative stars I would have. read more

My husband just was released after 2.5 weeks at Delta View after a long and critical hospital stay. I was very pleased with the level of care he received there. The nurses and staff were very compassionate and he received very good care and therapy while there. He also thought the food was good, and nothing like hospital food he'd gotten earlier. One of the nurses I thought was exceptionally kind and helpful was Satnam Kaur. She genuinly cares for the welfare of her patients and their family members. I would not hesitate to recommend this facility to anyone looking for a skilled nursing facility. read more

I was admitted to Delta View on October 17th thru November 8th. I had surgery for a broken femur and broken humerus all on the right side. The PT and OT were phenomenal. Indy was amazing along with all the physical therapist. It was because of them that I quickly progressed. Same for my OT peeps. Shout out to my girls Donna and Colleen and all the OT crew. You were great! It's because of all of you that I am getting better every day. And a big shout out to the best nurse, EVER Ricky. I could always count on him. He did everything he said he would do and was very dependable and reliable. You should be so lucky to have someone like that working for you. He was so friendly with all the residents and the visitors as well! He knew everyone by name. He just has a very cheerful disposition. I felt taken care of in the best way. RICKY, YOU ROCK!! The activities team were helpful as well. Lastly, I would like to thank all the C. N. A's. That were so helpful and came to my aid. There are just a few. If I could give negative stars I would. My grandfather was there for over a month. In that time they failed to remove him from medicine needed to perform surgery which delayed his care. They left him alone long enough to get bed sores. The facility was filthy. The social services person is the most unprofessional rude woman I have ever come across. He was taken to the emergency room because they failed to provide proper care and the facility falsified documents saying he was eating when in fact he was not. He suffered from malnutrition and dehydration in his care. He was then taken into the hospital for a surgery and was presumed to come back and they threw out his personal effects. The social services person , Yani, has been incredibly rude to my family and I. She refuses to call us back and has the nerve to complain about the walk to her office to look for his items when I, and my family, suffered a major loss. This place is a joke and will only hurt your family if taken there. I hope whatever supervisor sees this discusses with Yani the need for professionalism as she deals with families going through the toughest time of their lives. If at all possible do not bring your loved ones here. read more

My father was there about 10 months ago and writing now because it still bothers me months later and need others to know about this place. The food was terrible, it smelled, equipment all over the place even room was a storage for ??? not him, was placed in room with 2 other people and one being a crack attic that kept him up all night . THIS IS NOT THE BAD PART!!!!. He was taken to therapy room for his rehab and he coded (heart stopped). the therapist just put a pulse oximeter on his finger forever waiting to get a number for Hhis oxygen in his blood when he wasnt breathing. I screamed "he is coding" no one came for about 3 minutes then his nurse of the day came in rolling in with an oxygen tank with no tubing as if she was just going to open the tank to the room and that would revive him without even touching him. NO ONE CHECKED HIS PULSE OR RESPIRATIONS as the therapist continued to try and get a number from a pulse oximeter. idiot didn't realize in order to get a number the person has to be breathing with a pulse and NOT COMPLETELY GRAY and unconscious. The nurse did absolutely nothing as I kept yelling for help and no one else came. He miraculously on his own regained consciousness and color. immediately I made a formal complaint and was told nurse was registry and would not work there again and he was returned back to bed. He happened (by pure luck) to be wearing a heart monitor as he was discharged from hospital wearing one for testing. it showed his heart had unusual episode during that brief time. I found out by calling monitor company as this place did nothing , not even call the company that would give them answers they needed to know. They reassured me they would keep close eye on him, they would inform doctor and for me to go home. That night I got a call from heart monitor company telling me that they could see that my fathers heart was currently stopped. i immediately called this place to tell them to run to his room that his heart had stopped. First they told me that the room number he was in did not exist. I thought i was in a bad dream so asked them to then look up by his name. They said they didnt have a patient there by that name. I grabbed my keys jumped in my car and kept talking to them while i drove from pittsburg to antioch. I had them tell me what the name of facility I was calling in case I had called wrong place. they confirmed I was calling right place. I THEN HUNG UP THE PHONE AND CALLED 911 and had the fire department / ambulance go there to help him. I had to tell 911 what room he was in because the person that had answered the phone at facility told me everyone was on break and she was the only one there and she said my father was not only not there they didnt even have the room number he was in at that facility. I got there same time as fire department (drove fast) and we ran to his room. AFTER, HIS HIS NURSE CAME. Fire fighters were the ones who helped him after calling 911 because no one who was there would help him even though i had told them his heart had stopped. he was taken to hospital after paramedics revived him and was in need of a pacemaker. He is alive today by miracle of God and fire department as this place would of killed him. I say by miracle of God because his heart had been stopped for about ten minutes and was able to get his heart started and breathing after that long period. DO NOT SEND A LOVED ONE HERE. REFUSE KAISER OR ANYONE TRYING TO SEND YOU HERE. yES, KAISER PUT HIM HERE FOR REHAB. This place should be shut down in my opinion. The CNA's could get a job anywhere as they were ok but the people in high medical education (RN'S) are only working here because no way a hospital would hire such incompetent people. Hell my grandmother who never worked A job would know that in order to get oxygen from an oxygen tank you would need a tube or to check a pulse or to check to see if someone is breathing or to know if you have a room number of a place you are working at or to know how to look up a patient there by looking up a name. My grandmother does not know how to do chest compressions but obviously either do the RN working here. By law in california all nursing staff working in these facilities have to know CPR. In this place NOT one person knew how as I clearly saw with my father. . read more
