My uncle is at this facility and Deja needs to get a raise. She answered all my questions regarding my uncles food needs and/ or restrictions and got me connected to his nurse so that I can speak to him quickly. She was able to facilitate the DoorDash delivery as well. I thank her for her help and patience as I sent my love from Vegas. My daughter did her clinicals here for her program and the things she told me... let's just say my heart hurts for the people in here that have no voice. TERRIBLE care, disgusting food, inhumane conditions and treatment. The nurses are incompetent, the CNA's are violent towards patients, yes violent. Petal, Don and all the staff at station 3 have been true angels. We knew taking care of our grandmother at home was getting harder by the day; both physically, emotionally and mentally but we tried to keep her as long as we could, fearing a facility wouldn't care for her as good as we did. Upon transferring to San Miguel Villa, we were pleasantly surprised at how well she acclimated to her new environment and how caring and attentive the staff were to her. When we visit, it is very obvious she is well cared for and loved, especially by Petal and Don, the love is very much reciprocated. This place made me very sad. The actual doctor is so disconnected, the whole facility smells of urine, the alarms go off all day, there is no check in system for guests, they cram 4 patients in one little room, majority of the nurse buttons don't even work, medications get intermixed, very dark and old building, we had to bring our own extension cord to power the tv. It's horrible, the owners of their place needs schooling in how to run a his facility. However, kudos to the nurses- they are so kind, and trying their very best to manage their job professionally. But how can they tend to patients at their fullest when they are handling 15 patients at one time?! The nurse patient ratio is ridiculous. Lovely picture of the lobby. It ends there. Their room for covid patients was 4 beds in a room that should have maybe 2, divided by curtains. Their records showed a cognitive condition that my patient did NOT have, maybe that's why they basically wrote him off. We brought dentures that they never gave him. They told me not to bring his hearing aid and processor (cochlear implant) for safety reasons, which made communication so difficult. They weren't honest in recording his food intake. He was supposed to have PT, but how can you when confined to a 4 person room? They said he was getting medication that they never gave him. We brought him home where he could get actual care, he was getting worse there. The paperwork on release we were given to sign was for a different patient. On his first full day at home he was sitting up, enjoying TV and eating. read more

The staff are very nice and friendly, they are also committed in giving the best quality experience for the residents and the family. What I like with the staff is they are highly engaged team that works together, making sure everyone is on the same page and getting things done. I also formed good relationships not just with my department, but with all members of the team as well. Recently had a loved one there for 2.5 months, ostensibly for rehab. While there he was sharing a room with 4 other men which, okay, it doesnt need to be a palace and there are lots of people needing a bed. They lost all the garments we brought for him after assuring us multiple times that they would handle laundering them. The food is all Sysco industrial food which is not the most unreasonable thing but not much effort going on. The facility was getting a facelift so near constant construction of one type or another inside and out. It looked more like a facelift, not anything new, so flooring, paint, new desk for the nurses etc. The staff onsite is kind and they try but I think they have quite a few CNA and very very few RN, doctors, or other skilled medical staff. The nurse practitioner that deals with the medications is in another state and we didnt see a doctor even once for example The more damning aspect of the experience is that he got Covid, which isnt surprising given the lack of hygeine practices I witnessed, and they stopped the PT. Then when he was out of isolation, they "forgot" to restart so he sat in his bed not getting treatment for over a month before we finally confronted staff about why he was declining and we discovered that he was not getting the rehab he was there for. Someone from my family was visiting almost every day due to what we were witnessing and the only reason I wouldnt absolutely rail against this place is because they do have a lot of low level staff available and around. If you need this place because you need to recover from a joint operation or something relatively minor then maybe it works as barely sufficient but avoid it if at all possible. Our experience was corroborated by many health professionals in the area we have spoken to. Long story short, the facility is not particularly clean, its very full of patients, and be prepared to aggressively advocate for your loved one. read more

All together 20 years. When it became San miguel villa it went down hill. The owner didn't care if the patients was happy or not. She just wanted the beds filled. I had seen 2 residents where I work at now that stayed at San miguel both patients were upset and stated they hated it there. Not someone expecting a luxury stay- this review is purely based on the fact this place has a self proclaimed 5 star review and someone needs to level set this places own ego. And disclaimer- maybe it is 5-star in this category, but folks still need to know what that really means... 1. Website is misleading - images are stock photos. NOT EVEN THEIR DOCTORS OR THEIR FACILITY! They are renovating, but it's strictly cosmetic. (Lobby, hallways, flooring, paint.) 2. Don't expect communication or patient advocacy (Kept my mom hostage, would not discharge her because they wanted to keep getting paid by Medicare. Forget calling the front desk to reach your loved one. The answer I received was "don't they have a cell phone?" Followed by "ugh, I'll have to walk down there to see if she has a phone" 3. Patient rooms are a 2 star rating if judging cleanliness and comfort like a motel/ hotel (The bathrooms are shared, and lock from the outside...so yes patients can... and my mom did, get locked in. The showers are shared - ZERO dignity or privacy) 4. Care coordination is WEEKLY. This means even if you are well enough to get out, you need to schedule a care assessment or you won't be assessed until the weekly round. That is very tough to do as a able bodied advocate, let alone a senior in distress. (Mom is of sound mind and no other medical reason she needed to stay, however, they literally told us she had to stay or "they wouldn't get paid") 5. If diet and caloric intake is important, food choices, prep and care is probably prison level. (I wish I had taken photos, but think soggy bland oatmeal and cold water for breakfast and mystery meat/ spiral noodles and beets for dinner) 6. Lack of basic care or meeting acute care directives. And forget luxuries like bathing or even being offered toiletries...or even you know...personal wipes) (Aside from my mothers lack of basic care (i.e. no wound nurse, "they are out with covid" so no one checks your wounds!) , the poor lady in the bed beside my mom was never turned and literally laid in her own vomit until my mother had to get up from her bed and flag down a nurse.) Do NOT come here or send a loved one here unless you are planning to visit everyday and advocate. This is NOT somewhere to come if you or your loved one is not able to demand what they need and facilitate their own medical plan. read more

Full staffing, lots of Maintenance and housekeepers to keep environment clean and safe. Fun activities for staff as well as residents. From the time I first walked in this facility as an employee to now.... I must say this place have gone a long long way already. Not only has the building changed for the better (and still ongoing), but more importantly the care and the Management are constantly striving to improve the overall experience of each resident. This facility is not equipped to take patients who are bed bound. They don't have purewick to help keep you dry from urine instead they put them in diapers and no barf bags. I don't blame anyone but the owner of this facility. The nurses and CNA are awesome. Physical and Occupational Therapy are great especially with the amount of time they spend with patients. I only recommend this place if you are mobile and not bed bound. read more

Please don't have your loved one go here. My Dad was referred to this facility as part of the VA approved network. He has a hip fracture. On several occasions during his stay they tried to give him the wrong meds, had called him another patient's name. He had no medical id bracelet on during his stay. Unsafe shower, moldy conditions, waiting forever for assistance to get help to the bathroom. He had requested a transfer and was promised that his paperwork had been transferred out for referral for over a week. They never sent his paperwork, only did we threated legal action. Finally on his last day there, my husband showed up to pick him up for the front desk to tell him, that he had already been picked up...but he wasn't ....so they really don't have a clue who is in their facility or what med's to give them. So my wife has been at this facility over ten years. Previous owner was better than this corporate owner . They have improved since taking over. But no way 5star , constant employee change . Can't keep really good professionals (nursing) ,&they got a couple! If you can't keep really good people then it must be pay and benefits. No one answer the phone at the facility at any of the extension they provide. I would not recommend anyone to send their family member to this facility. The current long term care facility taking care of our bed bound Dad as of this writing. The staff has been kind and attentive and has monthly meetings with the family regarding his condition and progress. The only negative is the building is old, and the atmosphere is a bit institutional. ( ie. echoes in the hallways, and the occasional screaming from other patients.) But then again, this is a a nursing home. i think for the price, this is as good as it gets. read more

Why is this place still open? They are horrible. My family member is dying and they treated him with disrespect and did not care what happened to him. He passed out on the floor and they were not checking on him like they were suppose to and he ended up in the emergency room in a coma. They could not tell anyone how long he was laying there before they found him. And after he recovered they tried to send him back to the facility and they had given his bed away with no explanation. They never answer the phone and do not check on the patients like they need to. it is a god send that he won't be going back there but what kind of place does this to innocent people who need medical attention. I can't express enough what it means to someone like me who gets the kind of support I get from the staff at San Miguel. Both my parents were residents of San Miguel. My mom passed away last year and my dad is still there. Management had a staff meeting and advised the staff of her passing, when my sister and I walked in we were greeted by staff with hugs and tears. Now, my dad is there and he is struggling with the end stages of his disease. My father is a tough man to handle, he's been hard on the staff. But, what amazes me the most is how positive the staff is! No matter the situation! So, thank you to the staff of San Miguel Villa. You have no idea how you have touched my heart. My mother has been there for almost a month and the communication with the staff is horrible. I was on phone on hold for 42min waiting to speak to a nurse and each time I called after that my calls are answered but no one says anything they just put me on hold. the CNA have NO bed side manners, also was missing a nightgown and the patient in the bed next to her was wearing it. One of the most difficult things I've ever had to do was accept that I could no longer care for my darling husband at home when his Alzheimer's Disease progressed so rapidly that he became a fall risk and my own health started to decline. The only "bed" Kaiser could find at that time was at San Miguel Villa in far-off Concord, a 90-120 minute round trip from our home. This is a very large nursing facility, very institutional in appearance, nothing like a nursing home. We were not prepared for it, but my husband immediately accepted its hospital-like environment and felt safe. While it does not look like a luxury accommodation, it is always clean and very well maintained and work on improvements is ongoing. SMV turned out to be a Godsend for us. The staff there worked diligently to accommodate my special requests, and they took excellent care of my husband given the limitations inherent in such a large facility. Though most were pleasant, efficient and caring, certain staff deserve special mention: Nursing supervisor Alison, maintenance supervisor Robin, LVNs Brenda, Maikapoe and Earl at station 3 and Steven and Abel at station 4 and Jennifer of social services, along with CNAs Brenna, LeShea, Valerie and Romulo. It took 8 months of weekly phone calls to closer nursing homes to finally find a new place for my husband. We are very grateful for this move, which makes it so much easier to enjoy more frequent visits with my husband, but we will never forget the kindness of all the staff at San Miguel Villa! read more

I see some of the other reviews and folks have given a bad rating saying it's a "sad or depressing" place. Well.....it's a nursing home. When you visit your loved one in a nursing home of course its going to be sad and depressing. My mom has been an Alzheimer's patient at San Miguel for awhile now and for the first time in a few years i haven't had to worry if she is "she ok". She's in a safe environment with an extremely friendly staff who sincerely care about the patients. The staff keeps our family informed on her condition, and the challenges ahead for our mother. We are able to visit without restrictions. The last facility my mom was at she just stayed in the bed 24/7. Here she's in a lockdown facility but is able to walk around, go to the activity/meal room, and see and interact with other patients. It's the closest thing she's had to normal in quite some time. I am so happy my Mom is at San Miguel Villa. The staff is friendly, exceptional and accommodating. The facility is clean and fresh. When I visit, as soon as I walk in it smells fresh and is very clean. They do lots of activities with their clients to keep them entertained. They invite me to attend a patient update every quarter to report on my Mom's progress and address any issues. Ginger O'dell is one of my favorites. She is the Activities Coordinator. My Mom and I adore her. She is passionate about her job and genuinely cares about the clients. She's a wonderful lady and a perfect example of the wonderful staff at San Miguel Villa. read more

While this has been a very sad time for my sister to lose her partner of 30+ years to Frontal lobe dementia, the day staff have been very caring and above and beyond the call of duty. I will start by saying, This is not the first nursing home I have had a family member reside, so I can compare to the other 3. We are pleased with the care at San Miguel. One thing that impressed me the most is that whenever there was the slightest change in his care, they would call me right away. That gave me piece of mind. Big closed off section for "memory care" for Alzheimers' patients, which (due to the generally poor facility and atmosphere) looks and feels like a prison. While it can be hard to place a loved one in any nursing home, I have been pleased with the care and attention my mother has been given at San Miguel for the last 9 months. The staff genuinely cares for their patients, and the facility is well run by a kind director. I have been pleased with the nursing and support staff, and have not had to worry about my mother's well being. She has dementia with behavioral challenges, diabetes, and incontinence, and the staff continue to treat her with dignity and respect. While the facility itself seems older, it is clean and has some outdoor patio space for the residents to enjoy, with a blossoming vegetable garden. There are weekly entertainers, and the food seems good. My mom has no complaints, and neither do I! read more

My mother is a Alzheimer's / Dementia patient and it has been a tough road the last 5 or so years, obviously for my mom but also on mine and my brothers families. Once it became too much for us to handle she went to a couple of different places but we never felt comfortable until we found San Miguel. The staff is friendly and caring, there are activates several times every day and we are kept well informed on her status. We live about an hour away but it's more than worth the drive for the peace of mind we now have, that we have not had in a long time. A nursing home is sad and depressing for the most part by its very nature but given everything involved and the big picture, I would highly recommend San Miguel Villa. read more
