This doctor, in all frankness, completely and entirely ruined my life from lack of necessary medication information. I do not believe my brain will ever recover, and I experience the unbearable effects of that each day. So much so that I have finally been approved for disability as I can no longer process information and work any job. I never attempted suicide in my life until I was under his care. I find this very telling. He is kind and obviously intelligent to have a degree, and he is compassionate-he listens to his patients. I do not believe he ever intended to iatrogenically destroy my cognition. My gut feeling is that I got so damaged by his medication management simply because of his ignorance of the life altering injuries the brain can endure when titrating or stopping certain medications. If it is any huge indicator of this, he once told me that stopping certain drugs I was on for over half of my life do not have any withdrawal. I was dumbfounded in his office hearing that. I'm certain he helps most of his patients very, very much and gets them back to functioning without any drawbacks. Unfortunately I had a very different experience. I am in my thirties and bound to my bed because there is very little I can do without erupting into monumental attacks of fear, distress and physical pain endured from a cold turkey withdrawal under his direction. My life is gone, and now I lay in bed all day practicing radical acceptance of a mind and future lost. It is an unbelievable hell that never goes away. Since seeing him I have undergone innumerable amounts of medication changes by other doctors, 3 hospitalizations, and various therapies including TMS, Spravato (esketamine), DBT and a few different therapists. Each to no avail. I am absolutely broken, and I believe it is by no fault of my own.

Everyone could possibly have a bi-polar type condition. He kept it real with me as well as himself. So regardless of who he is living with (it is shacking, not Shaking...) if he was living with a woman, then she obviously was separated. However, the front office staff and office policies have not been a good experience. I recently moved out of state and attempted to access my records via the patient portal and could not access them. Per an email request, I received a phone call saying I could complete a form for my new physician (I've already done this) and if I wanted my records I could PAY A FEE for them to be provided via CD. I have two overwhelming concerns: 1) I and/or my insurance company have paid through the nose for medical care for the past 8 years; and 2) who uses CDs anymore-- new computers don't come with a CD drive! What century are you in? Everything is cloud-based now and precisely why I should be able to access my records via the portal. As an example, I can access my Emory and Mayo Clinic records today (some are over 2 decades old and others over a decade old). This is a terrible business practice and nothing but a MONEY MAKER for this practice. I'm hugely disappointed in this nonsensical practice. And, I would also be concerned about a medical office SNAIL MAILING private, medical records. Isn't this a HIPAA privacy concern?

In January 2025 I made an appointment for the earliest new patient appointment, which was in June 2025. I decided to wait for the appointment because I really appreciated the philosophy found on the website and definitely liked the glowing reviews. I had positive interactions with all staff I encountered and the clinic was clean and bright. The provider I met with suspected a new problem/diagnosis and so started a work up with labs and imaging. Clinic staff did follow up with me once labs resulted out and referred me to a specialist. The ordered imaging was performed ~2 weeks after my new patient appt but I never received follow up, from the NP or anyone else at Ponce Primary Care re: the imaging results. It has now been ~3 months since my last communication, specific to my care, with anyone in this office. I decided to reach out to discuss the standard of care, which I felt to be lacking. Dr. Chad Costley did return my call within an hour or so and he did verbalize disappointment with my situation. He assured me that my experience is unfortunately the exception to their typical patient experience. I was informed that the NP I had been assigned to was "only part-time and here less than a month". I did not ever receive any communication from Ponce Primary Care to notify me that my assigned provider was departing or to invite me to call to schedule with another provider for seamless continuity of care. I was told that my imaging results were never sent by the imaging facility I went to and that "this is where the wheels came off". That is absolutely possible but it was also not confirmed to be true at the time of the statement. The report not being in the patient chart does not mean it was never received by the ordering office or sent to the ordering provider. Sometimes things get scanned and assigned incorrectly, or not scanned at all. Lots of possibilities there. To me, that shows a lack of accountability for not holding a supervisory review of a departing provider's caseload/EMR task list and it also fails to consider that an out-of-state fax number associated with the ordering provider could be the problem. I waited 6 months for a new patient appointment and, whatever the circumstances, was lost in the shuffle. I will not be continuing with this practice, although I'm sure they have many satisfied patients as evidenced by their overall rating. My advice if you wish to establish/continue care here: Be able to advocate for yourself and for your own healthcare. Be knowledgeable about when to follow up on your care in case the practice doesn't. If the wait is still 6 months out for a new patient appointment when you try to schedule, do not think an extra special or VIP experience awaits you - realize that they just might be too busy to accept new patients.

He has absolutely no idea how to treat a patient. I have visited his office 3 times. Well, three strikes and your out. He never does a physical exam and always orders a ton of very expensive xrays and tests (which are done in house) but never explains why he is ordering them or even what is wrong with you. I have never received a call regarding the results of any tests that he has "performed" but you can bet I have received plenty of bills. The last time I visited he seemed annoyed that I was even there and spent the entire time smacking bubble gum and ordering xrays and blood tests after he wrote me a prescription. He never even asked me what was wrong. Finally I asked him what was wrong with me. He just shrugged and said " I don't know, could be viral, could be bacterial". I asked him if he thought it was the flu and he said "I don't know I guess it could be. I'll write you a prescription for that." He didn't even perform a flu test.
