I've seen Trevor twice - both times as a discovery session client for different issues. Trevor is highly skilled, effective, intuitive, and dedicated to his clients. After the first time, my chronic low-grade pain I had had for over a that limited my range of arm motion improved and then completely resolved. The second time I saw him most recently for a lower back/glute issue and while I might need a more consistent care, I felt improvement after just the first visit and left the session with a set of video exercises I can do at home. The discovery evaluation is a great value where you can get assessed without any pressure to continue treatment. Trevor will make the appropriate recommendations without any sales pitch to get treatment with him vs your insurance. After my discovery evaluation, I found that Trevor is genuine and knowledgable. I was receiving PT through my health insurance prior to Form & Function. There was a huge difference in physical therapy care between what I received through health insurance PT and Form & Function PT. Trevor's approach to treatment is conducive to a great recovery. He also uses technology to stimulate faster recovery and healing. You are also seen on time without having to wait. I've seen Trevor for over 40 sessions including PMEF. I had severe spinal instability due to an accident a decade ago. (Hip dysfunction, sciatica, and lower lumbar degeneration). Trevor's PT establishment is in a class of its own; it is what physical therapy should Actually be about! I am sure most of the people reading this are familiar with "standard" physical therapy places: mindlessly moving an arm or a leg around, laying down on some electric pillow that you could probably get at Walmart, and doing bird dogs till the cows come home. Quite the passive and uninspiring experience. Well, you will Not be getting a passive experience at Form and Function, and the benefits for me, 7 sessions in, have not been just inspiring, but life changing (and I am not saying this lightly). I came in with a "baggage" of 20+ years of on-and-off sciatica, an imbalanced body with a twisted, unstable pelvis, random muscular weaknesses, basically, a body that had long forgotten how to move its joints and engage its muscles fluidly and in sync with each other. Well, Trevor took on the daunting (not to him though!) task of "untwisting" me, and of teaching me how to strengthen and engage, but also relax, muscles that had been either constantly rigid or basically unused for years. During sessions, he utilizes a multitude of hands-on techniques, which are exceptionally effective, as he truly has the gift of knowledge And intuition when it comes to body functionality. That is followed by customized exercises, which are such an effective and fun way to strengthen and mobilize the body, that I actually look forward to doing those daily. I've turned my living room into a home PT gym, and I am feeling and loving the way my body keeps getting stronger and more mobile every single day! Shout out to Trevor and his team Form & Function (FF). What I really appreciate is that I reached out to them and I forgot to follow up for scheduling, however, Cassidy followed up with me the following day (Thur 20 Jun) and scheduled the Discovery (Eval) appointment for that Friday. FF admin is very easy and they provide you an online account with a welcome onboard folder. At time of Discovery, I was 5 weeks postop from my Achilles surgery... and I was very impressed how Trevor listened to my concerns and with the treatment provided within that short period of Eval time. I went home with my Achilles feeling really good. The following day I went to a soccer game and there was this Porta John that was left on the trailer-hitch which was maybe 2 to 3 feet off the ground. I really needed to use the restroom so bad that when I went to the front, my instincts kicked in and I mistakenly lifted off my surgical foot (supported with VACOped Boot of course) and I thought I will feel pain immediately but I did not, nor did I felt pain throughout the day.... and that's what sold me to Trevor and FF. Today I am 8 weeks postop with having received three 1 hour sessions, and let me tell you my Range of Motion (ROM) has improved dramatically. After each session I go home feeling really good and most importantly in the morning getting out of bed my ROM improves, which gives me a whole lot of confidence of the treatment for a speedy QUALITY.... recovery. Oh by the way, when I arrived I couldn't stand straight, as of this morning though I'm at 90 degrees standing flat footed... and WALKING out of my boot steadily with minor pain FF has reasonable treatment packages, and their flexibility regarding payments is an easy process. Lastly, at my Eval I conveyed to Trevor not to take it easy on me throughout my treatment plan. Be honest/patient with yourself and transparent regarding your current condition with Trevor because you're in great hands... literally magical hands! Form & Function motto is.... STAY IN THE GAME! After almost 12 years I returned to Form and Function PT due to excessive chest and lower back pain. This time I had an MRI and X-ray of my entire spine. I've been going to Form and Function for a few weeks now to address range of motion and pain in both shoulders. Trevor is very knowledgeable and has been great to work with, using various techniques such as deep massage, PEMF, and cold laser therapy. I'm not yet where I want to be but I'm definitely improving.
