So much goes through your mind when realizing you need hospice care for a dearly loved family member. This was my first experience needing hospice care in my lifetime. I contacted CareCHOICES via email at 9 PM on a weekend evening. A nurse replied within 30 minutes. Within an hour, I was speaking to her on the phone. We set up the first appointment on a Sunday for a nurse to come out and evaluate my family member. The following day on Monday, the intake nurse came and did an evaluation and we signed on with CareCHOICES. It was an incredibly difficult time. But CareCHOICES nurses were always available. And would come out within an hour or two when needed. Or would have pharmacy bring medicines quickly. Every nurse was supportive, caring, compassionate. They offered an amount of support that we didn't know we were going to need but very much needed. I highly recommend CareCHOICES for anyone faced with this difficult situation and wants to give their family member, the best support and care that can be given. read more

This facility took a terrifying situation and brought my husband back to life from a back injury...he was bedridden for 7 weeks, and he is now back on the golf course doing what he loves but is more careful from the first visit with Paula who assesses the needs of the patient to Ernesto who was his physical therapist carrying through with the assessment, first class, top-notch and very reassuring and comforting. My Mom was recently being cared for by CareCHOICES prior to her death a couple days ago. I consider us so lucky to have chosen CareCHOICES as our hospice agency because they are AMAZING!! A nurse came by the house to care for my Mom daily, we received a ton of useful equipment including a bed, lifting machine, oxygen, pads, gloves, medication and more! The nurses were so experienced with someone who is dying, they have all worked there for years, they made sure my Mom was comfortable and they taught me how to care for her when they were gone. And, you can call them 24/7 with any questions, which I did several times and I was always able to talk with a nurse. I would highly recommend CareCHOICES as a hospice agency - they are truly the best and made this really hard time so much easier. They are the best. read more

My uncle ended up passing away on 4/13/20 peacefully in his sleep. He started hospice on 3/30/20, he went very quickly and had a peaceful, beautiful, and fast passing right after Easter. No comfort meds needed. RIP uncle ray. He got his wishes to pass at home and not at a hospital or nursing facility. I couldn't imagine him passing at the hospital with no family members around. During covid, they would not allow any family in the hospital. I can't thank Care Choices enough for all of their support from the beginning to the post end. Always checking up on me and my family to make sure we are okay. I had complimentary therapy which helped especially going through it all alone during covid-19 pandemic. I def recommend them to anyone that needs exceptional comfort care for their loved ones. I def recommend any family member to ask for the booklet "Gone from my Sight" (the little blue book) It's a sad but very insightful book that explains the stages and process of death. Outstanding Service, Compassionate Nurses, A-1 Experience. My mom was in hospice care for 18 months, and she looked forward to and appreciated each visit from the Care Choices team. They provided for her needs, and went beyond the call of duty many times, to gather info for the doctors that my mom continued to see (or zoom with), to help my mom during Covid lockdown for over a year, helping out with an amazing variety of things that needed to be done since no family member or friend could enter. The Care Choices nurses coordinated well with the assisted living staff, and actually lined things out for them when they missed things. The Care Choices nurses were kind, patient, thorough, smart and reliable. The office team was excellent and caring. There was one episode when an urgent request for a visit was sent to a nurse who was not on duty, and we needed to follow up with them on that, but it was a miscommunication with their team and quickly corrected. We recommend Care Choices whole-heartedly, and their kindness as my mom completed her journey on this earth is greatly appreciated and will be remembered. read more

We just switched to CareChoices Hospice for my Father who is in his late stages of Alzheimer's. So far we have had a very pleasant experience and the prompt responses from the team at CareChoices. it has only been 5 days since we started and already we have had the Chaplin (Chip), the Social worker (Gregory), the case manager Nurse (Zorana), and Nurse (Carol), and the Health aid (Xochitl) visit us and they are all very kind and the communication is excellent! My brother and I chose CareChoices after careful thought, research, and a strong recommendation from a friend who had used them relatively recently and was very happy with them. Unfortunately, we did not have a good experience. The hospice contact was getting into arguments with our caregivers and was trying to slip business cards of some other caregiver. Was she getting a kickback from these other people? Not sure, but it didn't feel right, and made an EXTREMELY stressful time even worse/harder. We would have been finding another hospice provider if my dad hadn't passed away. I'd look elsewhere. read more

My experience having to go into this time with my mother in her last days was horrible and very emotional not knowing anything cause it's not something anyone would ever think of being faced with until it's right there. Although being something that couldn't be reversed my mother made a spiritually sound decision to go into the care of Care Choices Hospice and even though I prayed for my mother to get well and healthy again it just was something that wasn't happening and as I stayed watchful every minute for any kind of sign that things were gonna be okay it just wasn't and all along GOD had a different plan and was able to work through a lot of the care team with Care Choices namely Gregory and the Chaplain Garrett who came and held the ceremony for my mother at her celebration of life gathering where family co-workers from Optum health and friends joined to share all the great memories and moments of love and with my mother to always be cherished. What an invested provider / team of people who truly care about what they do in the sense of helping someone who is facing this end of life moment and in helping one to be in absolute peace and understanding that they are going to a better place. read more

Odette one of the representstives makes sures every single detail is handled to our expectations. After every visit the Medical Staff gives us a written report for the resident. We love Odette, she is a sweetheart. One of the owners Ed a philanthropist just won an award with a Local organization. He makes sure everything is in order. I can't recommend Care Choices highly enough! They took care of my mother when she went on hospice about five months ago. They took fantastic care of her (and she could be extremely difficult). Every single nurse who came was top notch, without exception. I mean that! Thuy, her case manager and RN, was absolutely incredible...caring, efficient, compassionate, and able to navigate my mother's moods, and desire to control things. When my mom needed something last minute, or an emergency came up...they came through and responded promptly. I am so grateful for their care for my mother in the most difficult days of her life (her final days). It chokes me up to think about how well they handled this most trying circumstance...and I'm a guy! Care Choices cannot be trusted. I am shocked by the lies and deception carried out by the company's social worker Linda. I now find myself unemployed and homeless due to her actions. I have been my mothers caregiver for 15 years and this has all suddenly changed. Recently Linda along with my brother Emilio lied to both my mother and myself. Linda insisted my mother needed medical attention and needed to go to a nursing home. My mom did not want, she refused this but Linda promised this was temporary and would only last a month until her wounds healed. Well when the day came for my mom to be transferred, Linda revealed that my mom would actually be going to my brothers house and will no longer be returning to our home. How can this be? Who approved this decision? And why was there no transparency? Linda failed to communicate the truth and never called me to inform me about this. I had to hear it from Emilio who said Linda decided my mom will not be returning to our home. I had to hear it from him? My mom is under my care I should've been informed directly. I lost my job as an In Home Support Service employee because my mom has been taken away and is no longer under my care. Emilio is not prepared to take care of my mother as he has no experience as a caretaker and does not understand the special care my mom requires. I have now determined that he is only interested in taking care of my mother for financial gain and money interest. How can Linda decide that this is whats best for my mom? Emilio is not qualified to care for my mom and that is concerning. This has been a terrible experience. I would like to file an official complaint against Care Choices and Social Worker Linda. I am in contact with a lawyer who will be assisting me. Please have management contact me directly. My name is Eliberto Ramirez. I would not recommend this company for any services. Care Choices Hospice is wonderful support for patients in declining health and for those in the last days of life. They are professional, knowledgeable, and compassionate. They get 5 stars plus! For the past 2 years Care Choices staff lovingly and faithfully monitored my mother's health and tend to her skin wound care. Her nurses and care givers, Zorana and Tiffany, also took the time after each care visit to update me about my mom's care by a phone call or text. They gave me the confidence that Mom was carefully being monitored and cared for. Dr Jason Abney, her primary care physician, always spent time and gave personal attention. In her last days, Dr Nguyen (her palliative care doctor) came everyday to visit her and comfort her family.   During her last week and a half she was on around the clock Critical Care. Every single caregiver had a special gentleness and passion to care for the dying. I was so impressed....I can't remember all their names, but want to thank each one with all my heart. They gave my mom comfort and dignity, and gave our family support and compassion. When you need hospice care for yourself or a family member, there are many hospice companies to choose from, but from my recent first-hand experience, I highly recommend that you select Care Choices. Highly recommend to seek another hospice agency. The chaplain has the worst bedside manners. When a family member passes you would expect empathy and sympathy, nope not this hospice agency. Save your loved ones the pain that this agency put my family through. All I can say is this group is beyond professional and I will give them not a score of 5 but a score of 10 plus for everything they did and their commitment. Rhonda Taylor was my Mom's Pallative Care Nurse she was a blessing and pleasure to work with. My Mom had Cancer and started having seizures and I had to make the decision to put her into Home Hospice with them for 24 hour care the last 3 days of her life. I want to thank the LVN's and Nurses who helped her Athena, Rose, Whitman, James, Darrell and Ruth. They helped make my Mom's transition more peaceful. And a extra special thanks to Chip the Chaplin who anointed my Mom and blessed her two times before she passed. Care Choices is by far one of the most superior Hospice organizations in Orange County.  Your company goes above and beyond where some of the others do not. I have never seen the level of care and concern given to patients like Care Choices provides. You are always willing to treat the common infection and provide intervention when it is really needed. The worst agency! Jenifer or however you write the name of this "nurse" it's the worst! She lied about my boyfriend (the patient) and "the company" didn't call to make sure it was true! They just decided to cancel and now we have to find another company! Do you think guys we have time to do this shit? My boyfriend is sick like everybody that takes your services! Carechoices showed compassion to both my mother and our family in a very difficult time. They had cared for mother for several months and always informed me of any changes in her status. When her health declined they assigned continuous care with 24 hour hour bedside nursing care. They kept her comfortable in her final days. All of the care givers seemed very sincere in the physical and emotional care they provided. We are especially thankful to Mimi who worked several day shifts and just happened to be the caregiver assigned the night my mother passed away. We were comforted to see a familiar sympathetic face. My husband recently had 19 days of hospice care with this company. While there were some very good direct nursing/home health staff experiences, their medical director, Mohana Kulkarni, M.D. made an initial medical error and refused to stand accountable for it or agree to be collaborative with our private physician, so it could quickly be resolved. This brings into the question Carechoices overall leadership and adherence with their own mission statements, so you may want to research other companies before making such an important choice. Additionally, it created a lack of trust and comfort on both sides---theirs and ours---which should not be a factor when your loved one is dying. Lastly, communications and supplies sometimes lagged, as did follow-through with the social worker we were assigned, but never heard from to schedule our meetings. read more

We contacted St Michael Hospice on the recommendation of two of our doctors. Within an hour of calling we had a nurse come to the house to explain what hospice is (and what it is not) and to walk my father through the process. He was dying from a very painful cancer and his oncologist had not been forthcoming with his prognosis; everyone in the family was trying to help and being ineffective and worn out in the process. But the hospice nurse spoke plainly and made it very clear what he ought to expect so we all could better understand his needs. Within hours of calling them we had O2 and a wheelchair delivered to the house. Anytime we called for help we had incredible fast service with medications, equipment, supplies, or an RN arriving speedily to ease whatever situation might arise. Hospice is not a place or a facility--- St Michael provides in-home care and the cost of it is completely 100% covered by MediCare. The nurses who run this organization are the best in the business. You still need caregivers at the house (and this does cost something) but the hospice people work with them and with the family, to provide the patient the most comfort and dignity possible. I only wish we'd called them sooner. They made the my father's end of life transition much more comfortable, and provided support to him and to us. After my father passed they assisted in all aspects of what to do next and have continued to follow up offering support. Yes of course you want professionals who know what they are doing, and we found everyone involved to be total pros, but most important they are kind, and they care. These people are angels. Final note - Mom passed away peacefully in her sleep. We know it was the right decision to dismiss CareChoices and hired a competent and compassionate hospice. My mother's suffering ended when we made that decision. The only regret I have is not firing them sooner! read more

St Michael's, now CareChoices...WOW. When my Father was dying in 2013, he had the opportunity to have Hospice care. He said NO. "Having Hospice means I give up". Actually in retrospect, I wish we'd signed him up for it as it would meant a number of things, one of which would have been that someone would have been by his side the moment he left this planet. However, I will not question respecting his wishes. After he passed away, I took full charge of watching after Mom so after some discussions with the head of nursing where she was residing, following her stroke in January, I asked for them to bring in a hospice group. Being named for St. Michael...well there was no way I was NOT going to get this group to help with my Mom. They were amazing! They visited her, made sure the Chaplain came and prayed over her, took great care of her, and the morning of the 17th at 3:24am when she passed, the called me to let me know so that I could come to her bedside, pray for her and then turn her over to the funeral home rep when they arrived. I cannot say enough great things about the levels of care they provide. About the human touch, about the respect, honor, and dignity preservation they put into place around my Mother. Mom had the red carpet and then some rolled out for her to walk the stairway to the sky, although I've been told she was flown on wings of Angels to see God and be let into Heaven. These people are amazing. If I had to do things all over again, I'd have engaged with them earlier and allowed them to help Mom even more than they did, but they say timing is everything so in this case...Perfect Timing. I highly recommend this company. They were so respectful of my father! They made me feel comfortable with the care my father was going to get and then received. No question was too small or unimportant. This is the type of support people need when experiencing severe illness and the process of dying. I wanted to take the time after my mom's passing to thank the people of CareChoices for everything. It was such a difficult time, I had such a negative perception of what I thought hospice was, and I could not have been more wrong. Everyone who was assigned to my mom was kind and compassionate and thoughtful. They were on sight 24/7 in a small apartment type room with my dad. You would have thought that would be uncomfortable but it was exactly the opposite. They checked on my mom and allowed my dad to relax and not worry and be able to leave the room. My mother just spent the last two weeks utilizing the hospice services of CareChoices. During her last week, she received 24/7 care from the most wonderful nurses I have ever met. They not only made sure my mother was clean and comfortable, they were comforting to the family as we sat at her bedside. Care Choices provided Home Health care for my sister from the time she got out of the hospital until the day she went into hospice. The nurse that helped us for 10 months was so caring, kind, and supportive. She was so important to us on this journey. Every person that we dealt with was caring and professional from the nurses, physical therapist, occupational therapist, social worker and chaplain. There are not enough words and not enough space here in this little review to express our family's gratitude and appreciation for the staff and management of CareCHOICES. Both of my husbands' parents were cared for during their final months by the team for North Orange County. I wish my own personal health care coverage measured up to the quality and integrity we saw EVERY SINGLE DAY. Our parents were afflicted with end stage Alzheimer's and heart failure and both prone towards bedsores. If we had any concerns or questions, answers or nurse visits were only a call away (24/7). Moving to hospice from an HMO made their care so much easier. The doctors and nurses came to their home and our struggles to transport them for care were solved. Also, several times a week a wonderful woman visited to sponge bathe both-which made them very happy-baths were near impossible even with a walk-in tub. The needs of the family were also taken into account with both a social worker and a chaplain on the team. My praises could go on and on, but you get the gist. CareChoices was absolutely negligent with my mom before she passed. She was in agony for hours. They came and did the paper work promptly, but then didn't deliver morphine for many hours while my mom suffered, (she was having a heart attack). Now my father's good friend is suffering, in pain, and unfortunately, also has CareChoices for his hospice. They send him a nurse once a week. Not acceptable. This is the worse hospice company I've ever run across. Avoid it at all cost! read more

If i could give zero i would!! In the initial evaluation, my mom (81 years old) who was starting home health was promised everything: OT, PT, RN for wound care, aide for bathing, social worker... the case manager absolutely dropped the ball on everything!!! I havent heard from her and my mom has only been bathed ONCE by the aide in two weeks!! The wound care order specified daily care and this agency only noted twice a week. There is no follow thru by case manager, no communication between disciplines, and no sense of care whatsoever for the patient. So disappointing!! I would absolutely NOT trust this agency!!!! They don't care about the patient. Ive had to make calls every day to gety mom the care she was promised and ordered by the doctor. Very upsetting!!! read more

Care Choices' intake staff was wonderful and took the time to assure my siblings and I that they would be there for my mother and make her journey into death comfortable and peaceful. Unfortunately they abandoned her in her final hours at her assisted living facility, with no medical staff, equipment or medications to relieve her from choking on her own fluids and gasping for air for hours, before my eyes until she died. I called Care Choices' office several times begging for help but was told matter-of-factly that they did not have the staff to come out and assist and would not for hours. Seeing my mother in such pain and distress in her final hours and moments, feeling helpless, and my pleas for help being dismissed is a Worst-Day-in-my-Life scene that replays in my mind daily . Like so many others, I can't say enough good things about CareChoices! I was so unset about my mom when she had to be taken to Hoag Hospital (another 5 stars for them). First to emergency and then admitted. I was advised to look for a hospice agency. I meet June at Hoag and she explained everything to me and took over. CareChoices was so efficient at everything. They took care of my mom and of me. I wish that I had written everyone's name down so I could thank them. It was a difficult emotional time for me.
