This is one of the best nursing homes in the state of Oregon! I've grown up around many of them because everyone in my family besides me is old, and/or work in the nursing field. So it's taught me what good and bad nursing homes look like. The level of care given here by the nursing staff is amazing. When my mother had a stroke and was leaving the hospital for a nursing home, I asked which one they were planning on sending her to, and they gave me the name of one I despise, I said you'll send her to Riverpark or home with me to do outpatient care. It's just that good. One of the things I love about this facility is how clean it is. There's never an overwhelming smell of urine or feces coming out of rooms as you walk down the halls or sticky floors, it's always clean. The food is actually pretty decent. Which surprises me even to this day. I had a fear of nursing home food for many years after going to a different facility as a small child and experiencing terrible food. This is literally the one place I can eat in a nursing home and enjoy it. Their physical therapy department is absolutely wonderful. They really care about their patients and getting to know them as well as getting them back to normal/optimal performance. I've had multiple family members stay here as patients after falls, strokes and much more. I know the level of care given to my family is exactly how they treat all of their patients and it's impressive to me. read more

I'm giving this a 2 star because most of the staff were cool! But about a week in, i noticed my stomach hurting bad! I threw up in front of a nurse who did nothing about it! I begged for bowel care {suppository} and was ignored! Finally the pain in my stomach got so bad, i asked to go to ER. at which point i was told it's prob menstrual pain! Although they didn't deny me Access to ER, they didn't encourage me. I finally went to ER AND HAD FOUND out my hernia perforated my bowels, causing my large intestine to die off! Was in ICU 2 days, severe surgery, wound vac, etc! Riverpark needed to be more compassionate! I'm now going to another rehab. read more

This facility has lost its way badly. The internet has been a problem for years, but now it barely works at all. If you have a game system it blocks it from the internet completely so I can't get any enjoyment any more. The staff has to stay late or just not chart on the patient because it is so bad. Now the nursing is terrible here, only a few nurses are holding this place together and get no appreciation for the hard work, while the rest are calling out not or doing the care needed to the patient. Cna's are lacking too, over worked and little training in new ones put by themselves. The rooms are filthy and the family/smoking area is a dump food all over the ground, tables dirty, the housekeeping have no leadership and no drive to work. Now I have a pressure that got so bad bone was exposed, from the staff not informing me on the regression. Now I am trying to heal one extra wound stuck in bed without the ability to play my game system. I hope your loved ones get the care they need. read more

UPDATE: To my review of Avemere Park on 2/14/2017. My visit with the daytime manager revealed that my mother was admitted here on her insurance instead of as the Medicare patient the hospital had sent her as. Had she been properly classified by Avamere she would have received twice as much physical therapy as she was getting. I was unable to get the manager to admit the mistake was theirs, which I know it was since I was standing right beside the social worker at Peace Health when she made the phone call and filled out the form fto have my mother's transferred as a Medicare patient. I started cleaning the room myself and made sure my mother got a shower each day and had her hair done. I brought outside food in, paid for out of my own pocket, for her and her roommate within the guidelines of each woman's dietary recommendations and restrictions. I stayed there morning, noon, and night and got my mother out 2 days earlier than originally scheduled. PT: The therapists in the PT room were great. They gave my mother confidence and made sure she clearly understood each action they instructed her to make. However they were older, more experienced people (some with their masters degrees) who knew something about motivating the frail elderly. I watched them with a number of patients, not just with my mother. The poor lady who was her roommate was so obese and out of shape she could barely do any of the moves the extremely young and hesitant in-room PT person instructed her to do, even when she was laying down. She was a very bright woman and a retired registered nurse herself. Rather than encourage or demonstrate how she could begin to move as instructed they just quit and moved on to the next thing she couldn't do. I watched as they charted their "session" and left her no better off than when she was admitted the day after my mother arrived. But I have no doubt her insurance company forms showed she'd "received physical therapy" and were charged accordingly. Evenings there were small groups of young CNA students who were led on tours and shown various patients as part of their schooling. They were young and eager and it was sad to see how much goodwill they had while in training, knowing that if they ended up in a place like Avamere they were going to be in an environment of don't care-don't ask-don't bother. One in which they will never be allowed to do their best work for their patients, until finally they'll just get jaded and indifferent. When I had my mother discharged her roommate wept. We felt bad for leaving here there. All of their employees smoke cigarettes in front of my apartment building (full of children and families) and park in our apartment parking. Staff is rude and unhelpful. They also tow vehicles from their lot, which has not one sign posted indicating any tow warnings or parking restrictions. Take your elderly family members elsewhere if they treat them any way like they treat everyone else! read more

This facility is always short staffed and they leave 25 patients to 1 Certified Nurses Assistant. Although there are some great nurses who care, the team is overworked. Some nurses have a tremendous lack of compassion and neglect their patients unless you are present. Sadly, I have witnessed what goes on here and it is an absolute disgrace. My friend has now passed and his life could have been prolonged if his wounds were cared for more often and if they were caught sooner. read more

This place is horrendous! Looks are extremely deceiving. Upon waiting for my loved to be admitted (again) I advised the Admissions Coordinator (R.A.) of prior problems with poor care, no care, poor quality of food and overall neglect. She rudely responded with "don't come in here expecting problems". I said I am not expecting problems I am trying to nip them in the bud before they begin. My family member had been overdosed by their sister company Hospice agency, Signature Hospice. Not one person would listen to family or friends concerns. One nurse Suzanne was not only rude and condescending, she yelled at family when issues were reported. She would be repeatedly asked to check on sores and hours later she would just leave to go home without checking on the resident. One time when family requested that she look at the wounds that they caused on prior admit, she yelled "I just did that an hour ago!" She did this right in front of Social Services Jamie(?) who did nothing about this woman's attitude. Needless to say I said it needs to be checked now, there is blood all over. Nurse Suzanne did not check the wound(s), she just went home. Another staffer said this is her normal behavior. Just days before a very nice Nurse Melissa was cleaning the wound and changing the dressing and she said what the? This didn't look like this the last time I came in here. I said that's because Suzanne is advised of issues of wounds and does not come in to address them, she just goes home and leaves it for some other shift to do. Expect your personal items to be stolen by staff and residents, even if you are in a room by yourself. This was reported to Social Services so they could retrieve the resident's personal property, but they did absolutely nothing. Social Services did nothing when our family member had a hot drink spilled down their back and was never cleaned up. Our grandparent was there for a short period and was scheduled to go home, but then we get a call that he mysteriously died. Constantly understaffed, forcing what staff they do have work double shifts and sometimes with no days off. They are a teaching facility and they do not tell you that, nor do they tell you that you can refuse to have students in the room. You can have two extra people standing around gawking at your naked body while they do cares taking away your dignity and pride. They have non-certified staff caring for your family members, some of whom have violated the residents. If you have family in any of these Medicare/Medicaid homes you need to be diligent about being there for a least 6 hours per day and come in on different shifts. You need to log incidences, who your reported them to and time and date and most importantly, take photos of your family member to document neglect and their decline. read more
