Now, as a combat veteran with disability (PTSD) and having a service animal, came to get service at this clinic for unrelated treatment. When it was my turn and my name was called by one of the treatment staff, I felt disrespected by this individual (James) that approached me on my appointment day at this clinic and had the audacity to ask me if I have a certificate for my service animal. Although, we have been in this clinic before, and no one cares about it. This individual must know that PTSD is not a sign of weakness it is a sign of Absolute Strength, PTSD is earned by doing what others Fear. So, to answer this ignorant staff member (James) question the answer is: No, a service animal does not need to be certified, registered, or licensed under the Americans with Disabilities Act (ADA) in the United States. What the Law Requires *No official paperwork: The federal government has no official test, registry, certification, or ID card for service animals. *No professional training mandate: Owners are legally allowed to train their own service animals. *No gear requirements: Service animals do not have to wear a special vest, cape, tag, or ID badge. *Task requirement: The animal must be individually trained to do work or perform tasks for a person with a disability. Online Certifications and Scams *No legal weight: Many websites sell online "service dog registrations" or certificates, but the Department of Justice does not recognize them. *Not allowed as proof: Businesses and public places cannot legally demand certification, ID cards, or special proof as a condition of entry. Allowed Questions If it is not obvious what service an animal provides, staff in public places are only allowed to ask two questions: 1. Is the animal required because of a disability? 2. What work or task has the animal been trained to perform?

I've seen Umair twice now for stretching, and he is incredible. He's professional, knowledgeable, and somehow knows exactly how to move and stretch my shoulders and arms in the most effective way.     Yelp needs a zero stars rating. Setting the appointment was…read morenothing short of a nightmare. It's standard to get X-rays before the appointment. They assume you know this. They don't ask if you already have X-rays. If they return a call, one person says, "hey it's me". If you return someoneo call, and tell the person, FOUR times, the name of the person you are calling for, she says, "I need to find out who called you". I witnessed a nurse giving pre-op and post-op surgical instructions in the waiting room! I know what pain meds his insurance will cover pre-surgery, they will give him something after surgery and immobilize his arm. He can use his current PT or a new one after surgery. This is all a HIPAA violation. There were 5 people in the waiting room who witnessed this interaction. I have severe medical anxiety due to medical trauma and supply a FULL, typed medicial history and ways to lessen my anxiety during the visit. It states to "read entirely before engaging patient". Nurse taking my BP missed the memo. She tried asking about allergies. Referred her to my paperwork. She asked another question; same response. She spoke to me during my BP reading (not supposed to). Met with the doctor. Went as well as expected. His attitude was apparent and he had no intention of assisting me. He can't understand a frustrated tone. He was more interested in chastising a patient in pain. If it didn't involve invasive procedures, he refers you back to your primary. I walked out angry and upset. Unprofessional office. I recommend avoiding this provider like the plague.

This is a BUSY practice with only two ortho docs so sometimes it is difficult to always see Dr. Centeno ( there are competent physician assistants) but with planning ahead it can be done.

When…read moreI began acupuncture in October 2024, my high blood pressure was 192/97 and I was dealing with low range of motion and pain in my right shoulder due to pulled/strained muscles. I had recently weaned myself off high blood pressure medication because of non-stop persistent coughing, and I was really scared. Dr. Shawne and her staff was very compassionate and listened to my concerns. Dr. Shawne assured me that with acupuncture, excercise, meditation, tai chi, and diet, my situation would improve. She also ordered a vitamin routine for me, which I began immediately. I continued with my acupunture treatments until the end of January 2025, when my high blood pressure reached l35/64, and my range of motion became normal. Everytime I lift my arm to grab something, I am so grateful that I was able to be under the care of Dr. Lopes. She is amazing and has given me back my life! Yvonne V.
