After 3 wks, I was told on phone your husband expired on Feb 7th. They had him on Albuterol nebulizer treatments he never in his life needed. They knew I told them never to put him on such treatments but they had already begun. Don't walk, run. read more

My mother-in law is in rehab here and we are so grateful. Because we have aging parents, I'm very familiar with many skilled nursing places in the Bay Area. Vineyard is so much better than most. My husband was here 2 weeks recovering from hip replacement. he needed help 24/7 was a fall risk yet they never came to help him to bathroom even when they told him he should not do it himself. They weren't on top of UTI kept saying current anti biotics needed to work I fought it for a week. After he got home he was out in another anti biotics needed because one he was on wasn't strong enough. Food was disgusting and my husband eats anything but not there. Rooms were filthy trash on floor. When he would ring for help it took over 30 minutes to get help. He had blood in urine I had to keep telling them. Not sure why they couldn't see it he had a catheter. Also they were not on top of dosage of meds. I wouldn't bring my dog to their facility. It's a shame. I had to come every day to make sure he was getting attended to so as a caregiver he has Lewy body dementia it was not a break for me. I had no sense of relief that he would be cared for there just the opposite. read more

My first thought is that they must be getting government money. People are 4 to a room, less than 6 feet apart, with only a curtain separation, televisions blaring, people screaming, and a lot of dementia. This all appears to be normal to the staff. The place is filthy. Food and crumbs on the tray table, in the drawers and on the floor; dated, broken furniture. We jsut got here. I removed my family member AMA when they couldn't get the medications straight, and I couldn't leave the family member there alone. Not a place you can rehab and heal from. We thought we were getting PT to get better. We did not stay 6 hours. Half of that time was leaving. read more

I visited my loved one often before they passed away, staff knew me; so then why didn't I get notified of their passing?! Why don't staff reach out for condolences, acknowledging our loss?! My loved one was miserable there before they passed; didn't have nice things to say about the unprofessional, rude, pushy staff. I didn't appreciate the EMT always hitting on me, when I declined ect. Nothing was done when their personal belongings were stolen. They were denied many times, of things they needed. Needless to say, I will not be making the mistake of sending another loved one there. read more

Everything! This facility has a lovely lobby but go beyond the lobby and it's dirty, poorly lite, the food was terrible, grilled cheese on while bread with an apple. The bathroom was dirty and I saw no rebab personal. They did it was Saturday so no professional rehab staff would be in until Monday, not acceptable! I am disappointed Kaiser recommended this facility. I'm still traumatized. read more

Terrible. They weren't watching my dad like they should've and he slid out of his wheelchair. I really regret having to send him there but I was sick and unable to care for him. I would NEVER send him there again. Please don't send any family there. Plus the communication is terrible. My dear friend has been a long-time resident at Vineyard Post Acute for almost three years. He has advanced cognitive issues and is bed-bound with fluctuating awareness. The care during the pandemic was extremely challenging with low staffing done primarily through registry temp CNAs and nurses. There were many issues with his care during this time. Communication is often challenging as they have frequent turnover with their Director of Nursing and it is hard for staff to keep up with all the notes in the computer. It has gotten better over the past year with regular staff CNAs assigned to the same halls so they get to know the patients. Nursing care continues to improve as long as regular assignments are also made with dedicated staff nurses. There are some exceptional staff including the wound care nurse, the maintenance supervisor and some CNAs with regular assigned shifts. Overall the food is adequate though they could improve some portion sizes. It is essential for any resident here to have an outside advocate to help resolve issues as it makes all the difference in achieving care that is satisfactory. Overall, I would say Vineyard lands somewhere in the middle of the spectrum with the potential to continue to improve with vigilant oversight and core team implementation of humanistic values. It is always the staff who are truly dedicated to serving the residents who make all the difference in providing compassionate and above average care. read more

I spent 2 weeks in this hell hole and was glad to leave when I did. The only redeeming thing about this shabby place are some of the CNA's that work there. CNA's are notoriously underpaid, undervalued and overworked. But a few of these folks are wonderful. Thank you to those of you who worked with me and got me cleaned up and fed. The office staff is bad. They don't listen, don't seem to care and are quite condescending at times. The kitchen staff seemed like slackasses to me and they don't really pay attention to dietary needs much at all. The place needs a good cleaning because there are splashes of fluids on walls and they don't seem to wipe everything down at regular intervals. They lost several articles of my clothing and never found them or replaced them. I went many times without eating because they couldn't get my food correct, EVER. Plus it wasn't good and I even had to order in at one point. Spent most of my time there in the most uncomfortable bed I've ever slept on and that's because the rehab portion of my stay was inconsistent and inadequate. Though I must say there are some cool therapists there. But they can get ify too. Some CNA's didn't clean others or myself well at all and when I left there there was an outbreak of an infectious disease. I had to be put in quarantine when I left here and went back to the hospital. There was even many issues with my medicine. Dosages being not given on time, wrong meds at the wrong time and even missing meds! They told me that I was out of one in particular, I should not have been out of it because I didn't take that much of it at all. How do I know? I documented everything that happened to me while I was there. This place stressed me out so badly during my stay that they re-agrivated my condition and made it worse! I even tried calling an ombudsman while I was there because I felt so mistreated. Unfortunately the office was closed during the holidays so I just had to deal. It was literally a nightmare for me the entire time I was there. I'm about 20yrs younger than the average client age and this was my life for 2 weeks. I heard things from residents and staff that I can't unhear. I can't imagine what the residents have to deal with day in and day out for years... If you love your family, don't place them here. They will hate it. There's much more I'm leaving out because I don't want this review to be the longest ever, but I hope you'll reconsider based on reading this. Just know we picked this place because of the glowing reviews online and on Yelp, but it is every bit the opposite. BEWARE. My stay was in Dec-Jan. 2019 Absolutely horrible. read more

My Sister had to do some rehab there from a bad fall, with a broken shoulder and other injuries. They were very accommodating and took very good care of her. Their staff consists of RN's and CNA's, which are professional and very kind and helpful, going the extra mile to care for elderly, sick patients. The food was fresh everyday, and they try to accommodate requests. The facilities are clean, lots of hand sanitizers and masks, and they really take good care of their clients. They set up meetings with families to discuss progress and help set up after release in home care. As a family member, one has to be proactive and get involved with the care. If you need to utilize one of these type facilities, I do highly recommend this place. Depending on the insurance, Kaiser will pay 100% for care up to 21 days, which was a blessing for our family. read more

THIS PLACE SHOULD NOT BE USED AS A REHAB CENTER I thought Kaiser sent me to a rehab center after my foot and leg surgery on july 24 2021 but I ended up in hell. How can they call this place a rehab center when it is simple a poorly run nursing home for Alzheimer / dementia patient. if only I had read the reviews I would have never set a foot in this place. But I must agree with Kandis the only redeeming thing about this place are some of the people that work there.A few of them were very sweet and kind especially the young one as for the oldest worker they were obviously burned out and could not wait to finish their shift and couldn't care less about the patients needs. The next day after my arrival I asked to be moved into a different room because my bed was so bad, I was in a cubicle with no light , the women next to me cried all night complaining she was in a lot of pain, and the other one was telling me over and over that someone had stolen her pillow and her rings. it is only when I was moved into the new room , yes I had more space and more light , that I realized i was not in a rehab center but a nursing home for Alzheimer people. This place is filthy , the smell is so bad that I did not want to get out of my room .In two days the bathroom we have to share with the room next to us did not get clean , no one bother to check on me during the night, and could not get anyone to help me to go to the bathroom . It is only in the morning that a young nurse changed my bed when she found me in tears on the side of it as for the food I feel sorry for the patient that have it everyday. I fell so sick , stress and unsafe that I demanded to be release immediately. I have a hard time believing that some people gave this place 5 stars . Thank god that most of those people don't even realized where they are. read more

COMPARING VINEYARD WITH HILLCREST REHABS: REHABBING AT HILLCREST IS LIKE STAYING AT THE FAIRMONT HOTEL, after coming from Vineyards, comparing the two. Vineyards is a horrible rehab by comparison, it's behind PVH. At Vineyards they are understaffed, very unhappy to the point of being negligent, from my observation. The night shift have their most grouchiest staff, one telling me to "QUIT YOUR WHININ,', as I shivered, with my teeth chattering. And "USE YOUR WORDS". I am 69, not 4. She would not have liked my words. Late at night, a CNA at Vineyards even refused to call 911 for my roommate, when she said she was dying. We both told him to call 911. He made it clear he did not want any more of my perceived interference. Roommate said they are supposed to take care of us, but instead it's "a place of Pain and Death". The CNA wanted her to wait until the nurse saw her first, to decide if she really needed 911. The aid said things move slowly around here. Should she have waited, she would have died. The nurse was on the other side of the facility. His plan would have killed her! I believed her, as did her daughter, who called 911 instead of our aid. She was taken to Memorial, bleeding internally. She did nearly die. She was in critical condition for many weeks. It was beyond negligent, it was traumatic for her, her family and for me. The aid returned to our room, rushing to her empty bed, then he ripped around and "accused" me of calling 911. He was angry. I reported him- G., to the ombudsman, Ruth Wells. So STEER CLEAR OF VINEYARDS. The first night I was over there, the CNA had to change me, my pad, my sheets. It is something that happens in a rehab, and I did not do it on purpose! This person angrily repeated the word Sh**, Sh**, Sh** !!! -as she changed me. It's nothing I did intentionally! It was such a relief to be at HillCrest! I stayed there for over 5 weeks, and so appreciate their help. The response time must be twice as fast at HillCrest vs Vineyards. There are still a couple of grouchy apples, but mostly there are people who actually care. I played roulette in picking HillCrest, and blindly, hit the jackpot. This is the only satisfactory rehab I've been to, out of all eight. It's the best so far, in my experience. I recommend it. . P'towner read more

In a word (minus the food) spectacular. The staff at Vineyard Post Acute took amazing care of my rehabilitating 88 year old father. My dad is not a primary English speaker and is hard of hearing. They did everything to accommodate him and helped him in every way. There are a couple knocks - 1, the food. I get it, they are a rehab hospital not a restaurant, but when your an old guy and incapacitated some of the only joy you look forward to each day is lunch and dinner. Next - the AC and heating system are part of an old building. So you are either hot or cold. We almost brought my dad a fan to help cool him off because the heat was almost too much for him. The kind CNA and nurses tried but this was really more of a limitations of an old building. Aside from that, his stay and especially the PT helped him walk better than before his accident. It took three days to get her name correctly. They continued to bring Apple juice when she & I asked them not to. Seems like every time I speak with her, she's been unhappy with the help. Left in the chair too long, not changed for an hour after request, pain meds not given regularly, so I have to call and request for the charge nurse to give her meds. She's 90 with a broken hip. Worst of all, my Aunt had a rash on her bum which wasn't treated for 5 days after I brought the medication her Doctor previously prescribed, because they had to first obtain Doctors approval. Now they say she's going to get a bed sore like its her fault somehow. 2-3 star with a new coat of paint. read more

Vineyard Post Acute in Petaluma took on 29 evacuees from Santa Rosa Skilled Nursing Facilities who were evacuated from the fires. These residents were triaged to our dining rooms and activities rooms. Our administrator, director of nursing and staff stepped up to care for these patients and residents. Staff from our sister facility assisted in working all shifts to cover the floors. Staff members who have been displaced because of the fires were encouraged to bring their children to work and our administrator sheltered and fed them. It was an amazing feeling of community and family to see how the staff and management at Vineyard Post Acute stepped up and out of their comfort zones during this disaster and great time of need. My husband has Alzheimer's disease and is a full-time resident there. After poor care at Our House he's gaining his weight back, has had PT and is still having deep heat treatments to his knees and shoulders. The smiles and caring attitude is wonderful at Vineyard as well as superior care! Music programs every day and a sunny / shady patio make life interesting and pleasant for those who want to take advantage of them. After being evaluated, my husband was found to need blended food, and is helped to eat each meal. I'm very pleased to have found such a good place with staff so caring! read more
