Infectious disease control failure. Elder abuse. Medication mismanagement. Title 22 violations. The facility accepted my family member for admission despite being unequipped to meet his medical needs, including mandated isolation due to highly infections disease. Upon arrival, the facility was unable to provide required IV antibiotics. Additionally, the facility placed the patient in a shared room with a shared bathroom, violating Title 22 § 73623, despite his infectious status requiring isolation. I observed the facility to be in a filthy and unsanitary condition. Staff falsely claimed that internal "protocols" allowed for the sharing of bathrooms for infectious patients. The facility cannot override the law with invented protocols. Isolation was required. Due to this neglect, we were forced to immediately remove the patient and return him to the hospital to ensure his safety. Complaints have been filed with California authorities. read more

My mom stayed at Broadway Villa Post Acute for two months to recover from her left shoulder replacement surgery, and I can't say enough good things about her experience. From day one, the entire staff was attentive, compassionate, and genuinely invested in her recovery journey. The physical therapy team was fantastic--they tailored her exercises to her specific needs and patiently guided her through each step, celebrating every bit of progress. The nurses and aides were always there to ensure she was comfortable and felt safe, going above and beyond to make her stay as pleasant as possible. I have been to a few SNF around the bay area during clinicals. And this is the first facility I will be working in. I am very excited and thrilled to work with them. Everyone is very enthusiastic and accommodating. I can see that they give an outstanding quality care to patients. The facility has a clean and organized environment which is amazing! My auntie and grandparents stayed here and they had a wonderful experience. Their needs were cared for and attended to and I am very happy to say that their experience is top tier in California. They provided many type of care from elderly, rehabilitation, and many minor disability care. This location is very modern and green which is an absolutely for the elderly. There are some spots that need a touch up but other than that the staff are the nicest and attentive. Its a very calming place to be treated. Thank you Broadway Villa, we will come again. read more

So I did a review a few months ago on this place and gave them 4 stars and had wonderful things to say about them. That has changed! The care and attention my mom is receiving has gone down hill right off the side of a cliff. I've been noticing the quality of care given to my mom has been dropping over the past couple months. Most recently my mom became ill and was in bad pain. A nurse named Faith called me after I called to see how my mom was doing. When Faith called the call connection was so bad I could not hear Faith on the other end. I said "I'm sorry but I can't hear what you are saying can you call me back from a different phone?" She hung up and called me back. Same thing again the call was cracking really bad. Again I said "I'm sorry but I can't hear what you are saying can you call me back from a different phone?" I heard her say "this is the only phone i have" after asking her to repeat what she said. Unbelievable out of all the phones in this place the one she was calling from was the only phone she can use? I say Bull_hit. Now during this time I am laying in a hospital bed in Sacramento. When I asked Faith what tests they were doing to determine what was wrong with my mom she said blood work and x-rays. She said they started giving my mom pain medication also. When I got out of the hospital I went to see my mom the next day (yesterday). When I got there at around 3:30pm my mom was in her bed in pain. I asked her nurse about her pain meds. The nurse looked up when she last was given some and said "the morning staff gave her some this morning, I haven't been in to see her yet" Do you see anything wrong with that answer? My mom was supposed to get regular pain medication every 4-6 hours. If she last received it in the morning she should have received more by now. At 4:30 the nurse finally gives her some. My mom got worse and seemed like she needed to throw up. Again I went to the nurse and asked if they were giving my mom anything for nausea. The nurse replies saying "we cant just give her medication. The doctor has to order it first." I said "she is on a large pain medication where a lot of people get sick when they take it. Maybe you should do your job and notify the doctor so he can order something for her." I then demanded that my mom be transferred to a hospital for a CT scan/MRI. They had to contact the doctor to get his ok first. They transferred her late yesterday to Kaiser to get a the CT scan/MRI. She had a urinary track infection and pneumonia. This place was just going to leave her lay there in her bed in very bad pain and scratch their heads because as the nurse Faith said "we don't know what's wrong with her because we run all the test and everything comes back normal." Side not: I was reading other reviews about this place and read the review by Gloria P. dated 9/7/2023. "The food is amazing with scratch base cooking!" What place is she talking about? The meals I've seen being served most of the time get thrown away. Like I said in a previous review of this place "Their meals here are really bad. I've seen a lot of the meals that are served and it does not surprise me that a lot of patients don't eat it. My mom is in a room with 3 other ladies and at times I've seen all 4 meal trays get rejected and thrown away." I received a call asking if I wanted them to save my moms bed for when she is released from Kaiser. I notified the person on the phone of the things that I'm having issue's. The lady said she would have someone contact me right away. I've still not heard from anyone. The facility it self is a bit run down and could use a makeover. read more

I posted at least two reviews of this place and i don't understand how they've removed them. Maybe it's because i mentioned the name of one of the managers who was extremely rude and unprofessional when i spoke to him about policies and they keep asking me to take it the social worker in their establishment. Well, the social workers have not gotten back to me in a month that I've been trying. Well there are lots of complaints about how they are not fair with allowing loved ones to be visited by family. They say anyone can come but they allow other hostile family members who got their before you to dictate how visits are performed. They allowed another visitor who dislikes me to not let me visit my loved one even when he was asking and crying in tears that he wanted to see my husband and i in front of them and the man hostile other family member. The other person was already there and started insulting us and they just allowed it. They have no policy in place like, after an hour it's someone else's turn to visit. They also knew we were having trouble visiting with this other family member present and had agreed to protect us with making apts which we did. But when we got there after an hours drive, we were accosted with insults by the other visitor and they just allowed it. This has happened twice to us and our loved one probably does not have much time left in this world. It's very sad and disappointing. read more

I would concur with the other reviews. We experienced poor direction from several of the people we needed guidance from. Tension and arrogance from the upper management. Often passive aggressive towards family members. The nurses and CNA's were caring and provided as much care as they were allowed. read more

My dad was transferred here from the Sonoma Valley Hospital. I do not see what they could do to make this a better experience. My dad was here for 3 months and I wish he could have stayed longer because of the excellent care and friendly helpful attention he and the rest of us received when we visited. read more

I have been a patient here for 13 years. During that time I have seen many changes, very few of them good. To let you understand a bit about the operation of this facility I will give some background. The facility is listed as a post-acute care facility. This means that the patients here range from recovering from operations to those who are in long care residency. That is the status I fall under. I had a sever stroke 13 years ago that left the right side of my body paralyzed. I need someone to dress me, change my diapers, and use a lift to move me from bed to wheelchair. My roommate suffers from COPE disease which many times leaves him short of breath and unable to draw a breath. When we have any problems we press a call light which should immediately bring an aide or nurse to our room to find out what is wrong and take appropriate action to alleviate the problem. Last night at 4:00am my roommate started SCREAMING he couldn't breath or move. Since he was unable to reach his call button I pressed mine to bring help. No one showed up for 40 minutes. Then when the Nurse came she brushed off his complaint that he could breath because he had been screaming and he could move because he rolled over to look at her. When I asked why it had taken so long to come to his aid she told me it was none of my business. I had seen animosity between the nurse and my roommate in the past. She gave him a breathing treatment and left the room mumbling under her breath "PEST" I was not supposed to hear that. I have seen the quality of service slide over my 13 years here. The food is terrible, they are way understaffed and the additude of SOME of the staff is terrible. They are just there to collect their paycheck. Don't get me wrong, there are some very dedicated people here trying to make our life more comfortable, but one bad apple can spoil the barrel. If anyone reading this review is considering putting one of your loved one's under the care of this facility think again. I stay because I can't afford anywhere else, not because I have good feelings about this facility. Do your family member a favor and find a better facility read more

This place is the ABSOLUTE WORST of all the post-acute care facilities my husband has been in. It would take PAGES for me to write about all the horrible things my husband suffered through at this facility, but here are some highlights: 1) one of the nurses was going to give him glucose syrup (when his blood sugar was ALREADY high) instead of insulin. 2) He fell and smashed his head on the floor and they refused to take him to the hospital. He was there recovering from a stroke. I had to FIGHT with them to get him medical care and it took more than a week before they grudgingly compiled! 3) The staff is horrid. We were lied to so many times about so many things that I finally refused to even enter the facility, but would wait in the parking lot to visit my husband when he came out. 4) I was treated like I was the one causing the problem when it was ongoing medical negligence from THEM day after day after day. It was truly horrible. I have reported this facility. to the AMA. 5) The day he arrived at the facility, he had been transported FROM Memorial hospital after a fall where he had AGAIN hit his head hard. Memorial didn't give him care, I was PROMISED by broadway villa he would be given care when he arrived, but he wasn't. The doctor showed up at the end of the evening for the very first time, poked his head in to say "hi, I'm doctor so and so" and left. THAT WAS IT! And that's pretty much the kind of care he received the entire time he was there. Shoddy care by shoddy people. The staff hierarchy are unbelievably arrogant and un-caring. ALL of our concerns were disregarded. The absolutely WORSE SNF in all of Sonoma County. Don't put your loved one in this facility unless you want them to be mistreated and suffer! read more

DO NOT BRING YOUR LOVED ONES HERE!! My grandmother was transferred here as she was ill and needed medical assistance. She was neglected. Unfortunately since it was during COVID we didn't get to see her in person, but we were still able to speak over the phone. She would complain about having to lay in her soiled linens for hours before they would come change her. Staff were nice but they were very short handed and you could tell every time you called. Yes, my grandmother was sick but she was completely conscious and coherent, she knew exactly what was going on. She thought she was going to a place where they had 24 hr care to assist her with her needs and make her feel comfortable. Unfortunately that did not happen and she ended back up in the hospital. 2 days before my grandmother passed she mentioned how they did not have enough support staff to help the patients and their needs. My friends father didn't have the greatest experience while there as well, but I leave that story for them to tell when they are ready. The only positive feedback I can mention regarding this facility is that they would set up scheduled visits through the window during COVID. I was a "rehab" patient there for 10 days recently and had many concerns. Leadership was poor on all levels as was moral. Staff were more attentive to the survey taking place (i.e.clean Floors), than patients. It took 2 tries and 3 hrs to answer my request to see my nurse. Confusion and lack of communication occurred on all levels from housekeeper to dietary to supervisors. Not sure why this facility is called "skilled?!". read more
