    I DO NOT recommend this place for anyone. The day shift were very nice to my father, but the night…read moreshift the staff didn't speak English and when my dad would hit the call light they would walk by and turn it off and keep walking. They did not give my father his medication as the doctor prescribed: it was either not enough or too much. One day I came to visit him and he had a huge bump and bruise on his head; bruises on his shoulder and his entire right side. I asked my dad what happen, he did not remember, when I asked the staff they stated that he fell out of his wheel chair. No one called us to let us know, it was very startling to come in and see your loved one look like he had been in a huge brawl and lost. He would have bruises on him all over the place. I understand that his skin was thin and may have bruised easily, but did they have to be so rough. I would watch his face when the turned him or moved him and his face showed that it hurt, they would just flip him like he was nothing. He was not nothing, he was my father. The nurses were very rude and would just walk away when you were talking to them, if you could get them to talk to you. Once my mother tried to talk to a nurse and she put her hand up and said "not now, I'm busy". My father when he first arrived did not like this place, every day he said that he had to get out of there. He would ask to participate in some of the activities but they would never come and get him (he was a double amputee). We asked them many times. After a while my dad gave up and asked us if he was just brought there to die. We tried to find other nursing homes for him to go to, my mother called every one of them, there were several that said they would take him they had the room all Sonoma health care needed to do was send them a referral. We would be so happy, then we would go visit dad and they would tell us that they were contacted by the other home and they made a mistake and they were sorry but they did not have the room for dad, my mom would just cry. I hated this! We couldn't even get him out. My mom would go home and call the place that said they had room for my dad and ask them what happen, each of the homes said exactly the same thing 'I'm sorry but he person that was being discharged suddenly got worse and could not leave". Funny all three of the facilities that said they had a room for dad said exactly the same thing, like they were reading from a script. He was there for 6 weeks, when he went in, he was laughing & joking around but within those 6 weeks, he gave up; he wouldn't even eat the last two weeks. My father passed away on 7/19/14; in the place he hated the most. The day shift seemed to treat my father well. But we were also there during the day. So I am not sure what happened when we were not there. Good Luck to whomever is reading this, I wish you the best. I do not recommend this place for your loved one.

They always provided guidance and help whenever needed. The facility is clean and food delicious -the special holiday buffets are amazing. My mother truly enjoyed activities from outings to playing bridge almost every day and more. She bonded with many there through fires, Covid and Increasing age. During these challenging times I saw the staff really step up and be there for the residents. I am forever grateful for the care Cogir gave my mother and for making the last years of her life full of friendship, health and enjoyment.

    My 95 year old mother was at Cogir for just under a year; April 2024- March 2025. Although I had…read moretoured other facilities and thought I had chosen the right place for Mom, I very much regret my decision taking her to Cogir. There are a few reasons for that regret, but here are the two main ones: 1. Out of the blue, without any notice or prior discussion, Cogir raised Mom's care cost sky high. There was no apparent reason for this hike and no explanations were given. Just an email stating the new amount was "due immediately ". 2. When seeking clarification or reasoning behind such huge cost increases, phone call after phone call, email after email were totally ignored by the management team. Day after day after day every message was disregarded. The first time this happened they ignored me for 7 days. The second time, it took them 6 days to finally respond. I found this "increase-then-ignore " style of management very stressful and extremely frustrating to deal with. Neither is a quality I seek in a management team. Especially one entrusted to my mother's care. I happily gave notice and left as soon as possible.

I came into this program at my rock bottom. My family found Align and I was honestly scared to even consider detox let alone a 30 day program. As soon as I entered the doors they welcomed me with open arms. The staff which consist of nurses and counselors are comforting and just genuinely want you to get better. I was honestly so scared to commit but I'm so glad I did. Stella is a wonderful cook and she cooks with her heart. The rooms are spacious and the whole build is top notch clean. We had about 5 groups a day and I learned so much about my recovery. I came out feeling stronger than ever. If you are looking for a program that feels like home and teaches you about addiction and recovery then I 100% recommend. I'm so glad to call myself an Alumni from Align.

I am really grateful to have found their office and to have put my trust into their healing hands! My posture has improved SO much from starting care here. I feel better, I sleep better, other areas of pain have gone away and I stand taller and straighter than I ever have! Dr H and K are super kind and caring. Before I came in, I constantly struggled with migraines, neck and severe back pain which extremely limited my ability to do things and live freely without pain. However with the help of Dr. Hodgson and his wife Dr. Lowry's expertise I have been able to wake up every day without worrying about having a debilitating headache or stiff neck. I love this office and consider these doctors a second family and this place a second home. They are genuine, authentic and as a patient I have always been able to tell that my care is their first priority.     I would strongly recommend looking elsewhere for hospice care. Continuum Hospice added stress to an…read morealready stressful situation--my mom passing. Care was inconsistent from the start. After enrolling on a Saturday, no nurse visited until Friday, leaving us on our own for five days. The nurse who did come called (with poor reception) to suggest stopping all medications, then never followed up--I had to track down the physician myself for guidance. I wasn't given the blue book--"Gone From my Sight" that explains the dying experience until the day before she died. Communication and clinical judgment were unreliable. When she had a severe reaction to morphine, it was dismissed by a nurse, and I had to escalate to the physician to get her medication changed. The day before my mother passed, I was told she was not transitioning. She died the next day. There were also clear signs of understaffing. They repeatedly offered help with bathing, but no one showed up for weeks--too late to be appropriate. After her death, the nurse who arrived to declare her death had her name wrong and lacked basic information we had already provided. So I was having to correct the spelling of her name and get my phone to give her the name and phone number of the mortuary. I had given this information to the social worker so it should have been in their system. A small thing one could argue. But did not make me feel cared for. Compassion for my mom passing was absent. It was totally transactional. I regret not changing providers earlier. I tried to initiate a change, but the paperwork was not sent in a timely manner. If you are not happy with a Hospice change at the first sign of trouble as transfers once someone is close to passing are more difficult. I should have been able to focus on my mom and dad during these difficult final weeks--not manage gaps in care.

The nurse (Allison) at…read moreVallejo Cogir and I talked to the head nurse (Angela) at Continuum on the phone twice - once at the hospital and once when my mother was brought to her apartment at Cogir. Angela agreed to provide servicing of my mother's medication throughout the night every four hours. We even discussed the hours that they would provide care during the night. Angela put none of this information in writing. This organization and head nurse cannot be trusted to do what they say they will do. Continuum says we did not call for help throughout the night. Of course we didn't because we were told by Angela they would cover us.
