    Continuum Hospice may be the most important review I have ever written. I cannot express the love I…read morehave for those that were involved with taking care of my mother during her final days. I went into this extremely stressful time in my life with the idea of tackling everything as a caregiver by myself. That was the wrong assumption of course as Continuum convinced me they were there for me and my family 24/7. First and foremost, Deb our amazing RN from Continuum, but more importantly our newly added family member at this point who was there when I needed it the most to help take care of my mother and make sure it was a stress free environment. Deb not only was very knowledgeable, extremely kind, and caring, but my mother felt at peace when she was there assisting in care and the emotional positive vibe that tagged along with her. I will never be able to thank her enough for everything, but made a promise to try my best to do so! Along with Deb came our amazing Social Worker (Kay), and aide Susanna, who both were just as important in the entire process. Obviously another stressful part was the medicine and equipment, which I was told up front to not worry and that my mother would be supplied with what she needed. They not only followed through with that promise, but exceeded it! I never had to plead my case for anything that we needed, it was either delivered the day of or overnight, which was amazing. Another huge aspect is the fact that they have a 24/7 line even during holidays with a multitude of nurses to assist whenever. They stuck by that, as we had called numerous times during the wee hours of the morning or very late at night and a nurse arrived within a 30 minute to hour window (Bravo!). Obviously I can go on and on about the wonderful job Continuum performed, but mostly I want to reiterate to those hopefully looking at this review for the purposes of hospice care that Continuum is the real deal.     I would strongly recommend looking elsewhere for hospice care. Continuum Hospice added stress to an…read morealready stressful situation--my mom passing. Care was inconsistent from the start. After enrolling on a Saturday, no nurse visited until Friday, leaving us on our own for five days. The nurse who did come called (with poor reception) to suggest stopping all medications, then never followed up--I had to track down the physician myself for guidance. I wasn't given the blue book--"Gone From my Sight" that explains the dying experience until the day before she died. Communication and clinical judgment were unreliable. When she had a severe reaction to morphine, it was dismissed by a nurse, and I had to escalate to the physician to get her medication changed. The day before my mother passed, I was told she was not transitioning. She died the next day. There were also clear signs of understaffing. They repeatedly offered help with bathing, but no one showed up for weeks--too late to be appropriate. After her death, the nurse who arrived to declare her death had her name wrong and lacked basic information we had already provided. So I was having to correct the spelling of her name and get my phone to give her the name and phone number of the mortuary. I had given this information to the social worker so it should have been in their system. A small thing one could argue. But did not make me feel cared for. Compassion for my mom passing was absent. It was totally transactional. I regret not changing providers earlier. I tried to initiate a change, but the paperwork was not sent in a timely manner. If you are not happy with a Hospice change at the first sign of trouble as transfers once someone is close to passing are more difficult. I should have been able to focus on my mom and dad during these difficult final weeks--not manage gaps in care.

My mother was not clean probably so I put her in the shower myself and cleaned her backside which was literally caked on. I felt very uncomfortable in that place no good vibes SO MY SISTERS AND I GOT HER OUT OF THERE. We went to Mother Love in Santa Rosa which is a real home, in the neighborhood, and Mother Love and her daughter fell in love with my MOM, When I first met them I said okay sisters this is the place, The vibes were so good in there, Suddenly my mother was herself again, Shining with the Holy Spirit, she hated Sunrise she felt scared there. If God isn't there then what was , She lit up when she stary with Mother love, Always protect your loved ones from darkness and soulless people when they are at that age. She died in Peace with Gods light. Cost of Sunrise with a caviler attitude rip off dont leave your loved ones there. Eight thousand dollars a month, Theres a place in hell for this place.

Disgusting! Sunrise Villa is NOTHING like that though! This is a fancy pants place! When you walk in you are blanketed by the amazing smells of the delicious food from the kitchen, and the dining room has chandeliers, cloth napkins, crystal glasses, and table service with huge menus for every meal! It's literally like a fancy restaurant for every meal for the residents! That cannot be beat! The entire place is immaculate, with no dirt or bad smells anywhere that we can detect, and that's a huge plus. Just the idea of assisted living usually conjures up negative things but Sunrise Villa creates a whole different experience, a wonderful one. My folks have only been there for one month, having come from a beautiful hilltop home on sonoma Mountain, and they are both already saying they are happy there. There are tons of amazing activities scheduled all day long, and their amazing two bedroom apartment gives them a living room, a bedroom,and an extra bedroom for my father's computer and office. On top of all that, Grace, who helped us through every step, is the most kind, loving, understanding, and patient person to help your family through this difficult time. She is amazing! Thank you Grace! Sunrise Villa is not cheap however, and you'll need plenty of money in order to stay there, but it's definitely worth it, and they have created an assisted living facility that takes the sting out of the painful family process that leads to the need for it. I recommend it highly, if you can afford it.

    My mother was a resident at Cogir of Sonoma Plaza for 12 years and I couldn't be happier with care…read moreand the facility we chose. The staff is incredibly kind, attentive and professional. They always provided guidance and help whenever needed. The facility is clean and food delicious -the special holiday buffets are amazing. My mother truly enjoyed activities from outings to playing bridge almost every day and more. She bonded with many there through fires, Covid and Increasing age. During these challenging times I saw the staff really step up and be there for the residents. I am forever grateful for the care Cogir gave my mother and for making the last years of her life full of friendship, health and enjoyment.

    My 95 year old mother was at Cogir for just under a year; April 2024- March 2025. Although I had…read moretoured other facilities and thought I had chosen the right place for Mom, I very much regret my decision taking her to Cogir. There are a few reasons for that regret, but here are the two main ones: 1. Out of the blue, without any notice or prior discussion, Cogir raised Mom's care cost sky high. There was no apparent reason for this hike and no explanations were given. Just an email stating the new amount was "due immediately ". 2. When seeking clarification or reasoning behind such huge cost increases, phone call after phone call, email after email were totally ignored by the management team. Day after day after day every message was disregarded. The first time this happened they ignored me for 7 days. The second time, it took them 6 days to finally respond. I found this "increase-then-ignore " style of management very stressful and extremely frustrating to deal with. Neither is a quality I seek in a management team. Especially one entrusted to my mother's care. I happily gave notice and left as soon as possible.
