My grandmother was admitted to Park View around October, and I started visiting her pretty often. From the very first time I walked through the front door, I was greeted by Alana with a kind smile that immediately made me feel welcomed. Every time I visited, Alana was incredibly helpful in showing me where to go and helping me find my way around the facility, even at different times of the day. What really stood out to me, though, was the way she interacted with the residents. She always seemed genuinely kind, patient, and caring. It was very clear to me that she wasn't simply there to collect a paycheck -- she truly enjoys helping people and making the residents feel cared for. One day, I asked my grandmother if she had ever interacted with Alana, and she immediately told me that Alana was really nice. She even mentioned that Alana had taken her on a quick walk. Hearing that meant a lot to me because, as a family member, knowing that someone is taking the time to make your loved one feel comfortable and cared for is incredibly reassuring. Alana made a lasting impression on me and my family. Her kindness, genuine personality, and willingness to go out of her way to help others are exactly the qualities you hope to see in someone working with elderly residents. Park View is very fortunate to have someone like Alana on their team. I'm incredibly grateful for the kindness she showed both my grandmother and me. If you're looking for a place where compassionate people genuinely care about the residents, I would absolutely recommend Park View. My father was a patient there for about a month last July to August. We placed him on hospice and he was arranged to be moved to a home care facility for end-of-life care, we were not present for the transport. The company that was booked arrived in a commercial vehicle and they claim Park View refused to give them a wheelchair to transport him. At this point my father was barely speaking and experienced drops in blood pressure whenever not in a laying position. They did not call my mother to inform her that he was being transported in what I would describe as a medically unsafe manor. In my opinion they never should have allowed him to leave their facility in such a way. He arrived extremely disoriented and weak and fainted once he arrived at the new home care facility. I would hope no ones loved one would experience something similar. read more

What an amazing facility with friendly, and attentive staff that showed so much compassion! I was so impressed with how concerned the team was about my dad and his well being. They were very informative on what care they were providing my dad, and kept me in the loop with everything they were doing for him. He received great PT and OT and had therapists that showed empathy, support, and encouragement. They also offered many activities to entertain the patients. Food is horrible and the staff comes in the rooms to take your vitals, change your "pads," take vitamins, and eat around 6:00 a.m. The CNA's are overworked and they despirately need help. To honor staff I wrote around 7 "WOW" cards because they were some great employees. You eat your meals in your room/bed but get out for PT and OT. I asked to sit in the lobby looking outside (it was too hot to be outside), and enjoyed those moments. Some residents scream at the top of their lungs and are ignored by the staff. I had 3 roommates within a 2 week stay. Two out of the three were exceptionally nice. enjoyed having my hair washed by a beautician for $5 ($2 tip). She visits multiple sites and I highly recommend her. read more

Amazing and caring staff. I believe Parkview to be the best of post surgery care facilities in the county. Their physical therapy therapists are highly skilled and supportive. It's very clean and friendly. It would be nice if one could continue with the same therapist instead of changing for every session. I can't say enough good things about their caring CNA's, nursing and therapy staff. They were the best. I just wish they had more of them as they did seem overloaded at times, I also want to compliment the food. Every meal had fresh fruit or vegetables. Amazing that they could deliver perfectly cooked fried eggs to so many patients for breakfast. Many places use liquid or powdered eggs so hats off to Parkview. I highly recommend them. read more

I was here in 2019 and then again in May of 2024. It has changed a lot. My CNA's Celia, Eileen and Martha were fantastic. My last stay was horrible but they have improved and changed the way things are done. No one wants to be in a skilled nursing facility but sometimes it is necessary to heal. The physical therapy department has also improved. Everyone's favorite Kenny !!! After 8 weeks I was able to go home and I miss the people that took care of me. So do I want to go back? No! If I had to I wouldn't kick and sream on my way like I did. read more

Its definitatly a 'SNiF'. I had a close family member there after hip surgery. They was rehabed and out in a few weeks. No known issues.

My sister in law was recently there for care pending surgery and while receiving chemo - she is immune compromised. She shared a room with another patient. Apparently the other patient's visitors were not required to wear a mask while visiting and they were unwell when visiting. She is being treated for pneumonia and had to be taken to ER. Prior to this she made requests for two days that staff contact a family member and that request was ignored. Food was ordered by the family for her and a member of the staff ate it. It was a traumatic experience for her. read more

My mom stayed there both times she had knee replacement and they took great care of her. My grandmother was there near the end of life phase and they were very kind (except one of my uncles called to change the first contact from my dad to him and they did, my dad it was my grandmother's power of attorney so what they did was not OK or the person working that night messed up because the next morning when she passed we did not get the call. He did the uncle and we were told maybe an hour later). my friend recently stayed there for medical issue was transferred from the hospital to postacute and as soon as she got postacute, her medical situation drastically increased in a good way and they took very good care of her and gave her instructions on when she's ready to go home and what to do with her medical needs read more

I will say, Park View is better than apple valley in sebastopol but not that great either. In the 25 days he was there, I wasn't given one update on my loved one progress or otherwise. I kept asking the director and he said they would do this or that but never did. They also lost all his clothes, a blanket and his seeing eye glasses. And when they finally released him gave us the wrong discharge paperwork and expect me to come back there for it. One non-critical incidental was the food was very bad - he lost a lot of weight being there. And they didn't notice for quite some time his swallowing was impaired and that he was experiencing aphasia, a brain injury commonly caused by stroke. In addition, they didn't do the allocated physical therapy on several scheduled PT days, that I am aware of. Yet his doctor scolded him for not working hard enough and then wouldn't release him to go home when we made it clear he was not doing well there and needed to come home! They made us wait another week meant we picked him up on Christmas Eve. The reason they gave for not releasing him was his mobility hadn't improved enough. The saddest part is his mobility is actually worse after staying here because he just laid in bed the whole time. By far though the most frustrating part was that no one told me anything and because my loved one has limited short term memory we were both clueless. Some good qualities about the facility was the staff was friendly and did not appear to be annoyed with the patients. They keep the place clean and there are only 2 people per room. And they allowed me to bring in my doggie while visiting. Hopefully this review will help prepare you for a stay here. Like I said, I'm not sure any of these homes are really great and it just shows how our culture doesn't take care of our elderly when they need it the most and are most vulnerable. It would be nice if they actually took care of their patients. My Dad came in because of an infection. He was stabilized by the hospital and sent there to recover his strength. They did not do the physical therapy he was supposed to be there for and could not tell me why it didn't happen. Every time I tried to talk to someone about his progress they said it was the first time they had dealt with him so they didn't know how he was progressing. He was there for a week and a half. The turnover rate must be unreal. He finally left when his infection went septic. He was only sent to the hospital at that point because we insisted on it. By then he was basically catatonic. He had been chatty and relatively normal not two days before. They kept saying they would have the doctor check on him tomorrow. By that point it could have been too late. I shudder to think what would have happened had we not checked on him so frequently and raised hell when something was wrong. Do not send your loved ones here if they actually need to be taken care of or recover . This place needs to be shut down. read more

Discharged elderly disabled very underweight due to being so short staffed in all areas causing more medical issues for the patient and seizures along with a life flight.. even with ombudsman and appeals process, it was all about admin issues. Shameful treatment with no remorse. read more

DO NOT let anyone you care about EVER spend one night at this place. The doctor, not really a doctor, a DO, is Hardly ever there. When he is there he prescribes drugs that your loved ones should never take and doesn't get permission from anyone. when you can track him down, he lies. he and his case manager never answer or return calls. I wouldn't send my worst enemy there. my family will be filing a complaint with whomever we can to get this place shut down! thank god we got my father out before they killed him! I highly recommend Park View Post Acute. When my mom needed rehab after a major surgery we interviewed 3 places and chose this place after talking to Ricci Erfe-Mejia in charge of admissions. My mom was there for over 3 weeks and she received excellent care. When she arrived she was wheelchair bound and when she left she was walking without any aid. I had to go out of town for a week and I was kept in the loop with all that was going on via emails. The staff, caretakers and doctors were all a 10! read more

The administration at this facility leaves a lot to be desired. The staff is caring and empathetic, but they cannot handle the work load they are given. They are so understaffed, it is impossible for them to provide quality care. It is all about the bottom line. If they were fully staffed, it would affect their profit. What is the ratio of staff to patients at this facility? When my 93 year old husband was there for 6 days, he was to receive therapy so that he could regain and improve his walking ability. The doctor said that he probably would be there 5 days. However, after having his first full therapy session, his feet were hurting and he could not stand on them. Unbeknownst to us, the administering doctor prescribed Norco for him, without seeing him. When we went in the next afternoon, we noticed that Mxxxx was disoriented and unresponsive, just wanting to sleep. There was a chalkboard next to his bed that read "PAIN MANAGEMENT THERAPY". I questioned that and the nurse told me that he was prescribed Norco for his pain. He did not take pain medication when he went into the facility and now it was clear that he was drugged and not able do any therapy because his balance and coordination was affected by the drug. This meant that he would have to stay longer than anticipated (until he was off the narcotic) which could add days or weeks to his stay. As long as they gave him this drug, his therapy would be postponed. My daughter and I instructed the nurse NOT to administer any other pain medication, except Tylenol. M.... then began to be responsive when the Norco wore off and we worked with him to get up and take steps, so that we could take him home by Monday. When we went on Monday, we told Dr. Hurwitz that we were going to take M xxxx home and wanted him to discharge Mxxx. His response was that he could not recommend discharge unless the Social Worker and Therapy team recommend he do so. He also told us NOT to go the Social Worker's office because she is too busy. My daughter ignored that advice and went to the office and was told that the Social Worker was not in that day. She persevered and insisted that the person in that office initiate the discharge papers. Had we not been diligent about visiting and observing, they would have continued the Norco and we wouldnt know when my husband would be cognitive enough to do physical therapy. How long does it take to get addicted to Norco? When would we ever be able to take him home if he didn't get therapy? These are questions we pondered. The only chance we had to get Mxxxx rehabilitated was to get him home and off of drugs. Mxxxx got P.T. At home and is now walking again, with a cane. If you need to have someone stay in this facility, it is important they have an advocate who visits often and stays informed on the treatment their loved one receives. D.S. read more

Our family very much appreciated Park View Post Acute while our dad was under their care for 5 1/2 weeks. Dad was well cared for as we always came upon him in good spirits, clean and comfortable. The PT, OT and Speech therapists always took time to answer our questions. They went above and beyond. The hair stylist was even another thoughtful person who helped our dad during his time there. Norma, Mady, Sylie (sorry if I misspelled names), Placido, Lorena, Alicia (the therapy director), Ricky (the admissions director) and an LVN (not assigned to our dad) who helped us during a particularly stressful event (I'm sorry I don't remember her name) But all of these wonderful caregivers make Park View a most excellent skilled nursing facility. read more

My Husband, needed additional health care after Surgical Hospitalization. What a wonderful experience. The clean, and welcoming skilled nursing facility. The staff were so friendly and professional. Skilled and knowledgeable of the care my Husband needed. The Rehab aided in his recovery, as well. Laura, Summer, were so kind as well as each person we encountered. I was recently a patient at Park View Post Acute. I received excellent care. The facility was very clean, and without odors that sometimes haunt the halls of hospitals such as this. The staff made my stay very comfortable and took care of my every need. The only problem that I encountered was during my first few days there. There seemed to be a lack of communication between the pharmacy and the doctor that had been assigned to me. I changed to Dr. Hurowitz, and everything was straightened out, all ended well. read more

After a bad fall, I was transported to Park View from the emergency room. I had no idea what to expect but I knew I wasn't ready to go home and take care of myself. I was fortunate to have a bed next to a window with a patio and a view. Lizards and cats scampered around outside. Overall, the staff was amazingly caring. I can list so many people who went above and beyond their basic responsibilities with a smile and a kind word. Nurses provided medications on time, and listened to my concerns both before and after surgery. CNAs brought coffee, opened and closed the drapes, cut up my food when I couldn't, gave me a shower, and encouraged me to get well. Even the housekeeper greeted me with a smile. The occupational and physical therapists helped me regain some level of independence with both encouragement and persistence. Coordination with my health care provider was excellent when it was time for me to go home including ordering medical supplies and therapy support. Nobody wants to end up in a rehab facility, but being at Park View was as supportive as it could be. There were a couple of minor inconveniences. The therapy schedule was quite flexible so the patient wasn't certain what time a therapist would arrive. The air conditioning blew strong and cold directly at my part of the room. Again, these were minor in the overall experience. I'm grateful for the care I received. read more

I was at park view from 6/24 to 7/7 recover from hip replacement and a broken femur. I was scared to go as I have never been to any facility. I told the cpa that first helped me about being scared and his response was comforting and their constant round the clock care put my fears to rest. It was clean and the care givers were all so loving and patient and happy! Some of them there for many years! They work fast and always let you know if they can't get to you right then due to finishing up w/ another patient they'd be right there never made you wait long at all.the staff all work together well. There was a good alternative menu available if you wanted something different. The PT department was very good and listened to you and found many alternative exercises fitting your needs. I highly recommend this facility and I would go back again if I need to go recover from surgery. RJArnett read more

It was almost a year since my husband got out of Park View Post Acute. Fortunately there was only two beds in the room he was in. The facility was clean and loved everything appeared to run smoothly. The staff was wonderful that took the time to treat every patient with respect helped the patient to enjoy their stay. They were very helpful to their patients family. They did rounds once a week with the patient and family members to explain the goals and the patients progress. This was very helpful to me in understanding my husbands progress. I was at Park View for the month of January 2020. I was pleased with the cleanliness of the place and the friendliness and professionalism of the staff. The floors were washed every day and rooms were deep cleaned every month. I felt cared for, cared about, and respected by the staff. It was great. I never heard an angry or unkind word from anyone. The doctor stayed on top of my case, as obvious by the comments he made and adjustments to my treatments. I would very much recommend Park View to anyone. read more

I recently spent 5 weeks at Park View and received excellent care and rehab there! Staff was friendly and caring. Nurses sent me back to the hospital when I was feeling very ill and literally saved my life. I had two blood clots and very low iron. After treatment I returned to Park View and continued my Rehab. You need to be shut down!!! My mother was admitted after a surgery to remove a blood clot from her leg!!! Lastnight she left unresponsive to the ER!! You people failed to realize she was septic and severely malnourished! She is now on a ventilator and fighting for her life! How do you not notice she hadn't eaten or drank anything in over a week?? How do you not realize she is not urinating??? How do you not realize her blood pressure and heart rate were irregular?? If you care at all about your lived ones avoid this place at all costs!! read more

My husband was here for six days of IV antibiotics. It's not the most glamorous place. Coming from a luxurious private room at Sutter Hospital in Santa Rosa, having to share a room with two other guys was a shock. The place is packed with patients in every bed and their visitors. There's always plenty of staff. And everyone was so warm and kind. It feels like a big family home. The staff is so caring and genuinely committed to making patients feel better. They were quick to respond and gave quality care. Whatever the shortcomings of this older facility, the homey feeling makes up for it in every way. read more

Here for pt after breaking my leg. And rest. The pt is great. The food marginal. Seems like there are no regulations on television hours. I have 2 roommates. TV on until late in the night and volume high. I'm tired of asking to turn it down so I put two pillows over my head. No visitors policy. People come in and out all day and night sometimes until 10pm. One lady on my room has visitors back to back 10 hours a day. There is no such thing as rest here. The noise level is a 9. read more

I highly recommend Park View Gardens Post-Acute facility. My 90 year old Mom was first there (at age 85) for physical therapy after breaking her wrist and arm after a bad fall. They took very good care of her, and the therapy sessions really helped. After 15 days she was able to go back home. I noticed that they also had long term care and kept that in mind for her future years. For about 5 years our family took care of her in her apartment and someone was there all day, but not while she slept. As her dementia progressed, she needed 24/7 care and we realized we would have to place her in a long term facility. This was a difficult decision for the family. I called various places, but kept coming back to PVG (Park View Gardens) because of her prior experience there, so decided to move her there. Ricci Erfe-Mejia, the director of admissions gave us a tour of the long-term care area. Some facilities smell of urine, a bad sign of how clean the facility is. I was impressed with the cleanliness at PVG and all the CNA's (Certified Nurse Assistants) moving about taking care of their patients. They also offered daily activities and have a music room for those that want or are able to participate. They even provide patient appointments to get your hair and nails done in the facility. The facility sits in a park-like setting, hence the name Park View Gardens. I took our Mom out there a couple of times to enjoy the sunny weather while she sipped on her hot chocolate. I could tell the facility was efficiently run and all the various employees in their respective office areas have been wonderful to deal with. They are great at keeping the family involved and updated with anything that comes up. Once our Mom moved in, my sister and I alternated days. Each day our Mom would see one of us, from the afternoon until she went to bed at night. I was also there most of Sat. until she went to bed and my sister most of Sunday. We didn't want her to feel like we dumped her there and abandoned her. The inside visits were really helping her spirits and PVG was very accommodating to her needs, as well as ours. Of course this was back in January thru March 13 of this year, before Covid19 changed everything. On March 14 there were county mandated signs all over the place that no one could go in anymore, our worst fear!! PVG has been fantastic! Since we could not go inside, they scheduled window visits. In April we also started Zoom sessions every Sunday, at 11:30am via the Activities team. This allows the rest of the family that live further away to visit with her as well and stay in touch. The CNAs are very caring, friendly and supportive, and we are grateful that they are there for our Mom. Whenever there are any issues, the nurses are very attentive to her needs. They also have doctors on call there that make the rounds visiting each patient. Everyone there, from the offices, the CNAs, therapists, nurses, doctors, the front lobby personnel, the kitchen staff and maintenance are very friendly, understanding and always willing to help. I really can't say enough about PVG. With Covid19 in the picture, we really appreciate the transparency in which they conduct themselves. The patients and staff are tested each week. Because of the nature of Covid19 now and then a staff member might come up positive and they isolate themselves and cancel all window visits to keep the patients safe. Recently a patient was found to test positive, we were contacted and told about it, they have isolated the person and their area with a plastic barrier, to insure the safety and health of the other patients. They are checking the patients every 4 hours and are very diligent about keeping them safe. They follow all the health regulated rules and are very transparent in keeping us updated. While it's hard to not see our Mom in person, it's very comforting to know all the efforts that PVG and staff put forth in keeping the patients cared for, safe and tested.
