It is a place where I would be honored in pursuing employment after completing my clinicals. During my clinical experience, I felt encouraged to learn and was consistently supported by staff who were willing to share their knowledge while maintaining a strong focus on proper patient centered care. Having a grandfather with Parkinson's disease deepened my understanding of the importance of patience, dignity, and compassionate care; values that Creekview consistently demonstrates. I am truly appreciative of the professionalism and empathy shown by the entire team, and I would feel very confident entrusting my own family's care to this facility. If you're looking for short-term skilled nursing, I'm not so sure. I brought my mom in here to recover from major surgery. They screwed up the medications for the first 2 to 3 days until I came in and sorted it out myself. And if you're doing this, just be aware, that if the hospital transfers a patient, it can take them a minimum of 12 hours to get the medication to their facility, but no one will tell you this. So as a result, my mother who is suffering from extreme pain due to major surgery, had to wait almost 12 hours for medication because once I realized what was going on, I had two traipse to her house and procure some myself. And even once I thought we sorted it out, they still kept forgetting to give her some of the meds she was supposed to have for her eyes. It was eminently frustrating. They treat everyone like a dementia patient whether they have dementia or not. This is a material problem for people who do not have dementia. If someone is trying to recover from something, it means they will not get the attention, exercise, etc. that they need to actually recover. They'll be told to stay in bed and not move. Not helpful. They are shortstaffed on skilled nurses for the facility. There's basically one per shift and that is the only person who is allowed to do much of anything other than very basic stuff. Another material problem in my point of view The facility itself is very nice, clean, with a variety of separate spaces that are all well maintained. The food was high-quality. There are enough people around that you can feel like at least someone is always on duty and attending to the needs of the residence. Everyone who works there is delightful. To be fair, they listen to my concerns and tried to rectify and respond in a timely manner. If this is considered one of the best of the best, I'm alarmed for what that means for my own old age. It was OK. Like I said, if you're using it for long-term Alzheimer's support it's probably fine.

Jennifer Watson as the new director assistant you are highly orgonized and efficient, honest , friendly, kind, approachable and show great empathy for the work you do to all who are around you. You are Profesional and an excellent leader at Care Meridian. I want to thankyour team at Care Meridian for all the hard work you do each and every day to attend to Veterans who sacrificed their lives for our freedom. I have known my adopted Orphaned Veteran for eight years and for the last eight months he's been in and out of ICU and different care facilities. The one recommendation I can give to anyone who has a love one in any care facility is that you have to be involved. Your love ones needs emotional support. Create a positive space that feels like home, hold their hand ,make them laugh, rub their feet, put their favorite music, be their cheerleader when they are trying to walk again etc. emotionally they need you physically their so they know they are cared about. As much as you want any facility to be everything to your love ones they just can't. Their busy giving their meds and helping them stay alive. My friend is a professional real estate person building care facility and it is big business so even though there's excellent staff in a Facility they are governed by the people that are above them and the stockholders want to make their money on their Investments. Which one of the hardest challenges is keeping staff due to lower wages as care facilities can't compete with hospital wages. If your love one becomes ill and needs your support get with your doctor do an FMLA for your job and you can get disability up to one year. Disability earnings is about as much as your take home so you wont be financially burdened while caring for your love. You can also hire a family member to go visit a few days of the week if you can't and learn what additional support you can give to your loveone to make their home away from home btop notch. These are some of the best RN Nurses that Pat and I have gotten to know that are top notch very caring and respectful and knowledgeable in the care they give to pat. Update 6/29/2019 i want to give a special thank you to Olivia cna you are warm and caring and you go above and beyond to make my husband stay comfortable. Many people don't understand what it's like to live with severe ptsd and I myself is still learning from pat. You want above and beyond last night as my husband awoke from one of his war nightmares and you were supportive and we appreciate it. His body was in the fight and flight mode which can be scarily to watch so I thank you from the bottom of my heart. 7/15/2019 for the récord Lenard who is contracted by CM does an excellent job with changing the trac and he friendly, kind and personable. The day nurses Maria, Chico, shelia, and Richard gave excellent care in wound changing and changing cánula and cleaning and bandage changes soon hubby will be going home. 07/27/2019 Hubby is home thank you Jesus and thank you to VA Palo Alto for removing trac , getting hubby off oxygen and peg removal. What have I learned in this journey . Just because this place is VA contracted I don't recommend this place for veterans who have PTSD. The staff doesn't have training on how to deal with a wounded war veteran with ptsd. This place will not rehabilitate your loved one as they advertise under a new name Nerorestorative in Pleasanton. Palo Alto VA did an excellent job sending my hubby to the nursing home only to have him in bed for 3 weeks as they just don't have the staff to do what patients need to thrive. The care facility allows staff sent from temp agencies without trac experience to attend to the patients this creating more room for error and the patients are the ones that suffer especially those who can't speak when they get the wrong meds or are hurt by careless nurses in the evening doing trac care. Their is no activity for the patients at Caremeridian/nerorestortive so depression is high in this place. My husband saw VA physiatrist who said in order for person to have good mental well being they need 5 things. 1. Sunshine 2. Pet 3. Companion/family 4. Care facility with activity to stimulate the brain 5. Music Bottom line I don't recommend this place only if your love one ready to die n just needs meds n wound care as their is excellent nurses among the disorganized staff sent from the agency . My husband is now home happy and wanting to put this all behind him as we fought hard to get him out the door so he rehabilitated himself. I was able to take him out 10am-8pm away from the facility 5 days a week to get him walking and breathing fresh air and sunshine.

    This place needs a new D O N . The turnover rate is 93% . Sanitary conditions doesn't meet State…read morestandards. The Cooks do double duty as Housekeepers which are against state regulations. This place needs to be SHUTDOWN. Actually 1star is being quite generous. Being a professionally trained Chef I shouldn't have taken a position beneath me under paid and over worked .I enjoy challenges but working for this corporation was the worse experience in my career. Employees are disposable . Ten to twelve hour plus shifts for employees at $ 15.00. Agencies employees make twice the pay as the employees of this corporation. I wouldn't place my dog in this rehabilitation center. FOR PROFIT CARE FACILITY MONEY BEFORE PATIENTS.

From the Executive Director to the Marketing Director to the Activity Director & Assistant Activity Director to the Chef and ALL the caretakers and those who maintain the facility and those who clean the rooms, and the front desk assistant........they are all very professional and so caring! This is a very tough industry to work in. The kind care all the caregivers provide my Dad is amazing and honestly unbelievable. They are all positive and upbeat with the residents and very focused on providing appropriate care while also adjusting to their needs. They really work to bring some joy to the residents in this very trying time in their lives. The communication between all departments is quick, consistent and thorough. They also do a superb job of keeping the family informed on how my Dad spends the day, what "kind of day" he is having, any health changes or challenges in his care, etc. It was a very tough decision to move my Dad to a memory care community. When I am at home at night I know that my Dad is where he needs to be and he is very well cared for. Blessings to all at Sunol Creek Memory Care....it is earned and deserved.

One lady yelled all day long. My husbands roommate was up all night with the lights on. So my husband was not able to sleep. The food was terrible. My brother survived a 6 month stay at Stanford…read morehospital where he underwent several surgeries ,endured a heart attack, Coma, lots of complications I was told each week by a different Dr that I should get him to Hospice to which I would explain that I heard that same suggestion each week and each week the new Dr would call me and say what a miracle that he recovered from from the prior week. The answer was NO. My brother had compilations from his feeding tube. It was placed because he forgot how to chew and swallow. It happens a lot after a coma. The bad thing was when his stomach would fill with fluids and he would have to get it pumped out by surgery. I was told about every 3 months.. So when my brother was released to Stratford, He was excited to be out of the hospital but scared also. Soon enough I was not so sure. My stomach and intestines resulting from kidney Failure. He was the last sibling I had as there were 4 kids total with me being last and my brother the oldest at 64. It started with the dude new staff members. Never answering requests for certain foods, not giving him proper care by not cutting his 1 inch long nails, or bringing him what he requested to eat or showers etc. then the Dr . He would only see a Dr online video chat. The staff that would give him random pills from this Dr he spoke with. When my brother would get his pills he would say " What is this?" They would answer back very shortly and yell " Your Medicine!" With No regard to tell him what it was for. My Brother asked me repeatedly to get him help with his Social Security and each time I would go up to the counter and request help for him. Not once would they. Also the director would always want to do a lengthy questionnaire for my brother regarding end of life and I would always tell her to let him fill it out and help him. It wasn't my business as he was getting better by the day. He started to go grocery shopping and I would take him. He hated their food and started buying his own. His stomach was really hurting him one day and I asked if they had asked or said anything about getting it drained. It was getting quite large in the belly area. He had no clue what I was talking about. I went to the front desk and let them know what was happening. It was poison for my brother. They called 911 that night and my brother went into emergency surgery . He stayed in hospital for 4 days before returning to Stratford.. I was very surprised they were not aware of it again. My brother couldn't quite walk on his own and asked for therapy one day along with me he was told it's not covered by insurance. He was bedridden. His clothes were always missing also. I Was busy around Xmas an ZNew Years day. Do We and had only taken him shopping for groceries and was felling pretty bad. I was heading out of town when he called me and said his stomach was hurting once again. It was filling up fast. He said he would let staff know. The next day I was headed to Reno for New Year's Eve. I called my brother and asked how things were going, he said his stomach was terrible. I asked if I should take him to hospital as an ambulance was not covered per the manager there at Stratford. My brother said "No, the staff said they would get him to hospital. I asked that he call me the next day for update. He agreed. 5 days later on Dec 31st my phone rang and it was Bebe the Director at Stratford, she said my brother was not responsive. I asked where he was as I was suprised she was calling me. She replied he was at Stratford. I said WHAT? He should have been in hospital as of 5 days earlier. She called me back and said she had no record of that request. I then told her I was coming to Stratford and to not touch any of his stuff. I got a call from Hospital telling me there was nothing they could do. It would require surgery I was told a few hours later on New Years Day that he passed. I kept getting calls from BeBe the director asking if he passed. It was very rude. I did let her know I was coming to retrieve his items. I wanted to see how much food he had eaten. I asked Bebe to please leave his area alone. I wanted to know how he left it a week solid. She assured me she would wait until I got there. Once I arrived, nothing was left. Everything was tossed. . I spoke to someone who said he was in pain and slept in his bed the entire time. No one checked on him. This person also was threatened by staff not to speak to me or he would e punished by his meds being held. Stratford killed my brother. They never did anything to help him. They ignored his request even for help with his social security benefits. He wanted to get outta there and would use those to help him. I will never forgive Raymond the front desk clerk or Be Be the director and the gal in charge of helping him with paperwork. I am upset she never raised a finger to help him.
