We've never seen an upcharge for a large ice cube! Ever! My wife emailed the GM after the fact, and the manager explained that the Old Fashioned is traditionally served with a large ice cube. The server should have asked my wife if she wanted a large ice cube with her Negroni. Hazelwood isn't a particularly cheap outing. They do appear to be nickel-and-diming in this situation.

At that point, though, after two international flights to get me to Minneapolis, I just wanted to be ready for my flight home.
