My family experienced repeated problems with severe understaffing that appeared to affect nearly every aspect of patient care. At times, it seemed there was only one nurse or aide responsible for as many as 17 patients. Whether that was the official staffing ratio or not, the level of care reflected a facility stretched far beyond its limits. The most upsetting experience was waiting well over an hour after using the call light for help with toileting. By the time anyone responded, my family member had already soiled herself. We ended up providing the care ourselves most of the time because we could no longer depend on timely assistance. On another occasion, a broken bathroom call light left her stranded for approximately 45 minutes, forcing her to attempt an unsafe transfer on her own and creating a serious fall risk. Unfortunately, these were not isolated incidents. During visits, I regularly saw multiple call lights going unanswered, families wandering the halls searching for help, and patients waiting far too long for basic care. At one point, while patients were waiting for assistance, several staff members were gathered in a back room eating together, leaving the floor seemingly unattended. Employees need to stagger breaks. The food was another major disappointment. This facility promotes itself as a high-end rehabilitation center, yet meals were consistently poor quality and lacked the fresh, nutritious foods patients need to heal. My family often felt compelled to bring outside meals, yet there wasn't even a microwave available for reheating food. I was also concerned by several operational issues, including the use of a shared open ice cooler instead of a more sanitary dispensing system. Delays in laboratory testing that could impact dialysis patients, where timely lab results are critical for safe treatment. There was no onsite lab and lab results were not posted or available for 2 days. What saddened me most was seeing vulnerable patients--including veterans and retired healthcare professionals--left waiting for basic needs such as toileting, hygiene, and assistance. No one recovering from surgery, illness, or dialysis should have to worry whether help will come in time. I understand that healthcare facilities everywhere face staffing challenges, but patients and their families deserve safe, compassionate, and timely care. My hope is that management addresses the chronic understaffing and operational shortcomings so future residents receive the level of care this facility promises. PS: The chief of culture Jim White owns casinos. This man has no clue about how healthcare is run or works.

If you're checking into a rehab center with kind and professional staff, this is the place to go. They really follow through on your requests. All rooms are private with private bathrooms and are disability accessible with good TVs. The beds are comfy with regular and air mattresses available. The food is excellent and if I needed anything, they brought it to me quickly. They have an all day menu available which is ideal if the daily menu items are not to your liking. The food is nutritious and you can ask for special diets like cardiac, diabetic and vegan. The PTs and OTs, like Linsey are all encouraging and really cheered me on. The equipment and rec room stand out. The staff all recommend exercises during your off time. The staff addresses your health goals. I really need to give a shout out to the excellent staff. Everyone quickly gets you what you need including the front desk receptioness . The RNs and LVNs who are outstanding and stand out for their professional and personal qualities are Danielle, Joanna, Robyn, and Ingris. The exceptional CNAs who really stand out for their care and hard work include Jasmine, Nicole, Rice, and Ashley. The nurse practitioners that particularly stand out for their care, listening skills, and explanations are Jenn, Lauren, and Sue. I was medically and physically taken well care of. I highly recommend you choosing The Center at Parmer for your skilled nursing and physical rehab needs.

    Just visited a friend who was transferred to this facility from hospital yesterday. She needed…read moreassistance to go to toilet and punched the call button. She did that several times. 27 minutes later I went to the nurses' station to ask if someone would please come help her. A nurse reluctantly came. We were both very disappointed and discouraged. She said dr advised her this morning to give at least 30 minutes' notice if she needed assistance. Sometimes that is not possible. My friend advised this has been the standard since she arrived. She said she Sat is room for 30 minutes after ambulance dropped her off yesterday with no pillows on bed, blankets, etc or even being acknowledged by any staff that she was even there. The care there seems to be totally lacking. We are trying to get her moved to another facility now. Thank goodness she has the mental capacity to advocate for herself.

Heartland wasn't a perfect match for Mom, but it was a great match. I checked Mom in based on prior reviews, so I had high expectations. There were some bumps along the way, but I was really impressed with how dedicated the people were to meeting Mom's needs, making her comfortable, and resolving any issues that came up. Mom spent about three months at Heartland paid for by Medicare as rehab and about three months self-pay in their nursing care. It's all the same care, it's just a matter of who is paying. She spent a week on their assisted living floor trying to live independently, but it just didn't work. What I was struck by, at all levels, was how approachable everyone was. From the aides to the administrator, everyone was there for Mom and for our family. I wish I could name each of their names (I have them all written down here somewhere), but I'd miss someone important and I don't want to do that. More than anything else, Mom wanted to get back to her apartment at her old assisted living center, but Heartland took much better care of her and she was just too ill to return to her old lifestyle. Ultimately, Mom died at Heartland. I could wish for a better ending to her story, but not a better ending to her life. She died comfortably, loved, and well taken care of. Thank you Heartland and each of you who worked with Mom. I would have no hesitation recommending Heartland. Visit for yourself. The building is a bit worn, but the staff are wonderful, organized, and they react quickly to the needs of their patients and residents.

She would switch my appoints without any notification. I was early for a appointment, she stated persistently, I should leave and come back, I did not look at the time, but by the time I turned around to go back and it was 15 minute too late for my appointment, I was so disappointed. I was told,now I would need to do exercises at home only. I am experiencing excruciating pain in my arm with no improvement. They are very unprofessional and have no regard or concern for the client. They need an overhaul and rethink about therapist they are placing the clients with. Experience is everything. Take my advice and run don't walk, find a center that is fully staffed with knowledgeable professionals for your physical therapy needs.
