Staff is exceptional from the welcome/release staff , to the physical therapists, to those providing other services like meals, meds, other needs. The key is the top level is very serious that the patients have the most positive experience so that the patient can focus totally on recovery. Amazing Staff and the facility is nice also.

My 76-year-old father was a resident at Ignite Medical Resort-Round Rock for 10 days before he…read moresuddenly and unexpectedly died overnight while in their care. I will not address the overall quality of care he received during his stay in this review. What I will address is what happened afterward -- or more accurately, what did not happen. At 1:30 a.m. on the morning of his death, I received a phone call from a constable, not from Ignite staff. The constable informed me that there had been a "pretty serious medical emergency" and that my father had been found unresponsive sometime between midnight and 12:30 a.m. He stated that staff had called 911 and that paramedics pronounced my father deceased around 1:00 a.m. I was in shock and did not know what questions to ask at that moment. It has now been three weeks since my father's death, and I have not heard from a single Ignite employee -- not a nurse, not a manager, not a care coordinator, not anyone. No one has contacted me to explain what happened. No one has offered condolences. No one has acknowledged that my father died while in their care. To be clear, I am not alleging that Ignite caused my father's death. However, the complete absence of communication is deeply disturbing and unacceptable. The expectation was recovery -- not death in the middle of the night with no explanation afterward. Why did no unit manager, social worker, or director of nursing contact me as the primary contact? Why was there no call explaining the sequence of events? A simple, humane conversation -- "The nurse entered his room to administer his midnight medication, found him unresponsive, we immediately called 911 and initiated life-saving measures, and we are so sorry for your loss" -- never happened. Instead, I am left to imagine what MAY have occurred. For a family who entrusted this facility with their loved one's care, that silence is unfathomable.

Mom has been here for a week and a half, and she's doing VERY well. The ONLY complaint I've had at…read moreall is sometimes I've come in and her dinner is there, but she doesn't know how to raise her bed up so she's sort of lying there unable to eat. I know they only take people who are able to feed themselves (she is), so the kitchen staff may not think about it when they drop off their trays. I've mentioned it to the nurse, so hopefully that will be remedied. Mom has made so much progress...it's really amazing for someone her age who has been through hell. I'm very impressed with this place generally...everyone is very kind. If I had it to do over I would probably hire a sitter to help with mealtimes, but the confusion is wearing off and she'll be home soon...just a few more days. Thank you, Rapid Discovery!

They provide supplies, keep my room clean, and support me in my efforts to regain my independence! The Center at Parmer has an excellent gym and therapy department! PT/OT encouraged me to learn skills to increase my strength, balance, and flexibility. The kitchen staff made food that was tasty and nutritious. Not gourmet, but hearty and varied. All the staff treated me as a person, not just as one of many! I highly recommend The Center at Parmer!

The next visit they just put me on a bike with no introduction on who they were (that they were the one I would be seeing instead of the PT) and what we were doing. They were so apathetic about what I was doing, and again, with zero conversation or explanation. There was another woman who had recently had knee surgery. They put her on some machine and left her there--not paying attention to form at all. The Assistants (??) were having very animated conversations over all of our heads... Honestly, I could have just watched videos on YouTube and learned more about what I needed to improve my situation. That is what I did. I let them know my concerns. The management did not seem to be concerned either. I was very disappointed.

My sister, fortunately, was referred to stay at Gracey Wood II a little over two years ago for hospice care. I have experienced a lot of different care centers for family members over my 77 + years but their facility is BY FAR the best and most consistent quality care center I have ever seen. Every time I would visit I had nothing but the best experience with their entire staff, protocols and cleanliness. They have been such a relief to me and my other family members to know that our sister was treated so well, humanely and professionally. She passed away two days ago and we will always be so thankful for the way they took such great care of her! Thank you to Gracey Woos II and their entire staff and administration! Heartland wasn't a perfect match for Mom, but it was a great match. I checked Mom in based on prior reviews, so I had high expectations. There were some bumps along the way, but I was really impressed with how dedicated the people were to meeting Mom's needs, making her comfortable, and resolving any issues that came up. Mom spent about three months at Heartland paid for by Medicare as rehab and about three months self-pay in their nursing care. It's all the same care, it's just a matter of who is paying. She spent a week on their assisted living floor trying to live independently, but it just didn't work. What I was struck by, at all levels, was how approachable everyone was. From the aides to the administrator, everyone was there for Mom and for our family. I wish I could name each of their names (I have them all written down here somewhere), but I'd miss someone important and I don't want to do that. More than anything else, Mom wanted to get back to her apartment at her old assisted living center, but Heartland took much better care of her and she was just too ill to return to her old lifestyle. Ultimately, Mom died at Heartland. I could wish for a better ending to her story, but not a better ending to her life. She died comfortably, loved, and well taken care of. Thank you Heartland and each of you who worked with Mom. I would have no hesitation recommending Heartland. Visit for yourself. The building is a bit worn, but the staff are wonderful, organized, and they react quickly to the needs of their patients and residents.

Our community service organization volunteers every month at Sedona Trace. The administrators and nurses were helpful in sharing any resident restrictions and locating the birthday folks for the month. The facility provides comprehensive and care and treatment plans for its residents. They have full-time In-House therapy and specialized skilled nursing.

He pushed me hard during PT, and gave me exercises to do at home. It's been a few months since he released me, and overall I'm feeling great. If I ever push myself to hard on my indoor bike and feel any sort of pain, I immediately do my at-home exercises and feel better within days. He really helped me! There were some minor billing issues, but the front office fixed them quickly when I called.

Be cautious with Endeavor Physical Therapy at Four Points (Austin, TX). I was directed to use their…read moreapp for home exercises but was later charged for "remote therapy" without clear notice or consent. Even two months after my last visit, I kept receiving new charges via text despite deleting the app. The office manager was unhelpful, and although accounting responded, the issue remains unresolved. Poor billing practices (H2 Health) and lack of accountability--would not recommend.
